Curing behavior of a nanocomposite as a function of polymerization procedure.

The purpose of this study was to analyze the curing behavior of a nanocomposite by assessing in real time the degree of cure at depths of 2 mm and 6 mm. The variation of hardness with depth, shrinkage stress, and curing time until gelation with 16 curing regimes was further investigated using one halogen and three LED curing units. In the present study, it was shown that the soft-start polymerization concept is still valid, even with high-power LED curing units. A soft cure polymerization resulted in reduced shrinkage stress while simultaneously keeping the degree of cure and mechanical properties constant. For all tested curing unit types, a short polymerization duration (10 seconds) was insufficient to cure the nanocomposite in deeper layers--since the variation of hardness with depth showed a discontinuity between the layers, thus accounting for a decreased hardness of up to 30%.

Silicones containing pendant biocides for antifouling coatings.

The preparation of biocide-incorporated silicone coatings for antifouling/fouling release applications is described. The biocide Triclosan (5-chloro-2-(2, 4-dichlorophenoxy) phenol) was modified with alkenyl moieties and incorporated into a silicone backbone through covalent bonds. The presence of the biocide on the coating surface was expected to deter fouling organisms from attaching to the surface of the coating. Allyl glycidyl ether was used to provide crosslink functionalities. Resins were cured using vinyl-terminated polydimethylsiloxane for hydrosilyl functionality and 1, 3-cyclohexane-bis (methylamine) for epoxy crosslinking functionality. Coatings were characterized by static water contact angle measurements and dynamic mechanical thermal analysis. Synthetic control over the incorporation of crosslink functionalities within the polymer resin allowed tuning of the surface of the coating and of mechanical properties. Resistance to macrofouling was tested by static immersion tests in the Indian River Lagoon at the Florida Institute of Technology from 15 October 2003 to 13 November 2003. Preliminary results showed that the coatings prepared from biocide-incorporated silicones with the appropriate bulk modulus significantly reduced macrofouling.

Intraoperative radiation therapy in patients with bladder cancer. A review of techniques allowing improved tumor doses and providing high cure rates without loss of bladder function.

Conventional external beam irradiation, using modern megavoltage techniques and doses that do not harm bladder function, will permanently eradicate local bladder cancer in 30% to 50% of patients, compared with 70% to 90% with cystectomy. In appropriately chosen patients, open surgery can safely provide excellent exposure for the selective delivery of more radiant energy directly to the tumor and less to the uninvolved portion of the bladder. Intraoperative radiation therapy, by either a removable radium or iridium implant or a large single dose of electrons, has been reported to be safe and can permanently cure the bladder of cancer and also preserve bladder function in more than 75% of patients with solitary tumors that invade into but not beyond the bladder muscle. With the increasing interest in and availability of intraoperative radiation therapy in the US, this curative and bladder-sparing treatment for operable patients with bladder cancer invading the trigone is appropriate for careful clinical trial.

Skin melanoma. Cured fraction and survival time as functions of thickness, site, histologic type, age, and sex.

BACKGROUND: Previously, nonparametric or semiparametric methods have been used to determine the relationship of various prognostic covariates with survival of skin cancer. Unfortunately, these methods do not readily distinguish between factors that modulate cure and those that modulate survival time among uncured patients. METHODS: The multivariate lognormal model can be used to detect the association of cured fraction and median survival time with specific prognostic covariates. This model was applied to survival data from 2004 patients with skin melanoma using the following prognostic covariates: thickness, site, and histologic type of the tumor and sex and age of the patient. RESULTS: This analysis revealed that a low cured fraction was associated with thick lesions and location other than trunk or extremity, whereas a short median survival time was associated with thick lesions and tumor located on the trunk. Advanced age was highly associated only with short median survival time. CONCLUSION: The lognormal survival model offers insight into the biology of skin melanoma by distinguishing the roles played by likelihood of cure and survival time. The differential associations of various covariates with these two parameters suggest that biologic mechanisms that govern cure are not identical to those that govern survival time.

Cure and improvement of renovascular hypertension after percutaneous transluminal angioplasty of renal artery stenosis.

In the present study we report our long-term experience in 82 patients with renovascular hypertension (48 with atherosclerotic stenosis, 34 with fibromuscular dysplasia) who were followed up for a mean observation period of 23.6 months after percutaneous transluminal angioplasty (PTA) of renal artery stenosis. Our results show a highly significant decrease in mean systolic and diastolic blood pressure. Cure rates were slightly higher in patients with fibromuscular dysplasia (41% cured, 47% improved) than in those with atherosclerosis (23% cured, 54% improved). Kidney function significantly improved in patients with cure, remained stable in those with improvement and worsened in cases classified as unimproved. These results document the good long-term effect of PTA on blood pressure and kidney function in patients with renal artery stenosis.

Use of conjoint analysis to assess web-based communications on functional foods.

This paper reports the results of conjoint analysis of communications about functional foods, run with women aged 40 and above, using messages found on the Internet. Each of 147 women evaluated 60 concepts, comprising 2-4 phrases per concept, with the phrases dealing with an aspect of functional foods. Three respondent segments emerged, based on the pattern of individual utilities, 'health optimists' (no deep interest in functional foods), 'food for life' (food for health), and 'food for cure' (looking to functional foods to help live a normal life). The utilities of the same elements differed far more across these segments than would be expected from conventional classification questionnaires.

Evaluating the efficacy of curing lights.

The use of light curing units has increased tremendously over the past few years with the introduction of photoactivated composite restorative resins. The advantages of light-cured composites are well documented especially the ease of placement of restorations as a result of extended working time and control of setting. Many of the advantages of these composites are dependent on the adequate polymerisation and therefore the source of the visible light, especially the wavelength and the intensity of the light source. The aims of this study were (i) to determine the intensity output of curing lights in state clinics, private dental practices and dental clinics run by trade unions using a radiometer and (ii) to relate their output to various factors that may affect the intensity of the light produced by each unit. The light intensity of each light curing unit was measured using the Efos Cure Rite radiometer (Cure Rite, Efos model #8000; Efos Inc, Mississauga, Canada). Mean readings ranged from a high of 448 to a low of 22 mW/cm2 for curing lights at the time of evaluation. Nineteen (54.3%) of the light curing units were functioning at optimal intensities, followed by 10 (28.6%) functioning at levels between 150 and 300 mW/cm2 and 6 (17.1%) functioning at levels far below that required to achieve adequate photocuring. No statistically significant differences were found when comparing intensity readings with age, last maintenance service and bulb replacement (P > 0.05). All of the clinicians interviewed (100%) expressed subjective satisfaction with the performance of the light curing units, even though 45.7% functioned below optimal intensities (P > 0.05). Not one of the clinicians expressed dissatisfaction with the performance of any of the light curing units. The results of this study showed that the light intensities f light curing units used within private dental practices, state and trade union dental clinics were inadequate for optimum curing.

Hemopoietic marrow function in chronic neutropenia of blacks: cure of aplastic anemia by allogeneic marrow transplantation from a neutropenic sibling donor.

A black patient with severe aplastic anemia is described who underwent successful bone marrow transplantation from a sibling with chronic neutropenia. During an evaluation to identify a suitable donor, it was found that the majority of family members tested had neutropenia, with no familial history of significant infections or related hospitalizations. In vitro hemopoietic culture studies of marrow from the patient's HLA-MLC-matched siblings showed normal numbers of pluripotential and committed hemopoietic progenitors; in vitro hemopoietic colony formation from the patient was markedly subnormal, consistent with the clinical picture of severe aplastic anemia. Following appropriate conditioning therapy, marrow transplanted from one of these neutropenic sibs produced full hematopoietic reconstitution. Posttransplant marrow culture studies of the patient showed restoration of a normal pattern of in vitro hemopoiesis. The in vitro culture studies and clinical experience in this patient support the concept that chronic neutropenia of blacks is not primarily a marrow progenitor cell disorder but, more likely, a manifestation of a genetically determined alteration in granulocyte kinetics.

Irritable bowel syndrome.

The irritable bowel syndrome (IBS) is very common in the community. Psychosocial factors other than the gut symptoms themselves may contribute to the decision of a few to seek medical care. Examination of these not only suggests that the manifestation of symptoms is multifactorial, but also offers clues for management of individuals. There is evidence for both physical and psychological contributions to IBS symptoms and both together may induce illness behavior. In some, many factors may be present, in others apparently none, but the more factors at work, the more complex the treatment. Management should take advantage of the known features of the disease. Its prevalence, recognizable symptoms, and benign nature underpin the reassurance value of a positive diagnosis. Psychopathology or antecedent stressful life events may indicate psychological treatments. Although drugs are unproved in the global treatment of IBS, certain agents may benefit specific symptoms and use the placebo response to advantage. Patients having more difficulty treating symptoms need continuing care. A graded response to IBS complaints implies reassurance and drug-free management in primary care, with increments of psychosocial support, psychotherapy, and the specific use of drugs in nonresponders. The goal for intractable cases should be improved functioning rather than cure.

Nitrite, nitrite alternatives, and the control of Clostridium botulinum in cured meats.

Historically, nitrite has been a component of meat-curing additives for several centuries. In recent years the safety of nitrite as an additive in cured meats has been questioned mainly because of the possible formation of carcinogenic nitrosamines. Nitrite has many important functions in meat curing including its role in color development, flavor, antioxidant properties, and antimicrobial activity. The inhibition of Clostridium botulinum growth and toxin production is an especially important antimicrobial property of nitrite. This review discusses the effects of processing, curing ingredients (especially nitrite), and storage of cured meats in relation to the control of C. botulinum. If nitrite is eliminated from cured meats or the level of usage decreased, then alternatives for the antibotulinal function of nitrite need to be considered. Several potential alternatives including sorbates, parabens, and biological acidulants are discussed.

Joint models for multivariate longitudinal and multivariate survival data.

Joint modeling of longitudinal and survival data is becoming increasingly essential in most cancer and AIDS clinical trials. We propose a likelihood approach to extend both longitudinal and survival components to be multidimensional. A multivariate mixed effects model is presented to explicitly capture two different sources of dependence among longitudinal measures over time as well as dependence between different variables. For the survival component of the joint model, we introduce a shared frailty, which is assumed to have a positive stable distribution, to induce correlation between failure times. The proposed marginal univariate survival model, which accommodates both zero and nonzero cure fractions for the time to event, is then applied to each marginal survival function. The proposed multivariate survival model has a proportional hazards structure for the population hazard, conditionally as well as marginally, when the baseline covariates are specified through a specific mechanism. In addition, the model is capable of dealing with survival functions with different cure rate structures. The methodology is specifically applied to the International Breast Cancer Study Group (IBCSG) trial to investigate the relationship between quality of life, disease-free survival, and overall survival.
