The role of lumbar puncture as a diagnostic tool in 2005.

Analysis of cerebrospinal fluid (CSF) obtained by lumbar puncture (LP) is fundamental to the management of inflammatory disease of the central nervous system (CNS), particularly that due to infection. This review summarises the role of lumbar puncture, anatomy and pathophysiology of CSF, techniques of obtaining CSF, indications, contraindications and complications of LP, methods of analysis and some of the implications of specific changes in CSF. The CNS is protected by unique immunological barriers, and has some unique responses to processes that breach these barriers. While clues in the epidemiology, history and clinical features of potential CNS inflammatory disease may be important in guiding early empirical treatment which may obviate adverse outcomes, many pathological processes cannot be distinguished or appropriately treated without analysis of CSF. When appropriate assessments are made of the indications, risks and potential to alter management, LP is a relatively safe procedure with a high diagnostic yield. Optimal performance and use of LP requires individual skill and judgment, and often benefits from close liaison with several disciplines, including emergency, intensive care, diagnostic laboratory, clinical imaging, neurology and infectious/ communicable diseases specialists. Understanding of the sensitivity, specificity and variation of CSF testing is critical to its effective use. Some CSF testing is sensitive, specific and timely, but other CNS disease processes will generate obscure and ambiguous results, and interpretation may benefit from liaison with experienced specialists in several fields. Polymerase chain reaction (PCR) testing has changed the practice of LP and is likely to generate further evolution. Some findings on CSF analysis may have implications beyond the individual patient - the consequences of the diagnosis of meningococcal meningitis, emerging pathogens such as West Nile virus or Nipah virus, and the identification of anthrax meningitis in the USA may be quite profound on both a local and global scale.

[Kinetics of the reaction of yolk cell surface in the loach to puncture and mechanic deformation].

Circumferential and radial components of the yolk cell surface movements were measured in the loach embryos at the late blastula stage within 40-50 min after puncture or indentation by an obliquely directed glass rod. The yolk cell surface was preliminarily marked by coal particles. It was shown that even closely located regions of the surface differed markedly in the rate and direction of their movements. In the vicinity of puncture, the yolk cell surface at first contracted in both circumferential and radial directions and then widened, but did not reach the initial values. In more remote areas, this surface continued to contract in the circumferential direction, but was extended in the radial direction. The degree of its contraction along different radii was unequal. The reaction to oblique indentation was anisotropic: the closest area of the yolk cell surface, located along the direction of indentation, contracted in both circumferential and radial directions and formed a fold "leaking" onto the rod, while the opposite area contracted in the circumferential direction, but extended in the radial direction. A conclusion was drawn that the yolk cell surface is a multivariant mechanosensitive system. Its active responses to mechanical influences obey the same patterns as multicellular embryonic tissues.

[Complications of tracheoesophageal puncture for Blom-Singer voice restoration after total laryngectomy].

OBJECTIVE: To investigate causes and treatment of complications of tracheoesophageal puncture for Blom-Singer voice restoration after total laryngectomy. METHODS: From 1986 to 2004, one hundred and fifty one cases with Blom-Singer technique after total laryngectomy for voice restoration were retrospectively analyzed. RESULTS: All cases were followed up from 6 months to 15 years. Among 151 cases, 138 cases got successful phonation and the total success rate of voice restoration was 91.4%. Of the 151 cases, 15 cases occurred complications. The rate of complications was 9.9%. The common complications included fistula granulations, infection, and leakage. Twelve cases got successful treatment accordingly, and the other 3 cases failed in phonation. CONCLUSIONS: The procedure of tracheoesophageal puncture for voice restoration is relatively simple and has low complications. this method had high phonation success and good phonation quality, which is one of the best way to make laryngectomee to speak.

[Puncture biopsy of the breast (debatable problems)].

The reliability of puncture biopsy of the breast is 65%. The procedure is not safe since it does not assure ablasticity. Puncture biopsy should be performed immediately before the start of treatment at institutions equipped for cytological, surgical and radiation treatment. The use of the procedure should be confined to cases in whom morphologic data are decisive for choosing therapeutic modality.

Puncture wounds: therapeutic considerations and a new classification.

Despite abundant documentation in the literature regarding the complications of puncture wounds, emergency room personnel and practitioners alike often undertreat this seemingly innocuous injury. The authors present a review of the literature, recommendations for treatment, and a new classification of puncture wounds to aid the clinician in the management of this injury.

[Computerized echotomography with target puncture biopsy in the diagnosis of thyroid gland lesions].

Combined investigation of 200 patients with different thyroid lesions was conducted. The program of investigation included computerized echotomography of the thyroid with spot puncture and rapid cytological examination. Of them 138 patients were operated upon. In 125 patients (87.4%) combined investigation including spot aspiration puncture biopsy and echotomography helped to establish morphological diagnosis of lesion, to specify indications for surgical intervention and to optimize its use.

[Primary and metastatic liver tumors--evaluation of tumor regression and response with regional cytostatic drug therapy by sonography and fine needle puncture histology].

Primary and metastatic hepatic tumours--evaluation of tumour regression or response under regional cytostasis with sonography and fine needle puncture histology. The results obtained through the use of regional liver perfusion with a 5-FU-BCNU application on non-resectable hepatic metastases of colorectal tumours following primary curative removal of the primary tumour and primary hepatocellular carcinomas display comparable results to those specified in the literature after the use of 5-FUDR. The effectivity of the cytostasis regime is checked using histological criteria. The ultrasonically guided fine needle puncture can prove the success of the cytostasis regime on the basis of cytomorphologic criteria more reliably than has been the case up to now. Patients displaying histological signs of tumour regression have a significantly longer survival time than those displaying no signs of regression (12 +/- 9.2 vs 4.5 +/- 2.2 months; p less than 0.05).

[The effect of laser puncture on the functional indices of the mammalian body].

Biologically active points (acupuncture points) and reflexogenic zones of Wistar rats of different age were exposed to He-Ne laser radiation with the subsequent control of acetylcholine esterase activity, electric parameters of skeletal muscles and heart, and physical activity. Laser puncture was shown to increase the sympathetic nervous system tonus which was manifested by the decreased acetylcholine esterase activity and increased heart rate. Laser puncture stimulated the growth of membrane potential and increased considerably the performance capability of animals, particularly of three-month-olds.

Puncture wounds of the foot.

Serious complications occurred in 29 of 887 children with puncture wounds of the feet treated over a four-year period at the Dr. Charles A. Janeway Child Health Centre. Osteomyelitis in one of the small bones of the foot was the commonest complication and occurred when a cartilaginous surface (physeal plate or articular cartilage) had been violated. Although systemic signs of osteomyelitis frequently are absent, this infectious process is refractory to medical management. The combined surgical and medical management of this complication is outlined. Management of puncture wounds in the emergency room should include a thorough history concisely recorded, tetanus prophylaxis, and cleansing, debridement, and probing of the wound. Antimicrobial agents are not routinely required but should be reserved for patients presenting late with cellulitis or an established infection. A semisynthetic penicillinase-penicillin appears to be the agent of choice until the results of microbiologic studies are available.

[Morphologic validation of the use of laser puncture in myopathies].

Conditions of hypokinesia from 10 to 150 days were produced in 24 4-month-old male rats. Biologically active points were irradiated by a helium-neon laser. Microscopically visualized destructive changes of muscle fibers were most pronounced on the 50-th day of hypokinesia (43.4% of muscle fibers). After laser puncture destructive changes reduced by the 50-th day to 7.2%. It was also established that after laser treatment microcirculation increased development of intramuscular connective tissue showed a delay, regenerative processes became activated. It is concluded that in the hypokinesia variant of myopathy laser puncture plays a structure-saving role and increases the restorative processes in the muscles.

Usefulness of double-puncture laparoscopy in the infertility work-up.

This retrospective study confirms that laparoscopy should be performed early in the infertility investigation when pelvic pathology is suspected. If performed, the procedure should be via the double-puncture technique. Otherwise, it should be done when the rest of the tests have proven to be within normal limits and there is only the possibility of peritoneal involvement (i.e., pelvic adhesions or pelvic endometriosis). Our impression is that the single-puncture technique seriously compromises the physician's ability to assess pelvic pathology adequately.

Visual field obstruction in single-puncture operative laparoscopy.

The use of the laser for translaparoscopic surgery has brought with it a renewed interest in single-puncture operative laparoscopy. Concern about the extent of visual obstruction created intraoperatively by the use of ancillary instruments through the accessory channel prompted this evaluation. Under normal operative conditions, a 5-mm instrument protruding 10 mm or more beyond the distal end of the laparoscope will obstruct 14.6% of the operator's visual field. Still greater curtailment of endoscopic visualization can be expected when large, modified laser probes are used. The double-puncture laparoscopic technique must be considered the method of choice for translaparoscopic conventional or laser surgical procedures.
