[Evaluation of a monitoring system for assessing animal health in dairy cattle].

A monitoring system to assess animal health in dairy farms was developed and applied on 44 farms in an epidemiological study. The objective of the study was to evaluate the applicability of the system for long term monitoring of health data. For this purpose, the quality of the collected data and the acceptance of the system by farmers and veterinarians were investigated. Farmers recorded all health events using a form. Collected data was entered into a central database on a monthly basis. The recording of health data was easy to do and did not take a lot of time. Most of the farmers (89%) used less than 20 minutes per week for the recording. This time need was independent of the herd size. 66% of the farmers found it useful to record health data, especially for improving their overview on health of each single cow. Treatments were reported in 81% of the cases, inseminations and calving in 80% of the cases. All events were reported correctly and precise.

Analytical performance of the new coagulation monitoring system INRatio for the determination of INR compared with the coagulation monitor Coaguchek S and an established laboratory method.

An evaluation of the INRatio Prothrombin Time Monitoring system for determination of INR was done in two centers with a total of 5 healthy subjects and 77 subjects on oral anticoagulation. The INRatio and the Coaguchek S were compared with an established laboratory method. The correlation coefficient of the comparison with the laboratory was r=0.954 for INRatio and r=0.937 for Coaguchek S. The mean relative deviation from the lab method calculated according to Hill was 6.87% for INRatio, which is rated "very goo", and 9.72% for Coaguchek S ("goo"). The imprecision in the normal range (INR=1.1) showed a coefficient of variation (CV) of 7.8% and a standard deviation (SD) of 0.09. In the therapeutic range (INR 3.9) the CV was 5.4%, the SD 0.21 and above therapeutic range (INR 5.3), the CV was 8.4% (SD 0.44), rated satisfactory. The concordances of the compared methods with the routine were 81% for INRatio and 79% for Coaguchek S, which is considered state-of-the-art. Most of the patients' perceptions of the INRatio were very positive. In the hands of professionals the INRatio demonstrated very good accuracy and precision and an excellent technical reliability. Further studies using INRatio for self testing by patients are warranted.

Experience with a medical waste portal monitoring system.

A portal monitor was installed to detect radioactivity in trash and laundry leaving the hospital facility. Experiences with selection, installation, calibration and detection capabilities are reviewed. Also reviewed are the topics used in training hospital employees on proper use of the monitor and response detected activity in trash or laundry.

Mead Johnson Critical Care Symposium for the Practising Surgeon. 2. Complications of monitoring systems.

Current invasive monitoring techniques, although valuable in the care of critically ill patients, also have disadvantages. Complications of these techniques include errors in interpretation of measurements and complications of venous access and indwelling lines. To prevent these, the author recommends a standard routine for inserting and changing catheters, regular calibration of transducers and insertion of Swan-Ganz catheters with the balloon inflated with 1.0 to 1.5 ml of air.

[Acceptance of a psychometric routine diagnostics and quality monitoring system among psychotherapists in inpatient psychosomatic rehabilitation].

OBJECTIVE: Psychotherapists are considered to be sceptical of empirical measures of quality assurance. It is generally recognised that the success of these measures mainly depends on how they are accepted by the people involved. METHODS: The acceptance of a quality monitoring system among psychotherapists was investigated with a standardised assessment instrument. RESULTS: Practical relevance and practicability of the system were judged positively on the following dimensions: "support of clinical diagnostics", "examination of the outcome", "reflecting on the therapeutic process", "documentation", "integration in the therapeutic process", "pressure to succeed and control" and "work loading". The factors "professional experience", "profession" and "sex" had just a small influence on the degree of acceptance of the quality monitoring system. CONCLUSIONS: Psychotherapists consider a practically relevant and economical quality monitoring system as useful.

Estimation of the costs of mastitis, using National Animal Health Monitoring System and milk somatic cell count data.

Data collected by the National Animal Health Monitoring System in Ohio for a 12-month period during 1986 and 1987 were used to determine the relative magnitude of costs associated with mastitis in the following categories: milk production loss, veterinary services, drugs, producer labor, and "other" factors. The cost of milk loss associated with mastitis that was reported by producers cooperating in the National Animal Health Monitoring System program was compared with estimates based on bulk tank somatic cell counts and individual cow milk somatic cell counts. Using producer-reported estimates, milk loss accounted for about one third of the total cost associated with mastitis. When estimates of milk loss were replaced by estimates based on bulk tank somatic cell counts, milk loss accounted for over 80% of the total cost of mastitis. Estimates of the cost of milk loss based on studies relating milk yield to somatic cell counts differed considerably. Consequently, it was unclear how to best estimate the relative magnitude of the milk loss component of mastitis costs.

Pediatric neuroanesthesia computerized monitoring system.

The authors describe the development of a computerized on-line monitoring system and its application during major pediatric neurosurgical procedures. The system acquires from monitor by serial communication the values of heart rate, systemic pressures, intracranial and central venous pressures, peripheral and central body temperatures and stores all these data on hard-disk. The system allows the collecting of clinical information, and other data concerning the metabolic and hemodynamic perioperative status of the patient. It also calculates in real time many derived parameters and stores all the measured data, the therapeutical administrations and their temporal relationship with the various surgical procedures on disk for statistical evaluation.

Methodological approach for an integrated environmental monitoring system relative to heavy metals from an incineration plant.

The use of an Integrated Environmental Monitoring System is an innovative and very important approach for the determination of environmental impacts due to a contamination source. In the present work, the methodological approach is described and applied to the case study of a MSW incineration plant. Heavy metals were chosen as Environmental Indicators. Gaseous emissions were measured and correlated to wet and dry depositions, soil and vegetation samples. Results show a good correlation between stack emissions and atmospheric depositions; less with soil and vegetation, but these results are important in order to design a standard procedure for an Integrated Monitoring System.

In vivo thyroid 131I monitoring system using an imaging plate.

The counting efficiency and detection limit of an in vivo thyroid 131I monitoring system using an imaging plate (IP) were estimated using an anthropomorphic thyroid neck phantom. The counting efficiency of the IP system was approximately constant for thyroid volumes between 11.7 and 20 ml and neck diameters between 10 and 14 cm. The detection limits were distributed from 288 to 451 Bq depending on the combination of neck diameter, thyroid volume and tissue thickness. The IP monitoring system gives a reliable counting efficiency notwithstanding the variation of thyroid volume and neck diameter. The IP system is a new option for thyroid 131I monitoring.

Diabetes monitoring system in The Netherlands.

Under the aegis of the European Regional Offices of the World Health Organization (WHO) and the International Diabetes Federation (IDF) a joint action programme to contest the growing burden of diabetes has been drawn up. The St. Vincent Declaration (1980) specifies targets for diabetes care, the reduction of complications, the integration into society and the education of the diabetic individual. The WHO/IDF have drawn up a data set to monitor progress in reaching the targets of the St. Vincent Declaration. The objective of the project Diabetes Monitoring System is to examine the possibilities for implementing a longitudinal-based and standardised diabetes monitoring system in the Netherlands. Although computerised recording systems for diabetes are seldom used and the agreement with the WHO/IDF data set varies substantially, the willingness of physicians to participate is clearly sufficient. This principally explorative project will hopefully lead to the implementation of a monitoring system in which a widely accepted data set is recorded to establish improvements in quality of care and to perform epidemiological research.

Water monitoring system for oil contamination using polymer-coated quartz crystal microbalance chemical sensor.

A water monitoring system with a new chemical sensor for oil contamination was developed. The sensor had an organic polymer film on a quartz crystal microbalance (QCM). The organic film was a hydrocarbon polymer and had high affinities for the organic compounds of petroleum products such as gasoline, kerosene, diesel oil and fuel oil. The monitoring system as composed of a sampling part, a purging part, a humidity control part and a sensor part. The oil in contaminated river water could be detected whose threshold odour number (TON) was less than three. The detecting time was less than 5 min depending on the oil kindness. This system was tested using artificially contaminated river water with the oils, to be found that the sensitivity was kept steady for longer than 6 months with 400 detections of diesel oil and heavy oil. Moreover, the oil kind could be discriminated with only one sensor device by analyzing the desorption response curves obtained by flowing a clean air on the sensor instead of the purging air.

Review of the Australian incident monitoring system.

BACKGROUND: A survey was conducted to assess the benefits and limitations of the Australian Incident Monitoring System (AIMS) as a programme to improve patient safety. METHODS: A 12-point questionnaire was sent to 12 current users of AIMS in November 2002. RESULTS: The AIMS provides a consistent system of coding, trending and monitoring of incident data. It promotes a patient safety culture and an awareness of system error. Other benefits include the building of teamwork and the implementation of strategies to reduce the prevalence and severity of incidents. The majority of respondents (83%) reported that AIMS investigations resulted in significant changes to equipment usage, medication prescribing or administration, clinical protocols, training programmes and falls risk assessment tools. Although 75% of users reported improvements in patient outcomes, these were difficult to measure. A major limitation of AIMS was the low rate of incident reporting by medical staff. Voluntary reporting systems did not capture all incident data and the information was often too generic for root cause analysis. There were difficulties benchmarking data and concerns were raised regarding the ownership of information. The programme requires ongoing resources to implement change strategies and to maintain incident reporting levels. On a scale of 1 (poor rating) to 10 (excellent rating) the mean benefit rating was 7.6. CONCLUSION: The Australian Incident Monitoring System is beneficial as a component of a clinical risk management strategy. Usefulness could be improved by increased participation by medical staff. The level of resources required should not be underestimated if the programme is to demonstrate improvements to patient outcomes. More recent versions of AIMS promise improved capabilities and will require similar evaluation.

Computerized automatic non-invasive blood pressure monitoring system.

A non-invasive, fully automatic blood pressure monitoring system was developed to make the diagnosis of hypertension more accurate and to help individualizing antihypertensive therapy. The system consists of two subunits: the automatic microprocessor-controlled blood pressure monitor (Nippon Colin Co. BP 203) and a microcomputer system (Commodore 64). The blood pressure monitor is fitted to the computer by our own interface and data transfer programme. Data-analysing, graphic displaying and plotting programmes were also developed by us. Circadian average blood pressure, its variability, the rhythm variabilities of chronobiological cosinor analysis: mesor, amplitude, acrophase and linear trend, have been calculated by means of these programmes. Results of blood pressure monitoring are demonstrated in a patient with essential hypertension and in a patient with renoparenchymal hypertension. The perspectives of this method in biomedical research and in clinical pharmacology are illustrated and discussed.

Costs of veterinary services and vaccines/drugs used for prevention and treatment of diseases in 86 Colorado cow-calf operations participating in the National Animal Health Monitoring System (1986-1988).

Eighty-six cow-calf operations involved in the Colorado National Animal Health Monitoring System were monitored for a 12-month period, and data were collected on the incidence, prevention, and costs of disease. The costs of veterinary services and vaccines/drugs used in the treatment and prevention of disease conditions in these beef herds were determined and expressed on a per cow basis. Beef producers in this study spent an average of $2.04 ($0 to $29.88) per cow annually on veterinary services for treatment of disease conditions. The cost of veterinary services was a relatively small percentage (5.4%) of the total mean cost of disease incidence. The reproductive tract disease class was the most costly class in terms of veterinary services for disease treatment ($0.99/cow). Dystocia was the disease condition with the largest veterinary treatment cost. The total mean annual cost of drugs used in the treatment of disease conditions was $1.22/cow. The enteric, miscellaneous, and respiratory tract disease classes had similar mean drug costs for disease treatment and ranged from $0.31 to $0.39/cow. The total mean annual cost of veterinary services for administration of preventive measures in these herds was $1.85/cow ($0 to $12.03). Pregnancy examination, breeding soundness examination in bulls, brucellosis vaccination, pulmonary arterial pressure test, and campylobacteriosis vaccination accounted for over 90% of the money spent for preventive veterinary services. Approximately 60% of the total mean annual disease prevention cost was attributed to the purchase of vaccines/drugs ($6.59/cow).(ABSTRACT TRUNCATED AT 250 WORDS)

Effectiveness of a prospective physician self-audit transfusion-monitoring system.

BACKGROUND: The purpose of this study was to search for a more effective transfusion-monitoring system than the existing system of retrospective peer review. STUDY DESIGN AND METHODS: This research used a study-control, preintervention and postintervention design, to evaluate the effectiveness of a prospective physician self-audit transfusion-monitoring system that functioned without the direct involvement of transfusion service physicians. This research also evaluated the effectiveness of issuing to physicians a memo with transfusion guidelines. Three process indicators were used to assess physician behavior at various stages of the blood-ordering process: 1) the number of crossmatches ordered per admission, 2) the transfusion-to-crossmatch ratio, and 3) the number of blood units returned to the laboratory after physician self-auditing. The study used two outcome indicators to reflect overall blood utilization: 1) the percentage of patients who received red cell transfusions and 2) the number of blood units transfused per recipient each month. RESULTS: The prospective physician self-audit system implemented at the study hospital did not reverse physician transfusion decisions, and the process of issuing to physicians a memo with transfusion guidelines at the control hospital failed to reduce blood usage. However, a transient reduction in blood utilization was observed at the study hospital. CONCLUSION: The reduction was hypothesized to be due to a Hawthorne effect, in which observed behavior is affected by the subject's awareness of the research study.

[Experiences with a mobile monitoring system for fetal and neonatal cardiotachometry].

A monitoring system for fetal and neonatal cardiotachometry which set up in own instrumentation at the Department of obstetrics of the Regional Hospital of Rostock. With aid of a wireless telemetric system is it possible to estimate the fetal condition before and during of caesarean section as well as at transfer of high risk neonates.

Fully automated health monitoring system in the home.

The aim of this study was to provide health monitoring for elderly and disabled persons at home by using fully automated signal measurement with personal identification to support daily health care and improve quality of life. We attempted to develop a home health monitoring system that did not provide any restrictions during sleep, bathing or elimination. A bedroom, bathroom, and separate toilet were built. The system consisted of monitoring devices and a computer terminal for collecting data. Data were automatically collected from monitoring devices placed in the bed, bath, and toilet and were transferred to a data terminal in the bedroom and stored for further analysis. The system was tested by a young healthy subject who stayed overnight in the rooms. Data were obtained without any trouble. The data acquisition system is now being used in experimental rooms in our laboratory and in a house located in Takaoka City, Toyama Pref. Japan. Data are being extensively collected.
