[The characteristics of the endocrine function of the fetoplacental system in pregnant women with chronic pyelonephritis and nephropathy].

A follow-up study of 100 pregnant women with chronic pyelonephritis and superimposed nephropathy has shown that the major determinant of placental insufficiency is late toxemia of pregnancy, not the underlying disease. Estriol levels correlated with severity and duration of gestosis and thus provided the best predictor of fetoplacental insufficiency. Findings on placental lactogen production were less helpful since the production of this hormone was decreased only in severe nephropathy. The onset and progression of placental insufficiency were associated with rising progesterone concentrations in blood, which appeared to be a compensatory placental event.

[Surgical tactics in obstructive-suppurative calculous pyelonephritis in children].

An analysis of results of decompressive nephrostomy for obstructive purulent calculous pyelonephritis in 30 children has detected different complications aggravating the course of the disease and process of treatment. The authors propose a rational variant of operation--primary nephrolithotomia with nephrostomy. Operations were performed on 40 children. Immediate results of treatment were good in 30 children, satisfactory in 9 children. One patient died.

[10 cases of xanthogranulomatous pyelonephritis].

Xanthogranulomatous pyelonephritis (XGP) is a chronic inflammation of the kidney usually associated with renal stones, recurrent urinary tract infections or endocrine disorders. A correct preoperative diagnosis is rarely made, since no specific clinical or radiological pattern is known. Differential diagnosis must include renal tuberculosis and renal carcinoma. Ten cases of XGP are reported. Clinical findings, radiological and pathological features, biolaboratory abnormalities are discussed.

Pyelonephritis in cows: 15 cases (1982-1986).

Medical records of 15 adult cows with a final diagnosis of pyelonephritis were evaluated retrospectively. Only 3 cows had obvious clinical signs that indicated urinary tract disease. Physical examination, including rectal and vaginal examinations, and urine chemical reagent strip screening detected abnormalities in all 15 cows and allowed subsequent confirmation by culture results of urine samples obtained using a catheter. Bacteriologic cultures of urine revealed Corynebacterium renale infection in 6 cows and Escherichia coli infection in 9 cows. Long-term treatment with antimicrobial drugs resulted in recovery in 9 of 11 treated cows. Of these, 4 of 4 cows with C renale infection and 5 of 7 cows with E coli infection recovered; the remaining 2 cows with E coli infection died.

[Xanthogranulomatous pyelonephritis: apropos of 13 cases].

We present 13 patients anatomopathologically diagnosed with xantogranulomatose pyelonephritis. This form of inflammatory reaction may give rise to major pre-operative diagnostic difficulties through being confused with renal neoplasias. Clinical evolution consists of long-developing lithiasis complicated with infection. We regard echography and C.A.T. as extremely useful elements both in accurate pre-operative diagnosis and in the locoregional evaluation of the lesion. Treatment is surgical and the prognosis is favourable.

[Immunohistochemical, immunocytochemical and electron microscope studies in experimental E. coli pyelonephritis].

Authors have studied in apostematous pyelonephritis induced by the ligation of the ureter and the intravenous injection of E coli bacteria the localization and elimination with time of the pathogen. The pathogen was demonstrated by light and electron microscopy, its parietel antigen was localized with the light microscopic peroxidase antiperoxidase and post-embedding electron microscopic immunogold techniques. Two days after inoculation the suppurative inflammation of tubulo-interstitial foci was observed; in the capillaries, interstitium, and tubuli, free and phagocyted bacteria were encountered. In the interstitium, in the proximal tubuli and in the capillary space of some glomeruli bacterial groups were observed. Intracapillary bacteria were attached by their outer wall to the surface of endothelial cells. In the tubuli this adherence occurred with pili or with the outer layer of bacterial wall. From the seventh day after inoculation macrophages containing PAS-positive globuli appeared in the interstitium. Under the electron microscope these globuli proved to be features composed of myelin figures of phagolysosomal origin. Globuli and the myelin figures possessed an E. coli antigenicity. Thirteen weeks after inoculation E. coli antigen positivity was found in the cytoplasm of inflammatory cells in the tubular walls and in the suppurative cylinders, The organism was apparently unable to eliminate the materials derived from the pathogenic microorganisms.

[Acute lobar nephronia (focal bacterial pyelonephritis). Apropos of 3 cases in children].

We present three cases of acute lobar nephronia or acute focal bacterial nephritis on paediatric patients. Both radiological and echographic pictures are described, emphasizing those aspects which, together with a clinical symptomatology, allow a differential diagnose with abscess and neoplasias renally settled. We think that, among those patients suffering from pyelonephritis, an early practice of renal echography followed by further controls, allow an early detection of lobar nephronia cases and assessing a response to the treatment.

[A case of emphysematous pyelonephritis with septic shock recovered by transurethral drainage of pelvis].

A case of emphysematous pyelonephritis with septic shock was present in a 58-year-old diabetic woman. The spontaneous production of gas was present within the right renal pelvis in kidney-ureter-bladder X-ray. The patient's condition deteriorated rapidly after admission, became complicated with acute renal failure, disseminated intravascular coagulation and acute respiratory failure. Transurethral drainage of the pelvis using a 6 Fr. UPJ occlusion balloon catheter and endotracheal intubation with respiratory assistance were performed as a life-saving procedure. The optimal therapy with surgical or conservative approach for such a severe condition is discussed. The use of transurethral drainage of pelvis as a non-invasive treatment is suggested.

Virulence of Escherichia coli strains in relation to their hemolysin formation, mannose-resistant hemagglutination, hydroxamate production, K 1-antigen and the plasmid profile in patients with chronic pyelonephritis.

Microorganisms initiating infections of the kidneys and the upper urinary tract are characterized by specific properties. In 96 patients with non-obstructive chronic pyelonephritis, Escherichia coli strains were isolated and their virulence properties studied. Most frequently hemolysin formation and mannose-resistant hemagglutination (n = 39/40%), less frequently the ability to produce hydroxamate/aerobactin (n = 18/19%) and the K 1-antigen (n = 13/14%) were detected. In 34 E. coli strains (35%) two and more virulence factors were found. A statistically significant correlation exists between mannose-resistant hemagglutination (evidence of P- and F7-F14 fimbriae) and hemolysin formation on the one hand and hydroxamate production and K 1-antigen on the other (confidence intervals are 0.56-0.75 and 0.75-0.89, respectively). Disease activity and pathogenicity of E. coli strains showed a statistically significant association (chi 2-value, DF 1: hemolysin 29.5, mannose-resistant hemagglutination 17.1, K 1-antigen 4.8, hydroxamate 0.5). The analysis of the plasmid profile yielded species in molecular weight range from 0-170 Md. A correlation between single virulence features and certain plasmid combinations failed to be demonstrated. The results revealed a close connection between certain virulence properties of E. coli strains and a specific uropathogenicity. The clinical significance of this observation should be further elucidated.

[Renocolonic fistula complicating vesicorenal reflux and xanthogranulomatous pyelonephritis in a 20-month-old child].

A case of vesico-renal reflux with xanthogranulomatous pyelonephritis complicated with a renocolic fistula in a 20 months old child is reported. Imaging in this case was crucial for the diagnosis and the work up; it included ASP, renal ultrasound, UIV retrograde cystography and CT. The treatment consisted in a right ureteronephrectomy after fistula's section.

[Status of the utero-placental blood circulation in pregnant women with chronic pyelonephritis and nephropathy].

Dynamic scintigraphy was employed in studying of uteroplacental blood flow in 75 pregnant females with variety of nephropathic severity and duration developing in presence of chronic pyelonephritis. Hemodynamic disorders in the mother-placental system in females with associated gestosis were found to depend both on its severity and duration. In females with continuous severe nephropathy the most pronounced hemodynamic impairment was revealed at the late pregnancy, (37-38 weeks of gestation). The earlier the treatment of placental insufficiency started, the sooner uteroplacental blood flow recovered.

[Xanthogranulomatous pyelonephritis. Value of magnetic resonance imaging].

Xanthogranulomatous pyelonephritis is a rare disease which frequently has a pseudotumoral appearance. Preoperative diagnosis is hazardous in spite of ultrasonography and CT scan correlation. Recently developed magnetic resonance imaging (MRI) identifies fatty tissue which is specific for XGP. The frequency of invasive investigations has decreased because of this new data and partial nephrectomy has become possible in localized forms of XGP. Magnetic resonance imaging provides an accurate reproduction of the lesions in three dimensions which allows detection of complications (abscess and fistulae) and a study of resection difficulties.

[Xanthogranulomatous pyelonephritis. Our experience in the last 10 years].

A retrospective study of 34 xanthogranulomatous pyelonephritis (XGP) cases operated on in our Service was performed. An analysis is made of the clinical signs, laboratory data, imaging diagnostic techniques and the value of cytology in the assessment of this entity. A very characteristic clinical and biological picture, although somewhat nonspecific of XGP, was encountered. The high sensitivity of simple abdominal X-ray films and I.V.U., as well as the high specificity of cytologic studies in the diagnosis of the disease, was shown.

[Indicators of blood coagulation and fibrinolytic activity of the urine in children with calculous pyelonephritis].

Parameters of hemocoagulation and fibrinolysis in the blood and urine were studied in 108 children with calculous pyelonephritis. Fibrinolytic inhibition and signs of hypercoagulation were noted. Changes in coagulation were found to be corresponding to the activity of the disease. The changes were mostly pronounced in the urinary fibrinolytic activity, with almost zero levels in those with bilateral nephrolithiasis accompanied by renal dysfunction. Fibrinolytic and anticoagulatory medication was found to be mandatory for the aforementioned patients.

[Changes in the level of urinary secretory immunoglobulin A in patients with recurrent pyelonephritis].

The urine concentration of secretory immunoglobulin A was studied in 58 healthy persons (26 women and 32 men) and in 55 patients (44 women and 11 men) with chronic recurrent pyelonephritis before the beginning of the antibacterial treatment. An immunoenzyme method on a solid phase was used (ELISA). In 21 patients a lowered urine secretory immunoglobulin A level (IgA) was found, in 19 patients it was elevated and in 15 patients it was in normal ranges. In the group of patients with an elevated secretory IgA level the antibacterial treatment was successful in 89.5% of the patients while in the group of patients with a lowered secretory IgA level the treatment was successful only in 61.9% of the patients. In 12 patients the secretory IgA level was examined after the treatment and in 72.7% of them it was changed--in 38.2% of the patients the level was lowered and in 34.5% of the patients it was elevated. The local immune response of the organism against infections is discussed.
