[Basic principles of mass screening of patients with asbestosis].

Complex hygienic, clinical and epidemiologic studies were carried out at 2 enterprises producing asbestos-technical products. It was established that the workers engaged in the production of such products were exposed to relatively low levels of asbestos-containing dust. The course, complications and outcome of asbestos-associated fibrosis were studied in 110 patients. The study revealed that asbestosis was characterized by its slowly progressing development. Asbestosis complications included lung tuberculosis, chronic intersticial pneumonia, and lung cancer. Proceesing from the epidemiologic survey higher risk of malignant neoplasms of the lungs was established in patients with asbestosis in comparison with those exposed to asbestos-associated fibrosis were studied in 110 patients. The study revealed that asbestosis was characterized by its slowly progressing development. Asbestosis complications included lung tuberculosis, chronic intersticial pneumonia, and lung cancer. Processing from the epidemiologic survey higher risk of malignant neoplasms of the lungs was established in patients with asbestosis in comparison with those exposed to asbestos-containing dust but having no occupational disease. The study findings were used for the substantiation of dispensarization principles for patients with asbestosis. The number of follow-up, laboratory and other examinations along with basic curative and preventive measures were pointed out.

Quality control in mammography: an initiative in France.

The ultimate effectiveness of any mass screening campaign is directly related to strict compliance with certain rules and technical protocols designed for radiological installations. Concerning radiological screening, the primary technical objective is to ensure reproducibly high quality images at low radiation dose to the patient. This paper describes the methodology followed for the implementation of a quality control (QC) programme of 48 mammography installations used within the context of the breast cancer screening campaign in the Bas-Rhin region of eastern France. In order to demonstrate the efficacy of such a programme, results of QC tests and procedures relating to each element of the radiological imaging chain are presented and compared for four control visits carried out at 6 month intervals over a period of 2 years. A reduction of 50% (from 16 mGy to 7.5 mGy) of breast entrance doses (normalized to a breast thickness of 4.5 cm) and a significant improvement of detectability of high contrast details are discussed. The importance of encouraging an extension of a similar process to the other screening experiments currently underway in France is emphasized.

A simple, rapid and accurate screening test for hyperchlororhoea.

A simple, rapid screening paper test for the quantitative estimation of Cl- concentration in feces is described. The measured time of decolorisation of the Chlorotrex indicator by aqueous extract of feces is inversely proportional to the chloride concentration. The technical details of the test are given. The test is very suitable for laboratory and mass screening.

[The diagnosis of osteoporosis].

The diagnosis of Osteoporosis on WHO Technical Report of Prevention and Management of Osteoporosis in 2003, in addition to present status in Japan, has been reviewed. Since Technical Report of Assessment of Fracture Risk and Its Application to Screening for Postmenopausal Osteoporosis was published by WHO in 1994, the field of osteoporosis has progressed remarkably. In particular, in regard to methods of measuring bone mass or density, diagnosis, assessment of fracture risk, and assessment of osteoporosis, this Technical Report describes evidences, confirmed from current many data, and provides informations available to osteoporosis practice. This Report also shows future direction and issues of osteoporosis which should be resolved.

Review of calcium methodologies.

A review of calcium methodologies for serum has been described. The analytical systems developed over the past century have been classified as to type beginning with gravimetry and extending to isotope dilution-mass spectrometry by covering all of the commonly used technics which have evolved during that period. Screening and referee procedures are discussed along with comparative sensitivities encountered between atomic absorption spectrophotometry and molecular absorption spectrophotometry. A procedure involving a simple direct reaction for serum calcium using cresolphthalein complexone is recommended in which high blanks are minimized by repressing the ionization of the color reagent on lowering the dielectric constant characteristics of the mixture with dimethylsulfoxide. Reaction characteristics, errors which can be encountered, normal ranges and an interpretative resume are included in its discussion.

A neonatal mass-screening for congenital adrenal hyperplasia in Japan.

A pilot neonatal mass-screening for congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21-OHD) was performed in the western region of Shizuoka Prefecture, Japan, using a simplified radioimmunoassay method for 'Disc-17-hydroxyprogesterone (17-OHP) determination. The results obtained during a 30-month period indicated that 3 infants out of the 34314 neonates examined were proved to have 21-OHD, and the incidence of homozygotes and heterozygotes were estimated to be 1:11438 and 1:54, respectively. At the time of recall, the concentrations of plasma 17-OHP and 21-deoxycortisol and their urine metabolites as well as plasma sodium levels were quite applicable to diagnosis, while the clinical signs that may be manifest in 21-OHD were of little value in this connection. Prematurity and perinatal complications of neonates tended to give false-positive results, being secondary to the function of the residual foetal adrenal cortex and non-specific stimulatory effects of various stresses. Despite several technical and practical problems to be solved, the present study demonstrated the importance and validity of a neonatal mass-screening program for CAH.
