Monitoring and prognostic evaluation of sex-mismatched bone marrow transplantation by competitive PCR on Y chromosome sequences.

In one case out of four, allogeneic BMT concerns a male recipient and a female donor. The monitoring of sex-matched BMT can be carried out by PCR amplification on Y-specific chromosome sequences (YCS), whatever the hematological disease. Twelve patients with sex-mismatched non-T-depleted BMT were first studied through a qualitative PCR, which gave semi-quantitative results. When the qualitative PCR revealed YCS, a competitive amplification was performed in order to estimate the YCS amount in the patient blood sample. For the purpose of the study, we classified the patients in two categories according to the results obtained 9 months after BMT. For 10 patients, we did not detect any YCS amplification after this time. These patients were in complete cytogenetic and clinical remission. For the remaining two patients, we always found male DNA in their blood samples. These patients were in cytogenetic remission but relapsed and died 21 and 25 months after BMT. Our results suggest that the persistence of male cells in peripheral blood, even at the low rate of 1% or 0.1%, 1 year after sex-mismatched BMT, is a bad prognosis.

Temporal artery catheterization for arterial access in the high risk newborn.

The hazards of umbilical artery catheterization necessitate the adoption of an alternate arterial cannulation site for monitoring arterial pressure and oxygen tension in the critically ill newborn. A newly developed method of catheter placement in the temporal artery not only provided arterial access for substantially longer than that reported for radial artery cannulation, but more than 50% of the infants in which it was performed weighted less than 1500 grams. Close adherence to the technique of arterial cannulation and catheter fixation is essential for prolonged arterial access. The method involves (1) Doppler location of the temporal artery, (2) a 3 mm transverse incision directly over the vessel, (3) bevel-down insertion of a winged hub teflon catheter with inner needle stylet, (4) securing the catheter with suture and collodion, and (5) maintenance of catheter patency with a constant infusion of heparinized solution. Temporal artery cannulation using this technique has been performed thus far 115 times in 100 infants with severe respiratory distress. The patients weighed between 540 and 3900 grams. Catheters remained operational for an average of 6.5 days (8 hours to 23 days), with 62% of all catheters providing arterial samples for more than 5 days. The cannulas worked well for arterial pressure monitoring, and pulse contour matched that of the umbilical artery in infants with catheters in both vessels.

Cigarette smoking, ambulatory blood pressure and cardiac hypertrophy in essential hypertension.

OBJECTIVE: To assess the role of blood pressure in the association between cigarette smoking and left ventricular mass in male and female subjects with essential hypertension. DESIGN: A case-control study with matching ratio of 1:4. PATIENTS AND METHODS: We studied 115 heavy smokers (> or = 20 cigarettes/day; 91 men) and 460 non-smokers (364 men) with essential hypertension. Subjects were matched by sex, age (within 5 years) and clinic systolic and diastolic blood pressures (within 5 mmHg). All the subjects underwent 24 h off-therapy non-invasive ambulatory blood pressure monitoring and echocardiography. RESULTS: By matching, clinic blood pressure was nearly identical in smokers and non-smokers (158/99 versus 158/98 mmHg). Daytime ambulatory blood pressure was significantly higher in the smokers than in the non-smokers (150/97 versus 143/93 mmHg), whereas night-time blood pressure did not differ between the two groups (129/79 versus 126/78 mmHg). Smokers had a higher 24 h but not clinic heart rate. Variability of systolic and diastolic blood pressure was slightly greater in smokers when expressed in terms of the standard deviation of the 24 h average (15.9/13.0 versus 14.6/12.2 mmHg), but not after correction for average blood pressure. Left ventricular mass was greater in the smokers than in the non-smokers (119 versus 110 g/m2), and this difference remained after adjustment for clinic blood pressure and other related covariates. However, when clinic blood pressure was replaced by daytime ambulatory blood pressure in the equation, adjusted values of left ventricular mass did not differ between the smokers and the non-smokers (113 versus 112 g/m2). CONCLUSION: In patients with essential hypertension, heavy cigarette smoking (> or = 20 cigarettes/day) is associated with a definite increase in left ventricular mass through a rise in whole-day blood pressure. A pressor mechanism of that type may not be detected by the standard measurement of blood pressure in the clinic, which would make ambulatory blood pressure monitoring a valuable diagnostic tool in this setting.

TPA and CA 15.3 measurements for breast cancer monitoring in a routine setting.

TPA and CA 15.3 concentrations were routinely determined in serum of patients treated for breast cancer during a 15-month period. ROC curves did not show differences in the ability to differentiate between NED and PD on the basis of matching tumor marker values. During monitoring of patients with NED, TPA levels showed fluctuations of more than 25% that were not disease related. We concluded that CA 15.3 is a more slowly reacting marker of tumor burden than TPA, which is an immediate indicator of cell turnover.

The use of the Actiwatch-Neurologica system to objectively assess the involuntary movements and sleep-wake activity in patients with mild-moderate Huntington's disease.

Huntington's disease (HD) is a neurodegenerative disorder characterised by cognitive, psychiatric and motor abnormalities including a range of involuntary movements. Currently, assessment of these movements involves the use of subjective rating scales such as the Unified Huntington's Disease Rating Scales (UHDRS) for bradykinesia and maximal dystonia and chorea, without any objective measures. As new therapies emerge, it is critical that an objective means of evaluating these abnormal movements is developed and we have investigated the use of a wrist-worn activity monitor, the Actiwatch-Neurologica, to determine whether these movements can be measured. In addition, this activity monitor and subjective reports were used to objectively measure the degree of sleep disruption in these same HD patients. Eight patients with mild-moderate HD and 8 age- and sex-matched control subjects wore the monitor for a period of 48 hours and recorded in a diary whether they were asleep or awake for each hour over the 2-day period. Assessment of various movement parameters revealed that HD patients exhibited significantly greater total and maximum activity levels and spent longer performing high acceleration movements while they were awake compared with controls. During sleep, patients not only showed significantly more activity and spent more time making high acceleration movements, but they also made significantly more movements than control subjects. These results demonstrate that the Actiwatch-Neurologica activity monitor can be used to objectively assess movements in HD patients during periods of high activity as well as during sleep.

Effect of BAEP monitoring on hearing preservation during acoustic neuroma resection.

We measured the effect of brainstem auditory evoked potential (BAEP) monitoring on hearing preservation in acoustic neuroma resection in 90 consecutive patients with monitoring compared with 90 historical controls matched for tumor size and preoperative hearing status. In small tumors (less than 2 cm), BAEP monitoring was associated with a higher rate of hearing preservation and a greater chance that the hearing preserved was clinically useful. Changes in the BAEP intraoperatively showed a good correlation with postoperative hearing status.

Reduced mortality rate in patients with severe traumatic brain injury treated with brain tissue oxygen monitoring.

OBJECT: An intracranial pressure (ICP) monitor, from which cerebral perfusion pressure (CPP) is estimated, is recommended in the care of severe traumatic brain injury (TBI). Nevertheless, optimal ICP and CPP management may not always prevent cerebral ischemia, which adversely influences patient outcome. The authors therefore determined whether the addition of a brain tissue oxygen tension (PO2) monitor in the treatment of TBI was associated with an improved patient outcome. METHODS: Patients with severe TBI (Glasgow Coma Scale [GCS] score < 8) who had been admitted to a Level I trauma center were evaluated as part of a prospective observational database. Patients treated with ICP and brain tissue PO2 monitoring were compared with historical controls matched for age, pathological features, admission GCS score, and Injury Severity Score who had undergone ICP monitoring alone. Therapy in both patient groups was aimed at maintaining an ICP less than 20 mm Hg and a CPP greater than 60 mm Hg. Among patients whose brain tissue PO2 was monitored, oxygenation was maintained at levels greater than 25 mm Hg. Twenty-five patients with a mean age of 44 +/- 14 years were treated using an ICP monitor alone. Twenty-eight patients with a mean age of 38 +/- 18 years underwent brain tissue PO2-directed care. The mean daily ICP and CPP levels were similar in each group. The mortality rate in patients treated using conventional ICP and CPP management was 44%. Patients who also underwent brain tissue PO2 monitoring had a significantly reduced mortality rate of 25% (p < 0.05). CONCLUSIONS: The use of both ICP and brain tissue PO2 monitors and therapy directed at brain tissue PO2 is associated with reduced patient death following severe TBI.

A two-dimensional electrophoresis-related laboratory information processing system: spot matching.

An approach for the computer-assisted analysis of two-dimensional gels has been developed as a part of our laboratory information processing system (LIPS). This approach relies in part on an algorithm for the pairwise matching of protein spots. The matching process initially matches spots based on a cross-correlational measure of how well neighboring spots align. While this first pass correctly determines most spot correspondences and noncorrespondences, it can make errors. Higher accuracy is obtained by monitoring the consistency of spot match decisions in a second pass, which demands that neighboring spot pairs that align spatially must also have been found to match in the first pass. Pairwise comparisons of gels are combined into n-way comparisons by matching spot lists of gels to "master" gel spot lists, which in turn are matched to higher level masters, resulting in a hierarchy of matched spots. After each pairwise match the results are reviewed and corrected with the assistance of a graphical match-editor. Results are given for 19 single-cell-derived lymphoid clones in which the presence of a mutation had previously been established, each processed in duplicate. Only one of 46 spot changes failed to be detected, which demonstrates that the strategy is sensitive and efficient for detecting qualitative spot differences.

[Explicit memory for type font of words in source monitoring and recognition tasks].

We investigated whether people can consciously remember type fonts of words by methods of examining explicit memory; source-monitoring and old/new-recognition. We set matched, non-matched, and non-studied conditions between the study and the test words using two kinds of type fonts; Gothic and MARU. After studying words in one way of encoding, semantic or physical, subjects in a source-monitoring task made a three way discrimination between new words, Gothic words, and MARU words (Exp. 1). Subjects in an old/new-recognition task indicated whether test words were previously presented or not (Exp. 2). We compared the source judgments with old/new recognition data. As a result, these data showed conscious recollection for type font of words on the source monitoring task and dissociation between source monitoring and old/new recognition performance.

Heart rate, blood lactate concentration and estimated energy expenditure in a semi-professional rugby league team during a match: a case study.

The aim of this study was to examine heart rate, blood lactate concentration and estimated energy expenditure during a competitive rugby league match. Seventeen well-trained rugby league players (age, 23.9 +/- 4.1 years; VO2max, 57.9 +/- 3.6 ml x kg(-1) x min(-1); height, 1.82 +/- 0.06 m; body mass, 90.2 +/- 9.6 kg; mean +/- s) participated in the study. Heart rate was recorded continuously throughout the match using Polar Vantage NV recordable heart rate monitors. Blood lactate samples (n = 102) were taken before the match, after the warm-up, at random stoppages in play, at half time and immediately after the match. Estimated energy expenditure during the match was calculated from the heart rate-VO2 relationship determined in laboratory tests. The mean team heart rate (n = 15) was not significantly different between halves (167 +/- 9 vs 165 +/- 11 beats x min(-1)). Mean match intensity was 81.1 +/- 5.8% VO2max. Mean match blood lactate concentration was 7.2 +/- 2.5 mmol x l(-1), with concentrations for the first half (8.4 +/- 1.8 mmol x l(-1)) being significantly higher than those for the second half (5.9 +/- 2.5 mmol x l(-1)) (P<0.05). Energy expenditure was approximately 7.9 MJ. These results demonstrate that semi-professional rugby league is a highly aerobic game with a considerable anaerobic component requiring high lactate tolerance. Training programmes should reflect these demands placed on players during competitive match-play.

Assessing high-risk infants in the delivery room with pulse oximetry.

Heart rate and oxygenation status are essential to the delivery room assessment of newborn infants. Pulse oximetry (PO) can capture these two variables but low peripheral perfusion, patient motion and the presence of ambient lighting can challenge the technology. New generation pulse oximeters claim to measure through these conditions. The objective of this study was to assess the performance of new generation pulse oximeters during the delivery room assessment and management of high-risk newborns. Part one (study A) compared the outcome of 25 infants simultaneously monitored with two new PO technologies (experimental group) to a matched population of 25 infants without PO-monitoring (control group). Findings from the experimental group revealed a significant difference between new PO technologies and an improvement in patient outcome compared to the control group. Part two (study B) evaluated the affects of PO-monitoring on 15 very immature infants. Use of Masimo SET PO was associated with a rapid acquisition and near continuous display of PO values, which proved valuable for initial respiratory care and to determine the need for more intensive procedures. Both studies revealed that near immediate postnatal PO monitoring of newborns in the delivery room was feasible and valuable.

A study of physiological responses during match play in Indian national kabaddi players.

OBJECTIVE: To determine the physical and physiological profile of kabaddi players and the physiological demands of playing a kabaddi match. METHODS: Maximum aerobic capacity (VO2max), maximum ventilation (VEmax), O2 pulse, respiratory equivalent (RE), maximum heart rate, and O2 debt were assessed on 16 players. The somatotype of the players was calculated by the Health and Carter method. Heart rate was monitored during a selection trial match on eight players who represented India in the Asian Games, 1994. From the playing heart rate, oxygen consumption (VO2) was computed through a heart rate v VO2 regression equation. Maximum lactate was evaluated from the blood samples collected at the end of the match. RESULTS: The average heart rate and oxygen consumption during the match were 146.5 (SD 9.25) beats min-1 and 2.25(0.59) litre min-1 respectively. During raiding the maximum heart rate attained varied from 162.4(11.3) to 177.4(4.2) beats min-1. Out of 40 min of match play a raider raided on average on 8.13(2.03) occasions. The average time per raid was 20.8(6.26) s. The match heart rate and oxygen consumption was 72.3-83.3% of the maximum heart rate, and 43.5-70.5% of VO2max respectively. Maximum lactate at the end of the match was 6.13(2.53) mmol litre-1. Kabaddi players had the somatotype of 2.68-4.71-1.83, with absolute back strength of 175.0 kg. VO2max and O2 debt were 3.59(0.36) litre min-1 [47.82(3.68) ml kg-1 min-1] and 5.3(1.85) litres (70 ml kg-1) respectively. CONCLUSIONS: Kabaddi is an intermittent sport. The rest pause during the game is sufficient for recovery. During raiding the main source of energy is anaerobic.

Physical demands of elite Rugby League referees, part two: heart rate responses and implications for training and fitness testing.

UNLABELLED: The purpose of this study was to assess whether any characteristic patterns of heart rate (HR) responses could be identified in National Rugby League (NRL) referees (n= 6) during matches played in the 2001 season. The data have been plotted and discussed, in order that exercise program planning practitioners may gain improved understandings of the physiological requirements for referees. Some specific training suggestions have also been made. METHODS: The HR was recorded every five seconds throughout six competition NRL matches; using a heart rate monitor with a built-in memory. RESULTS: The specific magnitudes of referees' HR mean values varied between individuals, possibly due to specific game intensities, referee fitness, and age. All referees however exhibited similar HR response patterns; characterised by frequent (13-20 per match) large transient upward and downward shifts (>20 beats.minute(-1)). Periods of elevated HR extended for between five sec and eight min at a time, and were further characterised as a typical cyclic wave of HR elevation and recovery (ranging from 99.2+/-12.4 beats.minute(-1) to 176.5+/-11.8 beats.minute(-1) [mean +/-95% CI]), with a work to rest ratio of 2:1. Steady state HR was not achieved at any time during any match. CONCLUSIONS: The findings of the present study, taken together with a recent motion analysis, indicate that Rugby League refereeing is a highly intermittent, variable intensity activity. Significant anaerobic contribution to performance appears likely. It is suggested that training and fitness assessment of athletes should reflect their specific demands; some specific recommendations have therefore been provided.

Shoulder posture and localized muscle fatigue and discomfort.

In many industries workers perform manual assembly tasks with hands postured above the shoulders. Awkward shoulder and arm postures are often viewed as acceptable given costs of workplace modification, postural exertions which are in compliance with current design recommendations, ready availability of strong workers, and numbers of electromyographic studies which fail to detect significant signs of localized muscle fatigue (LMF). An experiment was conducted to: (a) study the onset and severity of (LMF) in the shoulder when performing a stylus-to-hole Fitts reciprocal movement task under a range of postures, hand loads, ratios of work-to-rest, and task durations, and (b) to evaluate the efficacy of three techniques (i.e., changes in EMG behaviour, postural tremor, and cross-modal matching estimates) in detecting and monitoring posturally-based LMF and discomfort in the shoulder complex. Experimental findings showed that posturing hands above shoulder level significantly increased the risk of LMF and postural discomfort even in light-weight manual assembly environs where postural exertions are small, and that cross-modal matching estimates and postural tremor were more sensitive metrics of LMF in the shoulder complex than EMG RMS voltage and mean spectral power frequency metrics. The basis for experimental findings, as well as potential application of LMF metrics in future postural stress investigations, are discussed. Recommendations for workplace posture are provided for job designers facing work height decisions in manual assembly environs.

Intelligent diagnostic monitoring using trend templates.

In previous work we have defined our trend template epistemology for clinically significant trends and we have illustrated and tested a program TrenDx that monitors time-ordered process data by matching the data to trend templates. Our initial application domain was pediatric growth monitoring. In continuing work we have explored monitoring hemodynamic and respiratory parameters of intensive care unit patients. This application has highlighted the needs for advances in our representation and monitoring algorithms. In particular, we have added reasoning with uncertainty to the trend template epistemology, and a new control structure allowing numerical ranking of competing trend templates. Furthermore, intelligent monitoring in any medical domain requires a coherent framework for diagnostic monitoring. In this paper we show how TrenDx can be extended to a framework including sending alarms, changing clinical context, and filtering data streams.

Oxygen uptake-to-delivery relationship: a way to assess adequate flow.

Invasive and noninvasive monitoring facilitates clinical evaluation when resuscitating patients with complex haemodynamic disorders. If the macrocirculation is to be stable, then it must adapt to blood flow or blood flow must be optimized. The objective of flow monitoring is to assist with matching observed oxygen consumption (VO2) to pathophysiological needs. If an adequate balance cannot be maintained then dysoxia occurs. In this review we propose a simple schema for global reasoning; we discuss the limitations of VO2 and arterial oxygen delivery (DaO2) assessment; and we address concerns about increasing DaO2 to supranormal values or targeting pre-established levels of DaO2, cardiac output, or mixed venous oxygen saturation. All of these haemodynamic variables are interrelated and limited by physiological and/or pathological processes. A unique global challenge, and one that is of great prognostic interest, is to achieve rapid matching between observed and needed VO2--no more and no less. However, measuring or calculating these two variables at the bedside remains difficult. In practice, we propose a distinction between three situations. Clinical and blood lactate clearance improvements can limit investigations in simple cases. Intermediate cases may be managed by continuous monitoring of VO2-related variables such as DaO2, cardiac output, or mixed venous oxygen saturation. In more complex cases, three methods can help to estimate the needed VO2 level: comparison with expected values from past physiological studies; analysis of the relationship between VO2 and oxygen delivery; and use of computer software to integrate the preceding two methods.

[Continuous monitoring of mixed venous oxygen saturation during mechanical circulatory support of the left ventricle in heart surgery patients. Pathophysiologic principles and personal experience].

The clinical use of mechanical supports for cardiocirculatory function assistance has permitted to treat a higher number of low output syndromes insensitive to pharmacological therapy. One of the most important problems arising during mechanical circulatory assistance, is the ability to know if the performance of the artificial device is adequate to match metabolic tissues demands. Continuous monitoring of mixed venous O2 saturation (MSvO2) has been proposed as the most complete and reliable tool to verify this correlation. The Authors analyse the problems connected with this kind of monitoring during left ventricular assistance with centrifugal pump in cardiosurgical patients and present 6 cases taken from their clinical experience.

Therapeutic drug monitoring databases for postmarketing surveillance of drug-drug interactions: evaluation of a paired approach for psychotropic medication.

The present study investigated the potential of therapeutic drug monitoring data to document pharmacokinetic drug interactions with psychotropic medication, both in terms of methodology and applicability. It focused on 105 patients exposed to one of five agents known for their capacity to induce (phenytoine, phenobarbital, and carbamazepine) or to inhibit (thioridazine and levomepromazine) the metabolism of psychotropic drugs. These patients were matched by gender, age, and monitored psychotropic medication to 105 patients randomly selected from a pool of subjects nonexposed to target comedication. Such a paired approach was shown to be effective in reducing variability for a majority of substances. Power analysis suggested that eight to 10 pairs of exposed and nonexposed patients would effectively allow the detection of twofold effects of interacting substances. In keeping with the literature, analysis of the ratios of dose-normalized exposed to nonexposed concentrations indicated that phenothiazine comedication led to significantly higher concentrations of desmethylated metabolites but not parent compounds, when clomipramine, imipramine, or amitriptyline were administered. A similar, as yet undocumented interaction was observed for the tetracyclic antidepressant mianserine. In contrast, the present study revealed that maprotiline concentrations were increased, but its metabolite was largely unaffected by phenothiazine comedication. Increased concentrations were also observed for moclobemide, but not citalopram or its metabolite. In addition to its long recognized capacity to decrease haloperidol concentrations, carbamazepine was shown to induce the metabolism of clopenthixol and possibly flupenthixol. The consistency of such a picture substantiates the need to consider therapeutic drug monitoring databases as cost-effective and reliable tools for postmarketing surveillance.
