[Restoring the occlusion by direct techniques].

Today, extensive and generalized tooth wear is often a reason for restoring the occlusion. Adhesive techniques and direct composite restorations can reduce the need for more expensive indirect restorations. In this article the advantages and disadvantages of the direct and indirect techniques are discussed. Furthermore, the treatment sequence for direct restorations is described and illustrated by some case-reports.

[The direct composite crown].

The direct composite crown is a restoration replacing the original toothcrown in form and function. It can be an alternative for indirect gold or porcelain restorations if the oral health condition is not stable, if indirect restorations require a high biological price or if financial resources are limited. The longterm durability of these restorations is still unknown. As a direct composite crown can be considered as minimally invasive and can be easily replaced by an indirect restoration if needed, there are hardly any contra-indications for its use.

[The restoration of endodontically treated teeth: the minimally invasive (post-) core buildup?].

The rebuilding of endodontic treated teeth is a common situation in general dental practice. The final (post-) core buildup, with or without a crown, should protect the restoration as well as the tooth against fracture, should prevent debonding of the restoration, should not permit leakage and should provide an acceptable esthetic result. Several factors influencing these properties are described. The (post-) core buildup in relation to the concept of minimally invasive dentistry is discussed. In conclusion, there is no unambiguous guideline for restoring endodontically treated teeth. Standardised in vitro and in vivo studies are needed.

A comparison of all-ceramic restorative systems.

The list of manufacturers offering pressable systems continues to grow. Most new materials have similar compositions and properties. Recently, the area of pressable restorations has gone in a new direction with the introduction of "press to metal" technology. This approach is designed to take advantage of metal reinforcement and easily provide a reproducible all-porcelain margin to enhance esthetics and eliminate the "black line" from a metal margin.

Putting it all together: my journey in dentistry.

I have grown and evolved as a clinical dentist primarily because I have been exposed to other dentists who have superior skills. It has been my choice to do this, and it has been very satisfying and professionally stimulating. Unfortunately, marketing hype and not understanding the total picture is influencing many of our younger, inexperienced dentists. The purpose of this lecture/paper was to help our younger colleagues realize some of the pitfalls in our profession and guide them in a direction where they may also see the benefits and rewards of our great profession.

Direct adhesive restoration of anterior teeth. Part 1. Fundamentals of excellence.

Contemporary composite resin materials have significantly evolved, facilitating the delivery of aesthetic and functional restorations. The predictability of this treatment modality has been significantly improved by the introduction of adhesive protocols. This three-part series proposes a simplified protocol and the fundamentals for minimally invasive, aesthetic, direct adhesive restorations of anterior teeth. The first part of this series offers an overview of the state-of-the-art relating to composite restorations, both in situations deemed to be relatively uncomplicated and those that are more challenging.

Pediatric dental education and community service: a combined approach.

Dental educators nationwide have expressed concern regarding the decreasing pediatric clinical experience available to undergraduate dental students. Some educators have suggested that dental programs should utilize extramural clinics and rotations to enhance current patient pools. This paper presents a successful clinical program that is designed to 1) augment the dental education of predoctoral dental students, and 2) provide dental care for an underserved pediatric dental population in an urban community.

The composite resin-amalgam window preparation.

Amalgam restorations with a very visible mesiofacial aspect can be aesthetically modified. Clinical experience has proven the amalgam window preparation to be an economical, expedient, and effective treatment approach when patients request tooth-colored fillings in posterior maxillary teeth.
