[Surgical tactics in complicated pulmonary echinococcosis].

The authors analyse treatment of 326 patients with hydatid disease of the lungs. Complications were found in 112 (34.3%) of them. Broncho-fibroscopy was included in the complex of therapeutic measures in rupture of the cyst into the bronchus, and pleural puncture, therapeutico-diagnostic thoracoscopy and drainage of the pleural cavity in rupture into the pleural cavity. Temporary occlusion of the draining bronchus with Fogarty's catheter was conducted to prevent intraoperative aspiration complications. If organ-preserving operations are indicated, preference is given to A. A. Vishnevsky's echinococcectomy with a laser scalpel since the Delbet's method is fraught with the danger of cutting of the sutures due to acute inflammatory changes of the wall with the formation of a residual cavity or an abscess of the lung. A differentiated approach to preoperative management and the choice of the operative techniques reduced the incidence of postoperative complications and reduced mortality in complicated hydatid disease of the lungs to 0.9%.

[Reserves of pulmonary ventilation and gas exchange in patients with pulmonary echinococcosis].

An examination of 33 patients has shown that complicated course of Echinococcosis of the lungs and their multiple lesions results in certain exhaustion of reserves of the external ventilation system. A conclusion is made on preference of performing organ-preserving and organ-sparing operations, on necessity of dispensary observations and physical rehabilitation of patients operated upon for complicated forms and multiple lesions of the lungs.

[Surgical treatment of pulmonary echinococcosis].

Operation with the use of a laser for resection of the external fibrous capsule with re-expansion of the remaining cavity in patients with echinococcosis of the lungs is more effective than operation by capitonnage of the cavity. The main advantages of laser operations were the absence of closed cavities, deformities of bronchi, atelectasic areas of tissue around the fibrous cavity, and disorders of capillary blood flow and bronchial patency. All this allowed poor results of the operation and fatal outcomes in the main group of patients to be avoided.

[Choices of treatment in pulmonary echinococcosis].

Analysis of accumulated experience provides evidence that the approach to determining the principles of treatment must be differentiated and the principles applied in succession in patients with hydatid disease of the lungs. The choice of the most rational treatment is based on comprehensive appraisal of the condition of the pathological focus as well as on the character and degree of pathophysiological changes occurring in rupture of the hydatid cyst. In spontaneous ruptures of the cyst postponed operation is advisable, which is undertaken after a complex of non-operative measures aimed at relieving the patient from the critical situation and stabilization of the organism's immune forces is completed. Intraoperative ruptures of the hydatic cysts are prevented by successive application of optimal technical procedures and adequate anesthesia-resuscitation service.

[Pulmonary echinococcosis and effectiveness of its surgical treatment].

The problems of epidemiology and diagnosis of echinococcosis in the Turkmen SSR and organization of its treatment are urgent because of its prevalence. The number of diagnostic mistakes remains high. The modern diagnostic means provide early detection of echinococcosis. Higher quality of the diagnosis is possible with involving prophylaxis. The surgical method of echinococcosis treatment used mainly in surgical units of tuberculosis hospitals of the republic proved to be the most efficient. The efficacy of the surgical treatment according to our data amounted to 100 per cent. In some periods it amounted to 97 per cent.
