General practitioner's care of the elderly: studies of aspects of workload.

This paper reports trends in the volume and nature of general practitioners' contacts with the elderly patients on their NHS lists in Dundee during the 1970s. It forms part of a larger project involving 32 general practitioners collaborating in a continuing study concerning the introduction of the first health centre in Dundee.During the decade contacts with the elderly increased as a proportion of the general practitioners' total work, broadly commensurate with the increase in Dundee's elderly population. Indirect contacts, mainly reissue of prescriptions for longterm medication, showed a greater proportional increase than direct consultations. Home visiting of the elderly remained relatively constant, while house calls to other age groups declined. Hospital referrals remained a small proportion of the general practitioners' work. Entry into a health centre was not associated with obvious changes in patterns of workload.If more might be done in planning and implementing changes in care of the elderly at general practice level, such changes should be the result of consensus among the professionals concerned, and should take account of the views of recipients of care.

Evaluation method of mental workload under flight conditions.

In order to analyze the relationship between heart rate variability (HRV) and the flight tasks of pilots, HRV of various mental and physical work stresses were investigated. For the physical stress on the treadmill, S.D. of heart rate sequence showed a minimum value at medium stresses and remained there until the maximum stress. However, the centrifuge acceleration test presupposed special physical stresses, and S.D. was almost same as the resting findings. In the tracking tasks, which presupposed maximum interpretative actions, the increase in the mean heart rate (MHR) was small with the S.D. increased a greater amount than for resting results. Consequently, preflight checks and level flight phases presupposed medium interpretative actions and mental stress with reserve capacity. Takeoff and landing phases were considered as both high interpretative action and high emotional stress situations. Acrobatic and gunnery training flight phases were considered physical load, strong interpretative actions, and emotional stress tasks.

General practice--a quantitative study, 1. Workload and morbidity variation.

A unique continuous 13-year morbidity database provides the foundation for a statistical analysis of general practice in an urban environment. Based on the Royal College of General Practitioners classification of morbidity this paper offers an objective assessment of both the level and variation in morbidity activity in general practice. Specification demographic characteristics of the individual patients are utilised to identify factors which can constrain general practice. Knowledge of such influences may be utilised in practice management and preventative medicine to provide a higher level of primary health care.
