Effect of chloroquine and mepacrine on the spontaneous and evoked movements of the rat portal vein.

It has been known that chloroquine and mepacrine, through their parasympatholytic and spasmolytic effects, inhibit the gastric motility in rats. In the present study these drugs were shown to prevent by virtue of their antispasmodic property the spontaneous and induced movements in the isolated portal vein of the rat. As measured on isolated fundus strips of the stomach, chloroquine and mepacrine possessed almost the same pD'2 as had papaverine and drotaverine on the isolated portal vein. Chloroquine and mepacrine as well as the drugs used for comparison administered in vivo into the small intestine reached such concentrations in the portal vein blood which were shown to be capable of inhibiting the spontaneous and evoked movements in vitro.

[Twin pregnancies at the gynecology-obstetric unit of the medical center at Sibenik (1959-1978)].

In the period reported there were 244 (1.036%) twin pregnancies out of 23,535 deliveries. From 1959 to 1968 the incidence was 1.133% and from 1969 to 1978 0.946% (P < 0.05). Twins were most frequently delivered in the head presentation (48.36%) but also in such a way that the first twin was delivered in the head and the second in the breach presentation (26.22%), or both in the breech presentation (11.06%), or the first in the breech and the second in the head presentation (9.42%). Other presentations were far less frequent. Spontaneous delivery of the first twin was recorded in 71.31% and of the second in 54,50% of cases (P < 0.05). Surgery was required for the birth of the first twin in 28.69% and of the second in 45.50% of cases (P < 0.05). The birth weight of the first twin was below 2500 g in 38.11% and of the second in 40.98% of cases. The mean birth weight of twins was 2590.67 +/- 2 SD 977.29 g. The first twins were by 70.7 g heavier than the second ones. The perinatal mortality of the first twins amounted to 6.96% and of the second to 11.47% (P < 0.05), their total perinatal mortality amounting to 9.22%. The authors recommend domiciliary rest, preventive cerclage in the majority of cases, spasmolytics, and tocolytics (if needed) from the 20th week of pregnancy and hospitalization after the 28th week of pregnancy, as well as intensive care during and after delivery.

Erythromycin infusions for treatment of infections in the ear, nose and throat region.

Sixteen adult male patients (aged between 20-38 years) with ear, nose and throat infections, caused by bacteria sensitive to erythromycin, received daily infusions of 2 x 1 g erythromycin for 48 hours. The duration of infusion was 30 or 60 minutes, administered at intervals of 12 hours between infusions. Symptoms of intolerance such as nausea, retching, feeling of pressure in epigastric angle as well as abdominal cramps occurred as side-effects in many cases. A spasmolytic was given intravenously to twelve patients; it rapidly eliminated the symptoms. However, these side-effects were considered insignificant compared to the excellent clinical results obtained during infusion therapy. We, therefore, believe that 2 x 1 g/day erythromycin per infusion can be regarded as the drug of choice in chronic and acute ear, nose and throat infections. The rapid resolution of infections, which are otherwise difficult to treat, and the concomitant decrease in confinement to bed by about 8 to 10 days are the most important results in this study.

Clinical and manometric effects of nifedipine in patients with esophageal achalasia.

The effect of a new calcium antagonist, nifedipine, which has a spasmolytic activity on smooth muscle cells, was studied on the esophageal function of 20 patients with achalasia of mild and moderate degree. The study was carried out by using constantly perfused catheters and recording the pressure variations at the lower esophageal sphincter, before and after sublingual administration of 10-20 mg of nifedipine. The drug significantly decreased the lower esophageal sphincter pressure for more than 1 h. A clinical trial was also carried out by assessing the improvement of symptoms in achalasia patients taking sublingually a dose of 10-20 mg of nifedipine before each meal. After 6-18 mo of nifedipine therapy these patients underwent a placebo treatment, whereas an additional group of 9 achalasia patients was treated first with placebo followed by nifedipine. The nifedipine treatment gave excellent or good results in a large majority of patients of both groups. The moderate results were only 5 and the poor responses only 3. The clinical improvement induced by nifedipine was statistically significant when compared, not only with the pretreatment clinical state, but also with the results of the placebo treatment. No tachyphylaxis and few side effects were seen either during the manometric recordings or the longest periods of therapy. This study suggests that nifedipine may be advantageously used in the medical treatment of achalasia of mild or moderate degree.

Toxicological studies on the new veterinary antibiotic turimycin.

The acute toxicity of turimycin investigated in several animal species and following different routes of administration is low. In mice the LD50 values range from 320 mk/kg after iv injection up to more than 3000 mg/kg when administered orally. When lethal or sublethal doses of turimycin were given, animals either developed clonic convulsions and died within 24 hr, or recovered completely. The main pharmacological effect of turimycin is a reversible, dose dependent decrease in the blood pressure after intravenous injection of 10 to 15 mg/kg in cats and dogs. This may be due to the negative inotropic and chronotropic effect on heart muscle (isolated atria of guinea-pigs) and to the spasmolytic effect on the smooth muscle (rat ileum). A temporary decrease of the renal filtration rate after administration of turimycin is probably caused by its influence on the blood pressure. In rats the hexobarbital sleeping time was increased after repeated administration of higher doses of turimycin. In a chronic study on dogs oral doses of 50 and 125 mg/kg of turimycin, given in capsules daily for 12 months, produced no functional or histopathological abnormalities.

[The correction of the metabolic and microcirculatory disorders in patients with atherosclerotic and involutional depressions].

A total of 42 patients with atherosclerotic--and 34 with involutional depression were studied by conjunctival biomicroscopy for particular features of lipid peroxidation and state of microcirculation before and after treatment with vasoactive preparations, antioxidants and calcium antagonists. POL were found to be increased in patients with atherosclerotic depression irrespective of the syndrome, and in involutional depression with anxious symptomatology, as well as in those patients in stuporous state, this being accounted by a decreased provision of the organism with antioxidants as well as by an inadequate activity of antioxidant enzymes; high incidence rate of intravascular and vascular changes was also noted in the above patient populations. Tocoferolum acetatum and corinfar potentiated antiaggregational and spasmolytic actions of vasoactive preparations, which fact made for an earlier eradication of microcirculatory disturbances, decrease in the free radical oxidation activity and recovery of those patients getting out of the psychotic state.

[A simple method for management and treatment results of pregnant women with isthmo-cervical insufficiency].

A simple way of management of patients treated for isthmo-cervical insufficiency in the Department of Obstetrics and Gynaecology, Medical Academy in Bydgoszcz in the years 1980-1990 was subjected to own assessment. During this period the treatment was given to 684 pregnant patients with diagnosed isthmo-cervical insufficiency, laying a circumferential suture onto the cervix after Hervet, between 17 and 33 weeks of pregnancy. The accepted management after the procedure was limited to bed rest and administration of analgesic and spasmolytic drugs. Positive results of the established treatment were found in 95.4% of the patients with diagnosed isthmo-cervical insufficiency which made possible its routine introduction.

[Ultrasonic diagnosis of pyloroduodenal stenosis].

The method and semeiotics of noninvasive ultrasonic diagnosis of pyloroduodenal stenosis are described. Seventy-six patients with duodenal ulcer and 13 patients with gastric ulcer were examined, and also 18 patients with gastric carcinoma and 24 healthy individuals. The echographic picture of an empty stomach in compensated pyloroduodenal stenosis did not differ from that in healthy persons, though study of the motor-evacuation activity of the stomach revealed intensified peristalsis and increased gastric half-evacuation period. In subcompensated pyloroduodenal stenosis the stomach contained fluid and the period of half-evacuation of its contents was sharply increased. All patients with decompensated pyloroduodenal stenosis had an increased volume of the stomach which contained much fluid and food remnants. Pyloric spasm was differentiated by means of the spasmolytics test. Patients with a marked periulcerous inflammatory infiltration must be kept under dynamic follow-up to evaluate the degree of the stenosis. Taking into consideration the peculiarity of the echographic picture of the pyloric part, the neoplastic character of the pyloroduodenal stenosis may be presumed with high precision.

A combination of Populus tremula, Solidago virgaurea and Fraxinus excelsior as an anti-inflammatory and antirheumatic drug. A short review.

Anti-inflammatory properties of Populus tremula are mainly deduced from its components, the well investigated salicylates. Solidago extracts have spasmolytic, antihypertensive and diuretic effects. Fraxinus excelsior itself has hardly been investigated, but its coumarin components proved to have a variety of pharmacological properties, i.e. inhibition of T-cell activation and of the arachidonic acid cascade. Phytodolor N (abbreviated as PD) is a combination of the above mentioned plants. Various in vitro and especially in vivo studies proved its anti-inflammatory and antirheumatic properties, often comparable to non-steroidal anti-inflammatories, but with little or no side effects.

Treatment of renal colic by desmopressin intranasal spray and diclofenac sodium.

The vasopressin analogue, 1-desamino-8-arginine vasopressin (desmopressin), is a potent antidiuretic without the pressor effects of vasopressin. A total of 18 patients with acute renal colic due to stone disease received 40 microgramsf1p4mopressin intranasal spray with encouraging results. There was a significant decrease in the colic pain intensity from an initial mean visual analogue score of 67 +/- 17 mm. to 39 +/- 36 mm. within 30 minutes (p < 0.001). Eight patients (44.4%) had complete pain relief within 30 minutes of administering intranasal desmopressin spray. Nine of 10 patients who required intramuscular diclofenac sodium achieved complete pain relief within another 30 minutes. In other words, when intranasal desmopressin spray was administered before diclofenac sodium, 94.4% of the patients achieved complete pain relief and were discharged home. The mechanism of analgesic action of desmopressin in renal colic is uncertain. At the peripheral level, desmopressin may alleviate the acute renal colic through its potent antidiuretic effect or by relaxing the renal pelvic and ureteral smooth muscles. The central analgesic effect of desmopressin by stimulating the release of the hypothalamic beta-endorphin is proposed. We conclude that intranasal desmopressin spray can be used successfully in the treatment of renal colic. It may also replace prostaglandin synthetase inhibitors in treating renal colic with the advantage of avoiding the potential side effects. Further studies are needed to investigate whether the combination of desmopressin with analgesics or spasmolytic drugs offers competitive results compared with those achieved by prostaglandin synthetase inhibitors in the treatment of renal colic.

Acute central effects of the calcium channel blocker and antiglutamatergic drug caroverine. Double-blind, placebo-controlled, EEG mapping and psychometric studies after intravenous and oral administration.

In a double-blind, placebo-controlled study, the effects of caroverine (CAS 23465-76-1, Spasmium)--a quinoxaline derived from isoquinoline, the basic structure of papaverine, originally developed as a spasmolytic drug which may, however, exert central effects due to an unspecific calcium channel blocking action and glutamate antagonism--were investigated utilizing EEG-mapping, psychometric and psychophysiological measures. Fifteen healthy volunteers (8 males and 7 females), aged 20-35 years, received randomized and at weekly intervals 40 and 80 mg caroverine intravenously, 40 and 80 mg caroverine orally, as well as placebo. EEG recordings, psychometric and psychophysiological tests, as well as evaluation of pulse, blood pressure and side-effects were carried out at 0, 1, 2, 4, 6 and 8 h. Multivariate analysis of EEG mapping data demonstrated that caroverine exerted a significant action on human brain function, as compared with placebo, at all given dosages, as early as in the 1st and as late as in the 8th hour after both intravenous and oral administration during resting (R-EEG) and vigilance-controlled recording (V-EEG). Subsequent univariate analyses revealed, however, differential effects of caroverine--dependent on recording conditions and doses. In the resting condition, an increase of absolute and relative delta and theta power, a decrease of alpha and beta power and a slowing of the centroid of the delta and theta activity occurred, reflecting sedative action. The latter was slightly time- and dose-dependent (more sedation in earlier hours after higher doses). However, in the vigilance-controlled EEG, a dose-dependent decrease of delta and theta power, an increase of alpha power and an acceleration of the centroid of the delta and theta suggested vigilance-promoting effects, which were more pronounced in the later than earlier hours. Psychometric investigations confirmed these differential central effects. In regard to the noopsyche, an improvement was noted 6 h after the lowest oral dose. In the thymopsyche, a deterioration occurred after all doses except the highest oral dose, suggesting sedation. The latter was also seen after all active compounds as a trend in psychophysiological variables. The drug was well tolerated.

[A comparative evaluation of the results of using different means for stone-expelling therapy in urolithiasis].

A conventional approach to urolith elimination involving spasmolytics, water loading, terpene-containing drugs warrants efficacy in 80% of the cases. Diuretic tea promoted the stones evacuation from the ureter in 88.5%. Sinusoidal modulated currents with inductothermia and sound stimulation of the upper urinary tracts resulted in the stones elimination in 88 and 85.7% of the cases, respectively. A new method of urolithiasis treatment is suggested, tried and patented. It implies introduction of upper urinary tract sound stimulation with the frequency 2.4-3.3 kHz at the height of urinative effect of the tea. The resultant elimination of uroliths occurred in 129 of 134 patients treated (96.3%). The treatment is feasible both in inpatient and outpatient settings.

[Profile comment on clinical and experimental studies of pramiverine and pramiverine/metamizole (author's transl)].

The profile of the spasmolytic 4,4-diphenyl-N-isopropyl-cyclohexylamine-hydrochloride (pramiverine, Sistalgin) is outlined on the basis of fundamental research. Evidence is provided that a combination with metamizole is useful in the case of Sistalgin comp. Description of the clinical and clinico-experimental examinations confirmed the margin of indications for Sistalgin and Sistalgin comp. Reference is made to the applications with a view to facilitate endoscopic and radiological procedures. The tabulated list of 2368 patients demonstrates the positive ratio between therapeutic effect and undesirable cholinolytic concomitant effect of Sistalgin comp.

[The combined treatment of patients with atherosclerosis of the distal arterial bed of the lower extremities].

The authors summarize the experience of treatment of 178 patients with distal type of obliterated atherosclerosis of arteries of lower limbs. The 2nd stage of this malady was diagnosed in 91 (51.1%) patients; the 3rd stage--in 50 (28.1%); the 4th stage--in 37 (20.8%). 79 (44.4%) patients have received only conservative method of treatment, in 66 (37.1%) patients this method was accompanied by lumbar sympathectomy. Reconstructive operations with connection of one of the shin arteries to bloodstream were conducted in 33 (18.5%) patients. With the aim of improvement of distal artery channel visualization it is necessary to apply the digital subtraction angiography with the help of non-ionic roentgen-contrast preparations after the advanced application of spasmolytics.

[A new class of soluble guanylate cyclase activators, generating nitric oxide].

Derivatives of diazetidine-di-N-oxides have been found capable nonenzymatic generation of nitric oxide by a principally new mechanism of nitric oxide splitting at physiological values of pH. The effects of the synthesized compounds on human platelet soluble guanylate cyclase activity have been studied. Four of seven derivatives exhibited a distinct correlation between the ability of being decomposed with nitric oxide formation and soluble guanylate cyclase activation. Among those, 3-bromo-4-methyl-3.4-tetramethylene-diazetidine-di-N-oxide proved to be the most effective activator, its spasmolytic effect being commensurate with the glyceryl trinitrate activity. The data obtained open up new opportunities for a search into new vasodilatory agents among compound whose chemical structure provides nitric oxide generation and guanylate cyclase activation in human tissues. Besides, they provide evidence that 1.2-diazetidine-1.2-di-N-oxide derivatives are perspective objects for creating new vasodilatory drugs.

[Accelerated elimination of the stone fragments after extracorporeal shockwave lithotripsy].

As shown in experiments on 40 Chinchilla male rabbits, sinusoidal modulated currents (SMC) and ultrasound (US) are able to enhance bioelectrical activity of the ureteral smooth muscles. Activation of the muscular wall biopotential is followed by 0.15-0.2 s contraction. The above effect was used by the authors in rehabilitation of 120 patients after ECIL. SMC and US were included into the physiotherapeutic complex. The above patients whose concrements after the fragmentation diminished to less than 7 cm in size were divided into 2 groups. Sixty patients of group 1 received spasmolytic drugs, sodium chloride or iodine-bromine baths and dynamic amplipulse therapy. Sixty patients of group 2 in addition to the above treatment were exposed to local vibration and US. All the patients were given antibacterial drugs. The percent of patients with complete urolith evacuation in group 1 amounted to 91.7%, with partial evacuation 8.3% (96.7 and 3.3% in group 2, respectively). Dynamic nephroscintigraphy demonstrated that after relevant physiotherapy renal function improved, leukocyte and red cell count in the urine and ferrum, alkaline phosphatase levels in the serum decreased.

Pharmacotherapy of gastrointestinal motor disorders.

Pharmacotherapy of gastrointestinal motor disorders is complicated by the large variety of drug targets and their interactions, the complex relation between contractile patterns and propulsion and the occurrence of regional and species differences. A number of drug classes are discussed: cholinergic agonists/antagonists, dopamine-antagonists, calcium-entry blockers and spasmolytics, serotonin-5-HT4-agonists, cholecystokinin-antagonists, motilin-agonists, 5-HT3-antagonists, opiate agonists, and laxatives. The number of drug classes is increasing rapidly allowing a more specific treatment for the large number of gastrointestinal disorders related to disturbed motility. An interesting new development is the search for compounds interfering with transmitters of the non-adrenergic non-cholinergic nerves of the enteric nervous system a.o. nitric oxide (NO).

[The clinical pharmacological profile of pinaverium bromide].

Pinaverium bromide is a locally acting spasmolytic agent of the digestive tract. Its mechanism of action relies upon inhibition of calcium ion entrance into smooth muscle cells (calcium-antagonist effect). In humans pinaverium facilitates gastric emptying and decreases intestinal transit time in patients with constipation. Pinaverium is very effective in improving symptoms of irritable bowel syndrome (abdominal pain, gas, diarrhea or constipation). In this respect the drug proved to be significantly superior to placebo, at least as effective as trimebutine and on the whole more active than otilonium and prifinium bromide, being always extremely well tolerated.
