Papillon-Lefevre syndrome: report of two brothers.

Papillon-Lefevre syndrome was observed in two boys in the same family, and the topic of this report, the second to appear in the Greek dental literature. Both patients presented with hyperkeratosis of the palms and soles and severe periodontal destruction; the latter was more prominent in the younger boy. The children will lose their teeth before reaching maturity.

[Spontaneous correction of the rotation of the permanent central lower incisors].

On the basis of a series of dental models from children between five and nine years (twice-yearly examination) with final models being made of 319 of these children at the age of 16, determinations were made of the rotation of the permanent central lower incisors 'shortly after complete eruption of the cutting edge' at the age of 16. The study showed that spontaneous correction amounts to 53% and 60% respectively for an original mesio-lingual rotation (type less than 90 degrees) and for an original disto-lingual rotation (type greater than 90 degrees).

[Clinical and clinico-histological markers in chronic destructive adult periodontitis].

This study was designed to evaluate the significance and interrelationship of clinical parameters and their association with histologic changes in advanced destructive periodontitis. 158 patients with PDI greater than 4 (Ramfjord) were selected, evaluating the size, contouring, bleeding, consistency, colour and gingival pain. Epithelial ulceration of soft periodontal pockets were also evaluated. The results showed a statistically significant association between purplish colour and gingival fibrosis and advanced stage of the disease. Gingival bleeding on probing was the most important clinical parameter in advanced phases of the disease, either alone or in association with other parameters such as the presence of epithelial ulcerations. The Periodontal Disease Index (Ramfjord) has proven effective in the evaluation of generalized patterns of disease.

[Indications and technic of transdental fixation].

Transdental fixation, as closed implant, offers the most favourable preconditions of all dental procedures of implantation. Teeth where the root-crown relation is altered to the detriment of root length, which is held firm in bone, can with its help be made functional. Loosening and premature loss of teeth can be prevented. Further discussed is the indication for this procedure, especially in tooth fractures and luxations as well as the operative steps. Up to now transdental fixation has proved itself although the number of cases and the follow-up time are insufficient to allow final judgment.

[Expansion of the scope of indications for Wirz' transfixation method].

Transfixation after Wirz, a closed implantation, serves to conserve periodontally weakened teeth. The method can be recommended to the practitioner. It is subdivised into 6 phases which are described. The treatment method may be successfully enlarged clinically and topographically. 7 cases are demonstrated and show the scope of indication.

Patient evaluation.

Failure to diagnose periodontal disease is a frequent and unnecessary error. A new periodontal screening examination has been proposed that will accurately detect periodontal disease in approximately five minutes and can be incorporated into a routine dental examination. Simplified treatment planning based on the findings of the periodontal screening examination has also been proposed. Its use could significantly improve the quality of periodontal care for most dental patients.

Juvenile periodontitis (periodontosis).

Juvenile periodontitis, characterized by early age of onset, rapid rate of periodontal tissue destruction and selective tooth involvement in the absence of commensurate aetiological factors, is recognized as a distinct clinical entity. The present paper describes the clinical findings and some immunological characteristics in a group of thirty-four patients under the age of thirty. Clinical aspects studied included the influence of sex, race, family history and general health. Immunological findings relating to the level of immunoglobulins in serum and saliva, lymphocyte transformation in response to veillonella, bacteroides, fusobacterium and actinomyces species and to ultrasonicated plaque, and the inhibition of macrophage migration by these antigens. Differences were shown to exist between those patients below the age of 22 years and the group aged 23-29 years.

[Analysis of movement in partial prostheses and abutment teeth with differences in retainer].

When designing a prosthesis in a clinical setting that takes into consideration the protection of the abutment teeth and alveolar ridge, it remains difficult to carry out a method of connecting the abutment tooth and prosthesis based on objective grounds. This is because there are still many matters that remain unclear about dynamics of the stomatognathic system, including the prosthesis, when attached in the oral cavity. The author created the following three prostheses on a stimulation model of a tooth bone saddle that included an artificial mucous membrane and periodontal membrane: (1) A fixed bridge, (2) A telescopic denture (Konuskronen Telescope), (3) An Akers clasp denture. Following this, flat-controlled random noise was applied to each denture to create a dynamic load that exhibits a state closer to a clinical situation. Modal analysis was carried out with a laser doppler vibrometer and observations were made of the prosthesis and the abutment tooth apex to obtain visual clarification of dynamics, The following results were obtained. 1. As far modal resonant frequency, in mode #1, the bridge measured at 483 Hz and 561 Hz, the Telescope at 452 Hz and 600 Hz and the clasp at 581 Hz and 596 Hz. In mode #2 the bridge measured at 1,355 Hz and 1,370 Hz, the Telescope at 1,343 and 1,040 Hz and the clasp at 1,114 Hz and 1,079 Hz. In mode #3, only the bridge was detected and the recorded values 1,906 Hz and 2,026 Hz. 2. The influence on the modal resonant frequency due to the difference in abutment devices was limited. 3. Observation of animation indicated that the tendency for lateral movement of the abutment teeth increased going from the bridge to the Telescope to the clasp. 4. Displacement of the abutment tooth root apex decreased in reverse proportion to the strength of the connection between the abutment tooth and the abutment device.
