Simulation of appendicitis by tumor-like lesion of cecum.

Four cases are presented in which an initial diagnosis of appendicitis was made. At laparotomy, a cecal lesion simulating a tumour was found. Pathological examination showed inflammatory granulomatous tumour against foreign body, non specific ulcer, diverticulitis and acute solitary diverticulitis. When malignancy is excluded, hemicolectomy is not necessarily the best treatment and we propose conservative management for these patients.

[Intususception of appendical cystadenoma imitating a caecal tumor].

Appendical cystadenoma is one of the histological forms of the appendical mucocele. Preoperative diagnostics is difficult, however, due to potential iatrogennic rupture during a surgical procedure, is very important. The authors present a case review of a 62-year-old patient with itususception of the appendical cystadenoma, until now rarely reported in literature. Upon the preoperative diagnosis the appendical cystadenoma appeared as a caecal tumor or a tumor of the terminal ileum. A histological, intraoperative, diagnosis was established, followed by a final histological examination of the resected tissue. The authors discuss contemporary options of the preoperative diagnostic measures of the appendical tumors, stressing up all risks of aspiration cytology as well as risks of laparoscopic approach in this pathology management

[Cecum amputation in a blond d'Aquitaine bull].

A caecal torsion of 720 degrees was diagnosed in a one-and-a-half-year-old blonde d'Aquitaine bull. A partial amputation of the caecum had to be performed. The diagnosis and surgery of this case are described against the background of the usual dilatations and rotations of the caecum and ansa proximalis.

[The use of the TA-55 mechanical suturing device in the closure of the cecostomy. The authors' experience].

The authors confirm the actuality and validity of decompressive cecostomy in emergency treatment of large bowel obstructions. Taking into account that staplers have substantially reduced the rate of complications and inconvenient of gastrointestinal surgery, their use of cecostomy closure is proposed. Advantages obtained with this technique, namely lack of infections and incisional hernias, are underlined.

Nonspecific inflammatory pseudotumour of the caecum unrelated to the vermiform appendix.

A case of nonspecific inflammatory pseudotumour of the caecum unrelated to the appendix is reported. Symptomatology, radiological findings, and intraoperative findings suggested a caecal carcinoma. A right hemicolectomy was carried out. Histologic examination revealed an inflammatory mass with abundant fibroblastproliferation and collagen formation infiltrating the muscularis and adjacent pericolic adipose tissue. The caecal mucosa was entirely intact and normal. Histology suggested sclerosing mesenteritis. A correct preoperative diagnosis of nonspecific inflammatory pseudotumours of the caecum is difficult, but awareness of its existence might prevent too radical surgical resections. The differential diagnosis is discussed.

[Myxoglobulosis of the cecal appendix].

We report a case of mucinous cystadenoma of the appendix with myxoglobulosis associated to cecal adenocarcinoma. Appendiceal myxoglobulosis present typical gross and microscopical findings. The clinical diagnosis of myxoglobulosis should be followed by a careful evaluation of the patient in order to detect an appendicular neoplasm and/or an assoiciated occult intestinal carcinoma.
