Supporting tools for guideline development and dissemination.

This paper describes a methodology for representing clinical practice guidelines and facilitating their introduction into the medical routine. Since this methodology can be exploited in a www environment, it can represent the basis for sharing clinical guidelines both between different institutions and between human and software agents cooperating within a clinical context. In addition, the proposed guideline formalization is intended to deal with patient and organization preferences. This goal is achieved by augmenting the guideline with decision analytic models and by linking the guideline with an organizational model of the clinical setting. The designed framework allows guideline development, tailoring and implementation, real-time access to the guideline prescriptions and guideline validation.

Descriptions of the practice and organizational models demonstrate how each was tailored to the specific community. Using Andersen's behavioral model of families' use of health services and objectives for the year 2000, the impact on access to care and population outcomes are evaluated.

However providing a minimum of equipment and pediatrician and qualified paramedic personnel all the time. We noticed a statistically higher frequency of breastfeeding even after a time lapse (table 3), and a marked decrease of transfers to the neonatal pathology whit, especially among those neonates with minor pathology (tables 5 and 6). The fulfiment a direct connection with the family pediatrician and his services, through a complete and personalized exchange of informations, and a good degree of cooperation with the obstetrician, supported by the results discussed and evaluated together before (or him).

Nurses in corporate America: embracing power through influence.

Executive positions in corporate America offer nurse leaders the opportunity to influence product development and services delivered to ensure that the best possible solutions are provided to health care organizations, providers, and patients. This opportunity to "make a difference" is a critical component for nurses' attraction to migrating to the business side of the health care industry. However, making the transition from leadership positions in health care delivery organizations to corporate businesses carries big challenges. A major demand is for nurse leaders to adjust from direct span of control organizational models to matrix management structures used in complex business organizations.

Organizing for communication and integration.

Using the departmental philosophy as its foundation, an integrated organizational model was designed for a nursing department. This patient-centered model was implemented to streamline operations, focus resources, and provide a professional practice environment for nursing staff. Health team function: testing a method for improvement.

Health-care teams are functioning in the delivery of primary care. For purposes of this paper, the health-care team is composed of a primary care physician or physicians working with other health-care providers to deliver primary care. This definition represents an organizational model of health-care teams. Organizational specialists have proposed "team development" processes for improving health team function. This paper reports the results of an initial experiment designed to analyze the results of such a process. A modification of the "health-team development" process was used with one of three comparable teams at a family practice residency. Pre and post-team development data were taken. The team that experienced the modified team development process showed significant differences in the gain scores, compared to the "control" teams. Discussion focuses on the role of organizational technology in health-care teams and avenues of further analysis are presented. The modified format for the team development process is also reported and discussed.

Creating a healing environment for critical care.

Creating a healing environment for critical care requires an organizational as well as personal commitment. An organizational model for health and healing integrates five key elements: common value for the mind-body-spirit approach to wellness and health; patient/family centered relationships; an organizational culture that promotes personal growth; the availability of CAT as well as conventional therapy; and a physical environment that supports healing. CAT strategies to support a physical environment for spirituality and healing were outlined, followed by a discussion of strategies for building capacity for EI to alleviate the emotional labor of nursing and facilitate family centered care.

A critical analysis of recent Canadian health policy: models for community-based services.

As part of the thrust by Health and Welfare, Canada, to strengthen community health services, the National Health Research and Development Program commissioned a series of literature reviews. I undertook to review organizational models for community-based services, but said that this would be done in the context of the developing organization of the national health insurance program. With the help of colleagues I examined the literature from a number of different viewpoints. This article will present our findings on the development of policy and will trace the difficulties in making a shift towards providing more care in the community. We found that organization theories were helpful for explaining developments in the health service as a whole and the place of community-based services within it. We were able to use these theories to analyse efforts at restructuring. They provide explanations for the concurrent existence of policies of rationalization and cutbacks with policies of expansion in the area of health promotion.

Six areas of worklife: a model of the organizational context of burnout.

Burnout research has considered a wide range of organizational correlates of burnout. It is argued that integrating these various organizational qualities into a comprehensive model of organizational environments will further research on burnout. The primary themes in burnout research fit readily into six areas of worklife; workload, control, reward, community, fairness, and values. These areas are sufficiently broad to encompass the rich variety of research approaches taken in the field while being sufficiently precise to permit clear distinctions among them. The review identifies a consistent body of research on the relationships of these six areas with burnout. The review concludes by considering issues for a research agenda on the organizational antecedents of burnout and noting the implications of a focus on mismatches for interventions.
