Bilateral idiopathic inflammation of the optic nerve sheaths. Light and electron microscopic findings.

Idiopathic perioptic neuritis is a term used to describe noninfectious inflammatory disorders of the optic nerve sheaths, the causes of which are unknown. In the following report, a 68-year-old woman with bilateral visual loss was found to have chronic inflammation with necrobiotic granulomas of her optic nerve sheaths. The patient, who had no systemic condition known to be associated with necrobiotic granuloma, lost vision from infarction of the optic nerve parenchyma and from compression due to thickened meninges. Although there are similarities between the inflammatory reaction in this case to the necrobiotic dermatoses, the pathogenesis of this condition remains obscure.

Granuloma annulare with transfollicular perforation.

In recent years the phenomenon of transepithelial elimination has been found to be the underlying mechanism in a variety of skin disorders. "Perforating dermatoses" may show transepidermal or transfollicular elimination or a combination of the two. A case of papular granuloma annulare is reported, where in one lesion the pilary apparatus actively participated in the elimination of necrobiotic collagen from the dermis. To our knowledge, perforating granuloma annulare with a transfollicular pathway has not previously been described. Also, a classification of perforating dermatoses is presented.

[Histopathological research and immunofluorescence of AVND virus infection in cultured scallop Chlamys farreri].

The naturally infected scallops Chlamys farreri sampled during mass mortality in summer of 2003 was detected by means of histopathological and MAb-based immunofluorescence assay (IFA). The results of histological examination demonstrated that a series of histopathological changes including cell swelling, basophilic increase, disorder, partial sloughing and excessive sloughing were always observed in epithelia of many different organs, e.g. mantle, gills, stomach, intestine and kidney. Additionally, the infected tissues were applied for in situ detection of the "acute virus necrobiotic disease" (AVND) virus by means of specific MAb-based IFA, and the result demonstrated that this pathological changes or lesions were perfectly coincident with the positive cells (fluorescencing cells) . The positive cells were denser in some local area of epithelia, and exhibited serious pathological lesions, which would reveal the roles of this virus in pathogenesis and further confirm that the AVND virus is the main causative agent of mass mortalities among cultured scallop Chlamys farreri farmed in northern coast of China.

Radiologic evaluation of uncommon inflammatory and reactive breast disorders.

The radiologic features of breast lesions caused by immunologic, reactive, and noncurrent infectious diseases often mimic those of malignancy, frequently constituting a diagnostic challenge even if the underlying disease is known. Churg-Strauss syndrome mimics carcinomatous mastitis. Amyloidosis usually manifests as a suspicious mass, often accompanied by microcalcifications. Wegener granulomatosis and sarcoidosis often manifest as irregular masses, although sarcoidosis can also manifest as round, well-defined masses reflecting intramammary node involvement. Diabetic mastopathy is a rare but well-known entity in patients with long-standing insulin-dependent diabetes. Breast involvement by necrobiotic xanthogranulomatosis is rare and manifests as multiple bilateral asymmetric lesions. Multiple clustered hypoechoic tubular structures in a large hypoechoic mass seen after pregnancy can be suggestive of granulomatous mastitis. Mammary tuberculosis can manifest with a nodular, diffuse, or sclerosing pattern. A granulomatous inflammatory reaction must be carefully evaluated because it constitutes the major feature of a diverse group of diseases that includes vasculitis, granulomatous mastitis, tuberculosis, and carcinoma-associated sarcoidlike reactions. Core biopsy can play a major role in developing a differential diagnosis for these rare immunologic, inflammatory, or infectious disorders affecting the breast, and knowledge of these entities can, in the appropriate clinical setting, help the radiologist narrow the differential diagnosis, although cancer must be excluded definitively.

Necrobiotic xanthogranuloma with scleroderma.

I report the case of a 68-year-old man who presented with necrobiotic xanthogranuloma (NXG) with concomitant scleroderma, along with the presence of cryoglobulins and cryofibrinogen in the blood. Autoamputation of the distal fingers and parts of the ears also occurred, over time, with exposure to cold temperatures. This is an interesting constellation of findings in this rare disorder that raises questions concerning its pathogenesis. A review of the literature is included.

[35S-cysteine incorporation into protein synthesis of the liver and testes in subacute oral poisoning with zineb, maneb and mancozeb].

35S-cysteine incorporation in the protein synthesis of liver and testes is studied in groups of 20 white rats each, following 14--day poisoning with dithiocarbamates, at doses 1/5 LD50, as follows: group one, cineb - o,180 g/kg, maneb - 0,140 g/kg, and mankozeb - 0,260 g/kg. Protein incorporation in liver and testes is studied after decapitation of the animals, within two hours of intraperitoneal 35S-cysteine injection at dose 15 micro C per rat. Liver and testes undergo appropriate treatment after the method of Munro et al./10/, as modified by Sheitanov /9/, and counting is done with VAZ--310. The results point to a varying degree of cysteine incorporation increase in the protein synthesis of liver and testes. It is attributed to a variety of dystrophic and necrobiotic changes in the tissues of either of the organs, taking place under the effect of dithiocarbamate preparations. An enhancement of regenerative processes in the above mentioned organs, associated with intensified protein synthesis, is by no means ruled out. Differences established in the intensity of amino acid incorporation correlate with the degree of protein synthesis disorders, mostly in terms of the effect of maneb preparations.

Granuloma annulare with follicular perforation.

An instance of perforating granuloma annulare was observed in a 42-year-old female patient in which the necrobiotic material in the dermis was eliminated by a hair follicle. This had previously been described by the author in only 1 other case of perforating granuloma annulare. Transepithelial elimination may occur by the transepidermal and/or transfollicular route. Both pathways are possible in perforating granuloma annulare. Some factors are discussed which may be responsible for the predominance of one route over the other in various disorders of transepithelial elimination.

[Morphologic and morphometric studies of the liver of the white rat during inadequate feeding and aflatoxin poisoning].

The authors carried out morphological and morphometric studies of the liver of rats under malnutrition and chronic aflatoxin intoxication for 2 and 7 months. Malnourished animals demonstrated atrophic alterations. Animals exposed to aflatoxin B1 manifested severe dystrophic and necrobiotic alterations which did not depend on the nutrition regimen. In rats administered aflatoxin, blastomic processes were identified only at the 7th month. Whatever the diet.

[Morphology and pathogenetic problems of the crush syndrome].

Morphofunctional studies of muscles, heart, liver and kidneys after different periods of compression and decompression, as well as literature data indicate that crush syndrome is one of the most severe forms of traumatic shock. A wide range of pathologic effects of catecholamines and other shock-causing agents in response to the emotional stress and pain occurs already at the compression period and results in hemodynamic disturbances in microcirculation of organs and tissues with the development of dystrophic and necrobiotic processes, depression of the monocytic phagocyte system and immune system. The consequences of shock are mostly manifest after decompression: hypercatecholaminemia, hypovolemia, intoxication with myolysis and pathogenic microflora products result in aggravation of monocytic phagocyte failure, as well as immune system, intravascular coagulation, membrane penetration insufficiency, cell necrosis. Monocytic macrophage depletion favours the progression of hepatic necrobiosis, formation of renal failure and detritus organization in the muscles of the extremities. Hypercatecholaminemia and hypoxia (leading to electrolyte-imbalance contractures of myofibrillar apparatus, metabolism disorder and intracellular conductivity disturbance) from the basis for cardiac insufficiency. Inadequate cardiac function, in its turn, maintains hemodynamic and hypoxic disturbances in tissues. Changes in renal blood flow, hemofiltration and tubular system are shown to reflect different aspects of pathogenesis of the acute renal failure in crush syndrome.

[Changes in the venous endothelium after acute hemodynamic disorders].

Changes in endothelium of the inferior vena cava have been studied in 1, 2, 3, 5, 7, 9, 20, 30 and 50 days on 42 mature rabbits (6 of them used as the control) survived after orthostatic collapse. Fixation in "suza" fluid, after Carnoy or in the 15% formalin solution with a preliminary ultraviolet irradiation in 0.25% silver nitrate solution are used to reveal the cellular borders. Film preparations are prepared and stained with ferric hematoxylin after Heidenhain. The data obtained are statistically treated using the process approximation by means of calculating the sliding averages. The orthostatic collapse, accompanied with certain changes in hemodynamics produces, after the 2d day, a significant decrease in the arrangement of the cellular layer, development of clearly seen certain destructive and necrobiotic shifts in endothelium which are kept for a long time (50 days--time of observations). Beginning from the 5th--7th days, the restorative reaction takes place according to the cell hyperplasia type (increased mitotic coefficient in endotheliocytes) and hypertrophy (increased nuclear and cellular size, presence of binuclear elements), that reflects some processes of the intracellular renewal. The peculiar feature of the regeneration process is its late development and a prolonged course that depends on duration of the pathological shifts in the tissue. The processes observed in the endothelial layer result in an essential increase in the tissue polymorphism that characterizes its main property--plasticity.

[Perioperative intra-arterial irrigation perfusion for adjuvant therapy of acute intestinal circulatory disorders].

The extent of mesenteric infarctions caused by intestinal circulation disorders essentially depends on reactive vasoconstriction and oxygen radical induced lesions of enteric mucosa. Animal experiments indicated protective effects of an intraarterial flushing perfusion of mesenteric arteries with vasodilators and anti-oxidants. We carried out a transaortic perfusion of the superior mesenteric artery with lactated Ringer's-solution and vasodilators (papaverin, tolazolin, PGE1) in acute occlusion of mesenteric arteries in 12 patients (case reports). Three patients were treated successfully without reconstruction by conservative method alone. Only one patient (8.33%) died because of uncontrolled enteric ischemia. Angiological therapy is necessary in modern treatment of acute mesenteric ischemia. Surgical goals are elimination of central vascular occlusions and resection of necrobiotic areas of intestinal organs.

Bilateral periocular actinic granuloma in a patient with renal failure: a clinicopathologic study.

PURPOSE: We report a 50-year-old man with end-stage renal disease who has developed bilateral inner and outer canthal nodules. METHODS: The nodules were excised, fixed in 10% formalin and 2.5% glutaraldehyde, and processed for light and electron microscopy, respectively. RESULTS: Histological examination showed elastotic changes with associated granulomatous inflammation. The granulomatous reaction was centered around elastotic fibers, and some giant cells engulfed the fibers. Areas of dystrophic calcification were identified. A diagnosis of actinic granuloma was made. The differential diagnosis and histopathology of actinic granuloma, i.e. granuloma annulare, rheumatoid nodule, and necrobiotic xanthogranuloma, are discussed. CONCLUSION: This case of an actinic granuloma originating in the sun-damaged periocular skin provides further evidence that elastotic degeneration may be associated with a giant cell reaction. It is, however, unclear which antigens are responsible for initiating the granulomatous response and whether elastotic material on its own can elicit a granulomatous reaction.

Interstitial granulomatous dermatitis with arthritis.

BACKGROUND: Interstitial granulomatous dermatitis with arthritis is an uncommon systemic disorder involving the cutaneous and musculoskeletal systems. The eruption may mimic other dermatoses including granuloma annulare, erythema chronicum migrans, and the inflammatory stage of morphea. Key histopathologic characteristics, along with clinical correlation, allow accurate diagnosis. OBJECTIVE: We describe the clinical, serologic, and histologic features in three patients with interstitial granulomatous dermatitis with arthritis. METHODS: Skin biopsy specimens were examined and correlated with the clinical and laboratory findings. RESULTS: Erythematous, annular, indurated plaques on the extremities were present in two women. An erythematous, papular eruption on the head and neck was present in a third patient. All patients had myalgia and migratory polyarthralgias of the extremities along with various serologic abnormalities. Histologic examination revealed a dense lymphohistiocytic interstitial infiltrate involving primarily the reticular dermis. Foci of necrobiotic collagen were present. Vasculitis was absent. CONCLUSION: Interstitial granulomatous dermatitis with arthritis is unique multisystem disease with variable cutaneous expression. Abnormal serologic findings indicate a possible connection to collagen vascular disease.

Deep granuloma annulare (pseudorheumatoid nodule) in children: clinicopathologic study of 35 cases.

Deep granuloma annulare (DGA) is one of several lesions of skin and superficial soft tissues whose histologic character is a palisading granuloma with a small central focus of necrosis or necrobiosis. Unlike the other palisading necrobiotic lesions, DGA has a predilection for children in the first 5 to 6 years of life. A painless subcutaneous nodule(s) in the lower anterior tibial region or foot and the scalp, typically in the occiput, was the most common presenting feature in this study of 35 cases. Additional or recurrent lesions were reported in approximately 70% of cases with clinical follow-up. All lesions showed the presence of necrobiosis; however, one of the characteristic features was the multinodular character of the predominantly mononuclear cellular aggregates. The presence of vascular spaces at the periphery of the nodular profiles served as a clue to the diagnosis of DGA. The palisading arrangement of the mononuclear cells was evident only in those foci with central necrobiosis. A histiocytic disorder or fibrohistiocytic process was a common consideration in the differential diagnosis, especially in those cases with less apparent foci of necrosis. Palisading histiocytes with prominent eosinophilic cytoplasm and some nuclear atypism were problematic with regard to possible epithelioid sarcoma. Our study failed to identify any underlying or predisposing factors in the development of DGA. Despite the fact that DGA is a well-documented lesion in children, it occurs sufficiently infrequently that it is often not considered clinically when it presents as a subcutaneous mass or masses in a child. Its recognition by the pathologist is especially important as the occurrence of additional lesions in a high proportion of children can be anticipated without undue concern.

[HRCT of the lung in collagenoses].

Collagen vascular diseases, representing systemic soft tissue disorders, may cause a broad spectrum of pathologic changes of the respiratory tract. The type and extent of manifestations can vary considerably among individuals and entities. This survey describes the chest radiographic and, in particular, high-resolution computed tomographic (HRCT) findings of individual lesions of the respiratory tract. It includes fibrosing alveolitis (alveolitis, interstitial pneumonia, pulmonary fibrosis) and bronchial (bronchitis/bronchiolitis, bronchiectasis), pleural and vascular manifestations, as well as lymphadenopathy and abnormalities related to therapy. We present typical patterns of changes in progressive systemic sclerosis (PSS, scleroderma), systemic lupus erythematosus (SLE), mixed connective tissue disease (MCTD, Sharp syndrome), Sjögren syndrome, overlap syndrome and rheumatoid arthritis (RA). Furthermore, we describe findings which are specific for individual entities such as esophageal involvement in PSS, acute pneumonitis and pulmonary hemorrhage in SLE, lymphoproliferative disease in Sjögren syndrome and necrobiotic nodules in RA.

Metastatic Crohn's disease. Report of 3 cases with special reference to histopathologic findings.

BACKGROUND: Metastatic Crohn's disease is a rare, often unrecognized cutaneous disorder lacking definite histopathologic criteria. The purpose of this study was to document clinicopathologic and immunologic findings in 3 patients with metastatic Crohn's disease. The histopathologic findings are evaluated in correlation to those reported in the literature in an attempt to better define the histopathologic features. OBSERVATIONS: None of the patients showed signs of depressed cell-mediated immune response as evaluated with skin tests and T-cell subtyping of blood samples. One of the patients had antineutrophil cytoplasmic antibodies. Polymerase chain reaction, a highly efficient method of amplifying low levels of specific DNA sequences, did not show mycobacterial DNA in the samples studied. Granulomas of the sarcoid type with numerous foreign body and Langhans giant cells were the dominating features. In accordance with previous results, we found vascular involvement in 2 cases, manifested as granulomatous perivasculitis in both. We also found necrobiotic areas in all 3 cases. CONCLUSIONS: We propose that both necrobiosis and granulomatous perivasculitis be added to the histopathologic characteristics of metastatic Crohn's disease. Patients may even have a positive antineutrophil cytoplasmic antibody test result.

[Pleuro-pulmonary manifestations of rheumatoid polyarthritis].

Sensitive investigations such as pulmonary function tests, broncho-alveolar lavage or computered tomography at high resolution enable pleuro-pulmonary disease to be detected in nearly 50% of those patients studied who had rheumatoid arthritis (PR). The prevalence of these manifestations is most usually elevated in male PR sufferers, those who are sero-positive or have associated extra-articular signs such as sub cutaneous nodules. More recently there has been evidence of genetic risk factors linked to HLA grouping or Pi phenotype. Amongst the usual manifestations, the pleurisies and above all necrobiotic nodules, which are most often asymptomatic, sometimes pose difficult problems in differential diagnosis, particularly when they precede the articular disease. The diffuse interstitial fibrosis remains the most worrying specific complication due to the fact of its potential seriousness. The pathophysiology of this form of fibrosis is better understood since the introduction of LBA. In the absence of any specific controlled studies its treatment remains impirical and is similar to that given for diffuse or idiopathic interstitial fibrosis. Pulmonary vascularity, the bronchiolitis obliterans with organising pneumonia and apical fibrosis, very similar to Hamilton's syndrome, are much rarer manifestations. On the other hand non specific respiratory infections are the cause of death in 10-20% of cases. Bronchiolitis obliterans induced by D Penicillamine is the most severe iatrogenic manifestation, since corticosteroid therapy associated with immunosuppressive drugs enables at best a stabilisation of the alveo bronchiolar lesions. More recently there have been twenty observations of hypersensitivity pneumonia to low dose methotrexate. The prevalence of these pulmonary disorders during the treatment of PR is around 5%. However the respiratory contraindications of these drugs which are being used more and more and the methods of pulmonary surveillance under treatment are not yet defined.

Subcutaneous granuloma annulare of the penis in 2 adolescents.

Granuloma annulare (GA) is a benign inflammatory disorder of unknown etiology characterized histologically by dermal palisading granulomas with central degeneration of collagen (necrobiosis). There is a rare subcutaneous clinical variant, this occurring more frequently in children than in adults and very rarely involves the penis. We describe 2 cases of penile subcutaneous GA developing in adolescent boys who to our knowledge has not previously been described in literature. Both were initially treated with surgical excision. Circumcision was performed on one of the boys, with subsequent improvement with the resolution of most of the nodules. Granuloma annulare of the penis is very rare, with only 7 cases reported to date [Narouz N, Allan PS, Wade AH. Penile granuloma annulare. Sex Transm Infect 1999;75(3):186-7; Trap R, Wiebe B. Granuloma annulare localised to the shaft of the penis. Scand J Urol Nephrol 1993;27(4):549-51; Laird SM. Granuloma annulare of the penis.
