[Value of laboratory diagnosis of liver function from the insurance medicine viewpoint].

After a brief passage on the reliability and special features of the most important liver values, this paper deals with the diseases of the liver most frequent in everyday rating practice from the angle of specific laboratory patterns and enzyme processes, putting emphasis on prognostic aspects. Special mention is made of the most recent diagnostic findings in the field of virus hepatitis. Furthermore, the influence of alcohol on the liver values is explained in detail. In a résumé, GPT, Gamma-GT and cholinesterase are regarded as sufficient for classifying almost 98% of all diseases of the liver, the limited significance of liver values as "functional parameters" being stressed, however.

[Rational use of measurements and observations in clinical laboratory diagnosis].

Clinical chemistry possesses sufficient methodologies to ensure a fast transition between research and potential usefulness of measurements to be used in diagnosis. This is the case with future methods, such as DNA and RNA assays. The major challenge for the future is to develop methods that can ensure clinical performance and clinical efficacy of measurements potentially useful for the diagnosis, so as to select the still useful routine methods and refuse from the irrational ones.

Laboratory diagnosis of variant Creutzfeldt-Jakob disease.

The neuropathological and biochemical features of 33 cases of variant Creutzfeldt-Jakob disease (vCJD) diagnosed up to the end of 1998 are analysed in relation to the 646 cases of suspected CJD referred to the CJD Surveillance Unit laboratory from 1990 to 1998. Morphological studies of the central nervous system, lymphoid tissues and other organs were accompanied by immunocytochemistry; Western blot analysis of PrPRES was performed on frozen brain tissue. The findings were analysed in relation to clinical and genetic data. The pathology of vCJD showed morphological and immunocytochemical characteristics distinct from other cases of CJD. PrP accumulation was widespread in lymphoid tissues in vCJD, but was not identified in other non-neural tissues. PrPRES accumulation in vCJD brain tissue showed a uniform glycotype pattern distinct from sporadic CJD. All analysed cases of vCJD were methionine homozygotes at codon 129 of the PrP gene. No evidence currently exists to suggest that cases of CJD diagnosed in individuals who are MV or VV at codon 129 of the PrP gene represent 'human bovine spongiform encaphalopathy (BSE)'. Continued surveillance is required to further investigate this possibility, with the need to investigate autopsy tissues from suspected cases by histological and biochemical techniques.

[The early laboratory diagnosis of the pulmonary form of glanders and melioidosis by using rapid methods of immunochemical analysis].

The problems of the sanitary guarding of frontiers comprise such aspects as the early recognition of infectious diseases, even those which are considered exotic in the Russian Federation. These diseases include glanders and melioidosis, infections which are endemic for some frontier regions. The study performed on experimental models of the pulmonary form of glanders and melioidosis with the acute course of the diseases demonstrates that the use of such laboratory diagnostic methods as immunosorbent variants of the enzyme-linked, chemiluminescent and immunofluorescent immunoassays permits making reliable diagnosis as early as 3-12 hours after aspiration infection.

Laboratory diagnosis of dysfibrinogenemia.

Dysfibrinogenemia is a coagulation disorder caused by a variety of structural abnormalities in the fibrinogen molecule that result in abnormal fibrinogen function. It can be inherited or acquired. The inherited form is associated with increased risk of bleeding, thrombosis, or both in the same patient or family. Traditionally, dysfibrinogenemia is diagnosed by abnormal tests of fibrin clot formation; the thrombin time and reptilase time are the screening tests, and the fibrinogen clotting activity-antigen ratio is the confirmatory test. The inherited form is diagnosed by demonstrating similar laboratory test abnormalities in family members, and if necessary by analysis of the fibrinogen protein or fibrinogen genes in the patient. The acquired form is diagnosed by demonstrating abnormal liver function tests and by ruling out dysfibrinogenemia in family members. This article reviews the laboratory testing of dysfibrinogenemia and presents an algorithm for sequential test selection that can be used for diagnosis.

Contribution to the laboratory diagnosis of human cryptosporidiosis.

Human cryptosporidiosis is an infection caused by Cryptosporidium spp, a coccidial protozoan of emergencial pathogenicity and responsible for severe and prostrating watery diarrhea, mainly in immunocompromised patients. Smears of stools submitted to concentration and staining by carbol fuchsin technique has been used in our laboratory as a diagnostic procedure for cryptosporidiosis. The long time observing the smears in the microscope due to the small size of the forms and the low contrast of the staining led us to introduce some modifications in the original protocol for the acid-fast staining. The smears were treated with the carbol fuchsin solution for 3 minutes as recommended by LENNETTE et al., 1985 and the solution of the ethyl alcohol 70%-chloride acid 0.5% was used instead of the ethyl alcohol-sulfuric acid 5% recommended by HENRIKSEN & POHLENZ, 1981. Smears were treated with the discoloration solution for 2 minutes. These modifications promoted a better washing out of the excess of carbol fuchsin therefore increasing the dye efficiency. In such conditions, the visualization of protozoan oocysts on the slides examined became easier. They were observed by the contrast of their intense pink-reddish pigmentation against the blue background. It's worthwhile to emphasize that these modifications offer advantages when time and accuracy are concerned.
