This paper presents a preliminary study on the return to the workforce of employees with WMSDs of the upper limbs, and their reallocation to jobs with 'low exposure'. The study, which is still underway, involves a large engineering firm and includes some 100 workers affected by WMSDs. The trial involved: providing a definition of the criteria for characterizing 'accommodating' jobs (i.e. frequency of action < 20 actions/min; virtual absence of other risk factors such as force, awkward posture, inadequate pauses, etc.); effectively identifying jobs meeting such criteria (or jobs which, with minimal modifications, could be made suitable); classifying WMSD workers according to the type and severity of the disorder; matching WMSD workers with the jobs best suited to them; specific training for the workers and their supervisors; carrying out a follow-up of the return of WMSD workers to the workforce in organizational terms (i.e. the need for further modifications to equipment or procedures) and clinical terms (i.e. symptom patterns, acceptability of the condition). The preliminary results, 6-12 months after the start of the trial, are extremely encouraging, and show that when workers return to the workforce in jobs that fully meet defined criteria, a significant prevalence of 'improvements' are reported among the workers involved. The investigation will need to be extended but it already shows quite convincingly that it is possible for workers with what can be described as a 'reduced working capacity' to remain 'productive' (albeit in jobs featuring a lower exposure potential than the acceptable threshold for 'healthy' workers).
Psychoanalytic reconstruction has declined in theoretical and clinical interest as greater attention has been directed to the here and now of the transference-counter-transference field and inter-subjectivity. Transference, however, is based upon childhood fantasy, and is a new edition of unconscious intra-psychic representation and relationships. In this paper transference is viewed as a guide to reconstruction, but transference itself is also an object of reconstruction. Reconstruction is a complementary agent of change, which integrates genetic interpretations and restores the continuity of the self The patient's childish traits, features, fixations, and irrational childish fantasies and behavior point to the necessity for reconstruction. Reconstruction organizes dissociated, fragmented memories, potentiating the further retrieval of repressed memories. Reconstruction is essential to the working through and attenuation of early traumatic experience. Recapture of the past is necessary to demonstrate and diminish the persistent influence of the past in the present, and to meaningfully connect past and present. A case is presented in which reconstruction had a central, vital role in the analytic process.
Efforts to establish a minimal data set for the period of community integration after traumatic brain injury (TBI) have been the focus of a working group of leading researchers, clinicians and advocates attending the Galveston Brain Injury Symposium in 2001 and 2002. The purpose of the data set is to increase the consistency and interpretability of TBI research and programme evaluation by providing a common set of measures to document key aspects of community integration. It is intended to be sensitive to intervention, comprehensive, reflect satisfaction and outcome and promote future treatment/therapy. In deciding on the components of a minimal data set, the tri-partite model of outcomes was adopted (initially employed in psychotherapy, adapted to TBI) which focuses on the needs of the individual, society and the health provider. Scales are recommended for the respective components of the model.
PURPOSE: This article examines the effectiveness of differentiated rehabilitation programmes for patients with two distinct types of hemispatial neglect: body-centred and object-focused. We hypothesized that patients with body-centred neglect would respond to motor-control programmes designed for patients with limb apraxia, while those with object-focused neglect would require visually oriented therapy. MATERIAL AND METHODS: The article describes the rehabilitation of two patients treated by the authors 6-9 months after right-hemisphere infarct. Both showed significant left-sided hemispatial neglect: body-centred in one case (patient BC), object-focused in the other (patient OF). A modified AB-BA experimental design was used, where A represents visual training, and B is spatio-motor training. For patient BC, the sequence was A-B; for patient OF, B-A. Neglect was measured using standard tests for neglect and the analysis of drawings made during therapy by both patients. RESULTS: As hypothesized, patient BC showed no progress after conclusion of programme A, while after programme B she showed virtually no lingering neglect. In the case of patient OF, the results were exactly reversed. CONCLUSIONS: Body-centred neglect is essentially a defect in space formation, while object-focused neglect is a disorder of object formation. The patients described here both benefited from neuropsychological therapy for neglect, but not from the same programme.
PURPOSE: The purpose of this review is to summarize the literature related to the advances that have taken place in the management and rehabilitation care of limb amputation. RESULTS: Prostheses for the lower and upper limb amputee have changed greatly over the past several years, with advances in components, socket fabrication and fitting techniques, suspension systems and sources of power and electronic controls. Higher levels of limb amputation can now be fitted with functional prostheses, which allow more patients to achieve independent life styles. This is of particular importance for the multi-limb amputee. The rehabilitation of more traditional lower limb levels of amputation have also greatly benefited from the technological advances including energy storing feet, electronic control hydraulic knees, ankle rotators and shock absorbers to mention a few. For the upper limb amputee, myoelectric and proportional controlled terminal devices and elbow joints are now used routinely in some rehabilitation facilities. Experimental prosthetic fitting techniques and devices such as the use of osseo-implantation for suspension of the prosthesis, tension control hands or electromagnetic fluids for knee movement control will also be briefly discussed in this paper. CONCLUSION: It is possible to conclude from this review that many advances have occurred that have greatly impacted the functional outcomes of patients with limb amputation.
Functions of the prefrontal cortex (PFC) are fundamental to learning and rehabilitation after brain injuries, but the PFC is particularly vulnerable to trauma. We propose approaches to cognitive training that are hypothesized to specifically enhance PFC function. We present a theoretical framework that generates hypotheses regarding the effects of training on the functional integration of processes across distributed networks of brain regions. Specific outcome measurements that may be used to test these hypotheses in clinical trials are proposed. This neural network-level approach may guide cognitive rehabilitation and facilitate development of adjunctive biologic treatments to enhance the effects of training.
Substantial reductions in the in-patient census of state hospitals throughout the United States have led to the concentration of large numbers of formerly mentally ill individuals in sheltered living arrangements in local communities. These efforts represent a major change in providing care for the long-term chronic patient. How is the life of this individual affected by community placement? A survey was completed of 499 residents in 234 facilities representing all formerly hospitalized non-retarded mental patients between 18 and 65 years old in California's sheltered care facilities. Results indicate that the social functioning of individuals in the external community is enhanced primarily by the characteristics of the community in which they are placed. In contrast, the social functioning of individuals within the immediate environment of their sheltered living arrangements is most affected by the character of the placement itself. The particular characteristics of communities and facilities found to be most important in addition to the characteristics of the residents which impact on social integration are discussed in the study.

To determine the long-term psychosocial consequences of cancer related to work, a postal survey was conducted among 849 long-term survivors of cancer in the southwest Netherlands. Forty-four percent of the responders who worked at the time of the diagnosis of cancer returned to their job, 24% of them part-time. Fourteen percent experienced impediments at work after return. Absenteeism in this group does not differ from that in the year prior to the moment of diagnosis. There is a small decrease in promotional and financial prospects.
Extrachromosomal human T cell leukaemia virus type 1 (HTLV-I) proviruses are persistently maintained in HTLV-I-infected human promyelocytic leukaemia (HL60) cells even 24 months after viral infection. By successive recloning of these HTLV-I-infected clones, and by Southern blot analysis of their HTLV-I proviruses, we concluded the following. The copy number of extrachromosomal proviruses fluctuated, and this fluctuation was probably dependent on the epigenetic conditions in the host cell, HL60. The transient appearance of a high copy number of extrachromosomal proviruses was followed by an increase in the copy number of integrated proviruses. Persistence of extrachromosomal proviruses appeared to be caused by an intracellular rather than an intercellular mechanism.
In a depth process the unconscious presents images of destruction. In my analytic work with a female Mexican artist, the themes of destruction and creation alive in her psyche echo the motifs of the pre-Hispanic myth of Coyolxauhqui. I will illustrate the mythological amplifications of the patient's personal material with illustrations of her paintings. I will also discuss how this myth operates in a male patient, and between the masculine and feminine in Mexican culture. Similar images by the German artist Anselm Kiefer refer to the dismembering of the newly discovered lands and government violence against the youth of Mexico.
