[Epidemiological study of snoring--a questionnaire survey in factory workers].

There has been no epidemiological study of snoring in Japan, and we therefore performed a questionnaire survey (in about 7,000 adult men working at a steel-making factory at the time of the yearly health examination, and investigated the relationship between the severity of snoring and 17 items including age, obesity, family history of snoring, daytime hypersomnolence, hypertension, smoking, alcohol intake and traffic accidents. We classified all the subjects into three groups, no snoring, mild snoring, and severe snoring group. We defined severe snorers as persons who snored loudly in both inspiratory and expiratory phases and those who snored loudly with apnea. We found that aging, obesity, smoking and alcohol intake are risk factors for snoring. Compared with non-snorers, severe snorers were found to have a high incidence of family history of snoring, daytime hypersomnolence, and history of treatment of hypertension. No relationship was found between the severity of snoring and the occurrence of automobile accidents. The proportion of severe snorers over 40 years old with obesity, daytime hypersomnolence and morning headache was 0.25%, representing the group that may have obstructive sleep apnea syndrome. The probable incidence of sleep apnea syndrome in men may be considerably lower in Japan compared with that in either U.S.A. or Europe.

[Questionnaire survey on teleradiology for radiological technologists: analyses by age group and facility type in Hokkaido].

A questionnaire survey was given to 1,951 radiological technologists in Hokkaido to examine their views on teleradiology. The questionnaire consisted of questions about their understanding of teleradiology and interest in it as well as examining the need for teleradiology and problems with it. A total of 1,275 radiological technologists responded to the survey, a response rate of 65.4%. The returned questionnaires were analyzed according to different groups categorized by age of the radiological technologists and type of medical facility. Almost all radiological technologists knew about teleradiology. Among the different age groups, the higher age groups showed greater recognition and understanding of teleradiology. About 60% of radiological technologists were interested in teleradiology (no significant differences among age groups). When it was assumed that a teleradiology system would be introduced, (1) the working load for radiological technologists was estimated to be about 17,820 (+/-24,233) yen per month. (2) Sixty percent of the respondents considered that the system should be used for the reading and diagnosis of difficult cases. (3) Fifty percent of them thought that the system should be managed and operated by radiological technologists. (4) Many radiological technologists pointed out problems concerning initial costs and the facility's management system.

[Epidemiological studies of sleep apnea-hypopnea syndrome: a questionnaire survey of Taiyuan].

OBJECTIVE: To obtain the epidemiological data of sleep apnea-hypopnea sydrome (SAHS) in Taiyuan. METHODS: A questionnaire survey was performed in 6 028 people living in Taiyuan. The prevalence of SAHS was estimated by a two-stage procedure. In the first stage, stratified cluster disproportional random sampling survey was performed in Taiyuan. 6 028 questionnaires were send to random sample of defined population in the 4 sites selected from 2 districts. The response rate was 85.11%. During the second stage 476 of those highly suspected of having SAHS (ESS >/= 9) underwent all-night polysomnographic (PSG) studies. RESULTS: From the study population, 179 were diagnosed as having SAHS. The overall prevalence was 3.5% (male 4.7% and female 1.9%). CONCLUSION: The prevalence of SAHS was 3.5% among Taiyuan.

[Personal support and mortality: a cohort analysis of the National Questionnaire Survey 1984-86].

OBJECTIVES: The aim of the study is to investigate the association between personal support and mortality experience in Germany. METHODS: Data source is the Life Expectancy Survey of the Federal Institute for Population Research (Wiesbaden). This study comprises the National Questionnaire Survey of the German Cardiovascular Prevention Study (1984-86) and a mortality follow-up (1998). Included are 7240 persons with German nationality aged 31-69 years at baseline. 957 persons (13.2%) died during the observation period from 1984-1998. An additive index combining familiy status and number of persons available in emergency situations was administered as indicator for social support. The multivariate statistical analysis was performed with the cox-regression method. RESULTS: Persons with low or very low personal support yielded in both sexes significantly higher relative risks for mortality during the 14 years of follow-up. The relative risks in the regression models including confounding factors for low personal support were 1.49 (p < 0.01) in males and 1.67 (p < 0.01) in females, and for very low personal support 1.81 (p < 0.05) in males and 2.31 (p < 0.01) in females. CONCLUSIONS: These results are indicating an increasing importance of personal support for the mortality experience in the general population.

Attributes of specialist search dogs--a questionnaire survey of UK dog handlers and trainers.

The ideal and actual characteristics of specialist search dogs have been examined in questionnaire surveys of 244 dog handlers and trainers from the six main UK dog-using Government agencies. The ten most important characteristics were (ideal level in brackets): acuity of sense of smell (very high), incentive to find an object which is out of sight (very high), health (very high), tendency to hunt by smell alone (very high), stamina (very high), ability to learn from being rewarded (very high), tendency to be distracted when searching (very low); agility (high), consistency of behaviour from day to day (high), motivation to chase an object (high). Significant differences between actual and ideal levels were found for 22 of the 30 characteristics, predominantly in undesirable attributes, suggesting that there is scope for significant improvement in operational effectiveness.

[Misconduct in medical research. A questionnaire survey among project leaders in health region 4].

During the period 1986-92, a questionnaire survey was conducted among research project administrators whose study protocols were assessed by the ethical committee for biomedical research in Health region 4 (Central Norway). The questions referred to the scientists' attitudes towards medical research ethics in general, and their views on the work of the committee. They were also asked to comment on seven statements about scientific fraud and misconduct. This paper presents data on misconduct from the 119 scientists who completed the questionnaire, i.e. 70 percent of the 159 recipients. Some 40 percent claimed that scientific fraud is a problem in Norway, but 46 percent maintained that it is less so than in other countries. More than every fourth researcher (27 percent) knew of one or more cases of scientific misconduct, 42 percent stated that their knowledge was not publicly known, 18 percent felt that they had been exposed to misconduct themselves. A majority (60 percent) stated that a better system is needed to investigate claims of scientific fraud. The authors discuss these findings in relation to the personal characteristics and scientific qualifications of the researchers, and the current knowledge about the extent of scientific fraud in medicine.

Questionnaire survey of Japanese urologists concerning the diagnosis and treatment of chronic prostatitis and chronic pelvic pain syndrome.

OBJECTIVES: We performed a questionnaire survey to investigate various issues in the diagnosis and treatment of chronic prostatitis/chronic pelvic pain syndrome by Japanese urologists and to clarify the circumstances surrounding prostatitis in Japan. METHODS: Japanese urologists (n = 1869) were surveyed by mail using a 17-item questionnaire to determine current diagnostic and treatment practices for prostatitis/chronic pelvic pain syndrome. RESULTS: Only 1.5% (11/739) of urologists diagnosed chronic prostatitis/chronic pelvic pain syndrome using the 4-glass test, while most did so using the 2-glass test (voided bladder [VB]2 and VB3, or VB2 and expressed prostatic secretion [EPS]). Approximately half (55.2%; 412/746) did not perform urine cultures to differentiate chronic bacterial prostatitis from chronic abacterial prostatitis/chronic pelvic pain syndrome. Approximately half (46%; 343/746) did not count the number of leukocytes in VB2 or VB3/EPS to differentiate chronic abacterial prostatitis from chronic pelvic pain syndrome. Although many urologists (63.8%; 459/720) thought that chronic abacterial prostatitis/chronic pelvic pain syndrome was not an infectious disease, many chose antimicrobial agents as the primary treatment. More than half (52.2%; 384/735) of all urologists felt pessimistic about dealing with chronic prostatitis/chronic pelvic pain syndrome compared to treating benign prostatic hypertrophy or prostate cancer, because of the high number of complaints by patients and their own lack of confidence in diagnosing and treating the condition. CONCLUSION: There is much confusion and frustration among Japanese urologists about chronic prostatitis/chronic pelvic pain syndrome. Further studies are needed to elucidate its etiology and pathogenesis, and to establish guidelines for its diagnosis and treatment.

Ethical attitudes of intensive care physicians in Hong Kong: questionnaire survey.

OBJECTIVES: To examine the practice and ethical attitudes of intensive care doctors in Hong Kong and to compare findings with those from European studies. DESIGN: Structured questionnaire survey, modified from a similar questionnaire used in Europe. SETTING: Eleven publicly funded intensive care units in Hong Kong. PARTICIPANTS: Ninety-five doctors practising in intensive care units. RESULTS: Of the sixty-five respondents, sizeable proportions indicated that the admission of patients to the intensive care unit is often (25%) or sometimes (51%) limited by bed availability. About 69% to 86% of doctors admit patients with limited prognosis or poor quality of life, although all felt that these admissions should be more restricted. 'Do-not-resuscitate' orders are applied by almost all respondents, and 52% and 89% of respondents would discuss such orders with the patient or with the family, respectively. The withholding and withdrawal of therapy from patients with no chance of recovery to a meaningful life is common in Hong Kong (99% and 89%, respectively). A total of 83% respondents involved patients or families in the decision to limit therapy, compared with less than half in Europe overall. When the family wanted aggressive life-support despite doctors' recommendations to limit therapy, 62% of the respondents would still withhold therapy while only 9% would withdraw therapy. More than 60% of doctors feel comfortable talking to patients' relatives about limitation of therapy. Approximately 75% felt that euthanasia is unacceptable. Most respondents (94%) reported that medical programmes should include more extensive discussion on ethical issues. CONCLUSION: The ethical attitudes of intensive care doctors in Hong Kong are similar to those of counterparts in Europe. However, Hong Kong doctors tend to involve families more often in the discussion of end-of-life issues.

Use of lower-limb prostheses: a questionnaire survey.

The degree of use made of artificial legs by a sample of primary amputees is investigated by means of a questionnaire survey. Only four respondents indicated non-usage, but, of the remainder, about half indicated that they had problems which prevented them from using their prostheses "fully"; the vast majority of these, however, stood and/or walked on their prostheses every day.

How do general practices manage requests from patients for 'same-day' appointments? A questionnaire survey.

Same-day appointment requests are common and can be problematic for general practices that run appointment systems. In a questionnaire survey sent to 90 general practices in one health authority area (response rate 88%), a variety of management systems existed for dealing with same-day appointment requests. Managing the requests was found to be a significant cause of stress for many general practitioners. Registrars, locums, and practice nurses play only a small part in meeting patient demands and few practices operate telephone triage to help manage these requests.

[Questionnaire survey of AIDS examination recipients at government-run public health center regarding AIDS awareness promotion and HIV examinations].

An anonymous questionnaire survey was conducted among persons who had undergone screening for AIDS at public health centers under the auspices of 7 local government bodies (Hokkaido, Metropolitan Tokyo, Aichi Prefecture, Osaka Prefecture, Kobe City, Saga Prefecture, Okinawa Prefecture). There were 1,230 replies, for a response rate of 46.8%. Questions in the survey pertained to general knowledge of AIDS, information desired by the recipients, desired ways to foster awareness, and a desirable examination system. A comparison was made in terms of gender, marriage status (married, single) and age (less than 40 years of age, more than 40 years of age), respectively. The survey results were as follows. 1. Concerning the infection route, accurate response rate was low with regard to the mother-child infection, mosquito transmission, and use of contaminated needles. 2. Desired methods to promote AIDS awareness expressed among replies were given in the following order or frequency: "information via radio and TV," "spread of knowledge through telephone consultation," and "holding an AIDS Week or similar kinds of campaigns." 3. Information sought by respondents frequently included AIDS treatment, pathology, and spread. This trend was most obvious among those under 40 years of age. 4. As for the AIDS examination system, replies most often reflected the hope that the examination site would be the Public Health Center, the charge would be gratis, and that one could be examined at night during the week, and on days off. 5. For those undergoing examination where there was no actual concern of possibility of HIV infection, single persons, accounted for around 80% of the overall. 6. More than half of those who had been examined mentioned having experienced concerns about maintaining confidentiality at the time they were examined.

Short form 36 (SF36) health survey questionnaire: normative data for adults of working age.

OBJECTIVES: To gain population norms for the short form 36 health survey questionnaire (SF36) in a large community sample and to explore the questionnaire's internal consistency and validity. DESIGN: Postal survey by using a booklet containing the SF36 and several other items concerned with lifestyles and illness. SETTING: The sample was drawn from computerised registers of the family health services authorities for Berkshire, Buckinghamshire, Northamptonshire, and Oxfordshire. SAMPLE: 13,042 randomly selected subjects aged 18-64 years. MAIN OUTCOME MEASURES: Scores for the eight health dimensions of the SF36. RESULTS: The survey achieved a response rate of 72% (n = 9332). Internal consistency of the different dimensions of the questionnaire was high. Normative data broken down by age, sex, and social class were consistent with those from previous studies. CONCLUSIONS: The SF36 is a potentially valuable tool in medical research. The normative data provided here may further facilitate its validation and use.

Questionnaire survey of foot and mouth disease (FMD) and of FMD control by vaccination in villages in northern Thailand.

The authors describe a questionnaire survey of foot and mouth disease (FMD) and of FMD control by vaccination in sixty villages in northern Thailand. FMD was found to be common, with 90% of the villages surveyed reporting one or more outbreaks in the five years prior to interview, and 28% of villages reporting outbreaks every year. Outbreaks were more common in the wet season (June to September) and most outbreaks lasted for four weeks or less. The common sources of the most recent outbreak were the introduction of infected cattle and buffalo from a public market or surrounding village (25 of 60 cases), and commingling of cattle and buffalo with those of an infected neighbouring village (24/60). FMD was not perceived to be a major animal health problem, and there was substantial variation between villages in the percentage of animals vaccinated. The main reasons for not vaccinating were the fear that vaccination caused abortion, and the fact that animals were difficult to assemble for vaccination. It was concluded that vaccination cover needs to be raised significantly if FMD is to be effectively controlled.

[Present state and disadvantage of rapid diagnosis method in clinical microbiology--analysis of questionnaire survey].

To investigate the present state and the disadvantages of rapid diagnosis methods in clinical microbiology in Japan, We have conducted a questionnaire survey of 360 medical facilities accredited by the Japanese Association for Rapid Method and Automation in Microbiology (JARMAM). Major rapid diagnosis methods being used in clinical microbiology are, in the order of its use rate, influenza virus, adenovirus, Mycobacterium tuberculosis, Clostridium difficile toxin A, Rotavirus, hepatitis B virus, group A Streptococcus, RS virus, hepatitis C virus antibody. The fact found by this survey is that there is some gap between two groups, as one group views that the result of rapid diagnosis method can be considered as the final, another views that it is a supplementary diagnosis under conventional methods such as culturing. Some problems related to rapid diagnosis methods are also pointed out; how to interpret and report a test result obtained by the rapid diagnosis method when it is different from that of culture method, it can not perform antibiotic sensitivity tests, a problem of non-specific reaction, comparatively higher cost of rapid diagnosis kits. This survey finds that rapid diagnosis methods have greatly contributed to early medical treatment and appropriate therapy with its quickness, such as the use of rapid diagnosis kits for bacterial meningitis or viral infectious diseases requiring clinical urgency is highly necessitated even though some problems mentioned above exist.

[The significance of a reminder and physician's motivation for the response rate of a questionnaire survey in general practice].

The use of a mailed reminder to improve the response rate in a questionnaire survey was analysed in a general practice multicentre survey in Denmark, Norway, and the Netherlands. In total 14 general practitioners (GP's) handed out the questionnaire to 650 adult patients. A randomized sample of the patients who did not respond after three weeks received a reminder including a new questionnaire. In the group that received a reminder the response rate was significantly higher compared with the group that did not (79% vs. 62%). In Denmark, two of the four participating GP's were specially motivated. The response rate for these GP's was significantly higher compared to the normally motivated GP's (91% vs. 71%). Specially motivated GP's can increase the response rate and thereby minimise the effect of the reminder.

A questionnaire survey on nematode control practices on horse farms in Denmark and the existence of risk factors for the development of anthelmintic resistance.

A questionnaire survey to obtain information on endoparasite control practices and management on 68 Danish horse farms was undertaken in 1995. The study revealed that foals, young horses and adults were on average, annually treated 4.3, 4.0 and 3.7 times, respectively. The most commonly used drug from 1993-1995 was ivermectin. On average 2.4 different drugs were used annually. The most used method of weight estimation was eye measure: for foals by 78%, for youngsters by 81% and adults by 82% of the herd owners. The most commonly used weight in the dosing of anthelmintics was individual weights of the horse: 72% of the herd owners dosed their foals this way. 76% their youngsters and 75% their adults. Sixty two percent of the herd owners treated at turn out, 47% at housing, 57% treated when buying new horses, 26% treated when stabling visiting horses, 78% applied pasture change and 18% performed alternate/mixed grazing. Sixty one percent of the herds had experienced problems with diarrhoea and in 18% of the cases the suspected cause was considered to be endoparasites. 33% of the farms performed disease registration and on 25% Faecal Egg Count Reduction tests had been performed before entering this study. The herd owners obtained their formation about endoparasite control from veterinarians, meetings and papers in that order of importance.

Teenagers' knowledge of emergency contraception: questionnaire survey in south east Scotland.

OBJECTIVE: To determine the level of knowledge of emergency contraception among 14 and 15 year olds. DESIGN: Confidential questionnaire survey. SETTING: 10 secondary schools in Lothian, south east Scotland. SUBJECTS: 1206 pupils predominantly (98.7%) aged 14 and 15 in the fourth year of secondary school. MAIN OUTCOME MEASURES: Knowledge of the existence of emergency contraception; of its safety, efficacy, and time limits; and of where to obtain it. RESULTS: 1121 (93.0%) fourth year pupils aged 14-16 had heard of emergency contraception. 194 girls (32.7%) and 168 boys (27.5%) had experienced sexual intercourse. Of girls who had experienced sexual intercourse, 61 (31.4%) had used emergency contraception. Knowledge of correct time limits was poor, sexually active girls being the most knowledgeable. Pupils attending schools ranked lower than the national average for academic attainment were less likely to have heard of emergency contraception and more likely to have been sexually active. 861 (76.8%) pupils knew they could obtain emergency contraception from their doctor. 925 (82.5%) pupils believed emergency contraception to be effective but 398 (35.5%) thought it more dangerous than the oral contraceptive pill. CONCLUSIONS: One third of sexually active girls aged under 16 in Lothian have used emergency contraception. This may help explain the fairly constant teenage pregnancy rates despite increasing sexual activity. Scottish teenagers are well informed about the existence of emergency contraception. However, many do not know when and how to access it properly. Health education initiatives should target teenagers from less academic schools as they are more likely to be sexually active at a young age and are less well informed about emergency contraception.

[Questionnaire survey of full-time occupational health physicians in Japan].

In August 1987 the University of Occupational and Environmental Health conducted a mail questionnaire survey on occupational health physicians (OHPs) located throughout Japan. The objective was to obtain preliminary information concerning the actual situation of OHPs together with their opinions on post-graduate training in this field. A summary of the results based on 187 valid responses (valid response rate = 59.4%) is as follows: 1. Age distribution of the respondents was in excess of 55 years of age (mean age of 61.4 yr), suggesting aging of the work force. 2. Comparison of the present age with period in present position revealed that a significant proportion of the respondents assumed their current position following retirement from clinical service. 3. Routine work schedules such as clinical activities and making site visits within factory tended to be influenced by the number of employees. 4. Conduct of special physical examinations on workers in hazardous environments was rated the highest among the necessary abilities of OHPs. 5. It was found that 30-34 years of age and 2-5 years of clinical experience were the minimal qualifications for a full time OHP.
