A semiquantitative histological analysis was performed on 84 percutaneous biopsies 0-60 days post transplant from 66 grafts with clinical rejection, decreasing graft function of unknown etiology or persisting anuria. The presence of 1 or more of 3 histological lesions was associated with 100% graft loss within 1 year. The lesions were: infarction, glomerular capillary thrombosis, and arterial/arteriolar thrombosis. Since the detection of these severe lesions only permitted prediction of an unfavourable course in 72% of all grafts that were subsequently lost, a system was elaborated utilizing the sum of scores of 12 histological and 4 clinical parameters. This system failed to improve the prognostic value of the biopsy since diagnostic specificity as well as sensitivity was slightly lower by this method. The conclusion is that a graft biopsy performed during early graft failure has a high predictive value for graft loss if infarction, arterial thrombosis or glomerular capillary thrombosis is present; however the predictive value of a negative test is low.
The behaviour and fate of allogenic skin transplanted to rabbit deciduomata was investigated and compared with orthotopic and subcutaneous allografts. Allografts implanted in the decidua of non-sensitized hosts were alive and showed no evidence of immunological rejection 14 days after grafting. The surrounding decidual tissue showed no infiltration by lymphocytes. Allografts in the myometrium of nonsensitized hosts demonstrated immunological rejection as early as 7 days post grafting. Orthotopic allografts on non-sensitized hosts were rejected between 7 and 8 days. Allografts implanted in the decidua of pre-sensitized hosts were alive and histologically healthy 6 days after grafting but showed minimal survival at 8 days. Since rejected grafts were not usually infiltrated with lymphocytes it was concluded that humoral antibody, in the absence of a cell mediated effect, may have procured the destruction of grafts. Subcutaneous allografts in pre-sensitized hosts were rejected between 4 and 6 days after grafting and demonstrated a classical second set type of immunological reaction. The significance of these results was discussed and it as suggested that the decidua, possibly because of its structure, may serve to restrict invasion of its substance by maternal immunocompetent cells thus contributing to the immunologically privileged nature of pregnancy.

The peripheral blood neutrophils from 17 kidney transplant recipients were studied for their ability to phagocytose and kill Pseudomonas aeruginosa in vitro. Practically all of the percent phagocytosis values were within the normal range but many of the neutrophil samples demonstrated a reduced ability to kill ingested P. aeruginosa, particularly within the 3 month period immediately after transplant. Of the 17 patients studied, ten were found to have neutrophils with a reduced capacity to kill the test bacteria. These defects in bactericidal capacity were found in patients who received antihuman-lymphocyte globulin (ALG), large doses of methylprednisolone for rejection, and mostly in subclinical infection with Pseudomonas or other gram-negative bacterial species.
