Variables predictive of successful medical therapy in patients with unstable angina: selection by multivariate analysis from clinical, electrocardiographic, and angiographic evaluations.

Although unstable angina can be initially controlled with medical therapy in most patients, there is a high incidence of subsequent death, myocardial infarction, or need for coronary bypass surgery to control symptoms. Identification at the time of presentation of the patient likely to do poorly on continued medical therapy would be useful in advising consideration of surgical therapy. Since coronary arterial spasm may have a significant role in the pathophysiology of unstable angina in some patients, the recently developed calcium channel antagonists may therefore be of particular benefit in the medical therapy of unstable angina. One hundred thirty-eight patients were entered into a randomized double-blind study of the efficacy of adding nifedipine to conventional treatment of unstable angina (nitrates and beta-blockers) and were followed for 18 months. Of these patients, 104 underwent coronary arteriography. A multivariate Cox's hazard function analysis was applied to variables selected from the history, electrocardiographic (ECG) changes during chest pain, and from scintigraphic and coronary arteriographic data to determine those variables most predictive of response to medical therapy. The percentage of the left ventricular myocardium supplied by vessels with 70% or greater luminal stenosis was the most significant variable in influencing failure of medical therapy defined as sudden death, myocardial infarction, or need for bypass surgery. Whether or not the patient received nifedipine was the second most powerful variable, with the use of nifedipine reducing by half the relative risk of failing medical therapy. These were followed by cigarette smoking and presence of global ST segment changes during ischemia. After 18 months the nifedipine group had fewer patients failing medical therapy (p = .02), with fewer patients undergoing coronary bypass surgery (p less than .01). However, nifedipine did not appear to have a preventive effect against myocardial infarction or death. Kaplan-Meier actuarial curves confirmed that medical therapy was significantly less successful in the presence of increasing numbers of significantly stenotic vessels (p = .03). However, nifedipine provided a significant beneficial effect in patients with two or more stenotic vessels (p less than .01) and in whom 50% or more of the myocardium was supplied by vessels with 70% or greater stenosis (p = .01). Thus, although patients with advanced obstructive coronary disease have the greatest likelihood of unfavorable outcomes, the addition of nifedipine is of significant benefit.(ABSTRACT TRUNCATED AT 400 WORDS)

Multivariate analysis of risk factors associated with genital ulcer disease among incarcerated males in Sindh.

OBJECTIVE: To evaluate the potential risk behaviors associated with the lifetime risk of self reported genital ulcer disease (GUD) among prison inmates. SETTING: Prison inmates from 14 prisons of Sindh Province. METHODS: A cross-sectional study was conducted on 3395 prison inmates during July to December 1994. A questionnaire was used to assess the lifetime risk of self-reported GUD (whether or not the subject was ever affected with GUD up to present age) and to investigate demographic markers and risk behaviors for their possible association with lifetime risk of GUD using logistic regression analysis. RESULTS: The reported lifetime risk of GUD in the study sample was 11.4% (386/3395). In final multivariate logistic regression model the sexual behaviors which were independently associated with GUD were having sexual intercourse with female (adjusted OR = 1.7; 95% CI: 1.3-2.3, P=0.0002), sexual intercourse with a prostitute (adjusted OR = 1.5; 95% CI: 1.2-2.0, P=0.0008), sexual intercourse with man (adjusted OR = 2.2; 95% CI: 1.7-2.7, P=<0.001) and sexual intercourse with man during current incarceration (adjusted OR = 1.9; 95% CI: 1.2-2.9, P=0.0071). CONCLUSION: Health education needs to re-enforce monogamous relationship for high risk groups such as in our study. Although infrequent condom use was not a risk factor for GUD in this study, yet based on the results of previous studies, promotion of condom use should be the component of health education program (JPMA 50:115;2000).

A sequential approach to the diagnosis of coronary artery disease using multivariate analysis.

There has been considerable interest in recent years in enhancing the accuracy of noninvasive tests in diagnosing coronary artery disease. The recognition that no currently available test is a perfect predictor has led to the use of probability analysis as a means of assessing the presence or absence of coronary disease. In this article we present a multivariate approach to the diagnosis of coronary disease. One hundred forty-seven patients undergoing coronary angiography, thallium-201 imaging, and exercise ECG were studied. Patients were classified according to age, sex, and typical vs atypical chest pain. Sequential stepwise logistic regression analysis was performed to develop probability statements prior to testing, after exercise ECG, and after exercise ECG and thallium-201. The results indicate that this sequential approach can be used to develop strategies for the diagnosis of coronary disease in the same way as Bayes' theorem, while permitting integration of multiple characteristics into one model.

Multivariate analysis of the activity of rifamycins against rapidly growing Mycobacteria, to define an in vitro screening model for their activity against mycobacterium leprae.

In an attempt to select a group of rapidly growing Mycobacteria which could serve as a model for the in vitro screening of rifamycin derivatives for their activity against Mycobacterium leprae, the MIC values of 237 strains of rapidly growing Mycobacteria were determined for 4 reference rifamycins with known activity against M. leprae and 19 rifamycins with unknown activity against M. leprae. Study of the results by a multivariate statistical technique, the principal components analysis, defined a group of 16 rapidly growing Mycobacteria (7 strains of M. phlei, 4 strains of M. thermoresistible, 3 strains of M. fortuitum and 2 strains of M. vaccae) showing the lowest distance to M. leprae. The analysis detected among rifamycin derivatives with different activities M. tuberculosis, compounds which should be active against M. leprae, and allows the selection of a limited number of rifamycins for the in vivo screening in the mouse model.

Epidemiology of microbial keratitis in southern California. A multivariate analysis.

Two hundred twenty-seven cases of microbial keratitis reported in nonreferral county practice were studied. The staphylococci, Pseudomonas aeruginosa and Streptococcus pneumoniae, were the major isolates. A multivariate statistical model was developed to evaluate possible predisposing and outcome determinants. Several racial and age-related relationships were shown. The interaction of numerous local ocular and systemic factors played a fundamental role in causing disease. The authors found significant association between S. pneumoniae and topical steroid use, and direct and indirect linkage of S. aureus with diabetes and trauma, respectively. S. pneumoniae and Moraxella were risk factors for major complications (24% of cases); S. pneumoniae was related to enucleation and late perforation. Corneal exposure and prior topical steroids were associated with prolonged hospital stays. Hypopyon was associated with pneumococcal infection, 60 years of age or older, and trauma. The identification of groups at high-risk for microbial keratitis and problems of preventive management are discussed.

Multivariate analysis of the MMPI profiles of patients with multiple pain complaints.

The MMPI profiles of 123 male and 221 female patients with multiple pain complaints referred for psychological evaluation over a three-year period were examined using a multivariate clustering technique. Replicable subgroups were found in the patient cohorts in both the male and female samples. Multiple discriminant analysis and analysis of variance showed that the subgroups differed significantly from one another and within subgroup variation was small relative to between subgroup variation. It is suggested that pain patients are characterized by a variety of personality styles; subgroup membership may be an indication of behavior and personality dynamics which may have implications for treatment selection. Inferences as to possible treatment responses based on subgroup membership are made.

Social adjustment of the elderly residing in institutional homes: a multivariate analysis.

An attempt was made to determine the order of magnitude of a number of variables which a bivariate analysis had demonstrated to show some degree of correlation with the way in which institutionalized elderly individuals adapt socially. To determine the multivariate impact of all these variables on the degree of social adjustment to life in a home, a path model was developed that permitted detection of both the direct and indirect effect of the variables. It was found that loneliness was the main factor underlying failure to adjust. Furthermore, widowhood and being alone seem to have a considerable influence. The degree of disability and the age of the individual proved to have a smaller impact than we had assumed to be the case. The operationalized model explains 27% of the variance of the social adjustment in a home for the elderly.

Role of primary breast cancer characteristics in predicting positive sentinel lymph node biopsy results: a multivariate analysis.

HYPOTHESIS: Certain primary breast tumor characteristics can be used to predict positive sentinel lymph node biopsy (SLNB) results and thus determine which patients should undergo SLNB. DESIGN: Retrospective review of patients undergoing SLNB for invasive breast cancer from March 1, 1998, through March 31, 2001. SETTING: University-affiliated tertiary care hospital. PATIENTS: Two hundred fifty-seven consecutive patients undergoing successful SLNB. MAIN OUTCOME MEASURES: Correlation of patient age, tumor size, histological subtype, grade, lymphovascular invasion (LVI), host lymphoid reaction, border of neoplasm, characteristics of ductal carcinoma in situ, presence or absence of lobular carcinoma in situ, estrogen receptor, and her-2-neu oncogene status with positive SLNB results. Univariate and multivariate analyses were applied. Stepwise multiple logistic regression analysis identified variables predictive of positive SLNB results (P< or = .05). RESULTS: : Regression analysis identified tumor size and LVI as the only variables predictive of positive SLNB results. Results of SLNB were positive in 73 (28.4%) of 257 patients (T1a, 5/37 [13.5%]; T1b, 19/93 [20.4%]; T1c, 37/103 [35.9%]; and T2, 12/24 [50.0%]). Although LVI did not correlate with tumor size, it was highly predictive of positive SLNB results by means of the following equation: 1/(1 + e(-z)), where z = 0.3079 + 1.3814 (LVI), -1.1869 (T1a), -0.3235 (T1b), or +0.5724 (T1c). CONCLUSIONS: Tumor size and LVI were the only variables independently predictive of positive SLNB results. Lymphovascular invasion was the strongest predictor. These data show a higher than expected incidence of positive SLNB for all tumor sizes, which may be explained by results of immunohistochemical analysis of sentinel lymph nodes, and which suggest that all patients with invasive breast cancer should be offered SLNB. Further studies with a larger cohort of patients are warranted.

Multivariate analysis of laminar patterns of neurodegeneration in posterior cingulate cortex in Alzheimer's disease.

Posterior cingulate cortex is the site of earliest reductions in glucose metabolism and qualitatively different laminar patterns of neurodegeneration in Alzheimer's disease (AD). This study used multivariate analyses of area 23 in 72 cases of definite AD to assess relationships between laminar patterns of neurodegeneration, neurofibrillary tangle (NFT) and senile plaque (SP) densities, age of disease onset and duration, and apolipoprotein E (ApoE) genotype. No age-related changes in neurons occurred over four decades in 17 controls and regression analysis of all AD cases showed no relationships between neuron, SP, and tau-immunoreactive NFT densities. Principal components analysis of neurons in layers III-Va and eigenvector projections showed five subgroups. The subgroups were independent because each had a full range of disease durations and qualitatively different laminar patterns in degeneration suggested disease subtypes (ST). Cases with most severe neuron losses (STSevere) had an early onset, most SP, and highest proportion of ApoE epsilon4 homozygotes. Changes in the distribution of NFT were similar over disease course in two subtypes and NFT did not account for most neurodegeneration. In STII-V with moderate neuron loss in most layers, cases with no NFT had a disease duration of 3.5 +/- 0.9 years (mean +/- SEM), those with most in layers IIIc or Va had a duration of 7.3 +/- 1 years, and those with most in layers II-IIIab had a duration of 12.1 +/- 1 years. In STSevere, cases with highest NFT densities in layers II-IIIab also were late stage. Finally, epsilon4 homozygotes were most frequent in STSevere, but four statistical tests showed that this risk is not directly involved in neurodegeneration. In conclusion, multivariate pattern recognition shows that AD is composed of independent neuropathological subtypes and NFT in area 23 do not account for most neuron losses.

Multivariate analysis of relative growth in the limb bones of Arikara Indians.

The description of relative growth in the limb bones of Arikara Indians from a large archeological sample (N = 412 nonadults) is approached by (1) using a multivariate generalization of the bivariate scaling relationship, and (2) extracting the principal components of log shape. The two methods are shown to be complementary, and advantages accrue to both. Although most of the variability among individuals is related primarily to differences in size, significant age-related differences in shape are disclosed by both approaches. Negative forelimb growth allometry contrasts with positive hind limb allometry and results in decreasing intermembral indices with changing size and age. A within-limb, disto-proximal growth gradient is also evident, inasmuch as the proximal element within each limb grows faster than the distal. A functional link to the human specialization for bipedal locomotion and posture is apparent. A comparison between the ontogenetic results of this study and those from an analysis of static adult scaling from the same populations discloses no similarities between the two and suggests that one type of data cannot be substituted for the other.

Blood alcohol level among emergency room patients: a multivariate analysis.

This article examines the distribution of positive blood alcohol levels (BALs) among emergency room patients and the relationship between intoxication and other variables. Approximately one-quarter of the 640 subjects had positive blood alcohol levels and 16.3% had BAL readings above 0.10%, the legal limit for operating a motor vehicle in most states. Chi-square tests and logistic regression analysis indicated a significant relationship between employment status, gender, educational attainment, age and BAL. There were no significant racial differences. Analysis of the role of the presenting problem with BAL revealed that being involved in an accident was significantly predictive of a high BAL in a multivariate context. However, patients presenting with psychiatric problems had overall higher levels of intoxication than other patients. These findings have major implications for public policy formulation vis-à-vis public health delivery.

Multivariate analysis of risk factors for sensitization and early rejection episodes in a donor-specific transfusion plus azathioprine protocol.

A total of 205 patients who underwent donor-specific transfusions with azathioprine (DST+AZA) treatment from 1982 to 1987 included 25 patients (12%) who developed antibodies cytotoxic to donor T cells (+T warm crossmatch). When we examined a series of 14 variables in relation to the risk of sensitization using a univariate analysis, panel-reactive antibody (PRA) pre-DST of greater than 40%, pregnancy, female sex, male-to-female DST, and HLA antigen match all were significant (P less than .05) risk factors. Of these variables, only PRA pre-DST greater than 40% proved to be statistically significant (P less than .002) in a multivariate logistic regression model (overall P less than .001). Of those patients who went on to transplant with the donor-specific kidney, 41/170 (24%) developed early on or before day 5 posttransplant) rejection episodes. Univariate analysis of 17 variables in relation to early (less than 5 days) rejection episodes indicated no significant variables. Nonetheless using a multivariate logistic regression model (overall P = .09), HLA match (P less than .004), perioperative transfusions (P less than .05), and azathioprine withdrawal for greater than 1 week during DST (P less than .05) all correlated with the incidence of early rejection episodes. In a smaller multivariate model involving 47 patients for whom MLC/blocking factor data were available, the presence of plasma blocking factor at the time of transplant was associated with a decreased incidence of early rejection (P = .07). We conclude that the factors that influence the rate of T+ sensitization are distinct from those that influence the rate of early rejection episodes in a DST+AZA protocol.

Multivariate analysis of outcome of mental health care using graphical chain models. The South-Verona Outcome Project 1.

BACKGROUND: Short-term outcome of mental health care was assessed in a multidimensional perspective using graphical chain models, a new multivariate method that analyses the relationship between variables conditionally, i.e. taking into account the effect of antecedent and intervening variables. METHODS: GAF, BPRS, DAS (at baseline and after 6 months), LQL and VSSS (at follow-up only) were administered to 194 patients attending the South-Verona community-based mental health service. Direct costs in the interval were also calculated. Graphical chain models were used to analyse: (1) the associations between predictors (psychopathology, disability, functioning, assessed at baseline); (2) the effects of predictors on costs; and (3) the effect of predictors and costs on outcomes (psychopathology, disability, functioning, quality of life and service satisfaction) as well as their correlation. RESULTS: Psychopathology, disability and functioning scores at baseline predicted the corresponding scores at 6-month follow-up, with greater improvement in the more severely ill. Higher psychopathology and poorer functioning at baseline predicted higher costs and, in turn, costs predicted poorer functioning at follow-up. Outcome indicators polarized in two groups: psychopathology, disability and functioning, which were highly correlated; and the dyad service satisfaction and quality of life. Service satisfaction was highly related to quality of life and was predicted by low disability and high dysfunctioning. No predictors for quality of life were found. CONCLUSIONS: Graphical chain models were demonstrated to be a useful methodology to analyse process and outcome data. The results of the present study help in formulating specific hypotheses for future studies on outcome.

Group 6: Pleiotropy and multivariate analysis.

Analysis techniques using data on several traits simultaneously allow researchers to dissect the genetic architecture affecting correlated traits, and can increase the power to detect pleiotropic genes, i.e., genes that influence two or more traits. Several of the papers in this group from Genetic Analysis Workshop 13 presented promising univariate summaries of multiple traits that detected linkage signals that standard single-trait univariate methods did not. Other papers found linkage signals using multivariate techniques that univariate techniques missed, although this was not uniformly the case. Some papers also considered the correlation among measurements of a single trait taken at different ages to assess whether the genetic architecture of the trait changed over age. Applications of the Framingham Heart Study data identified major loci jointly influencing body mass index and high-density lipoprotein cholesterol, high-density lipoprotein cholesterol and triglycerides, total cholesterol and triglycerides, and various combinations of four traits involved in metabolic syndrome.

A multivariate analysis of familial associations of lipoprotein levels in the Lipid Research Clinics Collaborative Family Study: I. Familial correlation and regression analyses.

In view of the complex, intraindividual relationships among different lipoprotein levels (LDL-C, HDL-C, and VLDL-C), multivariate methods aimed at assessing joint familial associations and their possible determinants were performed in the white, random sample component of the Collaborative Lipid Research Clinics Family Study data (1,336 families with 5,097 subjects). After appropriate transformations and covariate adjustments of the data, several kinds of correlation and regression analyses were performed, taking into consideration variable family size and possible age and clinic differences. The association patterns across clinics and age strata were found to be homogeneous for the vast majority of comparisons. The results of multivariate analyses (especially the significant association of each lipoprotein among biological relatives), the persistence of parental levels as the best predictors for the same lipoprotein levels among the offspring, and the essentially unchanged partial correlation estimates as compared to ordinary correlations suggest strong influence of factors specific to each lipoprotein in the familial associations. But the highly significant intraindividual correlations and the nonnegligible cross-correlations among relatives also suggest the additional presence of common underlying factors for the familial associations, especially between HDL-C and VLDL-C and to a lesser extent between LDL-C and VLDL-C. The issues stemming from these analyses and the directions for further analyses are discussed.

Motor activity and the mesotelencephalic dopamine function. II. Multivariate analysis of genetically segregating generations.

Previous experiments on genetically different inbred strains of mice demonstrated parallel variations between the activity of regional brain tyrosine hydroxylase (TH) and locomotor behavior. Based on these associations, it was hypothesized that genetic variations in mesotelencephalic TH activity, an index of dopamine neurotransmitter function, would correlate positively with exploratory and locomotor behavior. In order to test this hypothesis, open-field motor behaviors and mesencephalic and striatal TH activities were analyzed by multivariate statistical methods in genetically segregating (C57BL/6ByJ X BALB/cJ)F2 and (C57BL/6ByJ X CXBI/ByJ)F2 generations. Factor analysis, based on correlation matrices of variables with significant genetic dominance or additive effects, demonstrated that locomotor activity and frequency of occurrence of various motor patterns were not correlated with mesencephalic and striatal TH activity. These results indicate that the assumption of a positive phenotypic correlation between spontaneous motor activity and mesotelencephalic TH activity does not hold in genetically segregating populations. Strategies and problems in revealing the behavioral consequences of genetically based variations in the mesotelencephalic DA system are briefly discussed.

Ovarian carcinoma: a multivariate analysis of prognostic factors.

Five hundred and fifty six women with invasive epithelial ovarian carcinoma were assessed for postoperative treatment between 1966 and 1976. The records of this group were reviewed retrospectively. Sixteen characteristics of the patient and tumor were analyzed for prognostic significance by univariate and multivariate techniques. Tumor grade, the presence of residual disease, and patient performance status are identified as stage-specific independent prognostic factors. These independent factors define patient subsets with good, intermediate, and poor prognosis within each stage. The results show that more effective treatment strategies are required for patients in poor prognosis subsets. Estimates of tumor grade, extent of residual disease, and performance status should be included in reports of treatment outcome.

A multivariate analysis of circulatory data and their relationship to later disease.

Measurements on 21 separate variables involving blood pressure and heart rate were obtained under casual, resting, and controlled conditions as part of the baseline evaluation of 1087 white males participating in a prospective investigation of possible precursors of premature disease and death. These measurements were subjected to a components-type factor analysis with orthogonal rotation of component vectors in an effort to construct statistically independent composites of greater predictive power and generality as well as to minimize problems in interpretation introduced by partially redundant variables. Multivariate and univariate techniques were afterwards used to test differences between groups of subjects who developed specific major disorders and those who remained in good health. Comparison of factor scores with original circulatory measures in a series of prospective analyses revealed that the former had the greater predictive power. Although only the hypertension group could be unequivocally differentiated from all others, a number of nominally-significant findings may provide leads for further research.
