Using visual latencies to improve image segmentation.

An artificial neural network model is proposed that combines several aspects taken from physiological observations (oscillations, synchronizations) with a visual latency mechanism in order to achieve an improved analysis of visual scenes. The network consists of two parts. In the lower layers that contain no lateral connections the propagation velocity of the activity of the units depends on the contrast of the individual objects in the scene. In the upper layers lateral connections are used to achieve synchronization between corresponding image parts. This architecture assures that the activity that arises in response to a scene containing objects with different contrast is spread out over several layers in the network. Thereby adjacent objects with different contrast will be separated and synchronization occurs in the upper layers without mutual disturbance between different objects. A comparison with a one-layer network shows that synchronization occurs in the upper layers without mutual disturbance between different objects. A comparison with a one-layer network shows that synchronization in the latency dependent multilayer net is indeed achieved much faster as soon as more than five objects have to be recognized. In addition, it is shown that the network is highly robust against noise in the stimuli or variations in the propagation delays (latencies), respectively. For a consistent analysis of a visual scene the different features of an individual object have to be recognized as belonging together and separated from other objects. This study shows that temporal differences, naturally introduced by stimulus latencies in every biological sensory system, can strongly improve the performance and allow for an analysis of more complex scenes.

The joint development of orientation and ocular dominance: role of constraints.

Correlation-based learning (CBL) has been suggested as the mechanism that underlies the development of simple-cell receptive fields in the primary visual cortex of cats, including orientation preference (OR) and ocular dominance (OD) (Linsker, 1986; Miller, Keller, & Stryker, 1989). CBL has been applied successfully to the development of OR and OD individually (Miller, Keller, & Stryker, 1989; Miller, 1994; Miyashita & Tanaka, 1991; Erwin, Obermayer, & Schulten, 1995), but the conditions for their joint development have not been studied (but see Erwin & Miller, 1995, for independent work on the same question) in contrast to competitive Hebbian models (Obermayer, Blasdel, & Schulten, 1992). In this article, we provide insight into why this has been the case: OR and OD decouple in symmetric CBL models, and a joint development of OR and OD is possible only in a parameter regime that depends on nonlinear mechanisms.

Soft mixer assignment in a hierarchical generative model of natural scene statistics.

Gaussian scale mixture models offer a top-down description of signal generation that captures key bottom-up statistical characteristics of filter responses to images. However, the pattern of dependence among the filters for this class of models is prespecified. We propose a novel extension to the gaussian scale mixture model that learns the pattern of dependence from observed inputs and thereby induces a hierarchical representation of these inputs. Specifically, we propose that inputs are generated by gaussian variables (modeling local filter structure), multiplied by a mixer variable that is assigned probabilistically to each input from a set of possible mixers. We demonstrate inference of both components of the generative model, for synthesized data and for different classes of natural images, such as a generic ensemble and faces. For natural images, the mixer variable assignments show invariances resembling those of complex cells in visual cortex; the statistics of the gaussian components of the model are in accord with the outputs of divisive normalization models. We also show how our model helps interrelate a wide range of models of image statistics and cortical processing.

Neurologic and radiologic manifestations of three girls surviving acute carbon monoxide poisoning.

We report the neurologic and radiologic manifestations of three adolescent girls with acute carbon monoxide poisoning. The girls were found collapsed and unconscious in a bathroom where liquid petroleum gas was being used as heating fuel. As hyperbaric oxygen therapy was not available locally, they only received oxygen supplementation via nasal cannula (4 L/minute) as treatment in the first 2 days. On transfer to a tertiary center in Hong Kong, evolving neurologic manifestations of visual acuity and field deficits, confusion, and focal motor weaknesses were observed. Focal infarctions were evident in cerebral computed tomography in one patient and cortical lesions on magnetic resonance imaging in all three patients. [18F]Fluorodeoxyglucose (FDG) positron emission tomography (PET) revealed additional decreased metabolism in the basal ganglia in two patients, which was typical of carbon monoxide poisoning. The neurologic deficits resolved completely at 3 weeks after the exposure, but psychologic symptoms succeeded. This report serves to alert clinicians to the varied neuro-ophthalmologic manifestations and psychologic impairment even with the same duration of carbon monoxide poisoning. PET might be more sensitive in detecting cerebral injuries specific for carbon monoxide poisoning.

Trans-orbital intra-cranial air gun injury.

PURPOSE: To describe a case of trans-orbital intra-cranial air gun injury with a discussion on air gun related morbidity and mortality. METHODS: Case report and literature review. RESULTS: The air gun pellet travelled through the orbit without penetrating the globe. It passed into the middle cranial fossa through the superior orbital fissure and lodged in the temporal lobe. The patient was managed conservatively with antibiotics and antiepilectis. CONCLUSIONS: Air gun design has changed in recent years resulting in an increased morbidity and mortality. Stricter legislation on the sale and use of these weapons needs to be implemented.

Bio-inspired nano-sensor-enhanced CNN visual computer.

Nanotechnology opens new ways to utilize recent discoveries in biological image processing by translating the underlying functional concepts into the design of CNN (cellular neural/nonlinear network)-based systems incorporating nanoelectronic devices. There is a natural intersection joining studies of retinal processing, spatio-temporal nonlinear dynamics embodied in CNN, and the possibility of miniaturizing the technology through nanotechnology. This intersection serves as the springboard for our multidisciplinary project. Biological feature and motion detectors map directly into the spatio-temporal dynamics of CNN for target recognition, image stabilization, and tracking. The neural interactions underlying color processing will drive the development of nanoscale multispectral sensor arrays for image fusion. Implementing such nanoscale sensors on a CNN platform will allow the implementation of device feedback control, a hallmark of biological sensory systems. These biologically inspired CNN subroutines are incorporated into the new world of analog-and-logic algorithms and software, containing also many other active-wave computing mechanisms, including nature-inspired (physics and chemistry) as well as PDE-based sophisticated spatio-temporal algorithms. Our goal is to design and develop several miniature prototype devices for target detection, navigation, tracking, and robotics. This paper presents an example illustrating the synergies emerging from the convergence of nanotechnology, biotechnology, and information and cognitive science.

A statistical model for binocular rivalry.

A probabilistic model is presented for the phase durations in binocular rivalry experiments. The hypothetical construct of inhibition or reaction inhibition is used to account for the length of the successive phases of left-eye dominance and right-eye dominance. In accordance with Hull's Postulate X.B. it is assumed that the inhibition increases linearly at rate a1 during periods of left-eye dominance and decreases linearly at rate a0 during periods of right-eye dominance. Two different versions of the proposed model are presented: the beta and the Bessel inhibition models. Inhibition fluctuates between the boundaries 0 and 1 in the beta inhibition model and between -infinity and +infinity in the Bessel inhibition model. The transition rates lambda1(t) for switches from a state of left-eye dominance to a state of right-eye dominance, and lambda0(t) for switches from a state of right-eye dominance to a state of left-eye dominance depend on inhibition: lambda1(t)=l1 (Y(t)), lambda0(t)=l0(Y(t)), where l1 is a non-decreasing function and l0 is a non-increasing function. In the beta inhibition model l1(y)=c1/(1 - y) and l0(y)=c0/y. In the Bessel inhibition model l1(y)=u1e(y) and l0(y)=u0/e(y). Special attention is given to the derivation of the expectation of the stationary phase durations.

Information throughput characteristics--a new method for evaluation of visual efficiency.

A method based on information theory for computer-aided assessment of visual capacity has been elaborated. In this method, the rate of recognition of visual targets is considered along with the accuracy of perception. Measurements are performed in a wide range of optical stimulus intensities (the Landolt ring size ranging from 0.125 to 1.57, in 10 logarithmic steps). For each step the information throughput (IT) is calculated and the results are displayed as a function of stimulus intensity. Thus, the IT characteristics of the visual system, with regard to motor response, are obtained. Within the reported range, they reveal low-pass properties of the system. Preliminary experiments show that these curves are useful in the evaluation of visual performance, especially in ergophthalmology. A recently developed IBM PC-based system for automatic measurements of the IT characteristics is described. Some aspects of application of the method are also discussed.

Improving quantitative trait loci mapping resolution in experimental crosses by the use of genotypically selected samples.

One of the key factors contributing to the success of a quantitative trait locus (QTL) mapping experiment is the precision with which QTL positions can be estimated. We show, using simulations, that QTL mapping precision for an experimental cross can be increased by the use of a genotypically selected sample of individuals rather than an unselected sample of the same size. Selection is performed using a previously described method that optimizes the complementarity of the crossover sites within the sample. Although the increase in precision is accompanied by a decrease in QTL detection power at markers distant from QTL, only a modest increase in marker density is needed to obtain equivalent power over the whole map. Selected samples also show a slight reduction in the number of false-positive QTL. We find that two features of selected samples independently contribute to these effects: an increase in the number of crossover sites and increased evenness in crossover spacing. We provide an empirical formula for crossover enrichment in selected samples that is useful in experimental design and data analysis. For QTL studies in which the phenotyping is more of a limiting factor than the generation of individuals and the scoring of genotypes, selective sampling is an attractive strategy for increasing genome-wide QTL map resolution.

Repeated hemorrhage in ciliated craniopharyngioma--case report.

A 49-year-old female presented with a ciliated craniopharyngioma manifesting as repeated intratumoral hemorrhage. Histological examination suggested that the hemorrhage originated from the many thin blood vessels in the cyst wall stroma associated with inflammation. Symptomatic hemorrhage in cystic craniopharyngioma may mimic pituitary apoplexy but the etiology is quite different. Minor hemorrhage may recur unless the cyst wall is totally removed.

Modelling and simulation in medicine and life sciences.

Modelling and Simulation in Medicine and Life Sciences' was the topic of a workshop held in Copenhagen at MIE'96 and of a session in Zakopane in 1997. A survey of some of the papers presented at these meetings and selected for this issue is made. The selection was made according to the topics, referring to modelling and simulation of fundamental biological processes: dynamic behaviour of the human respiratory system, wave propagation in normal and infarcted myocardium, electrical field generated by the heart, human chest conductivity, and depth perception in binocular vision.

Modified approach for the selective treatment of temporal lobe epilepsy: transsylvian-transcisternal mesial en bloc resection.

OBJECT: The authors propose a novel surgical approach for amygdalohippocampectomy (AH) in patients with temporal lobe epilepsy. Via a transsylvian-transcisternal route, the parahippocampal gyrus is directly exposed from its medial aspect, thus allowing a standardized en bloc resection of the temporomesial epileptogenic structures--the amygdala, anterior hippocampus, parahippocampal gyrus, and subiculum. Additional anatomical studies have been performed for standardization of this approach. METHODS: From 1990 to 1996, 32 patients presenting with medically intractable mesial temporal lobe epilepsy underwent AH via the transsylvian-transcisternal approach. Preoperative computerized tomography and magnetic resonance imaging revealed temporomesial lesions in 16 patients. Histopathological examination revealed cavernous malformations in seven patients, low-grade astrocytomas in four, hamartomas in three, and gangliogliomas in two patients. Specimens obtained in patients with no lesions were diagnosed as hippocampal sclerosis in all cases. No patient experienced permanent morbidity. Nine percent of the patients developed a temporary partial oculomotor nerve palsy. Only one patient developed a postoperative visual field deficit with a contralateral quadrantanopsia. With respect to seizure outcome, all patients benefited from surgery. At follow-up evaluation (mean 26.4 months), 80% of the patients were free from seizures (Engel Class I). Eight patients in this group were no longer receiving medication. Seventeen percent had experienced only one to several seizures since surgery (Engel Class II) and 3% reported a worthwhile improvement (Engel Class III). CONCLUSIONS: In contrast to previously described standard techniques for AH, the transsylvian-transcisternal approach presented in this study offers improved anatomical orientation and intraoperative control over the mesial temporal lobe and preserves the lateral as well as the laterobasal temporal lobe.

Symptomatic pituitary metastases.

OBJECT: The diagnosis and treatment of metastasis to the pituitary gland can be difficult to determine. The goal of this study was to analyze the clinical presentation, treatment, and prognosis of patients who presented with symptomatic pituitary metastasis. METHODS: The cases of 36 patients with symptomatic pituitary metastases were reviewed. The most common primary cancers were breast (33%) and lung (36%). The presenting symptoms included diabetes insipidus, anterior pituitary insufficiency, and retroorbital pain. The overall median length of patient survival following diagnosis of pituitary metastasis was 180 days. In 20 patients (56%), symptoms stemming from pituitary metastasis were the first manifestation of illness. Local control of tumor was associated with significant improvement in survival times (p < 0.05) and amelioration of disabling symptoms including painful ophthalmoplegia and visual field deficits. CONCLUSIONS: Aggressive treatment including both surgical decompression and radiation therapy improves the quality of life in patients suffering from symptomatic pituitary metastasis.

Pterional surgery of meningiomas of the tuberculum sellae and planum sphenoidale: surgical results with special consideration of ophthalmological and endocrinological outcomes.

OBJECT: The authors reviewed 47 cases of suprasellar meningiomas with special attention to ophthalmological and endocrinological outcomes. METHODS: All patients underwent surgery performed via a unilateral pterional approach between January 1983 and January 1998. Ophthalmological and endocrinological examinations were performed before the operation as well as 1 week and 3 months postoperatively. A special scoring system was adopted to quantify the extent of ophthalmological disturbances. Complete tumor resection was possible in all but one patient. There were no fatalities and the rate of visual improvement was 80%. The best prognoses were found in patients younger than 50 years and in patients in whom the duration of symptoms was less than 1 year. Before surgery, tumor-related endocrine disturbances were present in only three women who suffered from secondary hypogonadism; two of these patients recovered after surgery. Postoperatively, no patient needed replacement therapy for pituitary dysfunction. The overall tumor recurrence rate was 2.1% (one of 47 cases). For patients in whom long-term (> 5 years) follow-up data were available, the recurrence rate was 4.2% (one of 24 cases). CONCLUSIONS: In this series, complete resection of suprasellar meningiomas was possible through a unilateral pterional craniotomy and was associated with a low morbidity rate and no deaths.

[Prediction system of facial change].

UNLABELLED: Three-dimensional simulation of facial change in a patient with severe facial deformity is very effective in not only determining a treatment plan but also in obtaining informed consent. The purpose of this presentation was to introduce the outline of our prediction system of facial change and its preliminary clinical application. (Outline of this system) This system is composed of the following three parts: Input: Three-dimensional digitized data from modified Moire Topography and coloring data of the face taken by the three-dimensional photographing appliance. ANALYSIS: Reconstruction of colored simulation model and transformation by host computer with 32 bit CPU. Output: Display of full-colored simulation model by three-dimensional graphics (Results of clinical application) 1. Three-dimensional reconstructed graphics were well accepted for clinical use, especially for the prediction in orthognathic surgery. 2. Compared with other systems, the following advantages were found: a) Simplified digitizing system using the CCD camera b) Total access time, from initial data input to display of computed picture, was about 20 minutes c) Accurate reproducibility of facial form and color vision.

Orbital blow-out fractures: correlation of preoperative computed tomography and postoperative ocular motility.

BACKGROUND/PURPOSE: Although the management of orbital blow-out fractures was controversial for many years, refined imaging with computed tomography (CT) helped to narrow the poles of the debate. Many orbital surgeons currently recommend repair if fracture size portends late enophthalmos, or if diplopia has not substantially resolved within 2 weeks of the injury. While volumetric considerations have been generally well-served by this approach, ocular motility outcomes have been less than ideal. In one series, almost 50% of patients had residual diplopia 6 months after surgery. A fine network of fibrous septa that functionally unites the periosteum of the orbital floor, the inferior fibrofatty tissues, and the sheaths of the inferior rectus and oblique muscles was demonstrated by Koornneef. Entrapment between bone fragments of any of the components of this anatomic unit can limit ocular motility. Based on the pathogenesis of blow-out fractures, in which the fibrofatty-muscular complex is driven to varying degrees between bone fragments, some measure of soft tissue damage might be anticipated. Subsequent intrinsic fibrosis and contraction can tether globe movement, despite complete reduction of herniated orbital tissue from the fracture site. We postulated that the extent of this soft tissue damage might be estimated from preoperative imaging studies. METHODS: Study criteria included: retrievable coronal CT scans; fractures of the orbital floor without rim involvement, with or without extension into the medial wall; preoperative diplopia; surgical repair by a single surgeon; complete release of entrapped tissues; and postoperative ocular motility outcomes documented with binocular visual fields (BVFs). Thirty patients met all criteria. The CT scans and BVFs were assessed by different examiners among the authors. Fractures were classified into 3 general categories and 2 subtypes to reflect the severity of soft tissue damage within each category. "Trap-door" injuries, in which bone fragments appeared to have almost perfectly realigned, were classified as type I fractures. In the I-A subtype, no orbital tissue was visible on the sinus side of the fracture line. In the I-B subtype, soft tissue with the radiodensity of orbital fat was visible within the maxillary sinus. In type II fractures, bone fragments were distracted and soft tissue was displaced between them. In the II-A subtype, soft tissue displacement was less than, or proportional to, bone fragment distraction. In the II-B subtype, soft tissue displacement was greater than bone fragment distraction. In type III fractures, displaced bone fragments surrounded displaced soft tissue in all areas. In the III-A subtype, soft tissue and bone were moderately displaced. In the III-B subtype, both were markedly displaced. Motility outcomes were quantified by measuring the vertical excursion in BVFs. The interval between trauma and surgical repair was also determined. RESULTS: Among the 15 patients with a motility outcome in BVFs which was poorer than the median (86 degrees or less of single binocular vertical excursion), 4 patients (27%) had type A fractures; 11 patients (73%) had type B fractures. Among the 15 patients with a better outcome than the median (88 degrees or more), 10 patients (67%) had type A fractures; 5 patients (33%) had type B fractures. These differences became more defined as analysis moved away from the median. Among 5 patients with type B fractures and better than the median result in BVFs, 3 patients (60%) had surgical repair during the first week after injury. Among the 11 patients with type B fractures and less than the median result, 1 patient (9%) had repair during the first week. CONCLUSIONS: When the CT-depicted relationship between bone fragments and soft tissues is considered, a wide spectrum of injuries is subsumed under the rubric of blow-out fractures. In general, greater degrees of soft tissue incarceration or displacement, with presumably greater intrinsic damage and subsequent fibrosis, appear to result in poorer motility outcomes. Although this retrospective study does not conclusively prove its benefit, an urgent surgical approach to selected injuries should be considered.
