Of 118 twins born of 59 alloimmunized mothers from 1944 to 1984, 65 were Rh-positive and six died. One twin in one of the 59 sets and four sets of twins referred from elsewhere underwent intrauterine fetal transfusion. The Manitoba twin and two pairs of the four referred sets survived. Fetal transfusions were begun at 22 2/7 and 24 1/7 weeks' gestation in one surviving set and 23 3/7 weeks in both twins of the other surviving set. Three twins of the four who died were hydropic at referral. There would have been a 62.5% chance of survival if they had been referred at 23 weeks' gestation. ABO incompatibility in one twin of three sets had no effect on the amelioration of severity of Rh disease. In three sets, gross hemoglobin disparities were due to twin-to-twin transfusion. Differences in degree of severity of disease in the other four dichorionic sets may have been due to differences in sex, Rh constitution, amount of fetal erythropoiesis, and fetal hepatocellular function.

BACKGROUND: Umbilical vascular coiling may function as a rudimentary pump that facilitates venous return from the placenta. CASES: Three consecutive twin gestations with twin-twin transfusion syndrome were evaluated prospectively at delivery. The birth weights and umbilical coiling indices of donor and recipient twins were compared. The umbilical coiling index was determined by dividing the number of complete vascular coils in a given umbilical cord by the cord's length in centimeters. In each case, the recipient twin was larger at birth and had an umbilical coiling index value that was at least twice that of the corresponding donor twin. CONCLUSION: Differential umbilical vascular coiling densities among monochorionic twins may play a role in the pathogenesis of twin-twin transfusion syndrome.
Fetal liver length was measured by ultrasound in 14 pregnancies with twin-twin transfusion syndrome. The liver size was increased above normal values in both the donor (small) and recipient (large) twin. The liver length was significantly greater than in a control group of dichorionic twin pregnancies with discordant growth. Measurement of liver length may be useful in diagnosing twin-twin transfusion syndrome.

Most triplets are trizygotic because they result from assisted reproduction. Prognosis is generally good. We analyzed 15 sets of triplets who were conceived spontaneously. Six sets were monozygotic, 7 were dizygotic, and only 2 sets were trizygotic. Considered as 45 twin pairs, 25 pairs (56%) were monozygotic. Twenty percent of these twins died as a result of twin-twin transfusion. Spontaneously conceived triplets have high risks compared with those resulting from assisted reproduction. These risks result from a high proportion of monozygotic embryos, many of whom have monochorionic placentas with vascular anastomoses, causing twin-to-twin transfusion.
