Indirect assessment of cardiovascular "demands" using telemetry on professional football players.

Aerobic training is not a major component of th conditioning programs of professional football players. We used telemetric ECG monitoring to examine the cardiovascular "demands" of practice and game situations in six athletes who were selected to affect a variety of positions and aerobic capacities. Heart rates commonly considered high enough for an aerobic conditioning effect were rarely attained during a practice sessions. Maximum oxygen consumption testing confirmed that professional football players do not receive a sufficient amount of aerobic conditioning during practice. Telemetrically monitored heart rate response is much greater in a game situation than that experienced during practice. Telemetry results support injury statistics by indicating that aerobic conditioning should have a significant role in the training program of the professional football player.

The clinical features and prognosis of pseudoseizures diagnosed using video-EEG telemetry.

A total of 110 patients underwent diagnostic evaluation for attacks of uncertain origin by means of video-EEG telemetry and had a diagnosis of pseudoseizures confirmed. Eighty-six patients (78%) were female, mean age of onset 25 years, and mean duration of attacks was 3 years. Many of the patients had erroneously been thought to be suffering from epilepsy. The attacks could be divided into two broad categories: attacks of collapse (one-third) and attacks with prominent motor activity (two-thirds). In some patients, the attacks were associated with incontinence and injury. The differential diagnosis and clinical features of the attacks are described. Additional psychiatric features were present in 52 (47%) patients. Follow-up (for a median 5 years; range, 1 to 14 years) showed that 40% of these patients stopped having pseudoseizures. This favorable outcome was associated with being female, leading an independent life, a formal psychological approach to therapy and counseling, and the absence of coexisting epilepsy, but not with the duration of pseudoepilepsy, prior episodes of pseudostatus, the coexistence of overt psychiatric disease, or the clinical features of the attacks.

Measurement of blood pressure and heart rate by telemetry in conscious unrestrained marmosets.

A system is described for the continuous measurement of blood pressure, heart rate and motor activity by telemetry in conscious marmosets freely moving in their home cages. Consistent diurnal variations in these parameters were observed under standard conditions. However these parameters were sensitive to changes in the environment. Blood pressure values were similar to those measured by non-telemetric methods in conscious restrained marmosets while heart rate values were significantly lower.

Telemetry system for heart-rate and blood temperature using a microcomputer.

A radio-telemetry system using a microcomputer has been constructed to measure both the heart-rate (HR) and right atrial blood-temperature in dogs. The telemeter incorporates a thermistor temperature probe, which allows blood-temperature measurements in the range of 35 degrees-42 degrees C with an available resolution of +/- 0.02 degree C. The output of the receiver is connected to active filters, a frequency counter and an HR meter, and finally to a microcomputer, through an appropriate interface circuit, for data linearization and storage.

A new method for continuous chronic measurement and recording of blood pressure, heart rate and activity in the rat via radio-telemetry.

Accurate continuous chronic measurements of blood pressure from conscious laboratory rats are critical to many experimental protocols but have been difficult or impossible to acquire. A system consisting of an implantable radio-telemetry device, receiver, and computer-based data acquisition system that allows such measurements to be easily obtained has been developed. This system is capable of monitoring and recording arterial pressure (waveform, systolic, diastolic, and mean), heart rate, and activity from rats weighing greater than 175 gm for periods in excess of 6 months. Chronic patency has been achieved through a patented design which includes an antithrombogenic film and a gel membrane located at the catheter tip. Validation of telemetered measurements via carotid catheter has demonstrated the accuracy to be better than +/- 5 mmHg in 85% (N = 20) tested at 3 weeks post implantation, 86% (N = 15) tested at 8 weeks, and 78% (N = 9) at 12 weeks. Incidence of loss of patency was 2.3% (N = 44). This system has a demonstrated ability to obtain accurate continuous chronic measurements of arterial pressure free of the stressors associated with conventional systems.

Electrocardiographic and respiratory responses to viscerotropic and neurotropic strains of Newcastle disease virus measured by radio telemetry.

Two groups each of White Rock chickens from 6 to 23 weeks old were monitored by radio telemetry to determine their electrocardiogram and respiration response after they were infected with either a velogenic viscerotropic isolate of Newcastle disease virus (VVND) or the neurotropic GB strain of Newcastle disease virus (NGB). Significant changes were found in the heart rate, R wave amplitude, ST segment elevation, T wave amplitude, RS complex interval, ST segment duration T wave interval, TP segment duration, PR segment duration and TP interval of the birds infected with VVND, but no significant ECG changes were found in the birds infected with NGB. There were no significant changes in respiration rate in any birds.

A 4-channel telemetry system for hospital patient monitoring.

A 4-channel radio telemetry system has been designed and implemented for use in the operating room on patients undergoing surgery. 30 such systems are in daily use throughout the Duke University Medical Cener. Relative simplicity, long battery life and a DC-response capability are the important features of these systems. The four physiological channels used are; electroencephalogram, electrocardiogram, peripheral pulse, and arterial blood pressure.

Blood Pressure Determination

Clinical telemetry of EMG and temporal information during gait.

Multichannel telemetry has formed an integral part of the clinical assessment of children's walking problems. EMG signals and temporal information from foot switches are transmitted from a small belt-pack unit which provides almost complete freedom of movement for the child. Although patients with various crippling diseases have been studied, the investigation of problems resulting from cerebral palsy has been most valuable clinically. The effects of orthopaedic surgery to release or transfer muscles can be asssessed more positively and the causes of some unexplained gait patterns can be investigated more thoroughly.

Pulse code modulation telemetry in ski injury research. II. Preliminary results.

The excitation between the boot and the ski was measured in maneuvers skied over a variety of snow conditions. A precision pulse-code modulation (PCM) - frequency modulation (FM) telemetry system was custom built to transmit data from strain-gage force transducers mounted inside the test ski to a receiving station about 3 km distant. Field tests gathered cruising data from three basic maneuvers - snowplow, stem christiana, and parallel christiana. Some classic falls resulting from inadvertent release of the ski bindings were also recorded. For comparative purposes, the maneuvers were skied over a standard five-turn slalom course. Examination of the loading histories reveals that: (1) the data are nonstationary random; (2) boot compression for the test binding system exceeds 500 N and is caused by ski flexure; (3) an 80 Hz resonance of the particular toe binding mechanism is excited; (4) combined impulsive loading is significant; (5) the loading is related to the maneuver type; (6) bending components during forward falls exceed the tibia flexural strength even though heel binding release occurs at the laboratory setting, and (7) torsion and bending components exceed tibia fracture levels during elementary ski maneuvers.

Athletic Injuries

An ECG telemetry system for physiological studies on swimmers.

A low frequency telemetry system is described which has been designed to allow high quality ECG measurements to be made on active swimmers. The transmitting antenna is a small loop worn by the swimmer whilst the receiver is fed from a single wire loop around the swimming pool. Both ECG electrodes and the transmitter case are made in a waterproof form which allows the swimmer complete freedom of action to dive and swim normally. The transmitter can be constructed from normal miniature components although the present version has been assembled using thick film techniques which combine high reliability with a reduction in overall volume.

[Telemetry of blood pressure in healthy subjects and patients with tachycardia and hypertension under Prindolol treatment].

The effect of 0.4 mg i.v. Prindolol on arterial blood pressure and heart rate was investigated with continuous measurement by telemetry. The 19 healthy subjects and 13 patients were controlled at rest supine, standing and finally during exercise. Following Prindolol a significant decrease of blood pressure and heart rate was observed, in particular in the group suffering from hypertension and tachycardia and under conditions of exercise, when compared with the pretreatment values. No side effects were noted.

[A multichannel telemetry system for autonomic neural signals, blood flow velocity, blood pressure, and ECG measurements].

A radio multichannel telemetry system has been developed for use with chronically instrumented, unrestrained, small animals. The system can simultaneously record autonomic neural signals, blood flow velocity, blood pressure, and ECG. The system is time-multiplexed and pulse width modulation (PWM)/FM device, which employs two sampling frequencies. The system is designed with 10 standard low power integrated circuits, a 3 terminal voltage regulator, and a transistor. The size is 53 x 42 x 20 mm, and the weight, including two batteries is 40 grams. The system is powered by two lithium cells, which provide 60 hours of continuous operation.

[Clinical and neurophysiologic markers of epilepsy with frontal lobe seizures in a series of patients diagnosed by video-EEG-telemetry].

The general and ictal clinical features, including EEG findings, of 20 consecutive patients with frontal lobe epilepsy are presented. Diagnosis in all cases was based on video-EEG telemetry using strict criteria. Among the ictal motor phenomena, turning movements, dystonic, choreic and ballistic features prevailed. Ten patients vocalised during their seizures but loss of consciousness was rare, even if there was pronounced motor involvement. The duration of attacks generally was short, not exceeding 60 seconds in 18 patients, in whom sudden onset and cessation of seizures was noted. Ten patients showed inter-ictal epileptiform EEG activity and 19 had ictal abnormalities. The short duration and ictal clinical course may be helpful in differentiating frontal seizures from pseudoseizures.

Totally implantable dimension telemetry.

A totally implantable dimension telemetry system has been developed to instrument animals for chronic physiological research. Implantable signal processing electronics allow free-roaming animals with no percutaneous leads while retaining the long-term redproducibility of fixed implanted transducers. Two low-powered, custom-integrated circuits have been developed and assembled into an implantable package capable of measuring one dimension channel. The system has been operated in the amplitude modes of through-transmission and reflection as well as in a new Doppler-power configuration and aimed at determining interfaces between blood and surrounding structures. In a addition to single channel systems, these ICs are key elements in multimode, multidimensional implants capable of more accurate characterization of deep body structures.

Anatomy

An integrated circuit approach to totally implantable telemetry systems.

A series of totally implantable telemetry systems has been developed to determine such key physiological parameters as blood flow, pressure, dimensions, temperature, and bioelectrical activity in chronic research animals. Custom integrated circuits provide the signal-processing complexity and performance required to sufficiently instrument the animals for an accurate prediction of human responses. Additional implant and transducer technologies are necessary to complete the instrument package. Although the costs of these technologies are high, they are more than offset by the unique information obtained and overall reduction in the expenses of medical research because fewer animals can be studied over longer periods. A number of the IC-based implants are in use in several physiology and experimental drug studies where adequate reliability and performance could not be achieved by alternate approaches.

[Cariogenic potential of in-between-meals--results of oral plaque-pH-telemetry].

Using oral plaque-pH-telemetry the cariogenic potential of a sucrose-solution (10%), a sorbitol-solution (10%), of a roll with honey, a roll with jam together with sweetened coffee and the "Milchschnitte" has been examined. All sugar containing products cause a deep pH-drop in dental plaque. The duration of demineralization (time of plaque-pH below 5.5)-amounts to 75 min. after mouthrinsing with sucrose-solution, 80 min. after consuming a half roll with honey, 140 min. after eating a roll with jam and drinking sweetened coffee and 82 min. after consuming a "Milchschnitte". All these products shouldn't be used as in-between-meals or snacks without a consequent oral hygiene afterwards.

Dental Caries

A miniaturized ultrasonic flowmeter and telemetry transmitter for chronic animal blood flow measurements.

An ultrasonic blood flowmeter telemetry system, using only two integrated circuits and one transistor, has been designed for chronic unanesthetized animal studies. Standard components and construction techniques are utilized. The CW Doppler system employs LZT piezoelectric 10 MHZ transmitting and receiving crystals held in a lightweight polystyrene arterial transducer. The audio frequency, FM Doppler signal modulates an FM oscillator-transmitter to produce an FM/FM radio frequency carrier which is transmitted to an FM receiver at a remote location. The transmitted Doppler audio flow signal is demodulated at the receiver by a zero crossing detector. Power consumption is 48 mw. The 12.4 cc package volume can be further reduced for animal implantation.
