A patient presented an avulsion of left L5-S1 roots, with S1 meningocele, following a road accident. Saccoradiculography and CT scan imaging, even without contrast, allowed identification of total lesion in a suggestive clinical context. The characters of the CT scan images are emphasized. Images showed also the presence of pseudo-meningoceles and absence of roots.
The characteristics of evoked responses in pelvic nerve elicited with electrical stimulation of posterior, tuberal, anterior hypothalamus and preoptic area were studied in anesthetized and immobilized cats. The complex response in pelvic nerve consisted of waves I, II, III with latencies from 13 to 28, 52 to 96, 113 to 256 msec, resp. The evoked responses of lowest thresholds and shortest latencies were obtained from the posterior hypothalamus. These data suggest that hypothalamic parasympathoactivating neurons are localized mostly in the posterior hypothalamus. After high frequency (100/sec, 10-sec. period) stimulation of posterior and anterior hypothalamus which produced blood pressure increase, the mean component of pelvic discharge--wave III was depressed during the whole period of the pressure reaction, the inhibition being of baroreflex origin. Early components of the discharges after tetanic stimulation of hypothalamus were facilitated. Paired stimulation of the hypothalamus demonstrated that nearly 2.5 sec were required for complete recovery of the evoked test-responses in pelvic nerve.

The authors report 10 cases of pain in the outer surface of the hip related to damage of the perforating lateral branch of the subcostal or ilio-hypogastric nerve, which innervate this region. Anatomic investigation showed that there was a possibility of canal neuropathy, the possible mechanisms and signs of which are discussed. In general, treatment consisted of local infiltrations, but one patient underwent surgery with highly successful results.
Over an eight-year period, 50 presacral neurectomies were performed at Madigan Army Medical Center for chronic pelvic pain failing response to medical management. Each hospital record was reviewed and 45 patients answered a questionnaire grading the severity of their pain from 0 to 10 for dysmenorrhea, dyspareunia, and other pelvic pain before and after surgery. The results showed success rates of 73% in relieving dysmenorrhea, 77% in relieving dyspareunia, and 63% in relieving other pelvic pains. The addition of a bilateral uterosacral ligament resection to the presacral neurectomy did not increase the success rate. There was an 18% lateral pelvic pain recurrence rate, and no recurrence of dysmenorrhea. Complications occurred in 4%.
A new concept of anaesthesia in patients undergoing operations of the lumbar vertebral column is presented. Electrostimulation of four auricular acupuncture points is combined with controlled ventilation of nitrous-oxide oxygen mixture. This combination is comparable to similar anaesthesia procedures of other operative fields. In 30 patients the anaesthesia was performed as a "combined electrostimulationhypalgesia". In a control study of 30 similar patients a mixture of nitrous-oxide and oxygen only was used. For 120 min systolic and diastolic blood pressure as well as heart rate of both groups are measured and compared. The postoperative state as well as individual data from the patients are included in the investigation. The analgesia established by "combined electrostimulationhypalgesia" seems to be based essentially on the effect of nitrous-oxide as well as premedication, induction of anaesthesia, relaxation and kind of ventilation. Electrostimulation of auricular acupuncture points seems to induce stabilisation of the circulatory system. On account of our findings the relevance of electrostimulation in this method of anaesthesia must be judged very critically.
