[Ultrasound-guided percutaneous fine-needle puncture cytology in the diagnosis of malignant tumors of the liver].

Results of ultrasound guided percutaneous fine-needle puncture cytology in 142 cases of malignant tumors of the liver are reported. The positive cytology was noted in 118 (83.1%) (10 suspicious). The primary liver cancer comprised 99 cases. In 7 lesions, equal to or less than 3 cm in diameters, 6 were positive in cytology. In 20,3-5 cm in size, 18 were positive. In 15 false-negatives, 12 were larger than 5 cm in diameter. Among 43 cases of metastatic liver cancer, 34 showed positive cytology and 9 false-negative. Among 108 cases of benign hepatic diseases, in 49.1%, it was difficult to arrive at definitive diagnosis by ultrasonography only, but in 97.2% malignancy was excluded by ultrasound guided fine-needle puncture cytology. The suspicious false-positive result occurred only in 3 cases. In this series, there were 250 cases of malignant tumors and benign diseases. The overall accurate diagnostic rate was 89.2%. All the patients had been followed for more than 6 months. The differential diagnosis between malignant and benign tumors, causes of misdiagnosis and complications are discussed.

[Ultrasonically guided puncture of endometrial cysts--aspiration of contents and infusion of ethanol].

Ultrasonically guided transvaginal or transabdominal puncture of an endometrial cyst with a transvaginal or transabdominal sector probe was done safely and correctly. In 7 endometrial cysts in 5 patients, we aspirated the contents, flushed with saline solution and cleaned the interior cysts. At last 99.9% ethanol was infused into 6 cysts for 30 minutes. In all cases puncture of the endometrial cyst was successful and the contents of the endometrial cyst were collected. There were no major side effect and no recurrence of endometrial cyst. It was proven that the aspiration of contents and infusion of ethanol were superior methods for treating endometrial cysts.

[Ultrasound-guided cyst puncture of the human breast].

For mammadiagnosis sonography should be used only carefully directed as a supplement investigation. Excellent results in diagnosis, treatment and follow up examinations are obtainable, if sonography is employed together with mammography and puncture-cytology as triple-diagnosis. With the help of sonography localisation and puncture of not palpable cysts is possible. Together with pneumocystography the diagnosis is reliable, biopsies can be reduced. Follow up examinations must be done periodical. The employment of sonography reduces the numbers of radiographs. In a scheme our procedure is clearly arranged.

[Ultrasound-controlled percutaneous puncture of pancreatic pseudocysts].

The puncture of pancreatic pseudocysts with the help of ultrasound is an attempt of treatment without putting too much strain on the patient. In a number of cases a definitive emptying of cysts is successful so that a surgical intervention with application of an interior drainage can be avoided. Of 4 male patients of medium age treated 2 were successfully to be treated. The casuistics of one of the patients is reported. Under the point of view that a sonographically assisted intervention increases the security of the puncture therapy the complex of problems is outlined.

[Supraclavicular puncture of the anonymous vein].

The investigation concerns a series of percutaneous, supraclavicular punctures and catheterisations of the anonymous veins conducted in the course of 8 years. 9042 patients were involved. The specific advantages of the anonymous vein puncture and catheterisation pertain to the easy accessibility of the vessel in all circulatory conditions and to the reduced risk of complications (rate of pneumothorax only 0.2%).

Brachiocephalic Trunk↗

[Puncture of the brachiocephalic vein].

A specific central vein catheter for puncture of the brachiocephalic vein has been developed which is provided with a valve by which air-embolism and unwanted bleeding from the catheter are eliminated. Typical and often serious complications, which can develop on insertion of central vein catheter, can nearly be totally avoided by the puncture technique described.

[Comparison of bacteriologic urine studies following bladder puncture and bladder catheterization].

In 108 female patients with history of urinary tract infections bacteriologic work-up was done in urine taken by catheterisation. 85% of the urine samples were sterile, 15% had bacterial counts less than 10(4) ml. The incidence of contaminated urines was the same when the urine was controlled one and three weeks later again by catheterisation. When urine samples were taken by suprapubic bladder puncture at the time when the catheterisation was done, a significant higher percentage of the punctured urine was contaminated. The results were discussed in terms of the interpretation of bacteriologic data obtained by suprapubic bladder punction.

Transovale trigeminal cistern puncture: modified fluoroscopically guided technique.

A simple, safe, and rapid radiologic technique is described for trigeminal puncture via the foramen ovale. A radiopaque marker is placed 3 cm lateral to the labial commissure. Then, under fluoroscopic guidance, the patient's head is turned until the marker is projected vertically, over the foramen ovale. The needle can be advanced along the anatomically safe line determined by the foramen ovale and the lateral labial puncture site. Needle or patient misalignment is detected readily and can be corrected using fluoroscopic guidance. The technique is learned easily, obviates multiple needle passes, and takes less than 5 min.

[Percutaneous puncture of the pancreas controlled by computerized tomography].

The method of puncture biopsy under the control of computer tomography was used in 29 patients with diseases of the pancrease. Correct diagnosis was obtained in 27 patients. In 13 patients aimed biopsy was associated with the aspiration of cysts (in 7 patients) and drainage (in 6 patients). No complications of punctures of the pancreas under the control of computer tomography were noted.

Speech rehabilitation outcome after Blom-Singer tracheoesophageal puncture.

This study evaluated speech rehabilitation outcome and length of training needed for laryngectomized patients who underwent the Blom-Singer tracheoesophageal (TE) puncture procedure. Preoperative patient selection criteria included: 1) acceptable stoma size, 2) adequate motivation and manual dexterity, 3) absence of constrictor spasm. Training focused on coordination of breath control, articulation, muscle relaxation, and proper handling and maintenance of the "duckbill" prosthesis. Speech intelligibility in 12 patients was evaluated following the completion of their speech rehabilitation program, using the CID Everyday Sentences. Each patient was videotaped; the tape then was presented to unbiased listeners who recorded the sentences. The median percentage of intelligibility for the 12 patients was 89.5%; only one patient had a median score less than 60%. The mean length of formal training for the group was only 3.2 hours (range 1 to 7 hours). The percentage of patients attaining speech and the quality of their speech intelligibility was found to be higher than with esophageal speech, which is both time consuming and often difficult to learn. TE puncture followed by proper fitting and training in voice prosthesis usage improves speech rehabilitation outcome for the laryngectomized patient.

[Technique and use of fine needle puncture for thyroid gland (author's transl)].

Fine needle puncture of cold and warm thyroid gland nodes has its definite place in the diagnostic repertory of thyroid diseases. This method can be performed rapidly and easily. It has a low complication rate and imparts to the physician a maximum of information in association with in-vitro tests and scintigram. Indication and technique of fine needle puncture are described and discussed.

Puncture wounds of the foot.

Most puncture wounds of the foot heal satisfactorily even without treatment. Nonetheless, serious complications such as osteomyelitis can occur. Early cleansing and debridement are important in preventing complications. Penetration of cartilaginous areas is a common prelude to osteomyelitis. Puncture wounds can serve as a reminder to both patients and physicians to update tetanus immunizations.

[Blind liver puncture or laparoscopy?].

The discussions as to the priority of laparoscopy or blind liver puncture have not ended in a "either-or" decision, but, rather, have come out unequivocally in favour of both techniques. The procedure of blind liver puncture is suitable for the diagnosis and follow-up of diffuse diseases of the liver (fatty liver, acute hepatitis, with restriction, chronic persistent and chronic aggressive hepatitis, pre-alcoholic liver cirrhois damage). Laparoscopy is employed primarily in cases with equivocal liver palpation findings, for the initial diagnosis of chronic inflammatory, focal and tumorous diseases of the liver. For both techniques, there needs to be a careful weighing up of indications and contraindications, risks and limitations and they should not be carried out by any and every ward physician.

[Experiences with the Blom-Singer prosthesis following Blom-Singer puncture and following functionally disordered neoglottis phonatria].

The Blom-Singer puncture is a new operative technique which can help to make the laryngectomized patient speak again even years after laryngectomy. 34 patients were treated with a modified procedure. 5 patients with a good voice gave up despite intensive medical and speech therapy. Painstaking postoperative treatment is needed to make the Blom-Singer puncture successful. Patients who had a Blom-Singer prosthesis fitted after a neoglottis procedure spoke after only brief instruction without speech therapy being necessary.

[The Mancao cannula--a new system for the facilitation of peridural puncture].

A cannula system for epidural puncture is presented, that has been developed in order to prevent the risk of dural puncture and nerve damage and allows the epidural catheter to be withdrawn without the risk of tearing or cutting off the end. The cannula system ("Mancao-cannula") consists of a sharp outer and a blunt inner needle with a lateral aperture. First experiences in 400 epidural anaesthesias using the Mancao cannula are reported.

[Initial results of a combined sonographic and cytologic diagnosis of scintigraphically remarkable thyroid changes using real-time visual puncture].

The combination of sonography and biopsy in the diagnostics of the thyroid gland results in a reliability of 100%. Thus the routine application effective diagnostics and therapy is justified. The course control after emptying of the cyst is simplified, the therapeutic effect of the puncture can be revealed. It can be attracted as evident method which is poor in risk for the confirmation of an indication to operation. This form of examination is possible also then, when no sonographic device with a puncture is at our disposal.

[Pericardiac puncture. The most favorable anatomical approach (author's transl)].

On the basis of pericardiac punctures in 105 prepared patients and of pericardiac preparation in 92 adults, a simple low-risk method of epigastric pericardiac puncture is described. The needle is pushed at 33 degrees to the front level in about 1/2 cm below the left costoxiphoid angle and directed towards the right acromion. At a depth of about 4.5 cm the needle penetrates the pericardium. The left internal thoracic artery, the pericardial vessels, the arteria marginalis dextra and the anterior interventricular artery are usually more than 2 cm distant from the needle. When the pericardial infusion is small, the tip of the needle may come relatively close to the right coronary artery (1.3 +/- 1 cm).
