Biomaterials in orthopaedic surgery: effects of different hydroxyapatites and demineralized bone matrix on proliferation rate and bone matrix synthesis by human osteoblasts.

The effects of different biomaterials, used in orthopaedic surgery for bone substitution and/or prosthesis coating to improve fixation and durability of prosthetic implants, have been studied in vitro on cell growth and bone matrix synthesis by human osteoblasts. The materials were a bovine collagen matrix (Osteovit, B. Braun Melsungen AG, Melsungen, Germany) and two hydroxyapatite (Ceros 80, Robert Mathys Co, Instrumentenfabrik Bettlach, Germany and Ostilit, Howmedica International, Staines House, UK). Cell proliferation and bone matrix synthesis were assessed by incorporation of [3H]thymidine and [3H]proline, respectively. Cell viability in the presence of the materials was also morphologically controlled using phase-contrast microscopy. Exposure to Osteovit caused increased proliferation of human osteoblasts, whereas both Ostilit and Ceros 80 induced a decreased cell growth. Osteoblast bone matrix synthesis was increased by all the materials tested.

Effects of hydrogen peroxide solutions on artificial hip joint implants.

This study was designed to elucidate the erosive effect of hydrogen peroxide solutions on the materials used for total-hip arthroplasty (THA). As test materials, cross-linked polyethylene, Ti-6Al-4V alloy, and thermal sprayed hydroxyapatite (HA) were used. Changes upon soaking in 3% hydrogen peroxide, before soaking, 1 minute after soaking, 10 minutes after soaking, and 180 minutes after soaking were examined. Scanning electron microscope, Fourier transform infrared analysis, and x-ray diffraction were used for this examination. Hydrogen peroxide did not affect polyethylene, although notable changes in the Ti-6Al-4V alloy and HA did occur. These results indicate that caution should also be exercised to minimize erosion of prosthesis consisting of HA and Ti alloy when hydrogen peroxide solutions are used during total-hip arthroplasty.

Retention of oral microorganisms on cobalt-chromium alloy and dental acrylic resin with different surface finishes.

STATEMENT OF PROBLEM: The effect of surface finish of dental materials on the subsequent contamination by microorganisms. PURPOSE: This study compared the retention of Streptococcus oralis, Actinomyces viscosus, and Candida albicans on polished, sandblasted (fine and coarse) and electrobrightened cobalt-chromium alloy and dental acrylic resin to assess in vitro the effect of such techniques on prosthesis contamination. MATERIAL AND METHODS: Standardized cell suspensions were incubated with test materials for 1 hour at 37 degrees C, after which retained cells were counted by using image analysis (percentage area of a microscopic field covered by cells). RESULTS: Retention of bacterial cells was substantial (S oralis 12% to 20% and A viscosus 9% to 16%) irrespective of surface finish. Maximal retention was observed on cobalt-chromium alloy that had undergone fine sandblasting and electrobrightening (P < .01). For C albicans, an increase in surface roughness (0.15 to 3.53 microns) resulted in an increase in retention (3% to 9%). CONCLUSION: Cell size and the type of roughening significantly affected the retention of microorganisms on surfaces. Electrobrightening of cobalt-chromium alloy did not reduce the surface roughness or subsequent cell attachment.

Accuracy of survey questions for geriatric urinary incontinence.

PURPOSE: Risk factors, natural history, consequences, therapeutic responses and costs are all likely related to type of urinary incontinence, for example stress or urge. Yet few epidemiologic type specific data are available and only 1 study has been validated urodynamically. We compare the accuracy of a typical questionnaire used in a large epidemiologic study with the criterion standard of multichannel video urodynamic testing. MATERIALS AND METHODS: The questionnaire was administered before urodynamic testing to 132 subjects 65 years old or older, of whom 80% were women, all were mobile and none was severely demented. Responses to questionnaire items were compared to the criterion standard, singly and in combination, using a total of 4 a priori and post hoc strategies, including a computerized regression tree program. RESULTS: Overall, no analytic strategy correctly classified more than 67% of patients and none accurately classified even a single type of incontinence, including stress incontinence. CONCLUSIONS: Short questionnaires commonly used in epidemiologic studies correlated poorly with video urodynamic testing in incontinent older adults. Previously published information regarding prevalence of the types of incontinence should be reviewed in the light of these data.

Histopathological study of the use of freeze-dried allogenic dentin powder and True Bone Ceramic as apical barrier materials.

The biocompatibility of freeze-dried allogenic dentin powder and True Bone Ceramics (TBC), the latter prepared by incineration of bovine bone, as apical barrier materials was evaluated histopathologically in dogs. After pulpectomy, 80 root canals of 53 dog premolar teeth were prepared, and each material was tightly packed to the apical stop area. The remainder of the canal was filled with guttapercha by the lateral condensation method. Control teeth were filled by the lateral condensation method using gutta-percha without constructing apical barriers. In both the teeth filled with the dentin powder and those filled with TBC, multinucleate giant cells appeared on the bone resorption surface and the surface of the root canal wall, and marked bone resorption and positive tartrate-resistant acid phosphate staining were observed. Hard tissue had formed on the root canal wall and the filling material after 3 months. These findings suggest that the test materials are nonirritative and compatible with periapical tissue, and that they may be useful and clinically applicable as apical barrier materials.

The reproducibility of the allergy skin test scoring and interpretation by board-certified/board-eligible allergists.

BACKGROUND: Allergy skin testing is a cornerstone in the evaluation of the allergic patient. This seemingly simple test is subject to multiple variables that can affect the result. OBJECTIVE: To evaluate the degree of variability among board-certified/board-eligible allergists in the scoring and interpretation of allergen skin tests. MATERIALS AND METHODS: A series of allergen prick skin tests were digitally photographed and a questionnaire generated. Approximately 70 board-certified/board-eligible allergists were asked to grade each test item and to interpret them as positive, negative, or indeterminate or if they desired a followup intradermal test. RESULTS: Thirty-three interpretable responses were obtained. The majority of respondents (24) used a grading scale of 0 to 4. Agreement among physicians using a 0 to 4+ scale ranged from a standard deviation of 0.26 to 1.35, with greatest agreement on items with median/mode scores of 4+. The largest standard deviations were found on test items with median/mode scores of 1+ to 2+. Interpretation of the test items also showed greatest variation for those items with median/mode scores of 1+ to 2+. The number of intradermal tests requested ranged from 0 to 11 (of 22 test items). CONCLUSIONS: The results demonstrate interphysician variation in the scoring and interpretation of epicutaneous skin tests. A questionnaire such as the one used here may serve a useful quality control instrument to ensure reproducible scoring of skin tests. In addition, the results highlight the need for greater study on the clinical utility of intradermal skin testing when epicutaneous tests are negative or equivocal.

Adherence of Pseudomonas aeruginosa to inanimate polymers including biomaterials.

Cells of Pseudomonas aeruginosa were adhered to polymethyl methacrylate, polyvinyl acetate, polyvinyl chloride, polyhydroxyethyl methacrylate, mixed-acrylic, silicone, and natural latex materials. Planktonic bacteria and bacteria that adhered to the test materials were compared for their uptake of either L-[3,4,5-3H] leucine or [methyl-3H] thymidine during growth in a minimal medium. Leucine incorporation was reduced and thymidine uptake was negligible in adherent bacteria for up to 8 h following primary attachment by which time cells in the planktonic state showed active uptake of both substrates. These reduced uptake periods correlated with lag phases of growth of adherent cells as determined with a sonication-release plate count procedure and analyses of adenosine triphosphate (ATP). The extent of the lag phase of the adherent populations was dependent on initial densities of adhered cells and the nature of the substratum.

Vital marking of articular chondrocytes by retroviral infection using green fluorescence protein.

OBJECTIVE: One of the main open questions in chondrocyte transplantation is the fate of the implanted cells in vivo. We intended to establish prerequisites for such studies in animal models and to show the feasibility of this approach in rabbits. Isolated articular chondrocytes were retrovirally marked using green fluorescence protein (GFP) as a cell-specific marker in order to allow an in vivo follow-up of these cells. METHODS: Chondrocytes from rabbits, sheep, cattle and humans were isolated and infected with murine leukemia virus-derived retroviruses carrying the GFP gene. The influence of the host range of three packaging cell lines (PA317, PT67, PG13), start cell concentrations, number of cell passages and number of infection cycles on the efficiency of infection was investigated. Stability of GFP expression was followed by FACS analysis, confocal imaging and fluorescence microscopy. For in vivo follow-up of GFP expression we used marked allogeneic chondrocyte populations grown on scaffold material and implanted them into full-thickness defects in knee joints of rabbits. RESULTS: Retroviruses from all three packaging cell lines were able to infect rabbit and human chondrocytes, whereas only retroviruses released from PG13 cells were able to infect sheep and bovine chondrocytes efficiently. Optimization of the infection with these viruses resulted in efficiencies of 60-90% GFP-expressing chondrocytes. Populations of 100% marked chondrocytes were obtained by cell sorting. GFP expression stability of such marked chondrocyte populations was followed in monolayer culture and in 3-D culture on different scaffold materials. The expression of GFP was stable on all tested materials for at least 4 weeks. In monolayer culture GFP expression was stable for more than 8 months. In vivo, we observed stable GFP expression in the transplants during a four-week time course. CONCLUSION: Retroviral GFP gene transfer led to long-term expression in chondrocytes from rabbits, sheep, cattle and humans. Transgene expression and the number of implanted chondrocytes remain stable for at least 4 weeks in vivo. This method permits a rapid monitoring of chondrocytes and provides a basis for following the fate of these cells in vivo.

Comparison of human turning gait with the mechanical performance of lower limb prosthetic transverse rotation adapters.

Given the importance of minimizing transverse plane shear stress on soft tissue, several transverse rotational adapters (TRAs) are available for incorporation in lower limb prostheses. This study compares kinetic and kinematic data from human subjects during straight and turning gaits to the mechanical performance of several TRAs. Physiological data were collected from three individuals walking straight and turning at self-selected speeds around a 1 m radius circle. The average peak torques and range of motion for normal subjects while turning were 8.2 Nm and 26 degrees (outside leg), 11.8Nm and 20 degrees (inside leg), and 11.4 Nm and 20 degrees (right leg) during straight gait. Each TRA was mechanically tested without axial loading in a servo-hydraulic material testing system (MTS) over its rotational range at 0.5 dergrees/s and 60 degrees/s. The TRAs with axial compression were also tested at 0.5 degrees/s under a 736N (75kg mass) axial load. Applying these torques to the different TRAs yielded 3 to 35 degrees rotation, depending on the elastomer installed. Some TRAs had nearly constant stiffness, while others stiffened with rotation. The TRAs also varied in their average maximum stiffness from 0.4Nm/degree to 2.7Nm/degrees. Normal subjects exhibit interior vs. exterior asymmetrical torques and displacements; however, only one of the TRAs is designed to allow asymmetrical stiffness, and none have asymmetric ranges. Prosthetists and physicians can use these data to better interpret amputees' qualitative remarks and to prescribe the correct TRA and/or elastomer. This information also forms a basis for further design and development of novel torque absorbing prosthetic adapters.

The validity of the repeatable battery of neuropsychological status in acute stroke.

The construct validity of the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) was investigated in a sample of 210 acute ischemic stroke patients seen on an inpatient rehabilitation unit. Intercorrelations between the six index scores were found to be relatively consistent with previously published work. A principal components analysis yielded a two-factor (Language/Verbal Memory and Visuospatial/Visual Memory) solution that accounted for 61% of the variance. Correlations generated between the resulting factor scores, the Controlled Oral Word Association Test (COWA), the Visual Form Discrimination Test (VFD), Boston Diagnostic Aphasia Examination (BDAE) Complex Ideational Material Test (CIM), the presence of neglect as determined by Line Bisection Test performance, and the Mini Mental Status Examination (MMSE) supported the validity of these factors. A comparison of the obtained factor scores in a subgroup of 111 left and right hemispheric stroke patients showed that the left hemispheric stroke patients performed more poorly on the Language/Verbal Memory factor score than did right hemispheric stroke patients while the converse was true for the Visuospatial/Visual Memory factor score. Implications for the construct validity of the RBANS and its use and interpretation in clinical assessment are discussed.

Diagnosis of unilateral telencephalic hearing disorders. Evaluation of a simple psychoacoustic pattern discrimination test.

A new psychoacoustic pattern discrimination test (PPDT) has been validated for the diagnosis of telencephalic hearing disorders. In this test, regular sequences of noise bursts or click trains are presented dichotically, and randomly occurring monaural changes in intensity or click pattern have to be discriminated. The PPDT was administered to 48 control subjects and to 62 patients with circumscribed cerebrovascular lesions. Involvement of telencephalic auditory structures (TAS) was assessed from CT scans. Abnormality in the PPDT was highly correlated with the incidence of a TAS lesion. The most prominent abnormality consisted of an increased error rate (missed discriminations) on the ear contralateral to the TAS lesion, comparable to the ear effect described for former dichotic tests. In normals, no ear dominance, which might have confounded the interpretation of lesion effects in patients, was observed for our test material. Also, the influence of peripheral hearing loss on test results was small. Criteria for the clinical evaluation of the PPDT were developed and yielded a good sensitivity (76.1%) when related to the CT scan data. Abnormality in the PPDT was also confirmed by reduced auditory evoked dipole source potentials in the lesioned hemisphere. Disturbances in auditory language comprehension in aphasic patients were not significantly related to a positive PPDT result, but a questionnaire about hearing difficulties revealed a close association of PPDT abnormality and auditory perceptual disturbances. These occurred only in difficult hearing environments, for example, the cocktail party situation. It must be concluded that unilateral TAS lesions lead to auditory perceptual impairment and communication problems, which should be given adequate attention during neuropsychological rehabilitation.

A critical appraisal of neuropsychological correlates of Japanese ideogram (kanji) and phonogram (kana) reading.

Owing to the Japanese language's unique writing system, which consists of phonograms and ideograms, reading impairments of Japanese brain-damaged patients have attracted the interest of many researchers. Past case reports as well as some widely accepted handbooks and textbooks have concluded that a specific aphasia type or lesion site is associated with a particular impairment pattern of phonograms and ideograms in reading. However, the methodology and analytical procedures in previous studies were inadequate for reliable generalizations to be made. First, the test materials were unspecified or inappropriate, or the number of test items was small. Secondly, the conclusions were presented without providing individual performance data to support them. Thirdly, in associating patterns of reading impairment with lesion sites, only single cases were reported. The present investigation was designed to overcome the omissions of previous studies, and examined the ability to read 46 single phonograms and 46 single ideograms aloud in four groups of sufficiently large numbers of patients; namely, seven pure alexics, 23 Broca aphasics, 13 Wernicke aphasics, and seven patients with alexia and agraphia. Ours are the first data to demonstrate unequivocally no consistent linkage between aphasia type and the patterns of impairment of phonogram and ideogram reading. The impairment patterns were not uniform across patients even in the same aphasia group. A majority of the cases in each group showed that phonograms and ideograms were unselectively impaired. However, ideogram reading was more difficult in three cases in the pure alexia and Broca aphasia groups, respectively, and in one case in the Wernicke aphasia group. Phonogram reading was more severely disturbed in four cases among the Broca aphasics and in one case among the patients with alexia with agraphia. An apparent variability of impairment patterns characterized the Broca aphasic group. These dyslexic patterns did not appear to correlate with the site and extent of lesions identified by computerized tomography scans. Past reports linking a particular impairment pattern of phonogram and ideogram reading and a specific lesion site were studies of single cases, and their conclusions seem oversimplified. While sensory and motor dysfunctions can usually be neuroanatomically localized in individuals, impairments of certain high cortical functions, such as the reading of phonograms and ideograms, may not be correlated with damage to definite neuroanatomical structures.

Perceptibility curve test for digital radiographs before and after correction for attenuation and correction for attenuation and visual response.

OBJECTIVE: Two digital image processing methods, correction for X-ray attenuation and correction for attenuation and visual response, have been developed. The aim of the present study was to compare digital radiographs before and after correction for attenuation and correction for attenuation and visual response by means of a perceptibility curve test. MATERIAL AND METHODS: Radiographs were exposed of an aluminium test object containing holes ranging from 0.03 mm to 0.30 mm with increments of 0.03 mm. Fourteen radiographs were exposed with the Dixi system (Planmeca Oy, Helsinki, Finland) and twelve radiographs were exposed with the F1 iOX system (Fimet Oy, Monninkylä, Finland) from low to high exposures covering the full exposure ranges of the systems. Radiographs obtained from the Dixi and F1 iOX systems were 12 bit and 8 bit images, respectively. Original radiographs were then processed for correction for attenuation and correction for attenuation and visual response. Thus, two series of radiographs were created. Ten viewers evaluated all the radiographs in the same random order under the same viewing conditions. The object detail having the lowest perceptible contrast was recorded for each observer. Perceptibility curves were plotted according to the mean of observer data. RESULTS: The perceptibility curves for processed radiographs obtained with the F1 iOX system are higher than those for originals in the exposure range up to the peak, where the curves are basically the same. For radiographs exposed with the Dixi system, perceptibility curves for processed radiographs are higher than those for originals for all exposures. Perceptibility curves show that for 8 bit radiographs obtained from the F1 iOX system, the contrast threshold was increased in processed radiographs up to the peak, while for 12 bit radiographs obtained with the Dixi system, the contrast threshold was increased in processed radiographs for all exposures. When comparisons were made between radiographs corrected for attenuation and corrected for attenuation and visual response, basically no differences were found. CONCLUSION: Radiographs processed for correction for attenuation and correction for attenuation and visual response may improve perception, especially for 12 bit originals.

[Inhibitors of the polymerase chain reaction].

The information on the applied aspects of the polymerase chain reaction (PCR) is updated. In particular, the main inhibitor of PCR, considerably decreasing the sensitivity of the method both at the lysis stage of the tested material and due to the degradation of the DNA matrix and primers and/or to the direct inhibition of the activity of DNA polymerase, are described. The compounds, most frequently distorting the course of the reaction while testing clinical blood samples, bioptic samples, sputum, etc., are characterized. Testing concrete clinical material with the use of PCR was shown to require differentiated approach both at the stage of choosing the adequate method for the preparation of samples and at all other stages, including, e.g., the corresponding DNA polymerases or at the stage of heating for decreasing endonuclease activity or for IgG denaturation. Information on the causes of false negative results of PCR and the variants of their elimination, useful under practical laboratory conditions, is given.

The meniscofemoral ligaments: secondary restraints to the posterior drawer. Analysis of anteroposterior and rotary laxity in the intact and posterior-cruciate-deficient knee.

We have tested the hypothesis that the meniscofemoral ligaments make a significant contribution to resisting anteroposterior and rotatory laxity of the posterior-cruciate-ligament-deficient knee. Eight cadaver human knees were tested for anteroposterior and rotatory laxity in a materials-testing machine. The posterior cruciate ligament (PCL) was then divided, followed by division of the meniscofemoral ligaments (MFLs). Laxity results were obtained for intact, PCL-deficient, and PCL-MFL-deficient knees. Division of the MFLs in the PCL-deficient knee increased posterior laxity between 15 degrees and 90 degrees of flexion. Force-displacement measurements showed that the MFLs contributed 28% to the total force resisting posterior drawer at 90 degrees of flexion in the intact knee, and 70.1% in the PCL-deficient knee. There was no effect on rotatory laxity. This is the first study which shows a function for the MFLs as secondary restraints to posterior tibial translation. The integrity of these structures should be assessed during both imaging and arthroscopic studies of PCL-injured knees since this may affect the diagnosis and management of such injuries.

[Vicryl Rapide versus Safil Quick. A prospective comparison of two fast resorbing suturing materials].

Currently, there are two fast-resorbing suturing materials on the Dutch market, viz. Vicryl Rapide and Safil Quick. The aim of this study was to compare the clinical suitability of both suturing materials, which was defined as minimal discomfort or pain for the patient, good knot security and tensile strength, predictable resorption, minimal tissue response, and good wound healing. In a group of 120 patients (60 patients randomized in each group), who were referred for apectomy, the clinical suitability of Vicryl Rapide and Safil Quick as fast-resorbing suturing materials was prospectively studied. The results showed that the tested materials were equivalent with regard to most aspects of their clinical suitability.

The effect of 2 different diameter cast posts on tooth root fracture resistance in vitro.

Prosthetic treatment frequently involves the restoration of endodontically treated teeth (ETT), which in turn implies that there has been a reduction of the crown due to previous restorative treatment, trauma and/or endodontic manipulations. The prognosis of a restored ETT is very much dependent on pre-endodontic status of the tooth, the quality of the endodontic treatment, the amount remaining tooth structure and the bone support. The aim of this study is to measure fracture resistance of extracted premolar teeth restored with 2 different diameter posts. Extracted premolars were collected and stored in physiologic solution at 5 degrees C. The teeth were examined for fractures by direct and transillumination observation and 40 premolars were accepted for the study. The PC were modelled using the appropriate diameter plastic pattern for the post and Pattern Resin (GC) was used to model the core part of the PC. The PC patterns were invested and cast in cobalt chromium alloy and then cemented using zinc phosphate cement. Fracture resistances of the samples were tested in an "Instron 4301" (Instron Series IX Automated Material Testing System Version 5) with a loading speed of 0.5 mm/min. The data for each sample was recorded graphically. Both groups were analyzed using the t-test (p<0.01) in relationship to the mean value of that group. Post and cores significantly reduce the fracture resistance of the tooth and should be used only to secure retention and resistance form for full coverage crowns. Teeth with a larger diameter post have a reduced fracture resistance than teeth with a smaller diameter.

Quantitative scanning electron microscopy for the evaluation of thrombosis in extracorporeal circuits.

Methods devised in the past to evaluate blood-material interactions in extracorporeal devices assess small segments of test materials and do not analyze the entire circuit. In order to develop an effective method for the control of thrombosis in extracorporeal circulation, it is important to perform a quantitative evaluation of random samples of all the circuit elements. In an effort to quantify thrombosis in experimental extracorporeal membrane oxygenation circuits, random samples of silicone rubber membrane and polyvinyl chloride tubing were examined using scanning electron microscopy (SEM) and quantitative morphometry. The results indicate no statistically significant differences in the amounts of various adhered blood components between the tubing and the membrane. Greater than 60% of the examined surface had no blood components present. Platelet adhesion on the surface was predominant. Evidence of fibrin, gross thrombi, red cells, and white cells in small quantities were noted. It is concluded that quantitative scanning electron microscopy (QSEM) not only provides accurate information about the morphologic changes of blood cells due to surface contact, but also quantifies the individual elements and therefore assesses their contributions to thrombosis. This quantitative description will allow more meaningful comparison of blood-compatible materials and surfaces.
