Gluten enteropathy occurring in chronic hepatitis.

Steatorrhoea and subtotal villus atrophy due to gluten enteropathy was found to occur in a 27 year old female, who had histologically proven chronic active hepatitis six years ago. She had abnormal elevation of immunoglobulins IgG, IgM, and IgA, decreased level of complement C4 and a positive test for HLA-B8. The SGOT was persistently mildly elevated, but her latest liver biopsy (6 years after onset) showed only changes of chronic persistent hepatitis. She had good symptomatic and histological (jejunal biopsy) improvement with gluten-free diet.

Immune complexes in polyarthritis after Salmonella gastroenteritis.

In a case of polyarthritis after salmonella gastroenteritis the levels of circulating immune complexes (CIC) (C1q-binding assay) correlated closely with disease activity. This observation suggests that systematically absorbed salmonella antigens may lead or contribute to the formation of CIC. CIC activate both the classical and alternate complement pathway and may be important in the pathogenesis of the arthritis associated with salmonella infection.

[Testing a cellulose diacetate membrane for plasma exchange (author's transl)].

A cellulose diacetate membrane was tested in 60 plasma exchanges (PE) carried out in 16 patients. Access to the vascular compartment was obtained through one single puncture by inserting a catheter into the femoral vein, the blood circuit being provided with an alternating clamp. The exchange of 2 plasma volumes (72.3 +/- 16 ml/kg) reduced IgG, IgA and IgM concentrations to 30.9 +/- 12.1%, 31.6 +/- 14.5% and 28.3 %/- 15.0% respectively of pre-PE concentrations. Similarly, the C3 and C4 complement fractions and factor B were reduced to 32 +/- 12%, 33 +/- 15% and 36 +/- 16% respectively of pre-PE values. The membrane was used several times in 7 patients; its performance remained the same after 3 consecutive uses. In view of its effectiveness and ease of application, the cellulose diacetate membrane appears to be particularly suitable for plasma exchanges in intensive care units.

Serum complement levels in patients with cancer of the uterine cervix before and after radiation therapy.

With single radial immunodiffusion method the level of the third component of the complement was investigated in 153 cervix cancer patients. In some cases the level of the fourth component of the complement and total hemolytic activity of the complement were parallely determined. The C'3 values elicited a polarized distribution pattern in contrast to the normal distribution of the controls. Mean C'3 level increased simultaneously with the progress of the disease up to Stage III, and decreased only in Stage IV. On the contrary, C'4 level was stable and practically equaled the normal value. Total hemolytic activity of the complement in cervix cancer patients usually exceed normal values. The C'3 changes during radiation therapy contradicted its role as an acute phase protein.

Circadian and circannual aspects of the complement cascade - new and old results, differing in specificity.

Single cosinor analyses of several elements in the complement cascade suggest the occurrence of circadian rhythms in this immunochemical system. Some earlier results, obtained with less specific methods and reported as properdin, on reanalysis also reveal circadian and circannual rhythms in the aspects of immunology represented. The topic of the complement cascade as a whole is briefly reviewed in the content of modern knowledge.

[The effect of hemosorption on the glycoprotein concentration of the blood serum in inflammatory diseases of the large intestine].

71 patients with nonspecific diseases of the colon were examined before and after hemosorption for concentrations of seromucoid, orosomucoid, immunoglobulins G,M,A, C3 and C4 complement component fractions, haptoglobin, ceruloplasmin, alpha 1-antitrypsin, C-reactive protein, properdin, transferrin. It was found that hemosorption results in lowering of all the above concentrations in nonspecific ulcerative colitis, except seromucoid and orosomucoid in moderate and acute colitis. In response to hemosorption serum glycoproteins levels varied with the disease severity, its duration and metabolic activity of the liver. Concentrations of seromucoids, orosomucoid, alpha 1-antitrypsin changed in correlation with clinical manifestations of colon inflammation and may serve as criteria of hemosorption efficacy.
