[Textual research on luohuangzi].

By studying ancient and modern literatures and observing the material object, we have come to the conclusion that the fruit called "luohuangzi" in ancient times should be the seed of Sterculia nobilis rather than Tamarindus indica. We therefore suggest that "luohuangzi" be no longer used as another name for T. indica, and, in order to eliminate the confusion about the name and material object of "luohuangzi". Tamarindus should be called the "genus Shuandou" in Chinese instead of the "genus Luowangzi".

Divergent dislocation of the elbow in a child (a case report).

A rare case of simultaneous disruption of superior radio-ulnar joint and posterior dislocation of the same elbow in a 6 year old boy is presented. It was possible to achieve the stable reduction by means of closed manipulation, restoring normal function in 6 weeks time. The possible mode of injury is discussed. There are only 4 cases reported of such a divergent elbow dislocation in modern literature.

[Diseases of the lumbosacral portion of the peripheral nervous system (treatment and prevention)].

A critical analysis of modern literature and personal experience permits the author in a discussional aspect to speak on diagnosis formulation in patients with lesions of the sacrolumbar area of the peripheral nervous system. The paper contains detailed recommendations in relation to different forms of medicative therapy and prevention in this group of patients. The subsequent perspectives and further studies, as well as organizational questions are being discussed.

[Drug therapy following heart valve prosthesis].

Patients with prostheses of the cardiac valve are, related to the total population, only a small part. But their number permanently increases. Alone in our clinic since 1973 3,250 patients have been provided with artificial cardiac valves. Thus more and more physicians are confronted with the problems of these patients. The long-term results after replacement of the cardiac valves are very essentially influenced by the prevention and well-timed recognition of complications, respectively. For this reason the medicamentous therapy is demonstrated on the basis of own experiences and modern literature. It is clearly pointed out that after replacement of the cardiac valve a permanent cardiologic control is necessary. In detail is reported on the therapy of cardiac insufficiency including disturbances of the cardiac rhythm, on the prophylaxis of thromboembolism as well as on the prophylaxis of endocarditis.

[Ultrasonic diagnosis of gallstone diseases].

In the past decade the sonography has effected radical changes in the diagnostics of gallstone troubles and their complications. Today it is the method of choice in search for gallstones. Taking into consideration all advantages connected with the method and a sensitivity of more than 95% with an approximately 99 per cent specificity at present no other diagnostic method can compete with it. The reliability in the gallstone diagnostics above all depends on the experience of the investigator and the size of the concrement. Data in modern literature indicate a sensitivity of 70-90%, among our own patients it is in 376 cases nearly 86% (specificity 92%). In optimal technical and personal prerequisites the ultrasound diagnostics it is the initial method of choice also in the diagnostics of the bile ducts. The intravenous cholegraphy and ERC as complementary methods are used only in the second place.

[Clinical aspects of atopic dermatitis].

Discusses the diagnostic criteria of atopic dermatitis. Basing on a review of modern literature, suggests the following diagnostic signs as the basic ones: a certain clinical picture and its age-specific features, the disease onset within the first months or years of life, polyvalency of the allergic reaction, concomitant respiratory allergies, a family history of allergies, a seasonal pattern of the disease course, immunologic shifts.

[Establishment of the necessity for a review of the vascular endothelium: an introduction].

The vascular endothelium despite of its relative small total mass is distributed all over the organism and is obligatory for nearly all vital processes. The following review will show, how many structural and functional properties of the endothelium are accessible to modern morphological research and will summarize results obtained by the authors of this review in context with the modern literature. The intentions of the authors are to contribute to a better understanding of morphological results and to find new access to scientific and clinical problems of endothelium.

[Elaboration of approaches to quantitative evaluation of the health status of the population in mass screening practices].

The problems pertaining to the utilization of health status evaluation techniques insufficiently described in modern literature on preventive check-ups and dispensarization of the population are discussed. One of the approaches to quantitative diagnosis of risk factors of main pathologic syndromes and states carried out by means of Bayes formula and automation devices is proposed. The considered technique enables one to rank groups of healthy and practically healthy persons into arms according to quantitative indices and to differentiate them by priority, orientation and volume of the programmes of medical observation and examination.

[Progress in drug treatment of bronchial asthma].

On the base of modern literature a statement is given on the actual situation of bronchial asthma treatment and its tendencies. It intends to be an instruction for the doctors daily practice and shell help all asthmatics getting the new possibilities of treatment in time. Furthermore these recommendations intend to support a reasonable and scientific prescription of new drugs.

[The course and forms of epileptic attacks of the absence type in childhood].

In the modern literature the fits of absence type are divided into two groups: absence only and absence with generalized tonic-clonic convulsions, without considering whether absence episodes preceded the tonic-clonic seizures, or whether in early childhood tonic-clonic seizures appeared as first and absence attacks supervened later. In the light of an analysis of own material the authors believe this it is necessary to isolate a group of children in whom the absence attacks were preceded in early childhood by generalized tonic-clonic seizures, sometimes regarded as febrile seizures. In these children absence attacks appeared additionally usually at the age of 5-7 years, were mostly atypical. With prolonged duration and with evident motor component, simple myoclonias or automatism. These attacks are difficult to control. It may be possible that this is a separate form of childhood epilepsy, both from the standpoint of nosology and genetics. The course of this form is different and the prognosis is worse.

Lateral sinus pathology (22 cases managed at Groote Schuur Hospital).

One hundred and thirty patients with acute and acute on chronic mastoiditis were managed at Groote Schuur Hospital between 1980 and 1984. Twenty-two (16.9 per cent) had pathology of the lateral (sigmoid) sinus and 19 of these patients had cholesteatomas. Nine patients (40.9 per cent) had concomitant intradural sepsis and there were two deaths. The modern literature is reviewed and the pathology, presentation and management of lateral sinus pathology is discussed. This condition is rare and clinical features may range from subtle signs to gross toxaemia, torticollis and evidence of septic embolization. The otolaryngologist must be competent in diagnosing and treating this condition in all forms of its wide spectrum of presentation.

[Kinesigenic paroxysmal dyskinesia: an idiopathic variant and a case caused by hypoparathyroidism].

The authors present a brief review of modern literature on paroxysmal dyskinesia. Two cases of kinesigenic paroxysmal dyskinesia (KPD) are described. In the first case (a 22-year-old man) attacks first developed at the age of 14 years. The disease was sporadic in nature and was attended by tapeto-retinal abiotrophy. Attacks were fully eliminated by the treatment with diphenin (phenytoin) or finlepsin (carbamazepin). The second patient, a 14-year-old boy, presented moderate intellectual disorders, parkinsonism and KPD attacks in the absence of a tetanic symptom complex. Some of the KPD attacks in this case, as in the first patient, developed during running, with some of the paroxysms being attended by backward running. Paraclinical examinations made it possible to diagnose pseudohypoparathyroidism. Therapy with dihydrotachysterol and calcium preparations brought about a dramatic clinical improvement, in particular a complete cessation of KPD attacks.

[Behavioral approaches to communication disorders in schizophrenics].

Behavior therapy has used techniques of behavior modification based on the paradigm of operant conditioning to diminish some of the difficulties of communication present in schizophrenic patients. Individual programmes, then ward programs, have been applied to chronically institutionalized patients. They have emphasized the role of reinforcement and of social learning in behavioral approaches. More recently, these techniques have focused on the improvement of performance in social situations. As can be seen in the literature, modern behavior therapy is founded on a sociopsychological model in which environmental factors have a predominant role and focuses on maintaining behavioral improvement in the patient's day-to-day environment.

[Clinical, etiopathogenetic and anatomopathological considerations in primary cheilitis].

Among the wide range of stomatitis and mucositis there are lesions, defined as primary cheilitis, affecting mainly the lips. Specific topographic and structural features of the involved area determine their appearance. The Authors revient contact-related cheilitis, actinic-related cheilitis, glandular and suppurative cheilitis, granulomatous and exfoliative cheilitis. They point out the principle clinical, etiopathogenetic and histopathologic features. From their overview it arises, that in modern literature, funy detailed contributions are definitively lacking. Symptomatic medical treatments are a clear reflection of many etiological doubis. Surgical therapy is advised only in chronic actinic-related cheilitis and in deep-rooted glandular cheilitis. In granulomatous cheilitis surgical treatment is indicated once a permanent lip deformity occurs.

[Cytomegalic inclusion disease. Report of a case in a newborn].

A clinical case of this disease in a newborn is reported. The clinical signs and laboratory results were typical and were confirmed by pathological studies. The modern literature stresses the high frequency of this disease, specially in the U.S.A. and England, where reports reach 4% in pregnant women and 1% of newborns with cytomegalic inclusion disease. We do not know its frequency in Mexico.

[Origin of tissue basophils and basophilic granulocytes of the blood].

The paper deals with analysis of modern literature on origin of basophils and basophilic blood granulocytes. Data of radioimmunologic, electron-microscopic, cytochemical and morphological studies make it possible to conclude that these cells are of bonemarrow origin.
