[The Cerebral Function Monitor. Description, functioning and interpretation principles].

The Monitor of Cerebral Function enables continuous monitoring of the cerebral electrical activity and this over long periods due to the slow recording speeds. The cerebral electrical signals picked up by the electrodes attached to the scalp are registered in the form of a curve which fluctuates to a greater or lesser extent depending on the recording speed. The examination of the height of the curve with respect to zero and its amplitude indicates the voltage of the cerebral activity and yields information regarding polymorphism. It is thus possible to monitor variations in cerebral activity over a prolonged period during anaesthesia as well as during the revival phase with the Monitor of Cerebral Function, the electroencephalogram being reserved as an aid to precise diagnosis and for localization and close scruting of the anomalies.

Carrier and hapten functions in immune deviation. I. Contrary effects of hapten and carrier on cellular and helper functions of T cells.

Carrier and hapten functions have been studied in the immune deviation phenomenon. Delayed hypersensitivity to the carrier and anaphylaxis and Arthus hypersensitivities to the hapten and to the carrier were studied in guinea-pigs injected intravenously with large doses of carrier, homologous and heterologous hapten-carrier conjugates and subsequently immunized with the hapten-carrier conjugate in Freund's complete adjuvant. Pretreatment with DNP-BSA or with HGG were found to modify, in opposite directions, the hypersensitivity reactions induced by DNP-HGG in adjuvant. It is suggested that the hapten and carrier moieties of the antigen molecule might have antagonistic effects on the T cells responsible for cellular immunity as well as on T cells involved in helper functions for B cells.

Immunologic function of endothelial cells: guinea pig aortic endothelial cells support mitogen-induced T lymphocyte activation, but do not function as antigen-presenting cells.

The possibility that vascular endothelial cells (EC), like macrophages (M phi), can function as accessory cells necessary for mitogen- and antigen-induced T cell activation was examined. EC were enzymatically detached from the luminal surfaces of guinea pig aortas and then propagated in culture. Lymph node T lymphocytes were rigorously depleted of adherent cells, such that they completely lost the capacity to respond to mitogenic stimulation with phytohemagglutinin or concanavalin A. In this system, EC restored mitogen-induced T cell DNA synthesis as effectively as did M phi. This effect could not be explained by a facilitation of residual accessory cell activity within the responding T cell population, because EC restored mitogen responsiveness to T cells that had been treated with anti-Ia antibody and complement. Support of mitogen responsiveness could not be accounted for by secreted products of M phi or EC in the absence of intact accessory cells. In addition to the capacity to serve as fully sufficient accessory cells for the induction of mitogen-stimulated T cell proliferation, EC exerted a number of modulatory influences on T lymphocyte responses in cultures supported by M phi. When such cultures were supplemented with small numbers of EC, responses were dramatically augmented; larger numbers of EC resulted in marked suppression. At least part of these immunomodulatory effects could be accounted for by the activity of secreted products of EC. EC did not express detectable Ia antigens assayed either by indirect immunofluorescence, with the use of the fluorescence-activated cell sorter, or by complement-mediated cytotoxicity. Moreover, treating the EC population with anti-Ia antibody and complement had no effect on its capacity to support mitogen-induced T cell DNA synthesis. As would be expected from the lack of Ia antigen expression, EC were incapable of presenting antigen to primed T cells. They did, however, carry enough antigen into the cultures such that effective antigen presentation could occur when the cultures were supplemented with M phi that were syngeneic but not allogeneic to the responding T cells. Moreover, EC were capable of dramatically augmenting antigen-specific responses stimulated by antigen-pulsed M phi. There was no genetic restriction for this EC-mediated augmentation of antigen responsiveness. These results indicate that EC are capable of functioning as completely sufficient accessory cells for mitogen-induced T cell DNA synthesis and, in addition, are able to modulate ongoing M phi-supported T lymphocyte responses in both a positive and negative manner.(ABSTRACT TRUNCATED AT 400 WORDS)

Functions of human monocyte and lymphocyte subsets obtained by countercurrent centrifugal elutriation: differing functional capacities of human monocyte subsets.

Two monocyte subsets from the peripheral blood of healthy humans were isolated by countercurrent centrifugal elutriation (CCE), and their functional heterogeneity was assessed. Cell volume of the smaller monocyte population was (332 +/- 32 mu 3), compared with the larger monocytes (380 +/- 30 mu 3). Superoxide release by large monocytes exposed to opsonized zymosan was 1.5-fold more active than those of small monocytes. Furthermore, the production of colony-stimulating factor (CSF) and myeloperoxidase activity by the larger monocytes was 2-fold and 1.4-fold greater than that of small monocytes, respectively. Thus, CCE provides a useful means to isolate 2 subsets of monocytes with similar functional properties but quantitatively different activities. This suggests that the 2 subsets may be closely related in their developmental spectrum.

[The assessment of the function of the nervus intermedius by means of functional salivary gland-scintigraphy].

Using functional scintigraphy of the salivary glands, the function of the nervus intermedius can be assessed by estimating excretory quotients for both submandibular glands. This method is preferred to the standard salivation test method of Magielski and Blatt, despite a minimal exposure of the patient to radiation from the injected Natriumpertechnetat. The technical course of this investigation, along with the indications for its use, will be presented.

Membrane structure-function relationships in cell-mediated cytolysis. I. Effect of exogenously incorporated fatty acids on effector cell function in cell-mediated cytolysis.

The fatty acid composition of the membrane phospholipids of CTL memory cells restimulated with Con A can be altered significantly when selected exogenous fatty acids are present during the restimulation culture period. Such alterations also lead to significant changes in the subsequent cytolytic capacity of the regenerated CTL. Unsaturated fatty acids increase cytotoxicity, whereas saturated fatty acids lead to decreased cytotoxic function. The kinetics of the fatty acid effect on CTL function is the same as the kinetics of fatty acid incorporation into membrane phospholipids, suggesting that alteration of cytotoxicity results from altering membrane phospholipid fatty acid composition. The initial studies reported here suggest that this effect is mediated at the level of individual CTL, rather than through an alteration of CTL frequency in effector cell populations. Moreover, it seems unlikely that the fatty acids mediate their effect by altering membrane fluidity.

[Functional ability of the ileo-ureter determined by camera function scintigraphy].

The function of the ileo-ureter and the operated kidney was investigated by means of the camera functional scintigraphy in 23 dogs operated under different techniques. This non-invasive examination was carried out with two different radio-remedies (131I-Hippuran and 99mTc-DTPA) by a gamma camera microprocessor. The direct single-layered ileo-ureteric anastomosis showed the best urine transportation. This method has a great practical importance in the opinions of the authors.

Correlation between the morpho-functional organization of some portions of the visual analyser of chelonia and their ecology: I. Normal morpho-functional characteristics of the optic nerve and the tectum opticum.

Several methods (light optic, electron microscope and electrophysiological) were used to study the optic nerve and the tectum opticum of water turtles (two species) and of land tortoises (two species). As the result of study of the two levels of the visual analyser in four species of Chelonia three groups of signs of the morpho-functional organization were distinguished: signs common for the Chelonia under study; ecologically specific signs--common for water turtles and species-specific traits which are--different in species of the same ecological group. Specific features of structural-functional organization of the visually directed behaviour in Chelonia at the neuronal populations level were shown to correlate with the ecological features to a greater degree than with the systematic position of the species.

Correlations between the morpho-functional organization of some parts of visual analyzer and ecology in Chelonia: II. Morpho-functional characteristics of the visual nerve and tectum after unilateral enucleation.

The morpho-functional characteristics of the optic nerve and tectum opticum were studied electronmicroscopically and electrophysiologically after unilateral enucleation in 4 Chelonia species. Different types of degeneration of the myelinated and unmyelinated optic fibers and their endings in the tectum are revealed. Comparison of the types and times of the degeneration of retinal axons and their terminals with changes of the tectum evoked potentials at different times after enucleation gives ground to conclude that myelinated fibers produce nerve endings degenerating on the dark pattern and unmyelinated, all the remaining endings with the light, neurofilamentous, glycogenous, vesicular and other degeneration patterns. A stratal arrangement of the retino-tectal projections of various type fibers is demonstrated. Asynchrony of the process of degeneration of different groups of fibers and the relationship between the rate of degeneration and the diameter of myelinated fibers, as well as the absence of a relationship between the change pattern and the diameters of myelinated and unmyelinated axons are revealed. In land tortoises and water turtles differences are revealed in the set of major degeneration patterns and their endings also in the patterns of stratal distribution of various categories of the retinal fibers and tectum. A possible relationship is proposed between the above differences and ecological properties of the Chelonia groups under study. Also revealed are species-specific features of some substantial aspects of the morpho-functional organization of the levels of the visual analyzer in Chelonia.

Interaction of adrenaline and acetylcholine on cardiac pacemaker function. Functional inhomogeneity of the rabbit sinus node.

In the isolated rabbit sinus node, adrenaline induced a pacemaker shift toward the inferior part of the node (distance, 2.5 mm). Acetylcholine gave a pacemaker shift either toward the inferior part of the node (distance, 1.2 mm) or, at higher acetylcholine concentrations, toward cells near the upper part of the crista terminalis. After severing the superior from the inferior part of the preparation, we compared the chronotropic responses in the two parts. The node appeared to be functionally inhomogeneous: in the inferior part the adrenaline-induced acceleration was larger and the acetylcholine-induced deceleration was smaller than in the superior part. In the total preparation and the isolated inferior part, the acceleration due to adrenaline was suppressed by acetylcholine and the deceleration due to acetylcholine was enhanced by adrenaline. Such an interference could not be established in the isolated superior part. The acetylcholine-induced pacemaker also dominated during the simultaneous addition of the two neurotransmitters. We conclude therefore that the functional inhomogeneity of the sinus node explains the predominance of the effect of acetylcholine over that of adrenaline.

[Motor functions and their analysis in the developmental neurology. Part II: Spontaneous motility and straightening functions (author's transl)].

In a prospective longitudinal study, we investigated all low-risk infants on five occasions up to the end of their first year of life. In the second part of the report, we present the developmental changes of spontaneous motility and straightening functions. The consideration of behavioural state is important for the outcome of neurological testing. The changes of spontaneous motility (intensity, speed and quantity of motions, startles, tremor, prehension ...) are sufficient parameters for developmental diagnosis. Some functions (goal directed arm-motion, prehension...) are correlated to environmental influences. The usual practice of developmental diagnosis is insufficiently directed to developmental processes in the CNS.

[Pleuropulmonary aspergillosis with defective neutrophil functions--G-CSF effects on neutrophil functions].

A 66-year-old man was admitted to our hospital with productive cough and fever. Chest X-ray films showed pleural thickening of the right apex and calcification of the lung field. Antibiotic therapy was administered but no improvement was obtained. Chest CT scan revealed pleural thickening with effusion in the right apex. Needle aspiration biopsy was performed and Aspergillus fumigatus was isolated from pleural fluid. We began pleural drainage and intrapleural infusion of miconazole, with systemic administration of miconazole, amphotericin B and fluconazole. In spite of normal bone marrow findings, the neutrophil functions, NAP score and phagocytosis, of the patient were found to be subnormal. We commenced to treatment with granulocyte colony stimulating factor and his neutrophil functions improved (NAP score: 156-->309.5, phagocytosis: 64.9-->70.0%). However, his fever persisted and he died following cardiopulmonary arrest. Autopsy revealed Aspergillus mycelia invading the visceral pleura. No intrapulmonary lesions caused by Aspergillus were found.

[Cardia function following Billroth I and Billroth II gastrectomy (studies on the significance of duodenal passage for cardia function)].

To clarify whether the duodenum is important to the regulation of cardia function, patients with B-I and B-II resections were investigated manometrically. A test meal can stimulate the LES in patients with a B-I resection almost to the same extent as that in healthy test persons. Symptom-free patients with the distal stomach resection and gastrojejunostomy (B-II) do not show this pressure reaction. Therefore, the duodenal passage could be of importance to cardia function. In patients with a transformation operation of B-II in B-I, because of postgastrectomy symptoms a stimulation of the LES cannot be achieved either before or after the transformation operation. We cannot presently clarify the type of the cardia regulation since the IRG levels measured do not explain this phenomenon.

[Significance of gastric and duodenal motor function in guaranteeing full functional value of cholecystectomy].

Gastric and duodenal motor function was studied using ionoballoono-tensiokinesiography method in 77 patients with chronic calculous cholecystitis. Various periodic motor activity (PMA) infringements, both hypo- and hyperkinetic types, were diagnosed, mainly associated with duodenal hypertension, duodenogastric reflux and reflux-gastritis. Principles of pre- and postoperative correction of the PMA state for the guarantee of cholecystectomy functional full value were determined.

[The effect of radon baths at Khmel'nik health resort on the central hemodynamic indices, on thyroid function and on adrenal glucocorticoid function in hypertension patients].

As many as 63 patients with stage I, II hypertensive disease were examined. Characteristics of central hemodynamics were studied by means of tetrapolar thoracic rheography, the content of thyroid-stimulating hormone, thyroxin, triiodothyranine, hydrocortisone were determined by radioimmunologic method. All investigations were done before and after the patients having radon baths. Hypotensive effect of radon baths in hyperkinetic type hemodynamics was manifested by a significant drop in cardiac index and increase in specific peripheral resistance. In hypokinetic type hemodynamics there was a significant fall in average hemodynamic pressure, and improvement of the arteriole patency. The levels of thyroxin and hydrocortisone have raised significantly. The Khmel'nik health resort radon baths have hypotensive effect in HD patients, enhance the function of the thyroid gland and glucocorticoid function of the adrenals.

Functional MR imaging of perceptual and cognitive functions.

Functional MR imaging using the blood oxygen level dependent technique as an endogenous contrast agent localizes eloquent cortex when stimulated by a variety of motor or sensory tasks. To date, visual, motor, speech, and cerebellar studies have been conducted, revealing significant information regarding cortical localization and control of various functions. The technique is also an important management tool for localizing eloquent cortex contiguous to a tumor or arteriovenous malformation, so that injury can be avoided if ablation of the lesion is contemplated.

[Influence of radiotherapy and chemotherapy on the function of malignant tumor patients and regulation function of acupuncture].

The observation on the indexes of cortisol, estradiol, estriol and testosterone showed that incretory function of malignant tumor patients had different extent of pathologic changes, after radiotherapy and chemotherapy, making the change strengthened. Acupuncture can regularize this disorder of incretory function of patients treated with radiotherapy and chemotherapy to some extent.
