[Medical treatment of benign hypertrophy of the prostate].

Surgery is obviously indicated in a patient with benign prostatic hypertrophy and severe signs of urine retention, infection, and a bladder with diverticles and calculi. Inversely the patient with minor dysuria and a normal upper urinary tract usually needs simple counselling. The major question is how to manage patients with intermediary symptomatology. Much emphasis has been placed on measurement of quality of life, but cannot be used to determine a therapeutic strategy. Indeed, there is no precise criteria for indicating medical treatment of benign prostatic hypertrophy. Randomized, controlled and well conducted studies have shown however that the beneficial effect of certain drugs is greater than the placebo effect, but there is no common criteria to determine what degree of improvement justifies prescription. In addition, the cost of medical treatment, in terms of overall health care cost for the society, must be taken into consideration. Despite these drawbacks, several drug classes--plant extracts, alpha-blockers, 5-alpha-reductase inhibitors--have been used with confirmed effectiveness. The spectacular advances in pharmacology in the field of benign prostatic hypertrophy will undoubtedly have an impact on other areas of research, but the medical, economic and ethic justification of prescribing "comfort" drugs is one of the major challenges facing urologists today.

5-alpha Reductase Inhibitors↗

The abnormal development of male sex organs in the rat using a pure antiandrogen and a 5 alpha-reductase inhibitor during gestation.

The effects of a pure antiandrogen flutamide and the 5 alpha-reductase inhibitor finestaride on both prenatal differentiation of prostate and external genitalia were studied in the rat. In control male offspring the mean value of anogenital distance was 3.1 +/- 0.24 mm, and in control female was 1.2 +/- 0.10 mm. In control male newborn rats, histological sections at cranial portion of the urethra revealed prostate bud formation. Male offspring prenatally exposed to 6 mg/Kg/day of flutamide had a significant decrease in anogenital distance, but no alteration in prostate bud formation. At higher dosages of flutamide, the external genitalia were virtually feminizated and the prostatic budding was completely inhibited. In male offspring treated 'in utero' with doses of finestaride of 2, 8 and 16 mg/Kg/day, the anogenital distance became progressively reduced, but complete abolition of prostate development never occurred. However, in male offspring given finestaride at a dose of 2 mg/Kg/day concomitantly with flutamide at a dose of 6 mg/Kg/day, prostate differentiation was completely abolished.

[Variation in the diagnosis and treatment of benign prostatic hyperplasia in urological practice].

OBJECTIVE: To analyse practice variation among urologists and its determinants with respect to diagnostics and therapy choice in benign prostatic hyperplasia (BPH). DESIGN: Cross-sectional. SETTING: Stratified sample of 12 urological practices throughout the Netherlands. METHODS: On 670 consecutive, newly referred BPH patients > or = 50 years, data were collected about symptomatology, discomfort and sexual functioning (patient questionnaire), diagnostic procedures and outcomes (medical record), and (considerations for) therapy choice (urologist questionnaire). Characteristics of patients as well as of urologists were integrally studied to explain the variation. RESULTS: With respect to diagnostics, highest variation (interquartile ranges) was found for ultrasonography of the prostate (19-86%), kidneys (19-68%), and bladder (42-91%), and lowest variation for digital rectal examination (97-100%) and blood tests and urinalysis (88-100% and 86-99% respectively). For therapy choice, interquartile ranges were 24-42% (surgery), 32-49% (watchful waiting), 5-29% (alpha-blockers), and 0-17% (5-alpha-reductase-inhibitor). Variation in diagnostics was associated with characteristics of urologists and work setting as well as of patients. For differences in therapy choice, symptomatology, discomfort, sexual activity, peak flow, volume of residual urine, prostate volume, comorbidity, experience of the urologist, and the type of hospital were the most important explanatory variables. CONCLUSION: Variation in both diagnostics and therapy choice is considerable. This variation is associated with characteristics of patients as well as of urologists and work setting.

[Benign prostatic hyperplasia--its treatment with Proscar].

Benign prostate hyperplasia is a potentially serious condition affecting men in advanced age. The treatment is conservative and operative. Proskar of Msd is one of the drugs yielding encouraging results. It is a synthetic 4-azosteroid compound, and represents a new class of specific inhibitors of 5-alpha reductase, promoting stabilization of testosterone and the more active androgen dihydrotestosterone (DHT). A total of 35 patients undergo treatment with Proskar at 5 mg daily dose (one tablet) over a 6-month period. The study includes six patients with complete urinary retention. The results are assayed by the change in prostate gland volume, residual urine and maximal urinary flow, 6 PsA. Completely responsive to treatment are 71.44 per cent of patients, partial effect is recorded in 22.85 per cent, and no effect whatsoever--in 5.71 per cent.

Optimising the medical management of benign prostatic hyperplasia.

The ageing population is presenting an increasing demand on future healthcare services. In males, prostatic disease is one of the commonest disorders contributing to this. Alternatives to surgical intervention have to be considered as the preferred option for individuals and whether this be a therapeutic or a financial option to be taken. Two major medical alternatives are alpha-blockers and 5-alpha reductase inhibitors. The results of such treatment can be very beneficial in selected groups of patients. Side-effects with improved drugs electivity are reducing. Overall, while surgery still holds the gold standard, medical therapy has a significant role in the treatment of benign prostatic hypertrophy. Cost analysis may be a factor in deciding which treatment to have.
