Rupture of the posterior wall of the left ventricle is a rare but often fatal complication after mitral valve replacement. A method of repair is given that, although unsuccessful in its final outcome, may represent a worthwhile alternative surgical technique.

A case of autoamputation of the penis is presented. Although the unique vascular properties of the penis have permitted good results in a number of cases of amputation of the penis, it is certain that the use of microreimplantation gives better results with a minimum of postoperative complications.
This report regards a 55-year-old patient with an abdominally implanted, nonthoracotomy, cardioverter defibrillator system (Medtronic PCD 7217B) due to recurrent ventricular tachycardia and survived sudden cardiac death. Fifteen months after defibrillator implantation clinical signs of system infection occurred. The defibrillator and the two leads (Medtronic 6963 and 6966) were removed and treatment with intravenous antibiotic therapy was started. Thereafter the original resterilized ICD was re-implanted in a right subpectoral position with new right ventricular-and superior vena cava leads. This report shows that reuse of the relatively large Medtronic PCD 7217B with a new transvenous lead system in a retropectoral location is feasible and economically beneficial with regard to the high device cost.
Twenty five double kidney-pancreas transplantations were performed according to the total pancreas transplantation technique with drainage of exocrine secretions into the bladder via a vesicoduodenostomy. 72% of kidney-pancreas grafts were functional at one year and 59% were functional at four years. The authors observed a slightly higher rejection rate (0.56 versus 0.34) and a higher incidence of urinary tract infection (60% versus 35%) following double pancreas and renal transplantation than after isolated renal transplantation. Complications were rare: two venous thromboses and two cases of urethritis requiring of rediversion of the duodenum into the intestine. These good results, comparable to those reported in the international registry, reflect the value of the pancreatic and renal transplantation technique using a total pancreas drained into the bladder. It would probably be preferable to transplant patients earlier, when chronic renal failure secondary to insulin-dependent diabetes induces end-stage renal failure and the need for haemodialysis.

An original and reliable technique to culture growth plate chondrocytes was developed to obtain an abundant amount of mature and functional chondrocytes. Growth plates were provided from the epiphysis of 3 week old rabbits. The isolation of the chondrocytes was optimized by the use of trypsin and collagenase. The culture was realized according to the following conditions: seeding at 20,000 or 30,000/cm2 on type I collagen substrate and in Ham F12 medium without a supplementation of glucose or growth factors. After 7 days of culture, the implantation was to be carried out. Different implantation substrates were evaluated in vivo. Agar turned out to be the only substrate to provide strong and healthy chondrocytes 21 days after the grafting. In an other experimentation, the culture was implanted into surgically created defects in the growth plate area. In this case, the culture did not avoid the occurrence of an epiphysiodesis. However, the 6 weeks post operative histologic examination, showed that the implant remained viable, continued to maintain a proteoglycan rich matrix, and began to organize in ordered columns of mature chondrocytes.
