Eosinophilic granuloma with oral manifestations: a case report.

Eosinophilic granuloma is the most benign disorder of the triad commonly known as histocytosis X. In this article a case of a 6 year old female child with multiple eosinophilic granuloma with additional liver dysfunction and its oral manifestation is presented. This case demonstrated that oral findings, may be an early manifestation of the disease, definitive diagnosis needs to be determined by correlation of the clinical findings with histologic features. For the duration of 8 years the case has been followed up, there has been a progressive healing of the lesion, the clinical manifestations of the disease resolved with only chemotherapy and provided a very good prognosis.

Overdentures and the periodontium.

Several periodontal factors are critical to the prognosis of overdenture abutment teeth. This literature review outlines these factors and discusses the documented effects of long-term overdenture use on periodontal health. The efficacy of a professional periodontal maintenance program, which is coordinated with a home oral hygiene program, is related to the success of overdenture therapy.

[Dissertations 25 years later. 1. Mesial drift of teeth by occlusal forces].

More than 25 years ago the thesis 'Functional occlusion and mesial drift, an experimental study of tooth migration in monkeys' was presented. A literature review was carried out to assess whether the results have withstood the test of time. It was confirmed that occlusion plays an important role in mesial drift, whilst no direct evidence for transseptal fibre contraction as a causal factor has been put forward. Mesial drift is not merely a compensatory mechanism for loss of tooth substance, but may cause crowding, depending on axial inclination and occlusal morphology. This phenomenon has implications for orthodontic and restorative treatments.

Interdisciplinary treatment of a severe maxillary aesthetic defect: a case report.

The treatment of compromised dentition as a result of severe bone deficiencies in the aesthetic zone requires the use of predictable alveolar bone regeneration and restorative techniques. A multidisciplinary treatment approach based on thorough communication and treatment planning will allow the delivery of desirable periodontal and prosthetic results. By considering the patient's desires and existing condition, different treatment options can be presented to determine which best suits the patient's needs. This presentation describes a treatment protocol for restoration of the anterior region following bone augmentation, implant placement, and prosthetic rehabilitation.

Papillon Lefevre syndrome.

Papillon-Lefevre syndrome is a very rare autosomal recessive condition characterised by pronounced palmoplantar hyperkeratosis and severe early onset periodontitis, leading to early loss of teeth. Here, we report a case of Papillon-Lefevre syndrome with a brief discussion on treatment aspect.

Localized aggressive periodontitis in a six-year-old: a case report.

The purpose of this report was to describe an approach to diagnose and effectively treat a pediatric patient with localized aggressive periodontitis. A 6-year-old female presented with clinical and radiographic evidence of severe attachment loss around several primary teeth. She had no history of systemic disease, periodontal disease, or caries prior to the periodontal abscess that prompted her referral. Routine immunological tests did not reveal any functional defects, but DNA testing for periodontal pathogens revealed the presence of all 8 aggressive periodontal pathogens assayed. Treatment consisted of the extraction of 2 severely affected primary teeth, increased frequency of recall appointments, and administration of systemic antibiotics. The patient's periodontal condition was stabilized 18 months post-treatment, and the 8 pathogens were no longer at detectable levels. With a treatment goal of preventing disease progression into the erupting permanent dentition, this treatment regimen provides an effective alternative to more aggressive strategies.

The relationship between clinical dental status and oral impacts in an elderly population.

PURPOSE: To investigate the relationship between oral health-related quality of life and clinical dental measures in an elderly Greek population. MATERIALS AND METHODS: A cross-sectional survey was carried out of adults aged 65 years or older living independently in Athens. Data were collected through clinical examination and interviews. Oral health-related quality of life was assessed through the Oral Impacts on Daily Performance (OIDP) indicator. The sample consisted of 681 participants. Data analysis used non-parametric tests (Mann-Whitney, Kruskal-Wallis and multiple logistic regressions). RESULTS: The response rate was 87.8%. Dentate participants with 1-10 teeth were 2.05 (1.25, 3.35) times and those with 11-20 teeth were 1.81 (1.11, 2.95) times more likely to report oral impacts than subjects with 21 or more teeth. Participants with anterior tooth spaces were 2.86 (1.70, 4.80) times more likely to report oral impacts than those without anterior spaces. Participants with 0-8 natural occluding pairs (NOPs) were 1.72 (1.14, 2.58) times and those with 0-3 posterior occluding pairs (POPs) were 1.57 (1.04, 2.36) times more likely to experience oral impacts than subjects with 9-16 NOPs and 4-10 POPs respectively. Decayed teeth were not significantly related to the presence of oral impacts. Edentulous participants with inadequate denture adaptation were 2.59 (1.46, 4.59) times, those with inadequate denture retention 2.41 (1.39, 4.17) times and those with denture overextension 2.51 (1.10, 5.74) times more likely to report oral impacts than subjects without the respective denture deficiencies. CONCLUSION: Clinical indicators of oral health status were significantly related to measures of oral health-related quality of life.

[Experience with evicrol splints after eighteen months of use (author's transl)].

The authors, after discussing the general importance of the use of splints in parodontological therapy, give a detailed description of the manufacture of composite splints. The experience and results obtained after eighteen months of use of fifteen splints are discussed and compared with data reported in the international literature.

Sporadic Burkitt's lymphoma of the jaws: the essentials of prompt life-saving referral and management.

Burkitt's lymphoma is an undifferentiated non-Hodgkin's B-cell lymphoma. Three clinical subtypes are recognized: African (endemic), American (sporadic) and HIV associated. Sporadic Burkitt's lymphoma is a rare malignancy among western populations. This report describes a case of sporadic Burkitt's lymphoma of the jaws with an alarmingly rapid spread associated with acute renal failure. This type of rapid progression bespeaks the need for prompt recognition and life-saving referral by the dental practitioner. The clinical features of Burkitt's lymphoma involving the jaws include severely hypermobile, ectopically displaced and supra-erupted teeth. The purpose of this case report and review of the literature is to illustrate the clinical and histopathologic features of Burkitt's lymphoma to help clinicians recognize such cases readily and facilitate prompt and potentially life-saving referral.

Considerations in treatment planning of the periodontal patient.

Through the use of periodontal, orthodontic, and restorative procedures, this patient was restored to function with a stable dentition and minimal pocket depths. The patient was followed for 10 years. All areas exhibited a healthy periodontium with stability of all areas of the dentition.

[Studies of parodontal results obtained for selected teeth and groups of teeth with particular reference to their predictive potential regarding an overall assessment (author's transl)].

With the support of a Cluster Analysis and other statistical methods we have found out the relative efficiency of some periodontal partial recording selections. Overrating or underrating evaluations arise from the registration of periodontal symptoms and signs by partial recording of special teeth or groups of teeth. The results of our investigations have demonstrated the higher quantitative and qualitative coincidence of the RAMFJORDS teeth with the periodontal status as the frontal region of the lower jaw.

Severe localized periodontal destruction associated with cervical cemental separation.

Cemental separations and tears are secondary etiological factors that may lead to rapid periodontal destruction if the lesion communicates with the oral cavity and allows bacterial invasion. Although many cemental tears that occur on proximal surfaces can be diagnosed radiographically, separations on the facial or lingual surface that cannot be detected on radiographs may present a diagnostic dilemma. This article describes a case of lingual cemental separation on a maxillary incisor, with subsequent successful surgical correction utilizing an osseous graft.
