Role of electron microscopy in the evaluation of soft tissue neoplasms, with emphasis on spindle cell and pleomorphic tumors.

The current significant role of transmission electron microscopy in the evaluation of soft tissue tumors when correlated with conventional histological and immunohistochemical studies is discussed for the following entities: myxofibrosarcoma, storiform-pleomorphic fibrosarcoma (malignant fibrous histiocytoma), and myofibrosarcoma; dermatofibrosarcoma protuberans; hemangiopericytoma; monophasic synovial sarcoma; extrarenal rhabdoid tumor; soft tissue perineurioma; and gastrointestinal stromal tumors, notably the so-called autonomic nerve variant.

Extraskeletal fibroblastic osteosarcoma in a rabbit (Oryctolagus cuniculus).

A seven-year-old rabbit was presented with a non-painful mass in the right upper lip. Tissue samples from the mass at three different stages of the disease process were diagnosed sequentially as an osteogenic sarcoma, a fibroblastic tumour with rudimentary osteoid formation and, lastly, a fibrosarcoma. No bone involvement or metastatic disease was found. The final diagnosis was of an extraskeletal fibroblastic osteosarcoma, which is, to the authors' knowledge, the first reported such case in a domestic rabbit. Without the first two tissue samples, an incorrect diagnosis would have been reached; therefore, this disease may be underrepresented in the literature.

Antitumor activity of echinosporin.

Echinosporin isolated from a Streptomyces culture showed antitumor activity against rodent tumor models such as leukemia P388, P388/VCR, and fibrosarcoma Meth 1. It was marginally active against melanoma B16 and sarcoma 180. It was not active against Lewis lung carcinoma and xenograft MX-1. It inhibited the colony formation of HeLa S3 cells with a wide shoulder at low dose ranges. DNA, RNA, and protein synthesis were inhibited by echinosporin. It depressed WBC with nadir on day 3, but the recovery to the normal level after echinosporin injection was more rapid than that after mitomycin C.

[Effects of recombinant human interferon-alpha A/D on the growth of experimental tumors in mice].

The effects of recombinant human interferon-alpha A/D (rIFN-alpha A/D, a subtype of recombinant human leukocyte interferon with biological activities against murine tumor cells) on the growth of murine tumors were studied. rIFN-alpha A/D significantly inhibited the growth of mouse M5076 reticulum cell sarcoma, MOPC-104E myeloma, colon carcinoma 26 and Meth A fibrosarcoma by dose-dependent fashion. rIFN-alpha A/D also inhibited the metastases and growth of Lewis lung carcinoma and showed a synergistic effect with combination of cyclophosphamide. The antitumor activity of rIFN-alpha A/D was observed by intra-muscular, intravenous, subcutaneous, intraperitoneal injections or by the injection at the site of the tumors.

[Malignant change in fibrous dysplasia (author's transl)].

Report on 2 cases of poly-ostotic fibrous dysplasia combined with pigmented areas of the skin with secondary fibrosarcoma of the ulna or femur. They are analysed together with 49 cases published so far. The average age at the time of malignancy being diagnosed was 34.4 years. The average time from recognition of fibrous dysplasia to malignant changes was 17.7 years. In 14 cases local radiotherapy preceded the development of sarcoma. Femur and jaws were the most common sites. There were 30 osteosarcomas, 11 fibrosarcomas and 7 chondrosarcomas. The prognosis is bad: only 14 patients survived for 5 years. But malignant change is rare in fibrous dysplasia. It occurs in less than 1% of cases. Severe forms of poly-ostotic fibrous dysplasia which still progress in adults with a raised alkali phosphatase appear to show an increased tendency towards malignant degeneration.

Parallel antitumor, granuloma-forming and tumor-necrosis-factor-priming activities of mycoloyl glycolipids from Nocardia rubra that differ in carbohydrate moiety: structure-activity relationships.

Multiple intravenous injections (30 micrograms, ten times) in ICR mice of trehalose dimycolate and glucose monomycolate from Nocardia rubra, containing C36-48 mycolic acids, showed a prominent antitumor effect on a subcutaneously implanted sarcoma-180, an allogeneic sarcoma of mice with a significant granuloma formation in lungs, spleen and liver. On the other hand, mycoloyl glycolipids other than glucose monomycolate and trehalose dimycolate, such as mannose or fructose mycolate, showed no significant activity for tumor regression or granuloma formation in mice. Trehalose dimycolate and glucose monomycolate from N. rubra, and glucose monomycolate with C56-60 mycolic acids from Rhodococcus terrae also showed a distinctive priming activity for tumor necrosis factor (TNF), when lipopolysaccharide from Escherichia coli was administered as an eliciting agent. The TNF activity in the sera of mice was abrogated almost completely by anti-(murine TNF alpha) antibody with protein-A-agarose. Again in contrast, mannose and fructose mycolate from N. rubra and glucose monomycolate with C30-34 mycolic acids from Rhodococcus equi did not show such activities in mice. Meth-A, a syngeneic fibrosarcoma of BALB/c mice, was less sensitive to administration of glycolipids than sarcoma-180. These results indicated that the existence of a glucose or trehalose molecule was necessary for the expression of immunomodifying activities among various mycoloyl glycolipids differing in carbohydrate structure. However, since the administration of lipopolysaccharide was essentially required as an eliciting agent for the induction of TNF, while no eliciting agent was required for the antitumor activities, TNF does not seem to contribute directly to the antitumor activities of mycoloyl glycolipids in our systems. There was, however, a parallel structure-activity relationship among granuloma-forming, antitumor and TNF-priming activities, indicating that the structures of both the carbohydrate moiety and the mycoloyl residues influenced an initial step, such as macrophage activation, commonly and profoundly.

Micaceous and keratotic pseudoepitheliomatous balanitis and rapidly fatal fibrosarcoma of the penis occurring in the same patient.

A 55-year-old man had a rare, benign condition of the penis, micaceous and keratotic pseudoepitheliomatous balanitis, for 9 years before developing an aggressive soft tissue sarcoma of the glans. Five months after the appearance of this tumour, the patient died with widespread metastases. The sarcoma was small and superficial and the limited recorded experience suggested that local excision should offer a good prognosis. However, the histology showed poor differentiation and it would seem that this may be the important factor in the management and prognosis of penile sarcoma, as it is in sarcoma at other sites.

Feline intraocular tumors may arise from transformation of lens epithelium.

Feline ocular sarcomas are malignant intraocular neoplasms that are frequently associated with a history of ocular trauma. They usually present as fibrosarcomas, but some have both epithelial and mesenchymal features. The purpose of this study was to determine the cell of origin of a subset of feline intraocular sarcomas that display a mixed epithelial-mesenchymal phenotype, with elaboration of basement membrane-type matrix. We examined the morphology and histochemical and immunohistochemical phenotypes of nine feline intraocular sarcomas. Immunohistochemistry and in situ hybridization were performed to detect expression of crystallin alpha A. In addition, tumors were examined for expression of vimentin, cytokeratin, smooth muscle actin, desmin, melan A, neural cell adhesion molecule, S-100, glial fibrillary acidic protein, nerve growth factor receptor, and collagen type IV. Animals ranged from 7 to 17 years of age--no breed or sex predilection for tumor occurrence was present. Tumors were characterized by mixed epithelial and mesenchymal phenotypes, both of which elaborated basement membrane-type material and expressed vimentin highly. On the basis of collagen type IV and crystallin alpha A immunopositivity, we established that three of nine tumors were of lens epithelial origin. Expression of desmin and smooth muscle actin identified one tumor as a leiomyosarcoma. The remainder were undifferentiated sarcomas of myofibroblastic origin. This is the first report of lens epithelial neoplasia in clinical material from any species. The history and morphologic features of feline ocular sarcomas are reminiscent of feline vaccine-induced sarcomas. These tumors may share pathophysiologic similarities unique to this species.

Fibrosarcoma following radiotherapy for breast carcinoma: a case report and review of the literature.

The 17 patients with fibrosarcoma occurring after radiotherapy for breast carcinoma reported in the literature are the disastrous results of a treatment modality. All the reports concerning the sarcomas share the same feature-aggressive nature with a latent period of 4 years or more. A patient report of fibrosarcoma occurring 2 years after radiotherapy for breast carcinoma with the shortest latent period in the literature is presented and the literature is reviewed. The tumor was localized within an irradiated area and there was a clear histological distinction between the previous and current tumors. The tumor was found to encase the axillary artery and the brachial plexus. For a curative resection, a forequarter amputation was performed and the defect was closed by means of a posterior arm flap. Despite the aggressiveness of the tumors the patient had no distant metastases 1 year after the diagnosis of fibrosarcoma.

Anti-tumor effect of electrotherapy alone or in combination with interleukin-2 in mice with sarcoma and melanoma tumors.

Electrotherapy with direct current (DC) was performed on two murine tumor models, fibrosarcoma SA-1 and melanoma B16. Three Pt/Ir cathodes were inserted directly into the subcutaneous tumors and two anodes subcutaneously in the vicinity of the tumor. Significant tumor growth delay was achieved after electrotherapy and was dependent on DC intensity (0.6, 1.0, 1.4 and 1.8 mA). Melanoma B16 tumors were more sensitive to electrotherapy than SA-1 tumors. In order to enhance the antitumor effect of electrotherapy, combined treatment with interleukin-2 (IL-2) was performed. When both therapies were combined significant tumor growth delay and also higher curability rate was achieved. The results imply that electrotherapy can be an effective antitumor therapy and that the effects can be enhanced with additional IL-2 therapy.

Features of prostatic sarcomas in combined aspiration and punch biopsies.

Of malignant prostatic tumors, 0.1% to 0.2% are believed to be sarcomas; about 30% of these are observed in the first decade of life. We present the morphologic findings in three patients aged 55 to 70 years in whom an osteosarcoma and two leiomyosarcomas were demonstrated by combined aspiration and punch biopsies. The sarcomas clearly differed cytologically from the various forms of prostatic carcinoma. Some difficulties in the distinction of pleomorphic tumor cells of leiomyosarcomas from pleomorphic epithelial cells from the seminal vesicle may occur. One fibrosarcoma was diagnosed by punch biopsy alone in a 57-old-year patient. The incidence of sarcomas in our material was 0.17% of all malignant tumors of the prostate.

Sarcomatoid neoplasms of the breast: proposed definitions for biphasic and monophasic sarcomatoid mammary carcinomas.

Biphasic sarcomatoid carcinomas (SC) of the breast are defined as tumors that are composed of overtly carcinomatous and sarcoma-like elements and were known in the past as "carcinosarcomas." On the other hand, monophasic SC show features that are similar to those of sarcomas, but epithelial differentiation is detected in the former of these lesions by immunohistochemical methods. Adequate tissue sampling often is required to demonstrate both components of biphasic SC; if properly performed, this procedure greatly reduces the diagnosis of true mammary "sarcoma." The carcinomatous element of biphasic tumors is characterized either by features of a squamous carcinoma or an invasive ductal adenocarcinoma. Squamous differentiation is more often associated with a spindle-cell, fibrosarcoma-like, or malignant fibrous histiocytoma (MFH)-like sarcomatoid component, whereas adenocarcinomas usually are admixed with sarcoma-like tissues having "heterologous" properties. The overtly epithelial elements of biphasic SC are always immunoreactive for keratin, but areas that simulate sarcoma express this protein more heterogeneously. Both components may show positivity for vimentin, although the sarcoma-like elements do so more consistently. Monophasic SC is composed solely of fusiform and pleomorphic cells that are keratin-positive despite their mimicry of sarcomatous differentiation. This tumor variant commonly assumes a myxoid, angiomatoid, or storiform growth pattern, without recognizable carcinomatous foci by light microscopy. Prognosis and involvement of regional lymph nodes in mammary SC cases is controversial, because different tumors have been grouped together in the past for the analysis of these variables. However, SC generally has a less favorable outcome than that of ordinary invasive carcinomas of the breast.(ABSTRACT TRUNCATED AT 250 WORDS)
