Infants with myelomeningocele very often present cranio-cerebral anomalies such as Arnold Chiari malformation. Cranial ultrasonography allows early evaluation of these malformations and helps to detect complications in surgically treated children.
To examine patients suffering from diseases of the nervous system, use was made of a programmed apparatus complex that ensures an automated analysis of echosignals from different parts of the third and lateral ventricles. The complex is assembled on the basis of microcomputer. The method suggested has been demonstrated to be informative in the diagnosis of hydrocephalus and sizable injuries to the brain during mass screenings.
Sixty-one infants aged 0 to 12 months with documented purulent meningitis underwent transfontanellar ultrasonography at the Yaounde Central Hospital. Twenty-eight (45.90%) infants had evidence of complications including communicating hydrocephalus (20 patients; 71.43%), ventriculitis (5; 17.86%), abscess with ventriculitis (2; 7.14%), and cerebral atrophy (1; 3.57%). An analysis of the main epidemiological, bacteriological and clinical parameters found no correlation between the presence of ultrasound evidence of complications and age, sex, or color of the CSF. Conversely, a close correlation was found with both the time elapsed between onset and initiation of treatment and the severity of clinical manifestations (coma, seizures). The authors, recommend routine ultrasound encephalography in infants with meningitis, improved information of parents, and close collaboration between pediatric radiologists and neurosurgeons.
Intraoperative ultrasonography with real time sonographic imaging equipment is widely used and, in the neurosurgical operating room, has been really successful, initially in the brain and subsequently in the spinal cord as well. Its main applications i.e. accurate needle biopsies, localization of intracerebral masses, placement of ventricular and cyst shunts, can diminish the amount of potential damage to normal brain tissue. Moreover results of tumor resection may be intraoperatively confirmed and the end results are more accurate. No intraoperative complications and no postoperative infections result from use of ultrasound in the operating room. No evidence is given that the small amount of mechanical vibration emitted by the transducer damages the brain or the spinal cord. Surgical procedures, as a result, are faster, safer and better planned by virtue of informations obtained by ultrasonographic intraoperative examination, instead of simple preoperative imaging procedures.
