Protective effects of a new LTD4 antagonist (LY-171883) in traumatic shock.

We studied the effects of a selective antagonist of LTD4 (LY-171883, 2 and 4 mg/kg) in traumatic shock. Anesthetized rats subjected to Noble-Collip drum trauma developed a shock state characterized by a survival time of 1.7 +/- 0.3 h, a sixfold increase in plasma cathepsin D activity, and a fourfold increase in plasma myocardial depressant factor (MDF) activity. Administration of LY-171883 did not significantly inhibit the release of the lysosomal hydrolase cathepsin D during traumatic shock. However, LY-171883 (2 mg/kg) significantly attenuated the accumulation of MDF activity in the plasma (51 +/- 2 vs 37 +/- 3 U/ml), vehicle vs drug, respectively, and significantly (p less than 0.02) prolonged survival time to 2.7 +/- 0.2 h. Administration of the antagonist at a dose of 4 mg/kg further improved survival time (3.4 +/- 0.6 h, p less than 0.01) and additionally blunted circulating MDF activities compared to traumatized rats given only the vehicle. LY-171883 was found to antagonize the bronchoconstrictor effect of LTD4 given intravenously to anesthetized rats as well as the coronary vasoconstrictor actions of LTD4 in vitro. The beneficial effect of LTD4 antagonism in the present study is consistent with the concept that peptide leukotrienes are important mediators of the pathogenesis of traumatic shock.

Blood rheological abnormalities in the microcirculation during experimental traumatic shock.

OBJECTIVE: To find whether the principal indices of blood rheological disorders related to the microcirculation undergo drastic changes during the traumatic shock. METHODS: RBC aggregability and deformability, as well as the systemic hematocrit, were assessed in white laboratory rats with the techniques that provided us with direct and quantitative data. RESULTS: We found that in the experimental animals the RBC aggregability was sharply increased, the RBC deformability significantly decreased, while the systemic hematocrit underwent considerable lowering during the traumatic shock. CONCLUSION: The blood rheological properties are significantly disordered during development of the traumatic shock.

Administration of polymerized bovine hemoglobin improves survival in a rat model of traumatic shock.

The ability of purified and modified hemoglobin solutions to expand blood volume and act as oxygen and carbon dioxide transporters with no apparent toxic side effects has lead to an increased interest in their utilization in shock conditions. Although hemoglobin solutions have been shown to provide beneficial effects in endotoxic and hemorrhagic shock, they have not been studied in whole body traumatic injury. We investigated the effects of a polymerized bovine hemoglobin solution (HBOC-201) in a rat model of traumatic shock. Pentobarbital-anesthetized rats subjected to Noble-Collip drum trauma developed a shock state characterized by marked hypotension, a survival time of 102 +/- 20 min, significant increases in intestinal myeloperoxidase (MPO) activity and splanchnic vascular endothelial dysfunction characterized by an impaired vasorelaxation of the superior mesenteric artery (SMA) to endothelium-dependent vasodilators. Traumatized rats were treated with HBOC-201 or an equal volume of vehicle corresponding to 5, 10 or 15% of the calculated blood volume. Treatment with HBOC-201 10 min posttrauma prolonged survival, normalized mean arterial blood pressure and attenuated endothelial dysfunction of the SMA. However, administration of HBOC-201 failed to significantly attenuate increases in intestinal MPO activity following trauma. No beneficial effects were observed with the administration the vehicle. These data indicate that HBOC-201 exerts significant beneficial effects in traumatic shock states by normalizing systemic blood pressure and attenuating vascular endothelial dysfunction. HBOC-201 may serve as a useful adjunctive agent in the early treatment of trauma.

[Effect of mebikar on the experimental and clinical course of traumatic shock].

The effect of mebikar upon the course of traumatic shock was studied on the basis of experimental investigations and clinical observations. The authors have found a positive effect of the drug on indices of acid-base state, histostructure and cellular composition of the liver whose disturbance has a great pathogenetic significance in shock. The application of mebikar considerably improved the results of treatment in clinic and reduced lethality in experiment. It was supposed that the positive effect of the drug was associated with prevention of the development of circulatory disorders and with the maintenance of oxidation-reduction process and the histostructure of tissues.

[Effectiveness of the use of synthetic opioid peptide in rats with traumatic shock].

Experiments were conducted on 80 male Wistar rats. Traumatic shock was simulated by applying clamps to lower extremity soft tissue for 6 hours. After removing the clamps, experimental animals were intraperitoneally injected the synthetic leucine-enkephalin dalargin. The blood ionogram, the levels of urea, glucose, phosphorus, creatinine, lactic and pyruvic acids, blood plasma osmolarity, LDH activity were determined. In the animals untreated with dalargin, plasma concentrations of ions of potassium, phosphorus, creatinine, urea, osmolarity, LDH activity increased and the levels of glucose decreased when shock developed. At the same time the rat death rates were 70-80% 24 hours later. The administration of dalargin corrected the majority of the above-mentioned parameters and the death rates were decreased to 20%. It is concluded that dalargin has an early antishock effect.

[Energy exchange in mitochondria and contractility of bundles of glycerinated myocardial fibers in traumatic shock].

The state of energy exchange on the basis of respiratory chain reactions of myocardial mitochondria and contractility of bundles of glycerinated muscle fibers in traumatic shock, were investigated. The mitochondria isolated from animal myocardium at the terminal stage of traumatic shock were found to be capable to carry out oxidative phosphorylation at the level typical for healthy animal mitochondria. Parallel tensometric analyses of contractility of bundles of glycerinated muscle fibers showed that in rats and dogs with traumatic shock the most pronounced changes occur in isometric tension and specific power. At the same time relative fiber shortening under isotonic conditions is insignificantly reduced.
