Grid requirements for the integration of biomedical information resources for health applications.

OBJECTIVES: The goal of this paper is to identify how Grid technology can be applied for the development and deployment of integration systems, bringing together distributed and heterogeneous biomedical information sources for medical applications. METHODS: The integration of new genetic and medical knowledge in clinical workflows requires the development of new paradigms for information management in which the ability to access and relate disparate data sources is essential. We adopt a requirements perspective based on the user needs we have identified in the development of the INFOGENMED system to assess current Grid technology against those requirements. RESULTS: The gap between Grid features and distributed biomedical information integration needs is characterized. Results from prospective studies are also reported. CONCLUSIONS: Grid infrastructures offer advanced features for the deployment of collaborative computational environments across virtual organizations. New Grid developments are in line with the problem of multiple site information integration. From the INFOGENMED point of view, Grid infrastructures need to evolve to implement structured data access services and semantic content description and discovery.

[Importance of socio-cultural data for accessibility of health care and observance of treatment in leprosy. Example of the Zarma region in Niger].

From an epidemiological point of view leprosy remains a problem of public health. Various factors influence the accessibility to health care and the observance of treatment. The study carried out in Niger shows that beyond the stigmatism associated with the disease, the most important factors concern the difference between scientific and popular etiology and semiology. In the face of such problems it is necessary, from a theoretical point of view, to make anthropological enquiries based on the theme of the different representations of this pathology, and from a practical point of view, to examine the possibility of the participation of former patients in public health teams.

A caching approach for XML based medical data.

This paper discusses the representation and caching of medical data structured and stored as XML documents. Caching of XML medical data is proposed towards improving the medical data accessibility and availability. The representation of XML medical data is a tree like structure and the caching process is guided by frequency of access of the XML medical data records. Experimentation is carried out for an artificial workload of patient records and both cache and byte hit ratios are evaluated. The proposed caching scheme is proven to be quite effective and beneficial for accessing the medical data.

Designing a computerized data collection tool for access management.

Collecting data for continuing quality improvement can be a barrier for staff. Data collection can be stressful and time consuming. If not carefully planned it may not provide desired information. Satellite Dialysis Centers recently developed a new data collection tool for vascular access management that was incorporated into our clinical information system. The goal of this project was to collect meaningful accurate information concerning vascular access in a manner that would not increase the dialysis staff work load. This article will outline the measures taken by Satellite Dialysis Centers to automate data collection concerning dialysis access management, including how desired data were identified and strategies for ensuring data entry compliance.

[Evaluation of professional Internet resources' use by gynaecologists and obstetricians].

OBJECTIVES: The aim of this study was to evaluate the use of internet as the additional education tool by gynaecologists and obstetricians. MATERIAL AND METHOD: Prospective questionnaire was sent December 1999 to gynaecological departments of 35 hospitals in Wielkopolska region. Data were collected in MS Access data base until February 25, 2000. Analysis was performed using MS Excel packet. RESULTS: Questionnaires returned from 25 hospitals (71.4%) filled by 193 physicians. About 52.8% of them use the internet for searching professional information. Among internet users 77.4% search Medline data bases, 22.5% use discussion lists, 64.7% search for education possibilities, 67.6% are interested in precise professional topics, 31.4% read the OnLine papers and casuistic, 1.0% has the own www page. The importance of internet was stressed by 84.3% of internet users. Young physicians and those in specialising training are frequent users of internet. CONCLUSION: The access to internet is not satisfactory, but the physicians use all possible internet options.

[Renal cancer: descriptive analysis of a series of 267 intervened cases].

OBJECTIVE: To present the results of a descriptive analysis of 267 surgically treated renal tumors. METHODS: From January 1986 to October 1999, 267 patients (153 males and 114 females) with renal tumor were treated in our department. All data were introduced into the Access data base program and analyzed using the SPSS software. Descriptive analysis was performed and life expectancy was calculated with the Kaplan-Meier survival curve. Mean follow-up was 42.72 months. RESULTS: 56.4% were in the right and 43.6% were in the left kidney. 41.2% of the cases were incidentally discovered. In the symptomatic patients, the most common presenting feature was hematuria (51.8%). Radical nephrectomy was performed in 94.2%, partial in 3.1% and tumor resection in 2.7%. The lumbar approach was used in 75.5% of the cases. In regard to the histology, 88.7% were carcinomas. Clear cell was the most frequent cell type (91.5%). By grade, 55.8% were GI, 32.9% GII and 11.3% were GIII. By stage (according to the 1992 TNM classification), 64.3% were stage I, 15.4% stage II, 17% stage III and 3.3% stage IV. 80.5% showed no vascular involvement. The 5-year overall survival was 71.93% (mean 114 months; median 167 months). Currently, 76% of the patients are alive. CONCLUSIONS: Distribution by sex in our series was different to that reported in most of the studies. The tumor was incidentally discovered in a high proportion of the cases. The lumbar access was the most widely used surgical approach. The overall survival is similar to that reported by other groups.

A framework for scientific data modeling and automated software development.

MOTIVATION: The lack of standards for storage and exchange of data is a serious hindrance for the large-scale data deposition, data mining and program interoperability that is becoming increasingly important in bioinformatics. The problem lies not only in defining and maintaining the standards, but also in convincing scientists and application programmers with a wide variety of backgrounds and interests to adhere to them. RESULTS: We present a UML-based programming framework for the modeling of data and the automated production of software to manipulate that data. Our approach allows one to make an abstract description of the structure of the data used in a particular scientific field and then use it to generate fully functional computer code for data access and input/output routines for data storage, together with accompanying documentation. This code can be generated simultaneously for different programming languages from a single model, together with, for example for format descriptions and I/O libraries XML and various relational databases. The framework is entirely general and could be applied in any subject area. We have used this approach to generate a data exchange standard for structural biology and analysis software for macromolecular NMR spectroscopy.
