[Hemolysis indicators in the evaluation of the function of artificial heart valves after heart valve prosthesis].

Hemolysis of blood was studied in two groups of patients: the main group consisted of 75 patients with mitral, aortal and two-valves prostheses; a control group had 15 patients with rheumatic valve defects without prostheses. It was shown that hemolysis was minimal in patients with a satisfactory function of prostheses. It was higher after double prostheses and especially in patients with dysfunction of the valves. Lactate dehydrogenase is thought to be the most important and reliable test for the assessment of the hemolysis degree.

[Radiological aspects of acute respiratory insufficiency after heart valve prosthesis].

Analysis of the x-ray findings in 156 patients with acute respiratory insufficiency (ART) in the immediate periods after implantation of heart valve prostheses has shown that various pulmonary complications, such as pulmonary edema (in 84% of cases), atelectasis, hypoventilation (5.1%), hemothorax (6.4%), pneumothorax (0.6%) were the prerequisites for the development of respiratory disorders. Pneumonias were not the primary cause of ART but an additional factor for the respiratory disorder progress, for they develop in the presence of previous pulmonary changes. The necessity and possibility of establishing the pathogenetic mechanism of pulmonary edema (cardiogenic or noncardiogenic one) is shown. The authors emphasize the desirability of regular x-ray examinations as a supplementary method in monitoring the patients with acute respiratory insufficiency.

[Paravalvular insufficiency following heart valve prosthesis].

The features of the morphology of paravalvular insufficiency (PVI) were studied on the basis of analysis of 5,113 prosthetic replacements of the heart valves. From the clinical results and noninvasive and invasive data the authors revealed the peculiarities of the disorders of hemodynamics and functional condition of patients with PVI. Information on the specific features and the results of 35 repeated operations in PVI are discussed. Analysis showed 26.4% survival among patients 5 years after surgery. Bearing in mind the severity of the complication the authors emphasize the importance of its prevention.

Aortic Valve Insufficiency↗

[A mechanical heart valve prosthesis with a spherical shape and open center].

A mechanical valve prosthesis is described with central blood flow. The generations I and II of the mechanical valve obstruct the blood flow and cause turbulence with undesirable consequences. The opening in the centre of Simkovic-Bolf valve preserves the laminar blood flow, reduces the trauma of blood and myocardium stress. Our new valve represents a mechanical valve prosthesis of generation III. (Fig. 5, Ref. 16.).
