[An evaluation of the efficacy of chronotheray with heparin and kurantil in patients with chronic pyelonephritis].

The effectiveness of traditional therapy (TT) and chronotherapy (CT) with heparin and curantyl was comparatively studied in 80 patients suffering from chronic pyelonephritis. The blood clotting variables were determined every 4 hours before and 2.5-3 weeks after TT and CT in combination with antibacterial therapy. The findings suggest that CT is more effective than TT with heparin and curantyl. With CT, steady-state clinical effects, positive dynamics of laboratory findings and a greater trend to normalization of the circadian organization of hemocoagulation were achieved in earlier periods when lower daily and course doses of the agents were used.

Local immunie response in experimental pyelonephritis in the rabbit. III. Lymphocyte responsivieness to O and K antigens of Escherichia coli.

The response of circulating and infected kidney lymphocytes to the O (lipopolysaccharide) and K (polysaccharide) antigens of an Escherichia coli O6 K 13 H1 strain was determined. Both circulating and kidney lymphocytes showed significant incorporation of [3H-methyl]thymidine into DNA when incubated with the O antigen, whereas neither responded to the K antigen. The lipid moiety of the lipopolysaccharide was required for lymphocyte responsiveness. Upon sequential incubation of O antigen and fluoresceinated homologous antiserum, 24-30 per cent of kidney lymphocytes were shown to have surface receptors for O antigen, whereas none had surface receptors for K antigen. Although the K antigen is an important determinant of invasiveness of the upper urinary tract, it fails to elicit a cellular immune response or attach to lymphocytes from the infected kidney in experimental pyelonephritis.

[Adenovirus-induced kidney graft pyelonephritis following renal transplantation].

A 16-year-old female received a kidney transplantation from her mother 13 months before she suddenly noticed gross hematuria and painful micturition, and developed high fever with chills. The serum creatinine (S-Cr) level rose from 1.5 to 2.6 mg/dl, but there was no clinical sign of acute rejection. Despite the treatment with antibiotics and gamma-globulin, the the high fever and hematuria did not improve. The adenovirus antibody titer elevated from x8 to x1,024, while adenovirus was not isolated from the urine. On the 15th day of the disease, hematuria disappeared spontaneously and on the 19th day she became afebrile. The S-Cr level also was normalized spontaneously. Histological examination of the graft biopsy on the 14th day, showed severe tubulointerstitial nephritis localized in the renal medulla and full type intranuclear inclusions were revealed in tubular epithelial cells. From these findings, we diagnosed this case as adenovirus-induced kidney graft pyelonephritis associated with acute hemorrhagic cystitis.

[Physical factors in the therapy of secondary chronic pyelonephritis].

The authors have developed combined therapy of secondary chronic pyelonephritis with immune and urodynamic dysfunctions. The treatment includes physical factors: decimeter microwaves, gangleron-electrophoresis with sinusoidal modulated currents, sodium chloride baths, oral mineral water Moskovskaia. The adjuvant use of the above physical factors proved advantageous.

[The role of ultrasonic scanning in optimizing the therapy of gestational pyelonephritis].

Urinary tracts were examined by ultrasonic scanning (US) of the kidneys in 130 patients with gestational pyelonephritis. The scanning provided a rapid diagnosis of parenchymal and paranephric fat lesions, of urinary tract occlusion, location of concrements, congenital and acquired renal defects, abscesses, carbuncles. The follow-up US controls the inflammation, permitting differential treatment under reduced exposure to x-rays and radionuclides.

[Bacterial toxic shock as a complication of calculous pyelonephritis].

The causes of bacteriotoxic shock were examined in 33 patients (11 lethal outcomes). It has arisen as a complication of treatment given to 7830 patients for urolithiasis. Aggravation of chronic pyelonephritis, occlusion of the urinary tracts, urogenital mucosal and parenchymal injuries, low resistance to infection contribute to microbacteria and their toxins entering blood with resultant bacteriotoxic shock. Transcutaneous operative interventions, therapeutic and diagnostic procedures also produce high risk of blood infection with gram-negative microflora, especially in violation of asepsis and antisepsis rules. As shown by microflora tests, the dominating bacteria consisted of opportunistic agents which had acquired the resistance to antibacterial drugs. In view of rapid progression of bacteriotoxic shock therapeutic efforts should be concentrated on fast normalization of hemodynamics, recovery of urine passage, introduction of sorption detoxication, prevention of DIC syndrome. The schemes of combined antibiotic treatment adjusted to the kind of infectious agent are suggested.

[The clinical importance of histocompatibility antigens and their combinations in the diagnosis of chronic pyelonephritis].

Distribution of histocompatibility antigens and combinations thereof were investigated in citizens of St. Petersburg. All the examinees suffered from chronic pyelonephritis (CPN). The occurrence of antigens A1 and B17 in the patients was significantly high and associated with increased risk and etiological fraction. Combinations of antigens were frequently encountered in one locus: A1, A2, interlocus combinations A1, B5, A1, B7, A1, B17. Combinations of these antigens (especially A1 + B17) were indicative of high CPN risk. Frequent incidence of antigens A1, B7; A1, B5 may be due to the fact that in the above combinations they are responsible for higher CPN-risk. Increased incidence rate of antigens HLA-A1 and HLA-B17 evidences their involvement into CPN pathogenesis. Strong association of the disease with antigens A1, B17 seems to be typical for St. Petersburg citizens.

[The effect of Kuka mineral water on blood coagulability and fibrinolysis in patients with chronic pyelonephritis].

Chronic pyelonephritis (CP) is now ranking first among renal disorders, its incidence still tends to grow. Despite an impressive antibacterial armory, the efficacy of CP treatment remains low. In search of more effective and tolerable methods, the author directed his attention to CP balneotherapy. In addition to well-known health resorts where CP is successfully treated, Zheleznovodsk and Truskavets, a less known place is mentioned. It is the spa Kuka in the Transbaikal region. Mineral water Kukinskaya or Kuka (pH 6.18) comprises carbon dioxide, hydrocarbon-calcium and magnesium hydrocarbonates. The spa treatment was given to 65 CP patients. They underwent a general examination and evaluation of hemostasis. It was found that oral treatment with Kukinskaya mineral water produced a positive action on the blood coagulability and fibrinolysis. After the crenotherapy the pain persisted in 11.8%, leukocyturia in 9.9%, nephrogenic hypertension in 16.4% of the patients only. This proves the method effective for CP.

The disappearing kidney. A case of emphysematous pyelonephritis.

We describe a case of bilateral emphysematous pyelonephritis in which the left kidney could not be visualized sonographically because of multiple irregular air-fluid interfaces. This observation, which to our knowledge has been noted only once before in the literature, may be a diagnostic clue to this potentially fatal disease.

[Experimental pyelonephritis in rats: value in the comparison of antibiotics (author's transl)].

The author discusses the conditions which an experimental pyelonephritis must fulfill to provide therapeutic results which will allow the assessment of new therapeutic agents. He subjects the chosen model to the therapeutic effect of four antibiotics and three antimicrobial drugs. A numeric value is given to the bacteriuria recession slope observed during the experiment. A comparison of the results being obtained with the bacteriological and pharmacological data normally evaluated, the author demonstrates the value of the present study. The analysis of the results collected during the tests shows that comparison between antibiotics are of greater value when they are of the same family. Nevertheless, the experimental pattern, when applied to antibiotics as different as kanamycine and cephalosporins, give a very good correlation particularly when the overstepping coefficient, as proposed by Canetti, is calculated on the results expressed as free products.

[The determination of the chemiluminescence of stimulated blood leukocytes in assessing the activity of chronic pyelonephritis].

The activity of oxidant metabolism was studied in leukocytes from the whole blood of 23 patients diagnosed to have urolithiasis, chronic pyelonephritis in remission. Leukocyte function was evaluated by registration of luminol-dependent chemiluminescence. Barium sulfate microcrystals served leukocyte stimulator. Chemiluminescence was registered by chemiluminometer NPA-02. Diagnostic implication of chemiluminescence and other clinical-laboratory parameters was derived from the equations of multiple linear regression arranged in a model. It is shown possible to significantly assess chronic inflammation activity in renal tissue by whole blood chemiluminescence and ESR. Low functional activity of neutrophils recorded by chemiluminescence may indicate chronicity of renal tissue inflammation.

Ultrasonographic diagnosis of pyelonephritis in a cow.

A 2-year-old Holstein cow was examined because of a 2-month history of anorexia, decreased milk production, and a white vaginal discharge. Results of physical, laboratory, and ultrasound examinations were consistent with pyelonephritis. Through ultrasound examination, the right kidney and ureter were identified as structurally involved. Right nephrectomy was performed. Eight months after nephrectomy, the cow was clinically normal. In this cow, ultrasonography helped to determine disease severity, to direct the course of treatment, and to improve the accuracy of diagnosis and prognosis.

[The rehabilitation of children with chronic pyelonephritis and who have lived for a long time under radiation-hazard conditions].

Isolated and complex effects of environmental factors on adaptation abilities of the body, function of different organs and systems, composition of peripheral blood and course of disease were studies in 245 children aging from 6 to 14 years with remitted chronic pyelonephritis with normal renal function. Mineral water "Naftusia" (100-200 ml a day), lumbar applications of ozocerite, mineral baths and exercise therapy were used. Our findings give a reason to consider sanatory rehabilitation a promising method of health promotion and correction of functional disorders.
