The May 15, 1991, JAMA theme issue presented 14 different proposals for reforming the US medical care system. Though these proposals varied widely, they encompassed only three proposed methods for paying physicians: fee-for-service, salary, and capitation. When examined from a perspective that considers the incentives affecting practicing physicians, each of these methods has serious flaws. I outline these flaws and suggest an alternative method for reimbursing primary care physicians: that of combining capitation and fee-for-service. Specialists would be paid fee-for-service, but the cost of specialty care would be limited by giving each primary care physician a budget for such care. Consistent failure by a physician to stay within the budget would result in ongoing review, but not in direct financial sanctions. This system of reimbursing physicians is intended to balance incentives so that physicians can concentrate on making medical decisions with a minimum of distraction by third parties and by personal financial considerations.

Personal wellness not only is a matter of physical well-being. It is also based on financial security and planning. This is the case in making decisions about one's own personal health care system provisions. Each person has the right to decide about his or her health care system prior to becoming disabled and unable to make such decisions. In the case of incurable disease and comatic existence or needing life support, a person should have signed powers of attorney and living wills that will be adhered to when the need arises. This article will present these personal financial health care system provisions.

The Honorable Dirk Kempthorne served Idaho as a U.S. senator for six years prior to his election as governor in 1998. He has since served in this role with marked distinction--one poll lauded him as Idaho's "most popular figure" in the state's history. His leadership capabilities were recognized by the National Governors Association (NGA), which elected him chairman in 2003. Upon his election, Gov. Kempthorne asserted--and the NGA collectively agreed--to make long-term care for America's seniors and persons with disabilities the No. 1 item on the NGA agenda. Idaho's governor has since led the NGA through a series of hearings and fact-finding missions on long-term care culminating in a report that comprised the central focus of the NGA's Annual Meeting in July 2004. The following is a summary of Gov. Kempthorne's statements regarding long-term care and the reports he presented on the topic at that gathering. In recognition of his work, the Idaho governor has been selected to receive NAHC's highest accolade--The Claude Pepper Award--presented annually to the one person who has done the most to promote the interests of senior citizens and persons with disabilities within the past year.
