[Reproductive function of women and risk of the development of breast cancer].

The relationship between risk for breast cancer and certain factors of reproductive function was studied on a "case-control" basis. It was demonstrated that the traditional and well-known index of age at first birth cannot be considered a universal factor for all females who have borne different numbers of children. Indexes which take into account age at all births proved more reliable in evaluating risk of cancer in bi-, tri- and multiparae. Indexes of relative risk and the role of characteristics of reproductive function are discussed.

[Heterozygosity and fertility in the Northern Selkup tribe].

The relationship between heterozygosity for 11 polymorphic loci and fertility of women having passed the menopause was studied in North-Siberian tribe North Selkup. Women with low level of heterozygosity were characterized by the highest values of variance of the number of livebirths and the index of early reproductive loss.

Genetic drift in present human populations: survey of a Mediterranean region (La Alpujarra, South-east Spain).

This paper presents information on the opportunity for genetic drift in La Alpujarra, a rural population of the Spanish Mediterranean. This region is characterized by its historical isolation. The analyses were based on a survey of 45% of the population. The Effective Population Size (NE = 1986), the Effective Migration Rate (M(E) = 12.4) and the Coefficient of Breeding Isolation (CBI = 246.2) of Lasker & Kaplan (1964) were calculated. The value of the latter index indicates that genetic drift in the Alpujarrenian population is negligible at present because of the cessation of reproductive isolation due to the development of communication networks.

The association of pregnancy history with areal and volumetric bone mineral density in adolescence.

INTRODUCTION: Studies demonstrate that pregnancy may interfere with bone mineral density. Adolescence is a crucial time of life for bone mass acquisition and there are some questions as to the influence of pregnancy on bone mass at this age. OBJECTIVE: To evaluate the association between pregnancy history and areal (BMD) and volumetric (vBMD) bone mineral density in adolescence. SUBJECTS AND METHODS: A cross-sectional study of 119 adolescents ranging from 12-20 years of age was conducted; 30 of these girls had a history of full-term pregnancy. The adolescents were selected during a routine visit to the Adolescent Gynecology Outpatient Facility, completed a questionnaire, and had a physical examination to evaluate weight, height and Tanner stage. Bone mineral densitometry of the lumbar spine (L(1)-L(4)) and total body (TB) was performed to measure bone mineral density and body composition. RESULTS: The mean measurements of the area, bone mineral content (BMC), BMD and vBMD of L(1)-L(4) and the area, BMC and BMD of TB were not significantly different between adolescents with and without a pregnancy history, stratified by chronologic and gynecologic age. The percentage of adolescents with altered Z-scores was similar in both groups, and the prevalence ratio showed no association between pregnancy history and low bone mass (PR=0.52; CI 0.04-6.07). Upon multiple regression analysis, body mass index (BMI) and lean body mass (LBM) were the main factors associated with lumbar spine and total body measurements. Pregnancy history was inversely associated with areal BMD of L(1)-L(4) (R (2)=0.04) and vBMD of L(1)-L(4) (R (2)=0.04), accounting for only 4% variation in the lumbar spine. CONCLUSION: These data suggest that adolescent pregnancy seems to exert no significant influence on the acquisition of bone mass and does not appear to represent a risk factor for osteoporosis in the future.

The ethics of gene therapy and abortion: public opinion.

On the grounds that the public should be consulted in decisions concerning the legitimate scope of germ-line genetic therapy (GLGT), survey data on the ethics of GLGT were collected from a large (n = 1,403) representative national sample of Australians in 2002. The data show that opinion is quite divided over GLGT in the case of a 'death sentence' genetic defect: 36% would forbid it, 23% have mixed feelings and 41% would allow it. For less serious conditions there is more opposition to GLGT. Thus, 48% would forbid GLGT to remedy a minor physical defect and 52% would oppose GLGT to counteract a propensity to violence, but fully 73% would disallow GLGT for cosmetic reasons. The data also show that opposition to abortion is lower than opposition to GLGT in the case of a 'death sentence' genetic defect, but at about the same level as, or greater than, opposition to GLGT for less serious issues. The questions show good measurement properties, including low missing data rates, so they are likely to provide an accurate picture of the public's views on the ethics of GLGT. It is suggested that a system for monitoring public opinion on these issues be developed.

Urinary 6-sulphatoxy melatonin excretion during the human menstural cycle.

The excretion of 6-sulphatoxy melatonin in urine has been shown to provide a reliable index of melatonin production in man. Daily measurements made in two healthy ovulating women showed that both the total amount excreted/day and the proportion excreted during the sleep period remained relatively constant for each subject throughout the menstrual cycle. These data do not support the view that there are changes in melatonin production which correlate with cyclical reproductive endocrine function in the human.

Mid upper arm and calf circumferences as indicators of nutritional status in women of reproductive age.

Mid upper arm circumference (MUAC) is recognized as an effective means of screening for poor nutritional status in adults. The efficacy of calf circumference (CC) as a screening tool, however, is not well reported. We studied 2000 healthy women (age range: 15-49 years) in Mashed, Islamic Republic of Iran, to assess anthropometric measurements at reproductive age and the usefulness of MUAC and CC for estimating body mass index (BMI) and screening of women at nutritional risk. We found a strong correlation between MUAC and CC with weight and BMI, suggesting that MUAC and CC can be used to estimate BMI and detect nutritional disorders.

Uterine adnexal torsion with subsequent contralateral recurrence.

Uterine adnexal torsion and infarction is described in a 12-year-old girl who, six weeks later, suffered a contralateral recurrence, rendering her sterile. This is on the second known and reported case of this unusual sequence of events. The question of performing a prophylactic oophoropexy is addressed. A high index of suspicion and early diagnosis are mandatory in this condition to preserve reproductive function.

Effect of rosiglitazone on insulin resistance, growth factors, and reproductive disturbances in women with polycystic ovary syndrome.

OBJECTIVE: To evaluate the effects of rosiglitazone on insulin resistance, growth factors, and reproductive disturbances in women with polycystic ovary syndrome (PCOS). DESIGN: Prospective study. SETTING: Women with PCOS attending as outpatients of the Endocrine Division, Hospital Durand, Buenos Aires. PATIENT(S): Twenty-four insulin-resistant women with PCOS. INTERVENTION(S): Hormonal evaluations and a standardized oral glucose tolerance test before and after a 3-month trial of 4 mg of rosiglitazone daily. MAIN OUTCOME MEASURE(S): Serum LH, FSH, T, IGF-1, IGFBP-1, IGFBP-3, leptin, 17alpha-hydroxyprogesterone, insulin, and glucose concentrations. The area under insulin curve (AUC-insulin), the HOMA index (insulin resistance), the QUICKI index (insulin sensitivity), and the beta-cell function were calculated. Body mass index (BMI) and the waist/hip ratio were evaluated. RESULT(S): A significant decrease was observed in serum fasting insulin, AUC insulin, HOMA index, beta-cell function, IGF-1, LH, and waist/hip ratio. The QUICKI index and IGFBP-1 increased significantly. Serum sex hormone-binding globulin, androgens, leptin, IGFBP-3, and BMI remained unchanged. Twenty-two of 23 females had their menses restored, and three patients became pregnant. One patient was excluded because she became pregnant at the second month. CONCLUSION(S): Associated with the decrease in LH, rosiglitazone improved insulin-resistance parameters and normalized the menstrual cycle, which suggests that this drug could improve the endocrine-reproductive condition in insulin-resistant women with PCOS.

Association of reproductive factors, oral contraceptive use and selected lifestyle factors with the risk of ovarian borderline tumors: a Danish case-control study.

OBJECTIVE: The aim was to examine risk factors for ovarian borderline tumors overall, and according to histological subtype (serous vs. mucinous), in a large Danish population-based case-control study. METHODS: Ovarian borderline cases and controls were recruited from 1995 to 1999, and personal interviews were conducted. In all, 202 cases and 1,564 randomly selected controls were included. The analysis was performed using multiple logistic regression models. RESULTS: The risk of ovarian borderline disease decreased with increasing parity (OR=0.79 per birth, 95% CI: 0.63-0.98) and older age at first birth (OR=0.67 per 5 years, 95% CI: 0.53-0.84). Both a history of breastfeeding and use of oral contraceptives reduced the risk of borderline tumor, the effect being most pronounced for serous tumors. Increasing body mass index (BMI) was associated with elevated risk of serous borderline tumor (OR=1.05 per BMI unit; 95% CI: 1.00-1.10), whereas current smoking was a strong risk factor only for mucinous tumors (OR=2.10; 95% CI: 1.22-3.60). Finally, increasing consumption of milk (all types) was found to increase the risk of borderline disease (OR=1.04 per glass milk per week; 95% CI: 1.02-1.06), and increasing intake of total lactose also increased the risk significantly (OR=1.16 per 50 gram lactose per week; 95% CI: 1.06-1.26). CONCLUSION: The risk profile of ovarian borderline tumors is similar to that of ovarian carcinomas, and we observed significant etiological differences between serous and mucinous borderline tumors.

A prospective, randomized, controlled clinical study of a new subcutaneous, purified, urinary FSH preparation for controlled ovarian hyperstimulation in in vitro fertilization.

The aim of this study was to evaluate the clinical efficacy and safety of a new urinary follicle stimulating hormone (FSH) preparation (Fostimon) in patients undergoing in vitro fertilization-embryo transfer or intracytoplasmic sperm injection. Metrodin HP was adopted as a reference drug, as its purity and therapeutic efficacy are well known. Sixty normo-ovulatory patients aged 18-38 years with normal basal FSH and body mass index < 25 kg/m2 were selected for the study. After gonadotropin releasing hormone analogue pituitary desensitization, patients were randomized to receive either Fostimon or Metrodin HP at the initial dosage of 225 IU for 6 days. Thereafter, the dosage was tailored according to the ovarian response. Both drugs were administered by the subcutaneous route. The primary end-points were: number of follicles larger than 15 mm, levels of 17 beta-estradiol on the day of human chorionic gonadotropin (hCG) injection and number of oocytes recovered. The secondary end-points were: number of FSH ampules used, day of hCG injection and pregnancy rate. FSH kinetic curves were calculated during the treatment period with both products. Safety was evaluated by pre- and post-treatment blood biochemistry and hematology, and recording all side-effects. Local tolerance was investigated at each visit. None of the parameters evaluated showed a statistically significant difference between the two groups. Local tolerance was always recorded as good/excellent by both the patients and the physician. In conclusion, Fostimon proved to be an effective and safe drug for assisted reproductive cycles.

Ultrasonographical and hormonal description of the normal ovulatory menstrual cycle.

OBJECTIVE: To describe the changes in uterine volume, endometrial thickness and follicular size during the normal menstrual cycle by use of transvaginal ultrasonography and to correlate these changes with the endocrine events in the same cycle. METHODS: A group of 23 healthy women with normal body mass index and with a history of regular menstruations were monitored with repeated hormonal and vaginal ultrasonographical investigations during the menstrual cycle. RESULTS: Sixteen of the women fulfilled the hormonal criteria for ovulation. The mean length of the cycles was 28 days (range 25-30 days) with a mean length of the follicular and the luteal phases of 14.4 days (range 12-17 days) and 13.5 days (range 12-16 days), respectively. The estradiol level in serum the day before the peak of the luteinizing hormone varied between 490 and 1710 pmol/l (mean 1087 pmol/l). Ultrasonographically, the most clinically relevant changes were the increase in diameter of the dominant follicle and the growth of the endometrium. At ovulation the dominant follicle had a mean diameter of 21.4 mm (range 17.4-27.0), whereas the endometrium had a mean thickness of 12.8 mm (range 10.0-15.9). When analysed over the entire follicular phase, the serum estradiol concentration correlated both with the diameter of the dominant follicle (r = 0.93, p = 0.0001) and with the thickness of the endometrium (r = 0.79, p = 0.0001). However, no such correlations were found when only the last part of the follicular phase was analysed. In the luteal phase there was no correlation between the size of the corpus luteum and the serum concentration of progesterone. CONCLUSIONS: Vaginal ultrasonography is a practical and reliable method to monitor structural changes in the ovaries and the uterus during the menstrual cycle. The results are of clinical importance for a better understanding of the physiological changes and helpful when monitoring induction of ovulation in assisted reproduction.

Determinants of infant feeding method in relation to risk factors for breast cancer.

BACKGROUND: Breastfeeding is considered to be an important factor for maternal and children's health. However, the epidemiological findings related to the effect of breastfeeding on women's health, especially with respect to breast cancer development, are inconsistent. Determinants of infant feeding method may contribute to the inconsistency. METHODS: A total of 24,769 women aged 40-64 in Miyagi Prefecture, Japan, responded to a self-administered questionnaire survey in 1990. Using the data obtained from 22,085 parous women, we calculated odds ratios (ORs) for the choice of "breastfeeding only" during reproductive period. RESULTS: Late age at menarche (> or = 16 years, OR = 1.57) and high body mass index (BMI) at 20 years of age (> or = 24, OR = 1.31) were associated with the choice of breastfeeding only. Late age at birth of first child (> or = 28 years, OR = 0.29), history of breast cancer in mother (OR = 0.68), and high educational level (more than a high school education, OR = 0.53) reduced the possibility of choosing breastfeeding only. CONCLUSION: The results indicate that the choice of infant feeding method is associated with several breast cancer risk factors. Based on this finding, we should construct appropriate breast cancer risk models for parous women and investigate the changes in the effects of breastfeeding and other breast cancer risk factors among these risk models. Especially in a risk model controlling for breastfeeding, the effects of other breast cancer risk factors should be reevaluated. Through comparisons among different risk models, we may find the best-fitted risk model and identify the true effect of breastfeeding.

Preimplantation diagnosis after assisted reproduction techniques for genetically-determined male infertility.

One hundred and thirty-six cycles with a poor prognosis for full-term pregnancy underwent preimplantation genetic diagnosis (PGD) of aneuploidy. The mean maternal age was 31.8 +/- 2.5 years. Only patients younger than 36 years were included in the study with the aim of evaluating whether sperm indices have an effect on the chromosomal constitution of preimplantation embryos. No differences were detected in the percentage of aneuploid embryos; however a higher incidence of monosomies and trisomies was found in MESA-TESE embryos compared to the group of normospermic patients. In addition, an increase in the proportion of gonosomal aneuploidy seemed to be associated with the severity of the male factor parameters. The rate of de-novo chromosomal abnormalities in embryos from patients with a normal karyotype suggested an increased frequency proportional to the severity of the male factor condition, the proportion of monosomic and trisomic embryos, and the percentage of gonosomal aneuploidy increased accordingly. In the case of couples with a male altered karyotype, comparable frequency of chromosomally abnormal embryos, and monosomy and trisomy were observed irrespective of semen indices, gonosomal aneuploidy was only observed in one case where the patient had a karyotype with gonosomal mosaicism. These data confirm that the severe male infertility condition determines an increase in the rate of de-novo abnormalities, as anticipate by the follow-up of the children born after ICSI.

Tubal sterilization and hot flashes.

A cross-sectional study was conducted among women ages 40-60 years to assess the association between tubal sterilization and the occurrence of any, moderate/severe, or daily hot flashes. Although women with tubal sterilization were more likely to report hot flashes than were nonsterilized women, this association was largely due to differences in characteristics between the women, particularly body mass.

Endometriosis: medical therapy.

The management of women with endometriosis is complex and necessitates individualization of patient care. The most commonly used medical therapies are danazol, GnRH agonists, medroxyprogesterone acetate and gestrinone. Studies to date have shown these drugs to have equal efficacy in terms of reduction in laparoscopic score and relief of symptoms. However, their side-effects make them unsuitable for long-term use. The addition of low dose hormone replacement therapy to GnRH agonist regimens may allow prolonged use but the current cost of these agents is high. Low dose oral contraceptive pills deserve further investigation. The role of medical treatment for women with endometriosis and infertility is controversial. There is no place for hormonal therapy in such women with stage I or II disease. When expectant management fails, gamete intrafallopian transfer offers excellent results. For those with stage III or IV disease, surgery is preferable with adjunctive medical therapy in selected cases. If pregnancy does not ensue, in vitro fertilization and embryo transfer are the next line of management, and results are optimized by prior medical therapy and aspiration of endometriomas. Major advances have been made in the medical management of endometriosis. However, current treatment strategies are ineffective in eliminating the disease in most women. New approaches are required in both basic and clinical research in order to finally eradicate this often devastating disease.

Are health care providers who work with cancer drugs at an increased risk for toxic events? A systematic review and meta-analysis of the literature.

OBJECTIVE: A systematic review and meta-analysis was conducted to test the hypothesis that oncology health care workers are at an increased risk of cancer, reproductive complications and acute toxic events. DESIGN: A structured literature search of Index Medicus/ MEDLINE, CINAHL, EMBASE, the Cochrane Database of Systematic Reviews and Healthstar was performed from 1966 to December 2004 for human epidemiological studies evaluating the risk of toxic events in health care workers exposed to cytotoxic drugs. Raw data and adjusted odds ratios (OR) reported in eligible studies were combined using a random effects model to calculate point estimates and 95% confidence intervals (CI) for each potential risk outcome. MAIN OUTCOME MEASURES: Adjusted OR for congenital malformations, stillbirths and spontaneous abortions among health care workers exposure to cytotoxic agents compared to a nonexposed control group. RESULTS: The systematic review identified 14 studies evaluating the outcomes of interest, seven of which were suitable for statistical pooling. Due to lack of evidence, we were unable to estimate a pooled OR for the risk of cancer and acute toxic events. However, no significant association was detected between exposure to cytotoxic drugs and; congenital malformations (OR = 1.64; 95% CI: 0.91-2.94) and stillbirths (OR = 1.16; 95% CI: 0.73-1.82). In contrast, an association was identified between exposure to chemotherapy and spontaneous abortions (OR = 1.46; 95% CI: 1.11-1.92). CONCLUSIONS: The results of this systematic review identified a small incremental risk for spontaneous abortions in female staff working with cytotoxic agents. Health policy decision makers should effectively communicate the magnitude of this risk to their staff and implement cost effective interventions for its reduction or elimination.

A decade of Nepal's family planning program: achievements and prospects.

Knowledge and practice of family planning in Nepal increased considerably from mid-1976 through mid-1986, with significant improvements in the availability of and accessibility to contraceptives. However, a proportionately larger share of the achievements were made during 1976-81 than in 1981-86. The impact on fertility of contraceptive use during the decade was minimal. Knowledge and use of reversible contraceptive methods remained very low, primarily because of the continued strong emphasis on sterilization in Nepal's family planning program. Consequently, the welfare of many couples who may be in need of contraception for spacing births, but not necessarily for terminating childbearing, does not appear to be adequately addressed by the program. A more balanced emphasis on reversible methods of contraception does not necessarily imply that a substantially higher level of contraceptive prevalence would be required to reach the fertility target. Several steps to improve program performance and contraceptive method mix are suggested.
