The first paper reporting on a potentially important medical property of a non-antimicrobial tetracycline appeared in 1987. Since then, a literature of over 75 papers has supported the therapeutic potential of this class of compounds. In this review, this literature is grouped and organized with commentary on the data which has been published to date. The biomedical applicability of these discoveries obviously covers a wide range of medical conditions and clearly justifies their continued development.

The incidence, clinical features, histogenesis, and treatment of extramammary Paget's disease (EMPD) are reviewed. This unusual skin lesion is associated with an underlying adnexal neoplasm in about 50% of cases. Also, the incidence of distant organ malignancies of EMPD is higher than expected by chance. Even in the absence of a recognizable underlying cancer, EMPD may occasionally produce distant metastases, indicating the malignant potential of this condition. Histochemical, immunohistological, and lectin binding studies demonstrate that the cell of origin of EMPD is the exocrine cell of sweat glands. Although EMPD may arise from eccrine cells, derivation from apocrine cells appears more common. The treatment of the primary lesion, by wide margin excision, is fraught by a high recurrence rate. Chemosurgery may reduce local relapse of EMPD. The value of adjuvant radiation therapy is unestablished. Chemotherapy has induced remission in 2 cases of advanced EMPD and needs testing in clinical trials.
Understanding the effects of substance misuse by pregnant women and mothers of young children requires a knowledge of the epidemiology of women's drug and alcohol use and misuse, the treatment of pregnant and postpartum chemically dependent women, the impact of prenatal exposure on the offspring outcome and later development, and, finally, methodological issues related to these fields. This bibliography provides a list of recent and classic articles in these areas as well as information about the research in these areas.
Level of identification with one's culture has long been thought to be a predictor of substance use among ethnic minority populations. However, cultural identification is a complex construct that has been difficult to capture in a coherent theoretical way. Additionally, the various theoretical approaches have led to numerous and divergent methods of measurement. Although measurement is becoming more sophisticated, the evidence for a link between cultural identification and drug use remains elusive. There is evidence that immigrants to a new culture do change their pattern of substance use to match that of the host culture, but the mechanism of that change is not well understood. That culture impacts behavior is without question although how culture operates specifically with respect to substance use will require further effort.

Multimodal corpora that show humans interacting via language are now relatively easy to collect. Current tools allow one either to apply sets of time-stamped codes to the data and consider their timing and sequencing or to describe some specific linguistic structure that is present in the data, built over the top of some form of transcription. To further our understanding of human communication, the research community needs code sets with both timings and structure, designed flexibly to address the research questions at hand. The NITE XML Toolkit offers library support that software developers can call upon when writing tools for such code sets and, thus, enables richer analyses than have previously been possible. It includes data handling, a query language containing both structural and temporal constructs, components that can be used to build graphical interfaces, sample programs that demonstrate how to use the libraries, a tool for running queries, and an experimental engine that builds interfaces on the basis of declarative specifications.

The human major histocompatibility complex (MHC) locus has the greatest density of disease-associations in the human genome, including links to over 100 polygenic disorders. Its complex haplotype structure, rich gene density, and high degree of linkage disequilibrium combine to make deciphering the gene regulatory logic of the MHC locus extremely challenging. Employing complementary high-throughput CRISPR interference (CRISPRi) and activation (CRISPRa) epigenetic screens coupled with single-cell transcriptome profiling across three distinct human cell types, we identified hundreds of new connections between cis -regulatory elements (CREs) and their target genes in this locus. These CRE-gene links are largely cell type-specific and act as enhancers. Additionally, some CREs have complex features, including harboring both active and repressive histone marks, lacking chromatin accessibility, targeting multiple genes, or acting as silencers. Computational methods fail to predict a majority of these CRE-gene connections. These findings emphasize the potential for functional perturbation experiments to dissect complex loci and reveal shared and cell type-specific regulatory mechanisms relevant to genomics of complex diseases. Collectively, this study provides a unique resource for understanding the complex regulatory landscape within the MHC locus and supports the need for creating new models that encompass CRE-gene interactions, cell type-specific gene expression, and disease genetics in the noncoding genome.
