[Capillary hypoperfusion in diabetes mellitus: a comparative microscopic study on its pathogenesis and temporal manifestation in in vivo animal experiments].

In animal experimental alloxan diabetes changes the distribution of microcirculation with reduction of the nutritive capillary perfusion and simultaneous formation of hyperperfused functional microshunts. The nutritive capillary hypoperfusion is due to an increased precapillary vasoconstriction and a changed rheological behaviour of the blood with increase of the cohesive forces between the erythrocytes (aggregation of erythrocytes) and the intravascular cellular swellings.

[Rheological changes in the blood and disseminated intravascular coagulation (DIC) in systemic vasculitis].

Eighty-six patients with different versions of systemic vasculitis were examined for rheological blood properties and some immunologic characteristics. The relationship between the formation of immune complexes and disorders of rheological characteristics and cell aggregation was established with emphasis on the necessity of correcting these disorders by anticoagulants and deaggregating agents for the treatment of patients with systemic vasculitis to be successful.

[Hemostasis and pre-eclampsia].

A longitudinal study has been undertaken in 125 pregnant women between 25 and 40 weeks of gestation, to provide systematic information on the changes that occur in a wide range of haemostasiological and haemorheological variables. Fibrinogen, D-Dimer, Factor VIIIR: Ag, erythrocyte aggregation and plasma viscosity rose markedly throughout pregnancy. Antithrombin III and alpha 2-antiplasmin were unchanged during pregnancy. There were no significant differences between women in the pre-eclamptic group (N = 16) and the control group. HELLP syndrome (N = 7) was associated with high D-Dimer (p less than 0.05), and TAT (p less than 0.05), low antithrombin III (p less than 0.03), protein C (p less than 0.01) and platelets (p less than 0.001). Our results demonstrate that during pregnancy (also, however, in pre-eclamptic women) alterations of the coagulation system occur, but these changes do not affect the overall haemostatic balance. Findings in patients with "true" HELLP syndrome are consistent with an increased tendency for intravascular coagulation.

[Severe pregnancy hemostasis disorders: diagnosis and therapeutic procedures].

We evaluated 21 pregnant patients with severe disorders of haemostasis by retrospective chart analyses, 15 patients were treated with intravenous heparin (10.000 to 20.000 I.U.) during and after surgery. Eighteen of 21 patients developed renal failure and 8 died. Postoperative bleeding complications making laparotomies mandatory occurred in 10 patients. These bleeding complications are seen in relation to the heparin treatment instituted immediately after surgery. We do not favor the administration of heparin during the immediate obstetrical postoperative period, the more as Antithrombin III-concentrate is now available. Surgical intervention in order to achieve haemostasis is of paramount therapeutic importance. Drainage of infections remains furthermore the most important task in those obstetrical emergency cases where infections play a predominant role. To maintain or to re-establish the normovolemic perfusion ameliorates the rheological conditions which should monitored with care.

Detection of circulating T-activating enzyme in the serum of a patient having hemolytic-uremic syndrome and disseminated intravascular coagulation.

A three-and-a-half-year old boy suffering from streptococcal pneumonia developed hemolytic-uremic syndrome and disseminated intravascular coagulation (DIC). His red blood cells (RBC) were shown to be T- and Tk-activated; serial testing showed his mature RBCs as well as neocytes remained T-activated at 40 days. Anti-T was detected in his serum, with only one of two T-activated RBC samples. T-activating enzyme was shown to be present in his serum.

[Pulmonary microcirculatory bed and alveolar tissue in the postresuscitation period].

Structural changes in alveolar lung tissue microcirculatory bed and circulatory disturbances (edema, hemorrhage, thrombosis) were studied in postresuscitation period after experimental clinical death following acute hemorrhage and mechanical asphyxia. Circulatory disturbances proved essential in the pathogenesis of postresuscitation respiratory insufficiency, follow a stepwise pattern and in early postresuscitation (up to 7 days) are pathogenetically associated with coagulopathic changes. There are periods of primary, marked and delayed disturbances, and the period of reparative changes which may cover up to 30 days since resuscitation. It is emphasized that the pattern and the degree of lung tissue damage in the postresuscitation are largely determined by circulatory disturbances.

[Acute death from scalding: vital reactions (author's transl)].

After a boiler explosion on a ship 12 men with cutaneous scalding of 90-100% of body surface were found dead. 15 other fatalities occured after different intervals. This investigation deals with the histomorphologic changes in those 12 cases of instantaneous death. The most important findings consisted of: 1. Thermal injuries of the skin with multiple subcutaneous and corial bubbles, intravascular heat coagulation, and edema in the transitional zone to the undamaged submucosa. 2. Coagulation necrosis of the tracheal and bronchial wall extending to alveolar ducts in central parts of the lung. Marked congestion and edema in the deeper layers of the mucosa. Hemorrhagic alveolar and interstitial edema of the lung. Striking congestion of the pleura parietalis saving up the recessus diaphragmaticus. 3. Acute nephron-nephrosis and periglomerular microangiopathy. Interstitial edema of the myocardium, degeneration of myofibrils. Acute vacuolar degeneration and swelling of hepatocytes. Phagocytosis of detritus by the RHS. 4. Swelling of endothelium and edema of the vascular walls. Intravascular fragments of erythrocytes, platelet aggregation and fibrin thrombi. The fatal cours in these cases is considered to be due to a fulminant shock syndrome.

[Erythrocyte equidistance and heavy metals in the pathogenesis of disseminated intravascular coagulation in myocardial infarct patients at the hospital rehabilitation stage].

Blood levels of fibrin degradation products and free haemoglobin were studied in 108 inpatients with acute transmural myocardial infarction in the period of rehabilitation. At the same time contents of cadmium, vanadium, lead and nickel in blood cells and plasma were determined spectrographically. It was established that heavy metals take part in activation of blood clotting and haemolysis which decrease equidistance and accelerates sedimentation of erythrocytes.
