[A study on treatment of lung cancer by combined therapy of traditional Chinese medicine and chemotherapy].

From May 1992 to May 1995, 102 patients with lung cancer were divided randomly into two groups: group A, 48 patients, treated with combined therapy of TCM and chemotherapy, group B, 54 patients treated with chemotherapy alone. The protocol of chemotherapy used in the two groups was similar. The Chinese medicines were given according mainly to the Syndrome Differentiation. Four types were found in the group A: the Lung and Spleen Qi Deficiency type, the Lung Heat and Phlegm-Dampness type, the Lung-Yin and Stomach-Yin Deficiency type and the Qi stagnation and blood stasis type. The total effective rate according to the WHO standard was as follows: that of group A and group B was 52.1% and 35.1%; the 1 year survival rate of them was 69.4% and 66.7%, the 2 year survival rate was 56.2% and 15.8% and the median survival time were 13 months and 9 months, respectively. These results suggest that the elevation of median survival time and 2 year survival rate of group A might be closely related with the therapeutic effect of TCM, but the combined therapy of TCM and chemotherapy did not improve the therapeutic efficacy significantly.

[Relationship between basal core promoter combined point mutation of hepatitis B virus and TCM syndrome type].

OBJECTIVE: To investigate the relationship between basal core promoter (BCP) combined point mutation of hepatitis B virus (HBV) and TCM syndrome type. METHODS: One hundred and two patients with chronic hepatitis with positive HBV DNA and hadn't ever been treated by Lamivudine and interferon were differentiated according TCM syndrome differentiation into 5 types, two excess types (damp-heat blocking zhong-jiao type and blood stasis blocking collaterals type) and three deficiency types, gan-stagnation with pi-dificiency type, gan-shen yin-deficiency type and pi-shen yang-deficiency type. The serum HBV DNA, hepatic biochemical indexes, and the mutation of BCPnt 1762A-T and nt1764G-A combined point were determined, respectively. RESULTS: The variant strain positive rate detected in the excess type was significantly higher than that in the deficiency type, the highest rate appeared in patients of damp-heat blocking zhong-jiao type. CONCLUSION: BCP combined point mutation may be liable to happen in patients of TCM excess type, especially in patients of damp-heat blocking zhong-jiao type.

[Differential diagnosis in the preliminary evaluation of esterase responses of tongue coating cells].

The tongue coating cells were stained by Alpha-naphthol acetate esterase and naphthol AS-D chloroacetate esterase in 100 patients with different symptom-complex [five groups: dampheat, damp-heat due to blood stasis, Yin deficiency, Yang deficiency and Qi-blood deficiency] in view of TCM, and in 116 healthy persons. The responses of the epithelial cells of the tongue and the leukocytes to the two esterase stainings were observed. The results were: the number of nonspecific esterase was much higher in the patients, especially in the patients with Yin or Qi-blood deficiency, than that in the healthy persons (P less than 0.001); responses to ASDCE suggested that the leukocytes of the patients were different from those of the healthy persons in classification, more granulocytes in the healthy persons, and increased proportion of monocytes and lymphocytes in the patients, the most increased proportion of monocytes in the patients with Qi-blood deficiency and the most increased proportion of lymphocytes in the patients with Yang deficiency. The mechanisms of the different responses to the two esterase stainings in the patients and healthy persons, and the relationship between these mechanisms and the symptom-complexes by TCM were discussed.

[Immunological change in pigmentary degeneration of retina and its relation to traditional Chinese medicine differentiation of syndromes].

Using monoclonal antibody technique, T lymphocyte cell subgroups, immunoglobins, complements, and circulating immune complexes (CIC) were measured in 60 patients with pigmentary degeneration of retina (RP) and 40 normal subjects. The results indicated that T1, T4, T8, C3 and C4 of the patients were lower than those of the controls; T4/T8, IgM and CIC of the patients were more than those of the controls. The differences between the patients and the controls were remarkably significant. Similarly, by investigating the objective quota of TCM differentiation of syndromes, the authors divided the RP patients into Liver-Kidney deficiency of Yin and Spleen-Kidney deficiency of Yang groups, and compared them with normal subjects. The results indicated that the two groups underwent similar change of immunology. But they had their own characteristics. The Yin-deficiency group showed remarkable increase of T4/T8 and normal T4; the Yang-deficiency group showed remarkable reduction of T1 and reduction of T4.

Psychology and Chinese medicine: exploring the mental, emotional and spiritual disharmonies of the Kidney energy system.

Drawing on his perspective as psychiatrist and longtime practitioner of Chinese medicine (CM), the author introduces his interpretation of the energy concepts of CM as they pertain to the psychology of man. This discussion focuses on the Water Element and Kidney energy system which are associated with supervision the genetic and intrauterine developmental phases of evolution. A faltering of these energies is associated with both pervasive and subtle neurological disorders and a predisposition to the most severe psychological disturbances. The manifestations and consequences are examined with case studies.

[Study on the patterns of TCM syndrome differentiation of 900 patients with posthepatitic cirrhosis].

OBJECTIVE: To investigate the formula for TCM syndrome pattern differentiation in patients with posthepatitic cirrhosis. METHODS: Factors analysis, C mean clustering and fuzzy comprehensive assessment were used for analyzing the basic information, including clinical symptoms and signs, obtained from 900 patients with posthepatitic cirrhosis to establish the formula for syndrome pattern differentiation through collecting the syndromic factors. RESULTS: Multivariate statistic analysis showed the syndromic factors could be classified into two categories: the common characteristics related factors, which reflected the basic pathology of cirrhosis as qi deficiency and blood stasis, and the factors for differentiating 5 syndrome patterns (Gan-Shen yin-deficiency, dampness-heat accumulation, stasis-heat accumulation, Gan-qi stagnation with Pi-deficiency, and Pi-Shen qi-deficiency), which reflected the different pathological features of different syndrome patterns, showing the complexity and polymorphism of syndrome construction. And the functional formula for differentiating syndrome pattern was established. CONCLUSION: The differentiating formula of TCM syndrome pattern could provide a quantified criterion for syndrome pattern differentiation in posthepatitic cirrhosis patients.

Application of traditional Chinese drugs in comprehensive treatment of primary liver cancer.

The present paper is a summary for treating 50 cases of Stage II primary liver cancer by the Integrated method of traditional Chinese and Western medicine. It was found by comparative observation that the average survival time of the Spleen-Deficiency (SD) type patients was markedly longer than that of Phlegm-Dampness-Stagnancy (PDS), Qi- and Blood-Stagnancy (QBS) and Liver-Kidney-Yin-Deficiency (LKYD) types, indicating that the SD type patients might have a better prognosis, and that the Chinese herbal medicine was an alternative for treating intermediate and late carcinoma.

The principle and technique of using Chinese drugs in the treatment of hypertension.

Differential use of Chinese and western drugs, and combination of traditional theory of materia medica with the results of modern pharmacological research can be regarded as a general principle in the treatment of a disease, while combination of lowering blood pressure with preventing and treating complications is a specific method in the treatment of hypertension. As to the use of drugs based on differentiation of diseases and/or symptoms, drugs which have the action of relieving symptoms but also reducing blood pressure should be used as the first choice. For those drugs which have the action of relieving symptoms but can not lower blood pressure should not be used or should be used as less as possible. Drugs with action of raising blood pressure are contraindicated. When patients have the symptoms of hyperactivity of yang resulting from yin deficiency, Huang Bo ([symbol: see text] Cortex Phellodendri) and Zhi Mu ([symbol: see text] Rhizoma Anemarrhenae) are used together to enhance the effects of both nourishing yin to reduce pathogenic fire, and reducing blood pressure. This practice conforms to the TCM theory and also to the modern pharmacological research.

[Preliminary study on differential expressed genes in T lymphocyte of chronic obstructive pulmonary disease patients with Fei-qi deficiency syndrome].

OBJECTIVE: To study T lymphocyte related genes with differential expression in patients with chronic obstructive pulmonary disease (COPD) of Fei-qi deficiency (FQD) syndrome type by gene chips. METHODS: Lymphocytes in peripheral blood were isolated by Ficoll technique from blood samples collected from COPD patients of FQD syndrome type, Fei-yin deficiency (FYD) syndrome type, and also from healthy subjects for control. They were sorted and purified by flow cytometry, and the different expressed genes were screened from them by gene chip technique. RESULTS: There were 15 genes with high differential expression between patients of FQD type and those of FYD syndrome type, and between patients of FQD type and healthy subjects. CONCLUSION: Gene chip technique could be used for studying the gene expression profiles of TCM syndrome, and the T-lymphocyte related genes with differential expression in COPD patients with FQD were screened preliminarily.

Study on the pathological basis of classification of spleen deficiency in chronic gastritis.

BACKGROUND: Spleen in Traditional Chinese Medicine (TCM) is not actually the spleen in the anatomic sense designated in western medicine because its functions basically belong to the physiological category of digestive system in modern medicine, and it represents a macroscopic concept of digestion, absorption and nutrition metabolism. Spleen deficiency syndrome refers to the clinical phenomena such as hypofunction of digestion, absorption and nutrition metabolism. By integrating TCM with modern medicine, this paper is intended to explore the pathological basis of classification of spleen deficiency in chronic gastritis. METHOD: By means of optical microscope, scanning electron microscope (SEM), transmission electron microscope (TEM) and histochemical staining, we conducted histopathological and subcellular ultrastructural (nuclei and mitochondrial) analysis of gastric mucosa of 188 patients of spleen deficiency, and that of 42 voluntary blood donors without clinical symptoms. RESULTS: The gastric mucosa of patients with spleen Qi deficiency (SQD) and spleen yang deficiency (SyangD) could either be affected by organic lesion (type G-occurring on the basis of chronic superficial gastritis (CSG), chronic atrophic gastritis (CAG)) or unaffected (type F-chiefly belonging to functional indigestion); spleen yin deficiency (SyinD) and spleen deficiency with Qi stagnation (SDQS) both occurred on the basis of CSG and CAG; and the degree of mucosa inflammatory cells infiltration, the degree of decrease in glands propria, and the incidence of IMIIb in CSG and CAG were more serious than those of G-SQD and G-SyangD, P < 0.05 - 0.01. CONCLUSION: Spleen deficiency syndrome is likely to occur on the basis of organic lesion of gastric mucosa (disease with symptoms of both CSG or CAG and spleen deficiency symptoms), as well as on the basis of inorganic lesion of gastric mucosa (nondisease with symptoms, which is, despite spleen deficiency symptoms, there is no CSG or CAG). Besides, the clinical phenomenon of disease without symptoms (despite CSG or CAG, there is no spleen deficiency symptoms) occurres because of such factors as genetic diathesis and compensation. The lesion degree of CSG or CAG and the incidence of IMIIb of SyinD and SDQS are more serious than those CSG and CAG of G-SQD and G-SyangD.

[Study of color scale B-ultrasonography in syndrome differentiation of biliary tract diseases].

OBJECTIVE: To evaluate the relationship between modern medical detective method of imaging and Syndrome Differentiation of TCM. METHODS: Two hundred and sixteen biliary tract diseases patients were observed with color scale B-ultrasonography and fat meal tests. RESULTS: Eighty-four cases (38.89%) of damp-heat type in the Liver-gallbladder among the 216 patients had widened gallbladder, higher tension and weak bile echo-penetration through, 39 cases (18.06%) of type of Liver-Qi stagnation and Spleen deficiency had small transection, hypotension, slender type and low contraction rate, 13 cases (6.02%) of Blood-stasis type had small gallbladder volume and the contraction rate decrease significantly. There were 71 cases (32.87%) of Liver-Qi stagnation type and 9 cases (4.17%) of Liver Yin-Deficiency type, whose characters of the gallbladder were not prominent, so the identification of these cases depended largely on clinical differentiation. CONCLUSIONS: This study will facilitate the further extending the principle of inspection in the specific organs which may provide us with the objective basis for the differentiation of biliary tract diseases.

[Use of crude drugs in "PiWeiLun" in comparison to those used in "NeiWaiShangBianHuoLun".].

PiWeiLun (Hi-i-ron in Japanese) is a traditional Chinese medical formulary written in the 13th century that discusses endogenous disease. For curing endogenous diseases (Naisho) caused by functional deficiency of pi- and wei qi (Hi-I-Ki-Kyo), PiWeiLun was recommended for QingShuYiQi-Tang (Seisho-ekki-to), Ban-XiaBaiZhuTianMaTang (Hange-byakuju-tsu-tenma-to) and BuZhongYiQiTang (Ho-chu-ekki-to), which are described in NeiWaiShangBianHuoLun (Naigaisho-benwa-ku-ron).A characteristic of PiWeiLun is that it discusses combining the use of Phellodendri Cortex with sweet Qi-tonics (Hoki-yaku) as Ginseng and Atractylodis Macrocephalae Rhizome for improving false heat syndrome (Kyo-netsu) due to yin-deficiency (In-kyo). In PiWeiLun Cimicifugae Rhizome, Angelicae Radux and Atractylodes Rhizome are more frequently used than in NeiWaiShangBianHuoLun.The use of drugs presented in PiWeiLun may be useful to devise new crude drug formulations good for modern deficiency syndrome experienced by persons having post-operative complications.

Dialectic classification of syndrome diagnosis in traditional Chinese medicine used as new criterion for evaluating prognosis of patients with cervical cancer.

254 patients with cervical cancer were treated by radiotherapy from Jan 1979 to Dec 1984. All the patients were followed up at least for more than 3 years, of them 180 for over 5 years. According to various symptoms, picture of the tongue and pulse condition, the disease was classified into 4 types, namely: 1) deficiency of liver-yin and kidney-yin, 2) deficiency of splenic qi, 3) internal retention of damp-heat, and 4) stagnation of qi and blood or stagnation of liver qi. The survival rate of the various types was quite different. The patients of type 4 had the worst prognosis. The prognosis of patients with type 1 was significantly better than that of patients with type 4. The difference was of statistical significance (P less than 0.01).

Reduction of perception of chronic fatigue in an observational study of patients receiving 12 weeks of Kampo therapy.

OBJECTIVE: The aim of this study was to observe the influence of Kampo therapy on latent chronic fatigue of patients with chronic diseases. SUBJECTS: One hundred and seventy-three (173) consecutive patients with chronic diseases came to our department for the first time. DESIGN: This was a prospective study. Patients were divided into two groups: a chronic fatigue group (CFG) and a nonchronic fatigue group (NCFG). Based on Kampo diagnosis, both groups were prescribed Kampo formulae as an extract or decoction for 12 weeks. OUTCOME MEASURES: By using questionnaires, patients were assessed concerning their physical and mental types of fatigue, their sleep situation, and their attitude toward work or housekeeping, both before and after 12 weeks of treatment, according to Kampo diagnosis. RESULTS: The mental fatigue, physical fatigue, and sleep scores of both groups, and the work score of CFG, were decreased. The rate of reduction of the fatigue score was significantly greater in CFG than in NCFG. The factor responsible for this difference in fatigue score was physical fatigue. CONCLUSIONS: A reduction of the perception of chronic fatigue was observed in patients receiving 12 weeks of Kampo therapy.

The use of preventive strategies for bone loss.

Osteoporosis is a worldwide problem that is increasing significantly as the global population both increases and ages. While osteoporosis has been extensively studied in recent years, the utilization of traditional Chinese medicine (TCM) for the diagnosis, prevention and treatment of this condition has seldom been examined. This paper examines the theories and the literature that relate to diagnosis, prevention and treatment of bone loss at the time of menopause according to the principles of TCM. It also considers practical developments in these areas as illustrated by the authors' research findings in recent studies. TCM diagnosis attributes a number of different underlying patterns to menopausal bone loss. A very common pattern in this situation is a Kidney qi and yin deficiency pattern. TCM analysis can be used as an early determinant of those persons who are potentially at risk of bone loss. Acupuncture, herbal medicine and Tai Ji exercise can then be applied to prevent and treat osteoporosis. These treatments can be effective, if they are applied correctly. The therapies may also be used in the treatment and prevention of osteoporosis, as well as the general maintenance of women's health during menopause.

[Study of a qualitative diagnostic criterion for basic syndromes of traditional Chinese medicine in patients with primary liver cancer].

OBJECTIVE: To work out a qualitative diagnostic criterion for basic syndromes of traditional Chinese medicine in patients with primary liver cancer. METHODS: Based on the collection and analysis of related medical literature, clinical investigation, and experts' discussion, a preliminary qualitative diagnostic criterion for basic syndromes of traditional Chinese medicine in patients with primary liver cancer was formulated. Then it was used in clinic to be verified and revised repeatedly till it was improved to be a satisfied formal criterion. RESULTS: The basic syndromes listed in the qualitative diagnostic criterion for basic syndromes in patients with primary liver cancer consisted of two parts: excessive syndromes, including the syndromes of stagnation of qi, blood stasis, excess-heat and dampness, and deficient syndromes, including the syndromes of deficiency of qi, deficiency of blood, deficiency of yin and deficiency of yang. Each of the above syndromes could be diagnosed according to specific combination of its corresponding symptoms or signs. The clinical verification results showed that the total matching ratio was 73.92% between the diagnoses made according to the criterion and the diagnoses acquired from the experts' experience. CONCLUSIONS: The qualitative diagnostic criterion for basic syndromes of traditional Chinese medicine in patients with primary liver cancer is coincident with the experts' clinical practice. However, it needs to be further studied.

[Study on pathological rhythm of traditional Chinese medicine about circadian distribution of premature ventricular contractions in 240 patients with viral myocarditis].

OBJECTIVE: To explore the circadian distribution of premature ventricular contractions (PVCs) in patients with viral myocarditis (VM) according to the theory of chronomedicine in traditional Chinese medicine (TCM) in order to find out the scientific evidence for a reasonable administration time. METHODS: Two hundred and forty patients with VM were enrolled and electrocardiograms were examined for all by Holter monitor. The circadian distribution of PVCs was evaluated according to the theory of chronomedicine of TCM about time periods corresponding to five zang viscera. RESULTS: PVCs mostly happened in periods of 3:00-5:00, 5:00-7:00, 7:00-9:00 and 9:00-11:00, in which 5:00-7:00 was the most serious time. Less PVCs happened in periods of 17:00-19:00, 19:00-21:00 and 21:00-23:00. The frequency of PVCs in midnight also increased with aging. The time periods of the frequency peak of PVCs in patients with different syndromes were also different. In patients with syndrome of pathogenic heat invading heart (PHIH), the PVCs mostly appeared in midday. In patients with syndrome of flaring of fire due to yin-deficiency (FFYD), the PVCs were most serious in daytime and aggravated at noon. And in patients with syndrome of insufficiency of heart-qi and yang (IHQY), the PVCs mostly happened in early morning or midnight. CONCLUSION: There was a manifest regulation of the pathological circadian rhythm of PVCs in patients with VM. The circadian distributions of PVCs were variable in patients with different syndromes. Therefore the theory of chronomedicine of TCM may be used to guide the treatment based on syndrome differentiation.
