In this paper, we present the methodology we adopted in designing and developing an object-oriented database system for the management of medical records. The designed system provides technical solutions to important requirements of most clinical information systems, such as 1) the support of tools to create and manage views on data and view schemas, offering to different users specific perspectives on data tailored to their needs; 2) the capability to handle in a suitable way the temporal aspects related to clinical information; and 3) the effective integration of multimedia data. Remote data access for authorized users is also considered. As clinical application, we describe here the prototype of a user-oriented clinical information system for the archiving and the management of multimedia and temporally oriented clinical data related to percutaneous transluminal coronary angioplasty (PTCA) patients. Suitable view schemas for various user roles (cath-lab physician, ward nurse, general practitioner) have been modeled and implemented on the basis of a detailed analysis of the considered clinical environment, carried out by an object-oriented approach.
We present COMPUCELL3D, a software framework for three-dimensional simulation of morphogenesis in different organisms. COMPUCELL3D employs biologically relevant models for cell clustering, growth, and interaction with chemical fields. COMPUCELL3D uses design patterns for speed, efficient memory management, extensibility, and flexibility to allow an almost unlimited variety of simulations. We have verified COMPUCELL3D by building a model of growth and skeletal pattern formation in the avian (chicken) limb bud. Binaries and source code are available, along with documentation and input files for sample simulations, at http:// compucell.sourceforge.net.
Medication errors are significant and well-known problems in health care. Despite the evidence supporting the use of computerized physician order entering (CPOE) to help reduce medication errors, only a small number of hospitals in the U.S. have successfully implemented a CPOE system. Different authors have indicated that the utilization of order sets derived from best-practice standards can reduce medication errors and improve physicians' acceptance of CPOE systems. However, a variety of issues related to the development and continuous maintenance of best-practice order sets still need to be understood. This paper presents a model that supports an order set development process driven by clinical experts. Model requirements and details are presented and discussed.
Since machine-based exercise still uses local facilities, it is affected by time and place. We designed a web-based system architecture based on the Java 2 Enterprise Edition that can accomplish continuously supported machine-based exercise. In this system, exercise programs and machines are loosely coupled and dynamically integrated on the site of exercise via the Internet. We then extended the conventional health promotion model, which contains three types of players (users, exercise trainers, and manufacturers), by adding a new player: exercise program creators. Moreover, we developed a self-describing strategy to accommodate a variety of exercise programs and provide ease of use to users on the web. We illustrate our novel design with examples taken from our feasibility study on a web-based cycle ergometer exercise system. A biosignal-based workload control approach was introduced to ensure that users performed appropriate exercise alone.
This paper presents the architectural details of an evolving open source/open architecture software framework for developing organ-level surgical simulations. Our goal is to facilitate shared development of reusable models, to accommodate heterogeneous models of computation, and to provide a framework for interfacing multiple heterogeneous models. The framework provides an application programming interface for interfacing dynamic models defined over spatial domains. It is specifically designed to be independent of the specifics of the modeling methods used, and therefore facilitates seamless integration of heterogeneous models and processes. Furthermore, each model has separate geometries for visualization, simulation, and interfacing, allowing the model developer to choose the most natural geometric representation for each case. Input/output interfaces for visualization and haptics for real-time interactive applications have also been provided.
Many tools have been developed for visualization of RNA secondary structures using a variety of techniques and output formats. However, each tool is typically limited to one or two of the visualization models discussed in this paper, supports only a single file format, and is tied to a specific platform. In order for structure prediction researchers to better understand the results of their algorithms and to enable life science researchers to interpret RNA structure easily, it is helpful to provide them with a flexible and powerful tool.jViz.Rna is a multiplatform visualization tool capable of displaying RNA secondary structures encoded in a variety of file formats. The same structure can be viewed using any of the models supported, including linked graph, circle graph, dot plot, and classical structure. Also, the output is dynamic and can easily be further manipulated by the user. In addition, any of the drawings produced can be saved in either the EPS or PNG file formats enabling easy usage in publications and presentations.
In this paper, we present a neural network for solving the nonlinear convex programming problem in real time by means of the projection method. The main idea is to convert the convex programming problem into a variational inequality problem. Then a dynamical system and a convex energy function are constructed for resulting variational inequality problem. It is shown that the proposed neural network is stable in the sense of Lyapunov and can converge to an exact optimal solution of the original problem. Compared with the existing neural networks for solving the nonlinear convex programming problem, the proposed neural network has no Lipschitz condition, no adjustable parameter, and its structure is simple. The validity and transient behavior of the proposed neural network are demonstrated by some simulation results.
Stroke is a leading cause of disability in the United States and yet little technology is currently available for individuals with stroke to practice and monitor rehabilitation therapy on their own. This paper provides a detailed design description of a telerehabilitation system for arm and hand therapy following stroke. The system consists of a Web-based library of status tests, therapy games, and progress charts, and can be used with a variety of input devices, including a low-cost force-feedback joystick capable of assisting or resisting in movement. Data from home-based usage by a chronic stroke subject are presented that demonstrate the feasibility of using the system to direct a therapy program, mechanically assist in movement, and track improvements in movement ability.
This paper presents the basic features of software-based magnifiers used by some visually impaired people to read information from a computer screen. The paper briefly presents two major approaches to full-screen magnification for modern multiple window systems (the paper focuses on Microsoft Windows). This paper describes in detail the architecture and operation of a full-screen magnifier that uses Microsoft DirectX Overlays. This approach leads to a robust magnifier that has a low computational overhead. The magnifier has problems with video cards that use a YUV color model but these problems may be addressed by RGB to YUV translation software--an issue that is still to be investigated. The magnifier also has problems when the generic device driver, rather than the manufacturer's device driver, is installed on the system. The paper presents two further strategies for full screen magnification, namely, using multimonitor support and true type fonts for text enlargement.
In this paper the architecture of a hardware and software platform, for ultrasonic investigation is presented. The platform, used in conjunction with an analog front-end hardware for driving the ultrasonic transducers of any commercial echograph, having the radiofrequency echo signal access, make it possible to dispose of a powerful echographic system for experimenting any processing technique, also in a clinical environment in which real-time operation mode is an essential prerequisite. The platform transforms any echograph into a test-system for evaluating the diagnostic effectiveness of new investigation techniques. A particular user interface was designed in order to allow a real-time and simultaneous visualization of the results produced in the different stages of the chosen processing procedure. This is aimed at obtaining a better optimization of the processing algorithm. The most important platform aspect, which also constitutes the basic differentiation with respect to similar systems, is the direct processing of the radiofrequency echo signal, which is essential for a complete analysis of the particular ultrasound-media interaction phenomenon. The platform completely integrates the architecture of a personal computer (PC) giving rise to several benefits, such as the quick technological evolution in the PC field and an extreme degree of programmability for different applications. The PC also constitutes the user interface, as a flexible and intuitive visualization support, and performs some software signal processing, by custom algorithms and commercial libraries. The realized close synergy between hardware and software allows the acquisition and real-time processing of the echographic radiofrequency (RF) signal with fast data representation.
Two different mathematical procedures to compute source densities from cross-fire matrices in quantitative EM autoradiography are reported. BASIC programs for a desk-top microcomputer were written to fit the hypothetical silver grain distribution to the observed real silver grain distribution using a non-iterative linear process (least-squares procedure) and an iterative method, which minimizes the chi-square component between the observed and computed silver grain distributions. The latter procedure uses a Gauss-Newton algorithm.
Pyrolysis mass spectrometry was used to produce complex biochemical fingerprints of Eubacterium exiguum, E. infirmum, E. tardum and E. timidum. To examine the relationship between these organisms the spectra were clustered by canonical variates analysis, and four clusters, one for each species, were observed. In an earlier study we trained artificial neural networks to identify these clinical isolates successfully; however, the information used by the neural network was not accessible from this so-called 'black box' technique. To allow the deconvolution of such complex spectra (in terms of which masses were important for discrimination) it was necessary to develop a system that itself produces 'rules' that are readily comprehensible. We here exploit the evolutionary computational technique of genetic programming; this rapidly and automatically produced simple mathematical functions that were also able to classify organisms to each of the four bacterial groups correctly and unambiguously. Since the rules used only a very limited set of masses, from a search space some 50 orders of magnitude greater than the dimensionality actually necessary, visual discrimination of the organisms on the basis of these spectral masses alone was also then possible.
Efficient massive mapping algorithm (EMMA), an algorithm on efficiently mapping massive cDNAs onto genomic sequences, has recently been developed. The process of mapping massive cDNAs onto genomic sequences has been improved using more approximate mapping filtering based on an enhanced suffix array coupled with a pruned fast hash table, algorithms of block alignment extensions, and k-longest paths. When compared with the classical BLAT software in this field, the computing of EMMA ranges from two to forty-one times faster under similar prediction precisions.
