Kluyveromyces fragilis as an opportunistic fungal pathogen in man.

An immunosuppressed cardiac transplant patient with pulmonary infection due to the yeast Kluyveromyces fragilis is described. Isolation of this fungus from human sources is rarely reported, and previous reports of human infection are unavailable in the modern literature. The organism is poorly pathogenic even in immunocompromised hosts. In vitro susceptibility studies with several strains presented here indicate inhibition by 5-fluorocytosine and miconazole, and borderline susceptibility to amphotericin B. The patient was treated with amphotericin B, and recovered without sequelae.

[Suggestions for therapy with glucocorticosteroids in obstructive airways diseases (author's transl)].

Glucocorticosteroids are an integrating component of complex therapy in severe obstructive airways diseases. The well known risks of this treatment may be minimized by observing the following rules: 1. Rigorous indication for long term therapy 2. Careful attention to certain concomitant diseases and contraindications 3. Control of therapy by continuous specialized supervision in a dispensaire. On the basis of modern literature and personal experience with glucocorticosteroid therapy in 1210 patients with bronchial asthma and chronic obstructive bronchitis during the years 1972 to 1977 (34.9% of all patients in this group of diagnoses) are given some recommendations for practical handling of glucocorticosteroid therapy in obstructive airways diseases.

[Syphilitic aneurysms of the abdominal aorta. Considerations on 2 cases].

Abdominal aortic aneurysm have been considered in the modern literature only arteriosclerotic, apart from any possibility of superimposed bacterial infections. In 2 cases, after the hystological study on the aneurysmatic wall the Authors have found typical syphilitic lesions. both didn't have positive historical data and in one only serological tests were positive. This observation calls the attention on the importance of such an insidious pathology which is still present in spite of the decrease of the luetic epidemiology. It happens mainly in patients ignoring to be carriers of a remote infection or in those inadequately treated for it. Even it anamnestical and serological data are negative not only in presence of a thoracic aortic aneurysm but also in presence of an abdominal one, physicians must take syphilis into account in their diagnosis. The mistake comes often from an associated arteriosclerotic process present in elderly patients developing the aortitis a long time after infection. Therefore it is necessary to perform a careful hystological test of the aneurysmatic wall even if macroscopical examination suggests only arteriosclerotic lesions. An hystological evidence of syphilitic aortitis advises a long-term treatment aimed to avoid other localizations.

[Septicemia. Generalities on its diagnosis].

Considerations are made regarding the etiopathogenesis of septicemia and related factors in the patients and in the environment that favor the generalization of infections. Several clinical aspects, together with the frequency and distribution of infectious focuses in 128 patients with septicemia are evaluated. Considerations are made on clinical and laboratory findings that led to the diagnosis of septicemia and the current state of the investigation at our Hospital de Pediatría del Centro Médico Nacional, IMSS is discussed. A system of points is proposed giving values to the clinical and laboratory findings in order to establish the diagnosis of septicemia. The modern literature on the subject is reviewed.

[Evaluation of the MPEL for SO2 and NO2 and the development of new MPELs for atmospheric pollutants based on modern epidemiological, ecological and toxicological studies].

Proceeding from modern experimental and natural observations on the effect of sulphur dioxide and nitrogen dioxide on live organism and environment, the now, operative in Bulgaria maximum allowable concentrations of these contaminants are reassessed. The maximal single and mean diurnal SO2 MAC are proposed to be reduced to 0.15 mg/m3 and 0.03 mg/m3, while those of NO2 remain the same. Based on data available in modern literature and equations proposed by the authors for express determination of MAC for some atmosphere contaminants, MAC for some nonstandardized contaminants in Bulgaria are suggested: ozone (maximal single - 0.06 mg/m3 and mean diurnal - 0.03 mg/m3 asbestos (mean dirnal - 0.25 fibers/cm3), zinc (mean diurnal - 0.05 mg/m3, copper (mean diurnal - 0.01 mg/m3. The use of mathematical equations for determination of MAC of differrent hydrocarbons is suggested.

[Microtubules in the nerve cells: morphological and functional aspects].

The modern literature concerning ultrastructure and cytochemistry of microtubules in the nervous tissue is reviewed. Common features of cytological and biochemical organization of microtubules in different parts of the nervous system of the vertebrates and invertebrates are analysed: the similarity of ultrastructure of microtubules and their molecular organization (tubulin and its alpha- and beta-monomeres), the ability of microtubules to assemble and disassemble, to bind specifically with poisons--colchicine and vinblastine, participation of microtubules in the neuroplastic transport. The authors' data on space arrangement of microtubules within cytoplasm of the neuronal processes (dendrites and unmyelinated axons in the central and peripheral nevous system) are presented. Some literature and personal results concerning ultrastructure of neurofilaments and microtubules in the myelinated nerve fibres are also considered. The functional significance of microtubules in the nervous system is discussed with special reference to facts and hypotheses on a possible role of microtubules in the propagation of nerve impulse.

[Vitamins with antioxidant action and rheumatic diseases (a review)].

The review of the modern literature about a role of essential dietary antioxidants and first of all of vitamins E, C, P, A and carotenoids in protection of cell components against of damage by excessive strengthening of free radical oxidation is submitted. Expediency of inclusion of antioxidant vitamins in complex therapy of rheumatic diseases is discussed taking into account the failure in an antioxidant system and excessive strengthening of free radical oxidation as important symptoms in development of rheumatic diseases.

[Leiomyomatosis of the esophagus].

A rare case of dysphagia caused by leiomyomatosis of the esophagus is presented. Esophageal leiomyomatosis is triggered by diffuse proliferation of smooth muscle cells in the wall of the entire esophagus. Only 1.0% of all tumors of the esophagus are benign lesions; 0.4% of all esophageal neoplasms are leiomyomas. Only a few cases of leiomyomatosis-a special form of diffuse leiomyomas-have been described so far in the literature. Modern diagnostic imaging techniques such as CT and MR are described in esophageal leiomyomatosis.

[Thrombophlebitis of the lateral sinus. Apropos of a case].

About one case of septic sigmoid sinus thrombosis, the authors present various clinical and paraclinical elements of this rare pathology, whose entity are generally subtle for benign but also for serious forms. Although uncomplicated dural venous thrombosis carries an excellent prognosis with proper treatment, delay in diagnosis and institution of appropriate therapy may lead to serious, or even fatal, consequences. The modern literature is reviewed and the pathology, presentation and management of lateral sinus pathology is discussed.

[The clearance function of the bronchial epithelium].

Modern literature is reviewed in terms of different views on cytological basis of clearing function of the bronchial epithelium, on the whole, and of its separate parts. Different factors influencing the mucociliary clearance are described. The necessity to elaborate some better, more objective methods of clearance disturbances diagnosis is emphasized. Principal attention is paid to regenerative-restorative processes, which normally occur in the bronchial epithelium, and to expediency of influencing them.

Steroid cell tumor of the broad ligament arising in an accessory ovary.

OBJECTIVE: To determine the histogenesis of an unusual steroid cell tumor that occurred in the broad ligament. PATIENT AND METHODS: The tumor occurred in a 29-year-old woman who had evidence of virilization. The preoperative testosterone level was greater than 700 ng/dL, but it returned to normal after surgery. She was living and well without evidence of recurrent tumor 1 year after operation. The tumor was examined using light and electron microscopy and immunohistochemical stains. RESULTS: The tumor was composed of polygonal cells with prominent eosinophilic cytoplasm. No significant nuclear atypia or mitotic activity was identified; however, the tumor was large, and necrosis and hemorrhage were identified on gross and microscopic examination. A graafian follicle and a primordial follicle were present at the periphery of the tumor within the connective tissue stroma in one section, providing evidence for the presence of an accessory ovary. A separate normal ovary was present on the same side. The tumor mass was connected to the surface of the eutopic ovary by thin membranous tissue. CONCLUSION: To our knowledge, we report the first case in the modern literature of an extraovarian steroid cell tumor arising in the broad ligament, and we present evidence supporting its origin from an accessory ovary.

[The outcome of delivery in the breech presentation at the Stara Zagora Obstetrics and Gynecology Clinic during 1990-1992].

The outcome from the deliveries in breech presentation deserves close attention, because they are although 4% only from the total number of deliveries, but they exert important influence on the frequency of the operative deliveries by caesarean section and the level of the perinatal mortality. In the modern literature, the breech presentation is recorded as the one of the main causes, which leads to considerable increase of the caesarean sections frequency.

[The most common gastrointestinal problems in pediatric practice].

In this review the Author expose the most common gastroenterological problems in pediatric practice. The following illnesses are examined: infantile colics, recurrent abdominal pain, gastroesophageal reflux, vomiting, alimentary intolerances, coeliac disease, malabsorption syndromes, hepatic pathologies, acute diarrhoea, persistent postenteric diarrhoea, chronic constipation. For all problems are provided the actual indications of diagnosis and therapy on the basis of modern literature suggestions.

[Single coronary artery and acute myocardial infarction].

We describe the rare angiographic finding of the right coronary artery arising from the middle portion of the left anterior descending coronary artery in a patient with atherosclerotic coronary artery disease and acute myocardial infarction. This anatomic variation has been previously described only four times in the modern literature.

[Lipoma arborescens of the knee: contribution of x-ray computed tomography. Apropos of a new case].

The authors report a case of a synovial lipoma of the knee in a 40 years old man who suffered from chronic instability of the right knee associated to an extensive joint swelling and recurrent hydarthrosis. Ultrasonography and arthrography showed a synovial process and the computerized tomography showed an inhomogeneous low density due to its fat content suggesting a synovial lipoma arborescens of the knee. Final diagnosis was confirmed by histological examination of the arthroscopic biopsy. The authors discuss, through out their case and a review of the literature, modern investigation findings and indication for surgical treatment of this disease.

[Ectodermal dysplasia: multiple manifestations of a hereditary disease].

Ectodermal dysplasia is a hereditary disease that can manifest itself in many ways. Four cases, including two cases involving heterozygote twins, are presented. The clinical signs of the disease, as observed are compared to the knowledge offered in the classic and modern literature. Special attention is given to the intraoral manifestations of the disease, and to the solutions that a multidisciplinary dental team could consider to improve the masticatory function and the facial esthetics of patients affected by this disease.

[Normal-tension glaucoma. Its diagnosis and treatment].

The paper attempt a NPG definition in the light of the most recently data from literature. Modern investigation techniques (CDI, LDF, laser scan angiography) are summarized to support the presently most accepted pathogenic theory--the vascular theory. There are also reviewed the main clinical conditions to do differential diagnosis and main methods of its treatment.

[Gastroesophageal reflux disease refractory to medical treatment. Which approach: the pill or the scalpel?].

We analyze the modern literature regarding natural history, clinical picture and treatment of gastroesophageal reflux disease. The characteristics of a small group of patients with tendency to relapse are emphasized, analyzing the possibilities offered by pharmacological as well as surgical treatment. Finally, we analyse the current literature of our country situation.
