[Tuberous sclerosis with unusual retroperitoneal massive bleeding caused by monstrous hamartoblastoma in both kidneys. A case report].

This paper reports the case of a 22-year-old woman with tuberous cerebral sclerosis and monstrous angioleiomyolipomata in the two kidneys, who died of spontaneous retroperitoneal mass bleeding from the hamartoblastomata in both kidneys. The disease had not been diagnosed during life despite imbecility since early childhood and the presence of guiding peripheral symptoms in the form of Pringle's disease.

[The "negative" SISI-test (author's transl)].

The SISI-test has gained increased recognition during the past few years and can now be considered as the most important recruitment hearing test. In addition to the positive, negative and not determinable result there are frequent cases of incorrect negative interpretation. A "negative" SISI-test can be the result of insufficient or excessive hearing loss, missing adaptation, pathological threshold shift, aggravation, psychogenic deafness, cerebral sclerosis, imbecility etc. A negative result always requires additional audiometric differential-diagnosis.

[Angioarchitectonic characteristics of the cerebral cortex in oligophrenia].

New data are reported concerning the specificities of architectonic disorders in oligophrenia, being especially marked at stages of idiocy and imbecility. The authors feel that the insufficiency of the cortical blood supply as a result of the congenital and dystrophic impairments of the angioarchitectonics may be essential for the development of oligophrenia.

[Method of determining the concentrating ability of unschoolable children and adolescents capable of making progress].

The design, characteristics, and use of a process of determining the concentration power of unschoolable, but furtherable children and juveniles are reported. Diagnostically relevant quality criteria were determined and verified by tests of eighty-five children and juveniles. Standard values were not found. The experience gathered thus far shows that the method presented by the author in his present paper enables the capacity and development of imbecile and abnormality feeble children and juveniles to be diagnosed.

Portable insulin infusion devices: a new era in the treatment of insulin-dependent diabetics! Which patients should be treated?

The development of portable insulin infusion devices - in which the insulin dosage is programmed in advance (open loop system) are increasingly being used for long-term daily diabetes control. These pumps represent a new method for attaining significantly improved metabolic control (glycemia and glucose metabolites, HbA1, lipid and aminoacid metabolism) and an impressive influence on diabetic neuropathy, on general fitness and on quality of life of insulin-dependent diabetics. At least all insulin-dependent diabetics without noticeable residual insulin secretion should be treated by continuous subcutaneous insulin infusion (CSII) via portable pumps, that are 8 000-10 000 patients in the GDR and approximately 500 000 in the world. Considering special conditions (strict supervision by experienced medical staff, carefully drawn up adequate instructions, highly motivated and capable patients) we assume that at the present stage of technical development of the pumps infants, imbeciles and incooperatives should be excepted only.

[Dactyloscopy undifferentiated oligophrenia].

The paper deals with a study of dematoglyphics in 458 oligophrenic patients (265 boys and 193 girls) from 7--18 years of age and 373 normal children of the same age. In all the ill boys, irrespective of the degree of mental retardation there was a decrease of arc patterns and an increase of the whorle frequency. In girls with oligophrenia in the degree of debility there was a significantly lower amount of arc patterns and an increase of whorles, the frequency of loops did not differ significantly with that of the control group. In girls in whom the oligophrenic defect was of the degree of imbecility or idiocy there was a more frequent amount of arc and whorle patterns at the expense of a decrease of loop patterns, while in the control group there was a somewhat more frequent amount of arc an loop patterns and less frequent--of whorles.

[Psychomotor development in atrophic infants (author's transl)].

17 former patients with nutritional marasmus due to malabsorption syndromes of various etiology were followed-up at the age of 5 7/12 +/- 2 1/12 years for the assessment of their psychomotor development. As to the intelligence quotient there was no significant difference in comparison with a representative random sample of 200 preschool children. The mean IQ was 92 +/- 20 (n = 17). 2 children were moron (IQ 64 and 61, resp.), one child was imbecile (IQ 46). In two of them familial disposition of debility must be assumed. The deficit in the capability of the psychomotor functions was striking (fine motoricity, designing). 6 children showed mild forms of electroencephalographic deviations from normal (relative frequency 0.35).

[Experiences with expert assessment in guardianship law from the forensic psychiatry viewpoint].

Analyzing the expert opinions given by an institution of public health on the prerequisites of guardianship, we can discriminate between three groups of individuals: Juveniles reaching majority and young adults who suffer from some form of imbecillity, persons between circa 30 and 70 years of age suffering mainly from schizophrenic psychosis or alcohol dependency, and persons over 70 years of age suffering from dementia. Each group presents special problems for the psychiatric expert, assessment being most difficult in the middle-age group. Further general problems arising in expertising in guardianship cases are discussed, such as the specific role of the expert, prerequisites for assuming limited legal competence, admission to psychiatric hospitals on a nonvoluntary basis and cooperation with other institutions.

[Arson--a disorder of impulse control?].

Reviewing the literature the current knowledge about the motivational basis of arson is discussed. Fire-setters form a heterogenous group; they seem to commit their crimes for different reasons. Some of them are imbecile, some suicidal, and other showed autoaggressive behaviour in the past; sometimes a connection with sexual excitement can be observed. The criteria for the diagnosis "pyromania" used in the classification manuals DSM-III-R and ICD-10 are strict, in order to facilitate an operationalised diagnosis. Most of the fire-settings cannot be classified under this subcategory of "disorders of impulse control". However, impairments of impulse control are typical characteristics in most cases of arson.

The "lethal chamber": further evidence for the euthanasia option.

Historical discussions of the euthanasia or "lethal chamber" option in relation to people with mental retardation were presented. Positions were classified into those (a) dismissing the option, (b) presenting ambivalent views, and (c) showing outright support. Tredgold's forthright views in support of euthanasia for "idiots" and "imbeciles" were highlighted. Eugenic beliefs in the primacy of heredity over environment in determining mortality and the positive role of natural selection in race betterment were presented. I proposed that these beliefs were used to condone, if not encourage, the poor conditions characteristic of large, segregated institutions during the early decades of this century and their high rates of mortality from infectious diseases such as tuberculosis. The conclusion was drawn that euthanasia may thus have been implemented, albeit unofficially, in at least some institutions.

[From the shape of holidays for oligophren children and youth in the holiday camp at Pepelow at the Baltic Sea (Salt Bay) by rehabilitation (author's transl)].

It is reported from a holiday camp for children and youth arranged in summer 1975 at Pepelow on the Baltic Sea (Salt Bay) for the purpose of rehabilitation. By this shape of holidays the partical sphere of the process of training and education, namely the qualification of those oligophren ones in spending an ingenious leisure, should be noticed and contributed to educating those imbecile boys and girls, who are participating their holidays in a camp for their "relative independence*. The camp was a success for all the participators. Substantially contributed by appointed pedagogical assistors with a strong sense of duty, permanently accustomed to an exampled pedagogical-medical common task of rehabilitation.

Report on a 5 year old girl with the caracteristic features of the partial trisomy of the short arm of a chromosome no.4: short stature, microcephaly, hydrocephaly, enophthalmus, bulbous nose, deep set malformed ears, hypertrichosis, brachydactyly, hypoplastic ribs, abnormal EEG, imbecility. Trisomy originated from a reciprocal translocation in the father (46, XY, rcp (4;11) (p 13; q23)). q23)).
