[One-stage esophagoplasty without transfusion of donor blood components].

Six patients aged from 27 to 62 years were operated for subtotal burn strictures, cancer and glandular polyp of a low-thoracic part of the esophagus using a complex program of non-blood surgery. The following methods were used: collection of autoplasma before surgery by plasmapheresis, collection of autologic packet red cells, normovolemic hemodilution during surgery, use of perftoran for oxygen transport and recombinant human erythropoetin to stimulate erythropoiesis and raise preporative level of hemoglobin. Components of donor blood were not used in these patients. There were no complications associated with these methods. Partial failure of esophagol-colonic anastomosis on the neck was seen in 1 patient. The complex program permits one to avoid completely transfusion of donor blood components in esophagol surgery.

[Angiographic study of the colonic vessels with a view to esophagoplasty].

Variants of the colic vessels were studied angiographically in 34 cadavers, drawing up an original classification of the colic arteries arising from the lower mesenteric artery. Sixteen variants of the left colic artery were found, which were grouped into five main types, the classical type being encountered in only 22.6% of the cases. In one case the lower mesenteric artery was absent. In coloesophagoplasty better results were obtained with the isoperistaltic transverse colon than with the upper ileocolon in 64% of the cases and similar results in 20%; in 16% of the cases the latter operation was preferable.

[One-stage esophagoplasty with pathologically changed or operated stomach in patients with benign esophageal strictures].

Results of surgical treatment of 71 patients who have undergone extirpation of the esophagus with one-stage plastic surgery with pathologically changed or operated stomach are presented. All the patients suffered from benign diseases of the esophagus: burn and peptic strictures, peptic esophageal ulcers, cardiospasm of degree IV, benign tumors. Fifty-one patients had been previously operated on the stomach, 10 patients had burn deformation of the stomach, 3--benign tumors of the esophagus involving the stomach, the others--gastric and/or duodenal ulcers. In all the cases an isoperistaltic gastric tube was used for plastic repair of the esophagus. Postoperative lethality was 1.4%.

[Outcome prognosis of esophagoplasty of esophagus burns with colonic segment ].

Peculiarities of postburn esophagitis course and subsequent rehabilitation of patients with burn of esophagus were studied. The authors had systematized and proposed classification of postburn esophagitis, considering prognosis of autoplastic results using segment of colon for postburn esophageal impassability. Right calculation and logical interpretation, determination of significance of postburn esophagitis will provide success of autoplastic rehabilitation of patients with burn disease of the esophagus.
