[Drugs--of molecules and relationships].

Drugs have always been both medicine and poison. Their function is cure but they can also kill; they have desired but also adverse effects. Drugs can be defined as technical objects which use is inserted within a set of representations, practices and social relationships. Acting as a mediator--and sometimes as an issue--in the relationship, medication and its use are far from the rational management of drugs as defined by public health authorities. The matter at stake can also be the legitimation of illness or pain. As a means of legitimation, psychotropic drugs raise specific questions as there is no objectivation of symptoms, be them mood changes or pain, aside from the patient's account.

Vaginal vault prolapse--a case report.

Vaginal vault prolapse after hysterectomy is a distressing complication for both the patient and the surgeon. Successful repair of post-hysterectomy vault prolapse is one of the most difficult problems in gynaecological surgery. The aim of the surgery should be to restore coital function and cure the prolapse permanently.

Diaphragmatic resection in association with right hepatectomy.

Ten cases of combined diaphragmatic and hepatic resection for tumours involving both structures are described. A complete work-up, including ultrasonography and computed tomography, usually can predict potential direct spread to the diaphragm. Up to 50% of the diaphragm can be excised and reconstructed without the need for prosthetic mesh or tissue transfer. Diaphragmatic resection does not appear to cause long-term postoperative morbidity. Diaphragmatic invasion by primary or secondary tumours does not preclude resection for cure. Pulmonary function studies are not necessary if there is no serious pre-existing lung disease.

The concept of nursing. Part II.

Nursing is therefore, a profession worthy of respect, that which has a unique function of curing and assisting individuals, families and community:- sick or well to reach whole completeness or independency. It is the key and work-force of the health team, whose main aim is four fold preventive, promotive and restoration of health, and alleviation of suffering, to all regardless of sex, age, tribe.

Evaluation of humoral and cellular immunity in patients with urinary tract infections.

The cellular and humoral immunity of 15 patients with recurrent urinary tract infections was assessed during infection. We determined the reactivity of peripheral blood lymphocytes to microbial antigens (PPD, streptolysin O, tetanus toxoid, Salmonella typhi and Escherichia coli), to allogenic cells, and to mitogens (phytohemagglutinin, concanavalin A, pokeweed mitogen and jacalin). T and B cells were counted by the rosette technique. We also evaluated serum immunoglobulins and C3 and C4 levels by radial immunoprecipitation and specific antibodies for the etiological agent by direct agglutination, hemagglutination and indirect immunofluorescence. Urinary antibodies were assessed by the antibody-coated bacteria test (ACB). Some patients were retested for cellular immune function after cure. No significant alteration of immunoglobulin levels was detected, except for two patients who showed elevated IgM levels. C3 and C4 were elevated in one and in five cases, respectively. Specific serum antibodies for the etiological agent were present in all patients but their levels did not differ from those of normal controls. In only five patients were urinary antibodies detected by the ACB test. Patients' peripheral blood lymphocyte reactivity was depressed during infection for most of the microbial antigens and mitogens tested and returned to normal levels after cure. T and B cell numbers were within the normal range.

[Periodicity and rhythm in view of "functional relaxation"].

Starting from psychopathological and psychosomatic research, differences as well as interplay in periodicity and rhythm are worked out. While analytical psychotherapy deals with periodic-phasic disturbances, "functional relaxation" does so with rhythmical ones. Both processes in their dovetailing are made plain through case histories, in which periodic-phasic disturbances may also lead to rhythmical ones and vice versa. Treatment of bodily manifest disturbances of rhythm by "functional relaxation" cures or prevents periodically recurrent illness not only individually but does so with following generations too.

Therapeutic penetrating keratoplasty in nonhealing corneal ulcer.

Results of penetrating keratoplasty in 443 eyes of 439 patients of active infective corneal ulcers, from a period of 10 years, were analyzed. All these eyes were proven refractory to maximal medical therapy for extended periods of time, in some cases more than 6 weeks. As the surgery was performed during the active stage, the outcome was not favorable functionally. Clinical cure was obtained, however, by therapeutic penetrating keratoplasty in all but 27 of these eyes. Of unsuccessful cases, 16 could be saved with additional medical therapy, two led to phthisis bulbi, and nine required repeat therapeutic penetrating keratoplasty. Complications encountered included disturbances in intraocular dynamics such as extrusion of the lens and vitreous during surgery, secondary rise of intraocular pressure leading to damage of the optic nerve, loss of the eye due to above factors, and reinfection. The anatomical success achieved was 97% with graft clarity of 2+ or more in 39% of eyes. Useful vision, ie, better than or equal to 6/60, was possible in 33% of eyes. Thus, therapeutic keratoplasty in refractory corneal ulcer still has scope in a Third-World country.

[Motor disturbances in squint - pathogenesis and treatment (author's transl)].

Motor deviation in strabismus must be considered in the context of the different elements which determine it, that is, taking account of the nature of the deviation. Modifications in the muscle may on very rare occasions be due to a paretic origin, but they can also be of innervational origin or stem from muscular modifications secondary to the innervational deviation. To decide on operative procedure, it is absolutely essential to determine the aetiology of the deviation, for normal static and dynamic motility can only be achieved by an aetiological therapy. Since muscular disorders may be of various kinds, and given that in many cases, the deviation can be cured by functional treatment alone, the author does not feel that the precise measurement of the deviation by an instrument such as the synoptometer is indispensable, the best assurance of a post-operative oculomotor balance being the suppression of abnormal accommodation and the normalisation of the retinal correspondence before the surgical intervention. This affirmation is supported by the clinical and experimental data presented in this paper.

Proportion cured and mean log survival time as functions of tumour size.

We obtained maximum likelihood estimates (MLEs) of the proportion cured pi c and mean log survival time mu t for a sample of 4355 patients with intraocular melanoma whose survival times subsequent to treatment were assumed to follow a log-normal distribution. Following stratification by tumour size, MLEs of pi c and mu t derived for each stratum correlate inversely with tumour size. We then expressed pi c and mu t as continuous functions of tumour size and calculated MLEs for the parameters of these functions from the entire sample. This investigation documents for ocular melanoma a significant relationship of tumour size to both cured fraction and mean log survival time.

Lung function of Zimbabwean farm workers exposed to flue curing and stacking of tobacco leaves.

OBJECTIVES: To perform lung function tests on tobacco farm workers (TFWs) chronically exposed to flue curing and stacking of tobacco leaves and to compare them with vegetable farm workers (VFWs) who were not exposed to any known air pollutant (control). DESIGN: Comparative study. SETTING: Tobacco and vegetable farms. SUBJECTS: 20 TFWs and 30 VFWs. All subjects were male and the mean age, height and weight of the two groups were not significantly different. OUTCOME MEASURES: Lung function indices. RESULTS: Forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1) and peak expiratory flow rate (PEFR) of the TFWs were 3.28 +/- 0.51 litres, 2.68 +/- 0.74 litres and 6.41 +/- 2.08 litres/second, respectively. These figures were significantly lower than 3.97 +/- 0.83 litres, 3.09 +/- 0.71 litres and 8.62 +/- 2.74 litres/second, respectively, for the control subjects (P < 0.01, 0.05 and 0.01, respectively). However, mean FEV1 as a percentage of the FVC (FEV1%) of the TFWs was not significantly different from that of the controls FVC of the TFWs declined with duration of service (r = 0.74; P < 0.01). CONCLUSION: The results are indicative of restrictive lung defect in the TFWs and may be attributed to long-term exposure to flue curing and stacking of tobacco leaves. The results also suggest the importance of the duration of exposure in the aetiology of lung impairment in this environment.

Diminished ovarian reserve in female childhood cancer survivors with regular menstrual cycles and basal FSH <10 IU/l.

BACKGROUND: Approximately 70% of children treated for cancer are cured, however the treatment often impairs reproductive function. The aim of the present study was to assess ovarian reserve in women with an apparently normal ovarian function, who were cured of cancer in childhood. METHODS: Twenty-one female survivors with regular menstrual cycles and basal FSH <10 IU/l were included. The control group included 21 healthy age-matched regularly cycling women. On cycle day 2-5, ovarian volume and number of small antral follicles (2-5 mm) were evaluated with transvaginal ultrasonography. Repeated sonography, urinary LH testing, and endocrine assessments were performed during the actual cycle. Cycle length was calculated. RESULTS: The female survivors had significantly smaller volume per ovary (4.9 versus 6.8 cm(3); P < 0.01), a lower number of small antral follicles per ovary (4.5 versus 8.0; P < 0.01), and lower total number of follicles per ovary (8 versus 11, P < 0.01). Hormonal profiles were similar, but the mean cycle length of the female survivors was significantly shorter than in the control group (28.3 versus 31.0 days; P < 0.05). CONCLUSIONS: Childhood cancer survivors with regular menstrual cycles and basal FSH <10 IU/l seem to have a diminished ovarian reserve. Consequently, they may have a shortened reproductive span and an early menopause.

Squamous carcinoma of the head and neck: cured fraction and median survival time as functions of age, sex, histologic type, and node status.

The multivariate lognormal survival model can be used to determine the relationship of prognostic covariates to two important parameters of malignancy. Cured fraction and median survival time among uncured patients. Analysis with this model revealed that cured fraction is primarily a function of histologic type and node status, while median survival time is primarily a function of age and node status. Patient sex was also related to likelihood of cure, but this association was of marginal significance. The symmetric impact of node status on both cured fraction and median survival time is consistent with known biologic principles. The strongly asymmetric relationships of histologic grade to cured fraction and age to survival time suggest, however, that likelihood of cure and survival time may not operate by identical biologic mechanisms.

Late immune and haemopoietic functions in plasmacytoma-bearing mice cured by melphalan.

Alkylating agents can cause latent and permanent damage to the bone marrow. We compared the long term effects of melphalan on a number of immune and haemopoietic functions of plasmacytoma bearing BALB/c mice with that of normal mice treated with a similar dose of melphalan. The drug administered orally at a dose of 250 micrograms and 400 micrograms on day 14 and 24 following i.m. inoculation of MOPC-315 plasmacytoma cells resulted in cure of the mice. Their spleen cells showed a permanent impairment of MLR activity, T-cell number and IL-2 production as well as a mild suppression of NK activity for one year after cessation of melphalan therapy. The number of B cells was elevated. In contrast, plasmacytoma-free mice treated with melphalan retained long term normal immune functions, although shortly after melphalan therapy a temporary suppression was noted. On the other hand, melphalan was responsible for bone marrow myeloid stem cell damage since the number of myeloid progenitor cell (CFU-GM) colonies was reduced in both melphalan-treated groups compared to untreated normal controls. Plasmacytoma bearing mice had a shorter survival. These results demonstrate that some late sequelae of alkylating agents are not due to the drug alone; shorter survival and T-cell deficiency are related to the previous presence of the tumour.
