Cytohistologic diagnosis of neoplasms of the liver by ultrasonically guided fine-needle aspiration biopsy.

A procedure was developed for obtaining both cytologic and histologic specimens from patients suspected of having hepatic malignancy at the time of ultrasonically guided liver aspiration biopsy using a 22-gauge needle of 0.4 mm inner diameter. The fine needle and the syringe used were heparinized before use to avoid blood coagulation in the materials obtained. With heparinized needle and syringe it was possible to centrifuge the materials obtained and make good smears containing abundant tumor cells, and to obtain small tissue specimens. Histologic specimens obtained with the heparinized needle were fixed in 2.5% glutaraldehyde, postfixed in 1.0% osmium tetroxide, and embedded in epoxy Epon resin. Ultrathin sections were made and usually stained with Azan-Mallory stain. In 41 patients suspected of having hepatic malignancy, a correct diagnosis was made by a combination of these examinations. The sensitivity of this procedure for hepatic malignancy was 100%, and the specificity was 95.7%. Therefore, this procedure is a very reliable method for diagnosis of hepatic malignancy.

The yield of flexible sigmoidoscopy and double-contrast barium enema in the diagnosis of neoplasms in the large bowel in patients with a positive Hemoccult test.

BACKGROUND AND STUDY AIMS: In an ongoing screening study of 68,306 patients for early detection of colorectal cancer, those with a positive Hemoccult II test (H2) were examined with a flexible sigmoidoscope (60 cm) (FS) and double-contrast barium enema (DCE). The aim of this study was to compare the results of FS and DCE examinations in the rectum and the sigmoid colon, and to evaluate the benefit of DCEs. PATIENTS AND METHODS: 1831 FS and DCE examinations were performed on the basis of positive H2 tests (90% acceptance rate for positive Hemoccult tests). The radiologists were unaware of the endoscopic findings. One hundred and thirty-five patients underwent total colonoscopy due to abnormalities found on FS or DCE, or both. All patients were followed-up through various Swedish cancer registers (range: 50-145 months). RESULTS: The sigmoidoscope was fully introduced in 71% of cases, and the mean time for the examination was 5 minutes. The combination of FS and DCE detected 235 adenomas of 1 cm and more, and 81 cancers which were in Dukes stages A (n = 29), B (n = 22), and C (n = 23). Twenty-one cancers (26%) and 24 adenomas (10%) identified on DCE were above the rectosigmoid area. The rate of overlooked adenomas (> or = 1.0 cm) and cancers was 22 and five for FS in the rectosigmoid area, and 55 and 15, respectively, for DCE. Adenomas found in the rectosigmoid area were only rarely associated with more proximal adenomas of 1 cm and more (1%). CONCLUSIONS: This approach-screening by FS and DCE, and selective use of colonoscopy in case of abnormalities-leads to the diagnosis of significant colonic tumors (larger adenomas and cancers) in 17.7% of cases; only two of eighty-three cancers (2.4%) were overlooked with this method.

[Lung neoplasms. Cytological diagnosis based on puncture biopsy specimens].

Twelve-year experience gained in cytological diagnosis of lung tumors by examining puncture biopsy specimens in 2536 patients is analyzed. The results indicate that before cytological verification of the diagnosis, patients with squamous-cell and glandular lung cancers were examined at various institutions for up to 6 months and patients with nondifferentiated cancer for up to 3 months, the majority (57%) patients being of socially active age (45-60 years). In 1793 (70.7%) patients the cytological method was the only possibility of morphological interpretation of the pathological process in the lung before treatment. Analysis of informative puncture biopsy specimens helped identify the process in 2238 (92.4%) patients. Cyto-histological correlations showed high diagnostic sensitivity (98.7%), efficiency (97.3%), and specificity (86.0%) of examining puncture biopsy specimens by the cytological method.

[The puncture cytological diagnosis of neoplasms of the mediastinum (11 years of experience)].

Routinely stained cytological preparations of puncture biopsy specimens of mediastinal tumors from 870 patients are examined. Diagnostically informative material is investigated in 759 (87.3%) cases; malignant tumors are diagnosed in 538 (62%) cases, benign in 221 (25.2%). In 59 (6.8%) cases the cytological answers were descriptive, and in 52 (6%) puncture biopsy material was not informative for cytological verification. Diagnostic sensitivity of the cytological method is 94%, diagnostic specificity 71%, and diagnostic efficacy 91%.
