Though common, the exact incidence and clinical pattern of idiopathic pulmonary fibrosis (IPF) in Gulf countries are not known. The results of a four-year prospective study undertaken in two tertiary hospitals in Kuwait are presented. The mean age at diagnosis of the 52 patients studied was 55.40 +/- 11.87 years. Thirty-two (61.5%) patients were male and 22 (42.3%) were smokers. The mean duration of symptoms at diagnosis was 2.1 +/- 0.92 years. Digital clubbing was found in 34 (65%) patients. The mean FVC, TLC and TLCO were 57%, 64.4% and 55% of predicted normal, respectively. The FVC value showed a significant difference between smokers and non-smokers (p < 0.05). HRCT findings were abnormal in all patients. Typical histological and high-resolution computed tomography findings of usual interstitial pneumonia, desquamative interstitial pneumonia and non-specific interstitial pneumonia were observed. This study revealed that IPF is prevalent in Kuwait, with patterns showing some similarities to those established elsewhere. The response to treatment was not encouraging, especially in the usual interstitial pneumonia subtype.

BACKGROUND: An essential step in the process of tumor invasion and metastasis involves the degradation of tissue barriers in the extracellular matrix (ECM), particularly in the basal membrane (BM). Matrix metalloproteinases (MMPs) and tissue inhibitors of matrix metalloproteinases (TIMPs), in particular MMP-2, MMP-7, TIMP-1 and TIMP-2, play an important role in the process of ECM and BM degradation in connection with tumor invasion. The aim of our study was to assess the levels of MMP-2, MMP-7, TIMP-1 and TIMP-2 mRNA expression in colorectal carcinoma tissue samples and to correlate them with the stage of the disease. PATIENTS AND METHODS: The study included samples of tumor tissue of 38 patients with colorectal carcinoma and samples of tissue of 11 patients with benign disease. The expression levels of mRNA MMP-2, MMP-7, TIMP-1, TIMP-2 and glyceraldehyde-3-phosphate dehydrogenase (GAPDH), as housekeeping gene, were quantified in tissue samples using the method of reverse transcription real-time PCR. RESULTS: The levels of mRNA expression of MMP-2, MMP-7 and TIMP-1 were significantly higher in tumor tissue samples that in the control tissue (p<0.0005, p<0.0007 and p<0.0004). In addition the presence of mRNA MMP-2, MMP-7, TIMP-1 and TIMP-2 in tumor tissue samples in these parameters was significantly higher than in the control tissue (p<0.003, p<0.0001, p<0.0001 and p<0.05). CONCLUSION: This pilot study demonstrated that a significant difference in the level and in the presence of mRNA MMP-2, MMP-7 and TIMP-1 expressions between tumor colorectal and control colorectal tissues might be helpful for the prognosis of colorectal cancer.
BACKGROUND: Carcinoembryonic antigen (CEA) and cytokeratin 20 (CK20) are established tumour markers and the CEA pre-operative levels in serum have prognostic value. The aim of this study was to verify the usefulness of the estimation of CEA and CK20 gene expressions in tissues of colorectal cancers and their liver metastases. PATIENTS AND METHODS: Two hundred and twenty-two specimens of colorectal cancers, their liver metastases, other liver tumours and control tissues were analysed by reverse transcription combined with real-time PCR. RESULTS: The expressions of CEA and CK20 were significantly higher in tumours than in controls; there were differences between tumour types and no relationship to staging or clinical development was found. CK20 expression was inversely dependent on grading. The CEA expression in tumours did not correlate with the CEA levels in serum, but did correlate with serum tissue-specific polypeptide antigen (TPS). CONCLUSION: The measurement of CEA and CK20 gene expressions in tumours did not supply any new prognostic information, but raised the question of the mechanism releasing CEA into the blood.
Damp dwellings represent suitable conditions for extended indoor moulds. A cellulolytic micromycete Stachybotrys chartarum (Ehrenb.) Hughes is considered to be a tertiary colonizer of surfaces in affected buildings. Known adverse health effects of S. chartarum result from its toxins--trichothecenes or atranones, as well as spirolactams. Mechanism of their potential pathological effects on the respiratory tract has not yet been sufficiently clarified. The cytotoxic effects of complex chloroform-extractable endo- (in biomass) and exometabolites (in cultivation medium) of an indoor S. chartarum isolate of an atranone chemotype, grown on a liquid medium with yeast extract and sucrose at 25 degrees C for 14 d, on lung tissue were evaluated in the 3-day experiment. For the purpose, 4 mg of toxicants were intratracheally instilled in 200 g Wistar male rats. A trichothecene mycotoxin diacetoxyscirpenol was used as the positive control. Bronchoalveolar lavage (BAL) parameters--viability and phagocytic activity of alveolar macrophages (AM), activity of lactate dehydrogenase, acid phosphatase and cathepsin D in cell-free BAL fluid (BALF), as well as in BAL cells, were measured. Acute exposure to the metabolites caused statistically significant changes, indicating lung tissue injury in the experimental animals. Decreased AM viability and increased activity of lysosomal enzyme cathepsin D in BAL cells after fungal exometabolite exposure were the most impressive. As toxic principles were found predominantly in the growth medium, toxins were more likely responsible for lung cell damage than e.g. fungal cell wall components. S. chartarum toxic metabolites can contribute to the ill health of occupants of mouldy building after inhalation of contaminated aerosol.

Twenty-nine elderly were assessed on the attitudes toward aging before and after attending to an Estructural Educational Program (EEP), with a participative methodology. The program's objective was to strength and reorient the attitudes of aceptation, integration and communication. Subjects have been part of an urban sector of low social economical status from Cali, Colombia. A quasiexperimental research design, with a convenient sample was used. It was applied a questionnaire to each subject to collect biopsychosocial data, a Lickert scale before and after the EEP to measure attitudes, as well as an observational guide to measure individual and group behavior before and after each class. A statistical significative growth was observed on the attitudes toward integration, aceptation and communication (t22 = 2.46 p less than 0.05). Data indicated that elderly attitudes can be strengthened and reoriented through a EEP adjusted to the biopsychosocial characteristics of the group.
Most cephalometric analysis published to date are based on studies performed by orthodontists, focused on individuals in the growth and development stages, and based mainly on individuals with morphogenetic patterns different from those of the Latin prototype. Such studies concede little attention, if any, to the evaluation of the median (middle) third, which makes them insufficient to amply determine sites of structures found in this area of the face. In Mexico, the maxillofacial surgeon often needs to analyze individuals at the end of their growth stage, with very varied morphogenetic patterns, a result of wide ethnic interbreeding. This preliminary study aims to attain an analysis applicable to Latin individuals, whose morphogenic pattern tends toward the Caucasian prototype, and to establish standards from which to evaluate the middle third.

A case of right ventricular tumor in a 14 year old male is presented. Congestive heart failure and a systolic pulmonary murmur due to right ventricular obstruction were the main symptoms. The diagnosis was confirmed by echocardiogram (M mode) and right ventricular angiography. The surgical treatment was curative and the histological exam revealed a myxoma.
