Homozygous transfusion-dependent beta-thalassemia patients manifest cardiac, hepatic, endocrine, and metabolic disorders attributable to chronic anoxia and iron overload. Short stature, delayed sexual maturation, diabetes mellitus, hypothyroidism, hypoparathyroidism, and metabolic bone disease can and should be diagnosed as early as possible so that the intervention can be fruitful. Primary or secondary amenorrhea is due primarily to pituitary gonadotrope hemosiderosis, as attested by pathology data and the demonstration in vivo of nonstimulable follicle-stimulating hormone and luteinizing hormone release and secretion after the exogenous administration of gonadotropin-releasing hormone or its agonistic analogs. Ovulation can be achieved with the use of exogenous gonadotropins provided that the ovary has no siderosis (as seen in neglected patients) or damage induced by drugs used for bone marrow transplantation. Once pregnancy is achieved, it should be considered high risk and be dealt with or cared for by an expert team to ensure a successful outcome.

PURPOSE OF REVIEW: There have been recent legislative efforts in the USA to implement mandated parental involvement for minor adolescents seeking services for contraception and, in some cases, sexually transmitted diseases. RECENT FINDINGS: Research collecting information from adolescents in family planning clinics shows that the majority report that a parent knows they use the clinic for sexual health services. Similarly, a majority of minor adolescent females expects that they would use family planning clinics for prescription contraception, even if parental involvement were required. About one in five expected she would obtain contraception from a private doctor. A substantial minority, however, would engage in unsafe sex, and this response would be more common among teenagers whose parents did not know they used the clinic. Parental involvement laws for contraception might discourage some teenagers from using other services, such as testing or treatment for sexually transmitted diseases. SUMMARY: It is likely that mandated parental involvement laws would do little to decrease levels of teenage sexual activity but would, instead, increase rates of teenage pregnancy and, perhaps, rates of sexually transmitted diseases.
Afghan refugees in Iran, like many Muslims around the world, think that using contraceptives is against their religious beliefs. The majority of Iranians also thought so until a decade ago. Since then an all-encompassing social movement has emerged in Iran in which women and young people have played decisive roles. This movement has led to an attitude shift towards acceptance of family planning across Iranian society. High-ranking clerics, responding to the social movement, issued progressive edicts indicating that poor families could practise family planning. This paper reports the findings of focus group studies carried out among Afghan refugee men in two communities in Iran. The findings indicate that exposure to Iranian life, especially the rulings of Iranian clerics, have influenced Afghans' views on family planning. The findings also indicate that a system of justification is needed to help people avoid both social sanctions and individual stresses resulting from a controversial practice regarded as a sin for centuries.
